Background: Although breast cancer is the most common cancer in women, economic evaluation of breast cancer screening is not fully addressed in developing countries. The main objective of the present study was to analyze the cost-effectiveness of breast cancer screening using mammography in 35-69 year old women in an Iranian setting. Materials and Methods: This was an economic evaluation study assessing the cost-effectiveness of a population-based screening program in 35-69 year old women residing in rural areas of South east Iran. The study was conducted from the perspective of policy-makers of insurance. The study population consisted of 35- to 69-year old women in rural areas of Kerman with a population of about 19,651 in 2013. The decision tree modeling and economic evaluation software were used for cost-effectiveness and sensitivity analyses of the interventions. Results: The total cost of the screening program was 7,067.69 US$ and the total effectiveness for screening and no-screening interventions was 0.06171 and 0.00864 disability adjusted life years averted, respectively. The average cost-effectiveness ratio DALY averted US$ for screening intervention was 7,7082.5 US$ per DALY averted and 589,027 US $ for no-screening intervention. The incremental cost-effectiveness ratio DALY averted was 6,264 US$ per DALY averted for screening intervention compared with no-screening intervention. Conclusions: Although the screening intervention is more cost-effective than the alternative (noscreening) strategy, it seems that including breast cancer screening program in health insurance package may not be recommended as long as the target group has a low participation rate.
In this study, we aimed to identify the impact of socioeconomic characteristics on the health status of Chinese, which suggests that there might be the phenomena of health inequality in China. We used the year 2000, 2004 and 2009 pooled cross-sectional data of China Health and Nutrition Survey (CHNS), and utilized the Ordinary Least Square model (OLS) and Ordered Logit Estimation Method for this purpose. Empirical results showed that socioeconomic status and year dummy variables have a meaningful impact on Chinese health status. Therefore, we conclude that the phenomenon of health inequality has existed in China since 2000.
Objectives: One of the main objectives of the Targeted Subsidies Law (TSL) in Iran was to improve equity in healthcare financing. This study aimed at measuring the effects of the TSL, which was implemented in Iran in 2010, on equity in healthcare financing. Methods: Segmented regression analysis was applied to assess the effects of TSL implementation on the Gini and Kakwani indices of outcome variables in Iranian households. Data for the years 1977-2014 were retrieved from formal databases. Changes in the levels and trends of the outcome variables before and after TSL implementation were assessed using Stata version 13. Results: In the 33 years before the implementation of the TSL, the Gini index decreased from 0.401 to 0.381. The Gini index and its intercept significantly decreased to 0.362 (p<0.001) 5 years after the implementation of the TSL. There was no statistically significant change in the gross domestic product or inflation rate after TSL implementation. The Kakwani index significantly increased from -0.020 to 0.007 (p<0.001) before the implementation of the TSL, while we observed no statistically significant change (p=0.81) in the Kakwani index after TSL implementation. Conclusions: The TSL reform, which was introduced as part of an economic development plan in Iran in 2010, led to a significant reduction in households' income inequality. However, the TSL did not significantly affect equity in healthcare financing. Hence, while measuring the long-term impact of TSL is paramount, healthcare decision-makers need to consider the efficacy of the TSL in order to develop plans for achieving the desired equity in healthcare financing.
Purpose: The purpose of this study was to test a hypothetical model of health-related quality of life in patients with heart failure. The hypothetical model was derived from the Wilson and Cleary's model, the Rector's model, and published research findings. Methods: Data from 103 patients with heart failure were analyzed to determine the best multivariate health-related quality of life model given variables derived from the prior studies. The statistics programs SPSS 12.0 and LISREL 8.7 program were used for descriptive statistics and covariance structure analysis respectively. Results: The overall fitness of the path final model was good(GFI=.97, AGFI=.95, NNFI=1.06, NFI=.96, p=.96). Symptoms were directly affected by gender. HYHA Class was directly affected by only gender. Physical functioning limitation was directly affected by exercise. Health perception was directly affected by economics, symptom, and physical functioning limitation. Depression was directly affected by exercise and health perception. Heath-related quality of life was directly affected by physical functioning limitation and depression, indirectly affected by gender, economics, exercise, symptoms, NYHA Class, and health perception. This path analysis model explained 51% of health-related quality of life in patients with heart failure. Conclusion: To improve of health-related quality of life with heart failure patients, it is necessary to make nursing interventions for physical functioning and depression.
Rural Development Administration (RDA) has developed a manufacturing technology which can increase the amount of platycodin D in Platycodon grandiflorum. A study is needed to estimate the value of this new technology and predict the market demand for a new product. This study estimates additional amount of willingness to pay (WTP) using contingent valuation method (CVM) for a new product with the technology that RDA has developed. The survey was created under virtual health functional foods market of Platycodon grandiflorums. It was conducted with 1,000 adult males and females aged between 19 and 59 years old in 17 major cities and provinces nationwide from December 4 to December 7, 2018. The amount of WTP for the health functional Platycodon grandiflorum was drawn using maxinum likelihood estimation method. The estimated average and median WTP values are 21,933.85 won per person and 10,000 won per person, respectively. The independent factors, including the ratio of monthly average health function food consumption to income, the average monthly income level of a household, and existence of family members or relatives engaged in food and nutrition-related professions in a household, have been shown to have a statistically significant influence on the WTP. This study presents the potential magnitude of health functional food market and the value of the new technology based on health functional food market.
This study aimed to develop, and evaluate the effectiveness of, a home economics dietary education program that could be used to improve high school students' sustainable dietary competencies. To achieve this goal, learning objectives and elements were selected for the education program to improve the students' sustainable dietary competency. The content validity of this program was verified with numerous experts. After implementing the new educational program, its effect on high school students' sustainable dietary competencies was assessed through pre- and post-tests. The results of this study are summarized as follows. First, through the textbook analysis, the necessity of developing an educational program that can cultivate the three areas that constitute a sustainable diet (environment, health, and consideration) was confirmed. Second, a total of nine teaching and learning plans for sustainable dietary education programs were developed. From there, content validity verification was conducted by experts, indicating that the developed educational program was highly applicable in the field. Third, after the implementation, a survey was conducted based on a questionnaire consisting of 20 items related to three areas of sustainable diet, while the pre- and post-test assessment indicated statistically significant differences in all three areas: environment (pre: 3.40, post: 4.46, p<.001), health (pre: 3.15, post: 4.32, p<.001), and consideration (pre: 3.46, post: 4.48, p<.001). It is expected that the educational program developed in this study will be used as a basis for dietary education that fosters food citizenship in high school home economics courses.
The purpose of this study was to investigate the differences of middle school students' recognition of food and nutrition part in Home Economics by gender, school location and school type. The study was carried out through questionnaire about three category: perception (interests, practical usefulness, attitude change in food behavior), difficulty (nutrition and health in adolescence, balanced diet, principle of cooking and preparing food), necessity (nutrition and health in adolescence, balanced diet, principle of cooking and preparing food). The subjects were 1014 middle school students of 1st grade in Jeonbuk area; urban 334, suburban 339 and rural 341. The results of this study were as follows. 1) There was difference in ‘interests’ by gender (girls > boys), school location (urban, suburban > rural) and school type (coeducation > boys, girls). There were differences in ‘practical usefulness’ by school location (urban > rural) and in ‘attitude change in food behavior’ by school location (urban > suburban > rural) and school type (boys, girls > coeducation). 2) There was difference in difficulty in section of ‘nutrition and health in adolescence’ by school location (suburban > rural). There were no differences in section of ‘balanced diet’ by gender, school location and school type. But there were differences in section of ‘ principle of cooking and preparing food’ by school location (urban, rural > suburban) and school type (coeducation > boys). 3) There were differences in necessity for ‘balanced diet’ and for ‘ principle of cooking and preparing food’ by school location (urban, suburban > rural) and school type (coeducation > boys, girls). Characteristically, the boys had more necessity for ‘nutrition and health in adolescence’ and for ‘balanced diet’ than girls but girls had more necessity for ‘principle of cooking and preparing food’ than boys. It suggests that gender, school type and school location should be considered for an effective and practical curriculum of food and nutrition part in Home Economics.
This study was attempted to investigate the difference f eating behavior and nutrients intake according to Region. The summarized results are as follows; the economic values were most important scores and highest ranks for most of subjects, followed by health and social. subjects in Cheon Nam consider health values more important than those in Seoul, who consider social values more important than former. Food habits of subjects in both regions belonged to "Fair" group, the score of food habit it in Cheon Nam was higher than those in Seoul. Though Subjects in both regions showed similar in level of nutrients intake, Cheon nam are a little higher than Seoul(average 1~2%). This survey showed the more they concerned with health value, the better food habits and nutrients intake they had. The residents in Cheon Nam regarded health more importantly and had better food habits, and higher standard of nutrients intake than those in Seoul. in Seoul.
The purpose of this study was to analyze on effects of midlife ego-identity and crisis on quality of life in married women. The data were obtained through 394 midwife married women who live in Kwangju and Chonnam. The results were as follows. (1) The mean scores of ego-identity and quality of life were higher than the medium point. The mean scores of crisis was a little lower than the medium. (2) The ego-identity was significantly influenced by religion, health condition, social intimacy, marital relations, and parent-child relations. The crisis was influenced significantly by income, health condition, marital relations, and ego-identity. And the quality of life was influenced significantly by religion, health condition, marital relations, parent-child relations, and crisis. (3) On the result of path analysis, religion, health condition, marital relations, parent-child relations, and crisis directly influenced the Quality of life. Besides, income, social intimacy, and ego-identity indirectly affected the quality of life.
This study was conducted to investigate the effects of weaning behavior on infants' health status. 294 mothers who had infants, aged 4 to 12 months in Seoul, were selected by cluster sampling and answered by the special questionnaires. Result were as follows : 55.1% of mothers had commenced weaning by 3 months of age. Infants eatten commercial weaning food except for infant eatten only home made weaning food were 88.8%. The case that mothers couldn't make weaning food at the home were 56.6% because they didn't know how to cook of weaning food. Cereals were used frequently as infant foods while meat and fish were lesser used. The higher mother's education level had, the higher nutritional knowledge had. Nutritional knowledge was not influenced on selecting the kinds of weaning food. But the higher nutritional knowledge had, the more desirable weaning behavior mother tended to be had. The more desirable weaning behavior mother had, the more infant's health status tended to be improved. Therefore, for the desirable weaning behaviors of mothers and the improvement of infants' health status, nutritional education program including cook method and development of infant food is need.
