Utility of Serum Albumin for Predicting Incident Metabolic Syndrome According to Hyperuricemia

  • Background: Serum albumin and uric acid have been positively linked to metabolic syndrome (MetS). However, the association of MetS incidence with the combination of uric acid and albumin levels has not been investigated. We explored the association of albumin and uric acid with the risk of incident MetS in populations divided according to the levels of these two parameters. Methods: In this retrospective longitudinal study, 11,613 non-MetS participants were enrolled among 24,185 individuals who had undergone at least four annual check-ups between 2006 and 2012. The risk of incident MetS was analyzed according to four groups categorized by the sex-specific medians of serum albumin and uric acid. Results: During 55,407 person-years of follow-up, 2,439 cases of MetS developed. The risk of incident MetS increased as the uric acid category advanced in individuals with lower or higher serum albumin categories with hazard ratios (HRs) of 1.386 (95% confidence interval [CI], 1.236 to 1.554) or 1.314 (95% CI, 1.167 to 1.480). However, the incidence of MetS increased with higher albumin levels only in participants in the lower uric acid category with a HR of 1.143 (95% CI, 1.010 to 1.294). Conclusion: Higher levels of albumin were associated with an increased risk of incident MetS only in individuals with lower uric acid whereas higher levels of uric acid were positively linked to risk of incident MetS regardless of albumin level.

Comparisons of the prevalence and analysis of risk factors affecting gallstone disease on Jeju Island

  • The reported risk factors for gallstone disease (GD) are old age, female sex, obesity, metabolic syndrome and migrants. Many younger adults tend to live in Jeju City, where transportations are convenient and commercial activities are active. Whereas, older people tend to live in Seogwipo City, because they engaged in fisheries and agriculture. Rates of migrants and old age could affect the prevalence of GD among residents in two regions. Therefore, the purpose of this study was to compare the GD prevalence and analyze risk factors affecting GD including residencies. A total of 13,050 subjects who visited a single health medical check-up center on Jeju Island between 2012 and 2019 were included. We performed univariate and multivariate analysis to identify risk factors for GD. The prevalence of GD among residents were 5.7% in Jeju City and 5.8% in Seogwipo City, respectively. Multivariate analysis revealed that age (P=0.008), body mass index (P=0.044), high-density lipoprotein cholesterol (P=0.006) and gamma-glutamyl transferase (P=0.013) were independent factors affecting GD. The old age, mean higher body mass index, gamma-glutamyl transferase and mean lower high-density lipoprotein cholesterol were independent risk factors affecting GD. However, residencies did not affect the prevalence of GD.

Analysis of Five Years of US Immigration Medical Exams (5년간의 미국 이민 비자 신체검사 결과 분석)

  • Purpose: The United States of America have pre-migration screening program are mandated through legislation. The one of purpose of this program is to prevent the importation of certain communicable diseases. Examinations of migration applicants are performed by panel physicians who are licensed physicians in the countries of origin that have agreements with the government departments of the country of destination to undertake this activity. In recent decades, the number of US migrants has increased, however COVID-19 impacted this trend of international migrants. The Aim of this study is to examine trends of US applicants and outcomes of tuberculosis and sexual transmitted infections (STIs) screening in South Korean applicants for US visa. Methods: A total of 4,442 applicants participated in US visa health check-up in 2016 to 2020. Results: The numbers of applicant for US visa in males and females was 1,814 and 2,628. The positive results in tuberculosis and STIs screening was noted with 0.66% in nontuberculous mycobacteria (NTM), 0.03% in tuberculosis, 0.03% in gonorrhea, 0.08% in syphilis, and 0.74% in chlamydia. The age groups of 2 to 14 years showed higher positive rate (13.89%) in tuberculin skin test (TST) tests compared to the rate of interferon gamma release assay (IGRA) tests (0.44%, P<0.001). The positive ratio of NTM and abnormal chest X-ray (CXR) which required sputum tests among the applicants over 15 years old has trends to increase with age. The age groups of over 70 years showed high positive rate in NTM and abnormal CXR (4.10%, 20.51%). However, ages 15 to 49 years groups showed 0.00% to 0.30% in NTM and 1.08% to 3.91% in abnormal CXR. In the study population, the positive rates of STIs were 0.03%, 0.08%, and 0.74% in gonorrhea, syphilis, and chlamydia, respectively. Among the participants who underwent sputum smears and cultures were just 1 case of active tuberculosis (0.03%) and 25 cases of NTM infection (0.66%). Conclusion: This study found that rate of positivity in tuberculosis and STIs screening among South Korean applicants for US visa in 2016 to 2020. Regarding tuberculosis, the positivity of CXR among the aged 15 to 59 years applicants was lower than that among over 60 years old. Among 2 to 14 years, positivity of IGRA was very low. And IGRA test can reduce effort of further evaluation compared to TST test in South Korea, so IGRA test among young adults can be useful. High positive rates in STI among young adult could result in high active sexuality, especially chlamydia infection is most common. Therefore Chlamydia polymerase chain reaction should be considered formal protocol among the Korean applicant for US visa.

The Development of Web-based Nutrition Information Contents for Older Adults : Content Analysis and Card-sorting process (노인대상 영양정보 웹사이트 컨텐츠 개발 : 내용분석과 카드소팅과정(Card-sorting process))

  • This study was undertaken to develop web-based nutrition information contents for the older adults. Twenty six domestic web-sites were analyzed and then 12 foreign web-sites and 4 education materials for the elderly of foreign university were benchmarked. Also a lot of literatures on elderly education program were reviewed. A card-sorting task was performed with 8 older adults to ascertain how the target audience organized information about nutrition. The results were as fellows. Among 26 domestic web-sites, 2 sites(7.7%) were only for the elderly. Main topics of information contents for the elderly included 'Importance of Healthy Eating', 'DRI', 'Dietary Guidelines'. Four of twelve foreign web-sites were for the elderly nutrition education. Topics of 'Dietary Guideline', 'Meal Program' were found in 4 sites and 'Importance of Healthy Eating', 'Diet & Disease', 'DRI, 'Food Guide Pyramid', 'Nutrition Fact Labels' were found in 3 sites. Education materials of foreign university dealt with basic information on 'nutrient needs changes related with aging', 'Heart & Bone Healthy Eating Plan', 'Food Guide Pyramid'. Also topics on 'Eating on a budget', 'Eating Out Guideline' were included for practical use for the elderly. Based on card-sorting process, contents framework for web-site was developed and 4 main menus for framework were respectively named as 'Nutrition', 'Meals', 'Foods'. 'Check up Nutritional Health' by panel discussion. Finally we developed nutrition information contents for 4 main menus. We focused on helping older adults recognize the importance of healthy eating and apply the nutrition information to practical use. We expect that the developed framework of contents can be a guideline for indentifying the information needs of older adults in developing effective nutrition intervention program. And we suggest that the survey for target people should be peformed for the web-site to be user-friendly designed and the developed contents be evaluated and revised in the near future.

A Preliminary Study on the Wellness-Enhancing Effect of an App for the Development of the Self-Life Coaching App (셀프 라이프코칭 앱(App) 개발을 위한 앱의 웰니스 증진 효과에 대한 예비 연구)

  • Considering the necessity for individuals to check and manage their wellness to promote happiness, this study conducted preliminary research on the effect of wellness enhancement of the self-life coaching app. Thirty-six participants were randomly assigned to experimental groups 1 and 2 and a control group, respectively. In particular, the experimental groups were asked to perform self-life coaching for 4 weeks using a research app. Weekly feedback was provided to experimental group 1, but not to experimental group 2. The wellness index for workers (WIW) and the Korean wellness scale (KWS) were used as dependent variables. The analysis results of interaction effects between pre-, post-, and follow-up time points and groups reveal no statistically significant effects in both dependent variables, but the tendency of the effects was confirmed. Moreover, no difference was determined in terms of the presence or absence of weekly feedback in the experimental treatment effect. As a result of repeated measures variance analysis on KWS measured at five times, the wellness enhancement effect over time was statistically significant. Meanwhile, the polynomial trend analysis result confirmed the linear effect of the experimental treatment on wellness. The participation opinion survey also revealed that 84.2% of the participants perceived the activity to be helpful, and 78.9% responded that they would use it once every 1 or 2 weeks if the app is commercially available. This study is meaningful in that it suggests a methodology for utilizing the self-life coaching app so that anyone can use it to promote their wellness and partially confirms the effectiveness of this method.

Implant-assisted removable partial denture restoration in small number of residual teeth in mandible: A case report (하악 소수 잔존치 환자에서 임플란트 보조 국소의치 수복 증례)

  • The patient in this case presented with a desire to have new dentures due to discomfort with existing ones. At the initial visit, all of teeth were missing except for the mandibular left second molar. As the patient was 65 years old, treatment with dentures and implant-supported prostheses was possible under the national health insurance system, and the patient opted for the mandibular denture using implant. Temporary prostheses were initially provided for patient adaptation, and following successful adaptation period, the treatment progressed. A maxillary complete denture and a mandibular implant-supported denture using two implants in the canines were fabricated. The mandibular denture is a Kennedy Class II removable partial denture which consists of a six-unit porcelain fused to metal fixed dental prostheses supported by the implant in the canines on both sides and left second molar serving as the abutments. Despite severe bone resorption and insufficient abutment teeth, the patient expressed satisfaction with the treatment results. In cases with economic and anatomical constraints affecting the feasibility of complete denture, implant-supported overdenture, and implant-supported fixed dental prostheses, an implant-assisted removable partial denture using implant surveyed crowns proves to be a viable and effective alternative treatment option. Nevertheless, the current dearth of scientifically rigorous studies underscores the necessity for meticulous regular check-up and occlusal assessment.

A study on the compatibility of implant drivers (임플란트 드라이버의 호환성에 대한 연구)

  • Purpose: In this study, the diameter of each implant driver was measured and compared to find out the compatibility of implant drivers. Materials and methods: Drivers from 12 implant systems being used in Dankook University Dental Hospital were included in this study. The shapes of the implant drivers were segregated, and the effective length and the diameter of upper, middle, lower part of driver tips were measured (n=10). The measured data were mathematically analyzed for its compatibility. Results: A driver with the smallest diameter (1.17 mm) had the highest compatibility at the upper part of driver tip. This driver could be used for a bigger driver up to 1.35 mm in diameter. There were several driver groups which had the same diameter so as to be interchangeable each other. In the middle part, the smallest diameter measured was 1.2 mm and this was able to replace a driver up to 1.40 mm diameter. Since the diameter generally became thicker from upper part (the tip of driver) to the lower part (the shank of driver), some drivers with bigger diameter at the upper part so which was failed to show any compatibility became compatible with a driver which had smaller diameter at the upper part but wider in the middle part. The compatibility of torx shape drivers were affected by the inner diameter of the drivers not only by the outer diameter. Furthermore, the inner diameter of torx drivers decided the compatibility between torx and hex drivers. Conclusion: From the study it was found that compatibility in drivers existed among certain implant systems and to check its compatibility the diameter at a certain effective length should be measured. However, there has been not enough studies about long-term use of compatible drivers, so effects of using compatible drivers on drivers and implants are unknown. Therefore, usage in inevitable cases only is recommended and further study is needed.

Midwives' Perceptions of the Importance of Teaching the Lamaze Method of Childbirth Preparation, Their Practice of it and Inhibiting Factors. (조산사의 라마즈 산전교육에 대한 중요성 인식과 수행정도 및 저해요인에 관한 연구)

  • This study was done to investigate how importantly midwives thought of Lamaze Method of Childbirth, how satisfactorily they performed their Lamaze Method of Childbirth and what inhibited their activities from being performed if not done satisfactorily. The subjects of this study were 76midwives at midwifery in Pusan, Deagu, Kyungnam and Kyungbok. Data were collected by using questionaire from Sep. 1 to Sep.30, 1991. The measurement tools was maded by the researcher based on Kim's scale for prenatal nursing activities. The statistical analysis was done with based statistical values likes frequencies, means, standard deviations and advanced methods such as Pearson's correlation coefficient, 1-test, F-test, F-test by using SPSS package program. The results of this study can be summarized as follows ; 1. The results of Investigating perception according to importance degree and performance degree of Lamaze Method of Childbring showed that midwives were slightly below both perception according to importance(2.17) and performance(2.16) of Lamaze Method of Childbirth 2. Inhibiting factors of Lamaze Method of Childbirth were abscence of husband(3.08), lack of teaching room(2.71), and lack of books or literature of Lamaze Method (2.58). 3. There appears to be a positive relation between perception according to importance degree and performance degree of Lamaze Method of Childbirth (r=0.69, P<0.01). There appears to be a negative relation between perception according to importance degree and inhibit factors of performance (r=-0.38, P<0.01). There appears to be a negative relation between performance and inhibit factors of performance (r=-0.22. P<0.01). 4. The item of highest perception defree in prenatal education is 1st, Consultation about sexual life (2.53), End, illustration of the importance of a bath and perinal cleanliness and Avoidance of excessive respiration method and also its comfortable application during uterine contractility(2.45). The item of lowest perception degree is about pregnant symptom sign and predicted date of birth (E.D.C.). On the other hand, The item of highest performance degree is education about nurtrition during pregnacy. 2nd, Illustration of the Importance of a bath and perineal cleanness(2.45). The lowest one is leaching the pregnant woman and her family about the role of husband and her family and also teaching the couple to learn exercise and respiration method (1.84). Though importance perception degree and performance degree is low. Midwives who wert subject in this research shoves that they perform general items concerning prenatal education well. 5. In the relationship between perception according to importance degree of Lamaze Method of Childbirth and the general characteristics of subjects, the differences in mean number of visiting pregnacies for 1 month. 6. In the relationship between performance degree of Lamaze Method of Childbirth and the general characteristics of subjects, the differences in age, marriage, and mean number of visiting pregnancies for 1 month. With the above information we see the importance of midwife prenatal care education and high correlation between performance degree and perception degree. When Lamaze prenatal education is well performed the number of pregnant woman who have follow-up, check-ups increase. Therefore in this research we can validity that there is a relation between Lamaze prenatal education and patient follow-up. This research showes in a situation where if one does not maximally perform a prenatal education there is a possibility that the popular use of midwife activities may encounter obstacles so the education to save new knowledge and training for prenatal education is needed as a function of Lamaze prenatal education, when a special Lamaze education is well performed for the pregnant woman, who follows-up at amidwife clinic. With the above conclusion we can suggest : 1. Continued research which minimized obstacles to Lamaze prenatal education is needed. 2. The official method of midwives is necessary.

Characteristics of User's Behavior across Generations for space planing in General Hospital (종합병원 환경계획을 위한 세대별 종합병원 이용행태 특성분석)

  • This study is a basic research to suggest user-centered general hospital environmental design guidelines, which aims to analyze user's behavior characteristics across generation in general hospital. For this purpose, this study constructed an analysis tool through the literature review with regard to generation and behavior characteristics in general hospital. Besides, an online survey regarding user's behavior in general hospital was conducted targeting from 20s to 60s, 300 persons for each group, total 1,500 persons for about 3 weeks since September 1, 2016. The results of this study are as follows: (1) Based on the generation, there were significant differences in relevant categories of their visiting frequency, visiting purpose, visiting hour, transportation, companion, behavior during the wait and selection of a general hospital. (2) In all generation, they responded that they have visited once or twice per year. People in 20s and 30s responded that their visit for the hospital is to receive specific treatment, while other people in 40s, 50s and 60s visit the hospital majorly for routine check-ups. Therefore, it is imperative for a health check-up center to design an environmental plan that reflects the characteristics of elders in 40s, 50s and 60s. (3) People in 40s, 50s and 60s usually visit a general hospital in the mornings of weekdays, while generations in 20s and 30s responded that they mostly visit the hospital in the mornings of weekend. (4) When they visit a general hospital, people in their 20s are usually using public transportations, while people in their 30s to 60s are using their own vehicle. (5) People in their 20s majorly visited 'lobby'. In older generations, they tend to visit 'outpatient clinic'. Therefore, it is necessary to build an outpatient clinic environment that considers the elderly. (6) Patients majorly responded that they are using their cell phone, while waiting for their clinic call. In elder generations, they responded that they are more likely watching TVs, reading books/magazines or doing nothing. Therefore, it is essential to provide cell-phone related services and environmental supports. Visually attractive media can be utilized for this purpose.

Relationships between the Change in Obesity Rate and Risk Factors of Metabolic Syndrome among Middle School Students in Ulsan (중학생의 비만율 변화 및 대사증후군 발생 위험요소와의 상관관계)

  • The purpose of this study was to provide fundamental materials for improving school health promotion programs by investigating the relationships between the change in obesity rate and risk factors of metabolic syndrome among middle school students in Ulsan. We analyzed the routine health check-up data in 58,298 2nd-year middle school students in Ulsan, which conducted by the Planned Population Federation of Korea from 2007 to 2011. The overall and girls obesity rates gradually decreased throughout the first four years but increased again in 2011. The boys obesity rates were highest in 2008 and lowest in 2010. Throughout the research years, the incidence rates of over-weight and obesity were statistically significantly higher in boys compared to girls(P<0.05). Prevalence rates of the metabolic syndrome, assessed with the modified Cook criteria except high blood pressure, were 0.58%, 0%, 0.01%, 0.31% and 4.53%, respectively in total, low, normal, over-weight, and obese groups. It indicates that the prevalence of metabolic syndrome is increasing and directly related to the obesity rate among adolescents. Hypertriglyceridemia was the most common component (36.58%) of the metabolic syndrome among the middles school students followed by low HDL cholesterol (6.47%), abdominal obesity (3.72%), and high blood sugar (0.47%). Prevalence of the various components of the metabolic syndrome were significantly higher in the obese group compared to other groups. In this study we observed the decrease in obesity rates between 2007 and 2010, and developing obesity prevention programs is essential in order to maintain the trend. Additional study should be necessary to find the cause of increase of obesity rate in 2011 and so to create more effective school health promotion programmes.
