A Survey on the Urban Residents' Recognition of Eco-friendly Outdoor Playground - Focused on Apartment House Playground Users in Seoul·Incheon·Gyeonggi-

  • Urban outdoor playground serves as the space to promote the physical, cognitive, social, creative, emotional and linguistic development of children and the education center to stimulate the imagination and curiosity of them. It is also the place to be used as a basic park for urban residents and the local community space for associating with people of various levels. However, this playground as a banal space did not function properly before now. In 21st century, urban playground requires change and its various changes have been attempted. In particular, there is a need for the change into eco-friendly playground to provide children's health and urban healthy environment. Therefore, it requires much effort to construct and vitalize the eco-friendly playground adequate for domestic circumstances in Korea. This study derived the preferred planning items among eco-friendly playground planning elements focused on the people to use it and provided the data for the construction of local friendly, eco-friendly outdoor playground. For this purpose, we visited the playground of the apartment house built in 10 years with more than 300 households and surveyed 200 adults and investigated the residents' utilization and satisfaction on the current playground and the recognition and preference for eco-friendly playground.

네팔 돌카지역의 임신분만 환경 (A Survey on Status of Pregnancy and Delivery at a Rural Village, Nepal(Dolakha Bazar Area))

  • 생활수준과 교육수준이 매우 낮은 저개발국가에서의 임신분만 환경을 알아보고 건강관련 행위 실천(산전진찰, 시설분만, 제대기구소독)과 지식과 태도와의 관계를 알아보기 위하여 네팔 돌카 마을에서 지난 1년간(1994년 4월 13일$\sim$1995년 4월 12일)출산한 경험이 있는 산모를 대상으로 일반적 특성, 임신과 분만에 관련된 지식, 태도, 산전진찰률, 분만환경 등을 조사하였다. 이 지역의 임신과 분만에 관련된 지식수준은 낮은 편으로 대상자의 87.5%가 보건교육을 받은 경험이 없었다. 대상자의 29.6%는 임신시 흡연이 태아에 해가 없다고 하였으며, 42.8%는 임신시 음주와 약의 복용이 태아에 해가 없다고 답하였고, 17.1%가 제대절단시 소독하지 않은 기구를 사용해도 유해하지 않다고 응답하였는데, 교육을 받은 적이 없는 경우, 보건교육을 받은 경험이 없는 경우, 그리고 보건기관까지의 거리가 멀 수록 올바르지 못한 지식을 가지고 있었다. 응답자의 56.6%가 산전진찰이 필요없다는 태도를 보였고, 42.8%가 질출혈시 병원을 방문할 필요가 없다는 태도를 보였는데, 교육을 받은 적이 없는 경우, 보건 교육을 받은 경험이 없는 경우, 그리고 보건기관까지의 거리가 멀수록 바람직하지 못한 태도를 가지고 있었다. 또한 82.9%가 분만과정 자체가 불결한 것이라고 여기고 있었다. 한번이라도 산전진찰을 받은 경험이 있는 산전진찰 경험률은 28.3%였는데 교육을 받은 적이 없는 경우, 보건교육을 받은 경험이 없는 경우에 산전진찰 미경험률이 높았다. 시설분만율은 5.3%로 매우 저조하였고, 시설 외 분만인 경우 82.6%가 제대절단기구를 소독하지 않고 사용하고 있었는데 교육을 받은 적이 없는 경우, 보건교육을 받은 경험이 없는 경우, 그리고 보건기관까지의 거리가 멀수록 소독을 하지 않고 사용하고 있었다. 지식과 태도와의 관계에 있어서는 지식이 많은 군이 산전진찰의 필요성을 더 느끼며 질출혈시 병원방문의 필요성을 더 느끼고 있어, 임신과 분만에 관련하여 더 바람직한 태도를 가지고 있었다. 태도와 실천과의 관계에 있어서는 바람직한 태도를 가진 산모군이 산전진찰 경험률이 높았으며, 시설분만이 많아 실천율이 높았다. 다변량 분석에서도 지식이 태도에 양(陽)의 영향을 미치는 변수였으며, 태도는 실천에 영향을 미치는 유의한 양(陽)의 변수여서 실천을 위해서는 긍정적인 태도가 선행되어야 하고, 그러기 위하여는 먼저 올바른 지식을 가지고 있어야 함을 알 수 있었다. 이상의 연구결과로 볼 때, 저개발국가에서의 분만환경은 매우 열악한 편으로 모자의 건강수준 향상을 위해서는 예방적 기능으로서 보건교육 활동이 더욱 중요 성을 가지게 된다. 효율적 보건교육활동을 위해서는 보건교육을 보다 긴요하게 요하는 인구군이 어떤 계층이냐 하는 대상파악에 특별히 유념하여야 하는데 주로 교육수준이 낮고 생활이 어려운 인구층, 그리고 보건기관으로부터 멀리 떨어져 있는 사람들에게 중점적으로 교육을 시도하여야 하겠는데 이들 나라는 대부분의 국민이 교육수준이 낮고 생활수준이 낮은 편으로 보건교육시 단순한 지식의 전달에 그칠 것이 아니라 동기 유발을 가져와 행위변화를 할 수 있도록 보다 세밀한 연구노력이 필요하다. 또한 순회진료와 보건교육을 하는 의료캠프가 활성화되어야 하겠다. 향후 저개발국에 의료단을 파견하여 그 지 역의 보건문제 해결에 도움을 주기 위하여는 이와 같은 연구가 계속 진행되어 실천에 영향을 주는 요인들을 밝혀 지역보건의료 프로그램에 적극 활용해야 할 것으로 생각된다.

노인건강상담전화 운용과 가정간호사업 활성화를 위한 원격의료 시범사업 (Demonstration Project on Utilization of Telephone Consulting and Telemedicine System for Home Health Care of the Elderly)

  • Advanced countries such as the USA and Japan are eagerly seeking ways to improve health and welfare of the elderly. One of the services is home health care service using the telephone. Various types of services using the telephone have been developed, improved and are being utilized ranging from the basic consulting to emergency response systems in the area of health care for the elderly. A demonstration project was launched to study the feasibility of a consulting system and telemedicine for the elderly using the public phone system in Korea. For this project, a gathering site for the elderly was selected and those who visited this place were interviewed to find out what kinds of services they wanted and what kind of system they needed to provide the required services. Based on the users' requests and the surrounding environment, a telephone consulting facility was established at the Research Institute of Nursing Science at Seoul National University and consulting personnel was recruited, trained and posted at the center. An Application program for home health care nurses to use when they visited the patients at their homes was developed. This system operates on a notebook Computer and allows nurses to communicate with a doctor at a local hospital through a modem and telecommunication line. These systems were implemented for three months and problems which developed during operation of the systems were identified and progressively modified. Through system evaluation, it was found that a consulting system using phone service will be an invaluable system for the welfare of the elderly in the future. But in order to meet the elderly's need, more services than mere consultation are needed. That is, communication with physicians and hospitals are needed. Thus, when there is any need for physicians' attention, physicians or hospitals should be contacted directly. Similarly for telemedicine, when the home health care nurse visits elderly patients she can assess the patient's problem and provide nursing care, access a physician or hospital to refer her patient to or consult directly using the telecommunication the system. The above mentioned system is a basic form of futuristic telemedicine for the elderly and those who have chronic disease problems. This kind of system will be of great value when it is used on the national information super-highways in the future. In order to get to that stage, of course, this project needs great improvement in the technical, academic, and legal aspects.

Endotracheal intubation by inexperienced trainees using the Clarus Video System: learning curve and orodental trauma perspectives

  • Background: The ideal alternative airway device should be intuitive to use, yielding proficiency after only a few trials. The Clarus Video System (CVS) is a novel optical stylet with a semi-rigid tip; however, the learning curve and associated orodental trauma are poorly understood. Methods: Two novice practitioners with no CVS experience performed 30 intubations each. Each trial was divided into learning (first 10 intubations) and standard phases (remaining 20 intubations). Total time to achieve successful intubation, number of intubation attempts, ease of use, and orodental trauma were recorded. Results: Intubation was successful in all patients. In 51 patients (85%), intubation was accomplished in the first attempt. Nine patients required two or three intubation attempts; six were with the first 10 patients. Learning and standard phases differed significantly in terms of success at first attempt, number of attempts, and intubation time (70% vs. 93%, $1.4 {\pm}0.7$ vs. $1.1{\pm}0.3$, and $71.4{\pm}92.3s$ vs. $24.6{\pm}21.9s$, respectively). The first five patients required longer intubation times than the subsequent five patients ($106.8{\pm}120.3s$ vs. $36.0{\pm}26.8s$); however, the number of attempts was similar. Sequential subgroups of five patients in the standard phase did not differ in the number of attempts or intubation time. Dental trauma, lip laceration, or mucosal bleeding were absent. Conclusions: Ten intubations are sufficient to learn CVS utilization properly without causing any orodental trauma. A relatively small number of experiences are required in the learning curve compared with other devices.

보건진료원의 정규직화 전과 후의 보건진료원 활동 및 보건진료소 관리운영체계의 비교 분석 (Comparative Analysis of Community Health Practitioner's Activities and Primary Health Post Management Before and After Officialization of Community Health practitioner)

  • 정부는 보건진료원으로 하여금 지역주민들에게 보다 더 의욕적으로 양질의 보건의료서비스를 제공하도록 하기 위하여 1992년 4월 1일부터 보건진료원을 별정직 공무원으로 정규직화 하였다. 본 연구는 보건진료원의 정규직화가 보건진료원의 업무활동과 보건진료소의 관리운영체계에 미친 영향을 분석하기 위해 경상남도와 경상북도의 보건진료소 중 집락추출법과 단순확률추출법으로 50개소를 뽑아 보건진료원을 대상으로 직접 면담조사하고 제반기록 및 보고서에서 필요한 자료를 발췌하였다. 조사기간은 1992년 1월 1일에서 3월 31일까지(정규직화 이전)와 1993년 1월 1일에서 3월 31일까지(정규직화 이후)였다. 보건진료원들의 96%가 정규직화를 원했는데 그 이유는 신분보장과 보수가 좋아지리라는 것이었다. 정규직화 후 보건진료원직을 자랑스럽게 생각한다는 사람이 24%에서 46%로 증가하였다. 신분보장에 대해서는 항상 불안하다는 사람이 30%에서 10%로 감소하였다. 정규직화 후 월평균 급여액은 802,600원에서 1,076,000원으로 34% 증가했으며 90%가 만족한다고 했다. 업무 내용별 자율성 인지정도는 업무계획, 업무수행, 진료소관리(재정)운영, 업무평가 영역에 대한 자율성 인지도가 정규직화 후에 증가되었다. 보건진료원의 활동내용 중 지역사회 자원파악, 지도작성상태, 지역사회조직 활용정도, 인구구조 파악정도와 가정건강기록부 작성은 정규직화 후에 특별한 변화는 없었다. 또한 집단보건교육, 개인보건교육, 학교보건교육의 실시도 정규직화 후에 변화가 없었다. 그러나 가정방문 실시현황은 1인당 월평균 13.6%회에서 정규직화 후에는 27.5%회로 늘었다. 모성보건 및 가족계획 사업 그리고 예방접종도 정규직화 후에 타기관에 의뢰하는 것이 더 늘었다. 통상질병관리 가운데 성인병관리는 3개월 동안 1개 진료소당 평균 고혈압환자는 12.7%명에서 11.6명으로, 암환자는 1.5명에서 1.2명으로, 당뇨병환자가 4.3명에서 3.4명으로 줄었다. 각종 기록부 비치상황은 장비대장, 약품관리 대장, 환자진료기록부는 100% 비치되었으나 기타 기록부는 그렇지 않았고 정규직화 후에도 변화는 없었다. 보건진료소가 보건소로부터 지원을 받는 내용은 약품 14.0%에서 30%로, 소모품 22.0%에서 52.0%로, 건물유지 및 보수가 54.0%에서 68% 로, 보건교육 자료가 34.0%에서 44.0%로 증가하였고, 장비는 58.0%에서 54.0%로 감소했다. 보건진료소의 월평균 수입은 진료수입이 약 22,000원 증가했고, 국비 또는 지방비 보조금이 4,800원에서 38,508%원으로 증가했으나 회비 및 기부금은 줄어 총수입은 약 50,000원 증가했다. 지출총액은 큰 변동이 없었다. 보건소로부터 3개월 동안 받은 지도감독 중 지시공문을 받은 진료소가 20%에서 38%로 늘었고, 방문지도는 79%에서 62%, 회의소집은 88%에서 74%로 감소하였다. 전화지도는 보건진료소당 평균 1.8회에서 2.1회로 늘었다(p<0.01). 면보건요원과의 협력관계가 있다고 한 보건진료원은 42%에서 36%로 감소하였다. 보건소장과의 관계가 좋다는 보건진료원이 46%에서 24%로 감소하였고, 보건행정계장과 관계가 좋다는 사림이 56%에서 36%로 감소하였다(p<0.05). 보건진료소 운영협의회 회장과의 관계가 좋다는 사람은 62%에서 38%로 감소되었고 보건진료소 운영협의회가 보건진료소에 별로 도움이 안된다와 전혀 도움이 되지 않는다는 사람이 정규직화 전과 후에 각각 92.0%, 82.0%였다. 운영협의회가 필요 없다는 사람은 정규직화 전에 4%에서 16%로 증가되었다(p<0.05). 보건진료원제도 발전을 위해 제안된 사항은 보건교육중심의 활동, 보건진료소운영의 자율성 보장 보건소에 경험이 풍부한 보건진료원을 두어 지도감독하게 할 것과 사용하는 약품의 종류를 늘려 줄 것 등이었다. 이상의 결과로 보하 정규직화 후 보건진료원의 역할, 기능 등의 업무활동의 변화는 거의 없었으나 신분보장과 봉급에 대한 만족도는 향상이 되었고 또한 자율성도 증가하였다. 보건소의 지원은 약간 늘었으며, 지도감독체제에서 지시 공문의 증가로 사무보고 업무가 많아지고, 근무 확인을 위한 전화감독은 늘었으나 업무치진을 위한 행정직 지도 또한 기술적 지도는 거의 없었다.

The clinical utilization of radiation therapy in Korea between 2009 and 2013

  • Purpose: The purpose of this study was to estimate the clinical utilization of radiation therapy (RT) in Korea between 2009 and 2013. Materials and Methods: We analyzed open claims data from the Health Insurance Review and Assessment Service. The subjects were patients who had diagnostic codes C00-C97 or D00-D48 according to the 10th revision of the International Classification of Diseases, with procedure codes indicating RT treatment. Results: The total number of patients who received RT in 2009, 2010, 2011, 2012, and 2013 were 45,571, 49,593, 54,671, 59,172, and 61,485, respectively. Among them, the total numbers of male and female patients were 20,780/24,791 in 2009, 22,711/26,882 in 2010, 24,872/29,799 in 2011, 27,101/32,071 in 2012, and 27,941/33,544 in 2013. The five cancers that were most frequently treated with RT between 2009 and 2012 were breast, lung, colorectal, liver, and uterine cervical cancers. However, the fifth most common cancer treated with RT that replaced uterine cervical cancer in 2013 was prostate cancer. The three leading types of cancer among the male patients were lung, colorectal, and liver cancers, whereas in female patients, they were breast, uterine cervical, and lung cancers. The type of cancer most commonly treated by RT was cancer of the central nervous system in patients aged 20 years or less, breast cancer in patients aged 30-50 years, and lung cancer in patients aged 60 years or more. Conclusion: Data from this study provided the clinical utilization of RT in Korea between 2009 and 2013.

노인 관절염 환자의 보완.대체요법 이용실태 (Study on the Utilization of Complementary Alternative Therapy in elder Arthritics)

  • The purpose of this study was to investigate the rate of utilization, kinds and effective complementary-alternative therapy in elder arthritics, and then utilize the results as basic data for nursing intervention for elder arthritics. Study subjects consisted of 157 elder arthritics over 60 years old, data were collected through a structured questionnaire and face to face interviews. Data collection was done from July 2001 to August 2001. Subjects were sampled out from outpatients of department of rehabilitation of a university hospital in S city, outpatients of a local hospital in D city, and outpatient at public heath center in K and S city. Sexual distribution of subjects showed male 19.1% and the female 80.9%. The diagnosis distribution showed degenerative arthritis at 91% and reumatoid arthritis at 8.9% Duration of arthritics was 10 years over by 46.5%, duration of hospital treatment was 1-5 years by 41% The degree of pain by arthritis pointed out a mean point of 3.37 on a 5-point numeric scale 94.2% of subjects have experience complementary-alternative therapies used. Of the kind the subjects used, physiotherapy occupied 38.2%, Oriental medicine 36.3%, physical exercise 35.7%, nutritional therapy 22.3%, animal diet 8.9%, herbal diet 3.8%. The hardest thing due to arthritis represented disability in daily life by 59.8% and the pain problem by 30.5%. In conclusion, results of the study reveal that elder arthritics have used physiotherapy, Oriental medicine, physical exercise. Concrete strategies for nursing intervention about these complementary-alternative therapy are required to the established soon.

Insect meal as a feed ingredient for poultry

  • Shortage of protein feed resources is the major challenge to the world farm animal industry. Insects are known as an alternative protein source for poultry. A wide range of insects are available for use in poultry diets. Insect larvae thrive in manure, and organic waste, and produce antimicrobial peptides to protect themselves from microbial infections, and additionally these peptides might also be functional in poultry feed. The feed containing antimicrobial peptides can improve the growth performance, nutrient digestibility, intestinal health, and immune function in poultry. Insect meal contains a higher amount of essential amino acids compared to conventional feedstuffs. Black soldier fly, mealworm, housefly, cricket/Grasshopper/Locust (Orthoptera), silkworm, and earthworm are the commonly used insect meals in broiler and laying hen diets. This paper summarizes the nutrient profiles of the insect meals and reviews their efficacy when included in poultry diets. Due to the differences in insect meal products, and breeds of poultry, inconsistent results were noticed among studies. The main challenge for proper utilization, and the promising prospect of insect meal in poultry diet are also addressed in the paper. To fully exploit insect meal as an alternative protein resource, and exert their functional effects, modes of action need to be understood. With the emergence of more accurate and reliable studies, insect meals will undoubtedly play more important role in poultry feed industry.

  • 일부 도시 영세지역 주민의 상병양상과 의료이용형태를 파악하기 위하여 1988년 12월 한달간에 걸쳐 대구시 남구 대명 8동 영세지역 677가구 2,591명과 대조지역 688가구 2,682명의 주민을 대상으로 면접조사를 실시하였다. 두 지역의 연령별 인구 구성은 0-4세, 65세이상의 인구가 영세지역이 높았으며, 성비는 차이가 없었다. 교육정도와 경제수준은 영세지역은 낮았고, 의료보호 대상자는 영세지역이 많았다. 15일간의 급성질환 이환율은 영세지역이 1000명당 57.1로 대조지역의 24.2보다 유의하게 높았다. 두 지역에서 9세 이하 연령에서의 이환율이 높았으나 연령 증가에 따른 이환율의 뚜렷한 증감현상은 보이지 않았다. 질병분류별 급성질환 이환건수는 두 지역 모두에서 남녀 공히 호흡기계 질환이 가장 많았으며, 영세지역의 경우 감염 및 기생충 질환이 두번째로 많아 대조지역과는 다른 양상을 보였다. 급성질환 이환자의 15일간의 평균 이환기간은 영세지역 6.6일, 대조지역 5.4일로 영세지역이 약간 길었다. 급성질환 이환자의 15일간 의료기관 이용은 1차 이용에서는 영세지역의 경우 약국이 58.1% 그리고 대조지역에서는 의원이 36.9%로 가장 많았으며, 2차와 3차 이용에서 영세지역은 약국과 한의원, 대조지역은 병원과 의원이 가장 많았고, 미치료율은 영세지역이 8.8%로 대조지역의 4.6%에 비하여 훨씬 높게 나타났다. 급성질환 이환자의 지역별 의료이용행태에서 1차 이용에 있어서는 동네내 이용이 영세지역 73.6%, 대조지역 64.6%로 가장 많았다. 급성질환 이환자의 의료기관 1차 이용시 병원을 이용하는 이유로 가장 많은 것이 영세지역은 '단골'이었으며, 대조지역은 '가까운 거리'였다. 의원, 보건소 및 약국을 이용하는 이유로 가장 많은 것은 두 지역 모두 '가까운 거리'였다. 1년간 만성질환 이환율은 1000명당 영세지역 83.0, 대조지역 28.0으로 유의한 차이를 보였으며, 연령이 증가함에 따라 이환율도 증가하는 경향을 보였다. 만성질환 이환건수는 영세지역에서는 남자의 경우 신경계 질환이 19.6%, 여자의 경우는 신경계 질환 18.3%, 근골격계 질환이 18.3%로 가장 많았고, 대조지역에서는 남자의 경우 심혈관계 질환이 23.1%, 여자의 겨우 위장관계질환이 27.3%로 가장 많았다. 조사 시점전 1년간 만성질환의 평균 이환기간은 영세지역 9.8개월, 대조지역 10.4개월로 대조지역이 약간 더 길었으며, 평균 활동 제한 기간도 영세지역 1.9개월보다 대조지역 2.1개월로 다소 길었다. 만성질환 이환자의 의료기관 1차 이용에 있어서 영세지역은 약국 이용이 24.2%, 대조지역은 병원외래 이용이 34.7%로 가장 많았으며, 2차, 3차 이용에 있어 영세지역에서는 각 의료기관을 고루 이용하였고, 대조지역에서는 병원 이용이 두드러지게 많았으며, 미치료율은 영세지역과 대조지역이 각각 34.7%, 16.0%로 영세지역이 월등히 높았다. 만성질환자의 지역별 의료이용행태에서 1차 이용에 있어서는 동네내 이용이 영세지역 38.3%, 대조지역 45.3%로 두 지역 모두에서 가장 많았다. 만성질환 이환자의 지역별 의료기관 1차 이용시 병원, 의원, 보건소 및 약국을 이용하는 주된 이유는 '가까운 거리'였으며, 한의원을 이용하는 이유는 '좋은 치료결과'와 치료의 명성'이었다. 이상의 결과로 영세지역의 사회경제적수준이 상병과 의료이용행태에 영향을 미친 것으로 생각된다. 따라서 영세지역 주민을 위한 보다 적극적인 보건의료 서비스의 개발과 합리적이고 적절한 의료이용을 위한 지도 및 계몽과 아울러 사회경제적인 요소들의 개선을 위한 노력을 병행해야 만이 효과적인 보건의료 사업이 될 것으로 생각된다.

농촌(農村) 주민(住民)들의 의료필요도(醫療必要度)에 관(關)한 연구(硏究) (A Study Concerning Health Needs in Rural Korea)

  • Today most developed countries provide modern medical care for most of the population. The rural area is the more neglected area in the medical and health field. In public health, the philosophy is that medical care for in maintenance of health is a basic right of man; it should not be discriminated against racial, environmental or financial situations. The deficiency of the medical care system, cultural bias, economic development, and ignorance of the residents about health care brought about the shortage of medical personnel and facilities on the rural areas. Moreover, medical students and physicians have been taught less about rural health care than about urban health care. Medical care, therefore, is insufficient in terms of health care personnel/and facilities in rural areas. Under such a situation, there is growing concern about the health problems among the rural population. The findings presented in this report are useful measures of the major health problems and even more important, as a guide to planning for improved medical care systems. It is hoped that findings from this study will be useful to those responsible for improving the delivery of health service for the rural population. Objectives: -to determine the health status of the residents in the rural areas. -to assess the rural population's needs in terms of health and medical care. -to make recommendations concerning improvement in the delivery of health and medical care for the rural population. Procedures: For the sampling design, the ideal would be to sample according to the proportion of the composition age-groups. As the health problems would be different by group, the sample was divided into 10 different age-groups. If the sample were allocated by proportion of composition of each age group, some age groups would be too small to estimate the health problem. The sample size of each age-group population was 100 people/age-groups. Personal interviews were conducted by specially trained medical students. The interviews dealt at length with current health status, medical care problems, utilization of medical services, medical cost paid for medical care and attitudes toward health. In addition, more information was gained from the public health field, including environmental sanitation, maternal and child health, family planning, tuberculosis control, and dental health. The sample Sample size was one fourth of total population: 1,438 The aged 10-14 years showed the largest number of 254 and the aged under one year was the smallest number of 81. Participation in examination Examination sessions usually were held in the morning every Tuesday, Wenesday, and Thursday for 3 hours at each session at the Namchun Health station. In general, the rate of participation in medical examination was low especially in ages between 10-19 years old. The highest rate of participation among are groups was the under one year age-group by 100 percent. The lowest use rate as low as 3% of those in the age-groups 10-19 years who are attending junior and senior high school in Taegu city so the time was not convenient for them to recieve examinations. Among the over 20 years old group, the rate of participation of female was higher than that of males. The results are as follows: A. Publie health problems Population: The number of pre-school age group who required child health was 724, among them infants numbered 96. Number of eligible women aged 15-44 years was 1,279, and women with husband who need maternal health numbered 700. The age-group of 65 years or older was 201 needed more health care and 65 of them had disabilities. (Table 2). Environmental sanitation: Seventy-nine percent of the residents relied upon well water as a primary source of dringking water. Ninety-three percent of the drinking water supply was rated as unfited quality for drinking. More than 90% of latrines were unhygienic, in structure design and sanitation (Table 15). Maternal and child health: Maternal health Average number of pregnancies of eligible women was 4 times. There was almost no pre- and post-natal care. Pregnancy wastage Still births was 33 per 1,000 live births. Spontaneous abortion was 156 per 1,000 live births. Induced abortion was 137 per 1,000 live births. Delivery condition More than 90 percent of deliveries were conducted at home. Attendants at last delivery were laymen by 76% and delivery without attendants was 14%. The rate of non-sterilized scissors as an instrument used to cut the umbilical cord was as high as 54% and of sickles was 14%. The rate of difficult delivery counted for 3%. Maternal death rate estimates about 35 per 10,000 live births. Child health Consultation rate for child health was almost non existant. In general, vaccination rate of children was low; vaccination rates for children aged 0-5 years with BCG and small pox were 34 and 28 percent respectively. The rate of vaccination with DPT and Polio were 23 and 25% respectively but the rate of the complete three injections were as low as 5 and 3% respectively. The number of dead children was 280 per 1,000 living children. Infants death rate was 45 per 1,000 live births (Table 16), Family planning: Approval rate of married women for family planning was as high as 86%. The rate of experiences of contraception in the past was 51%. The current rate of contraception was 37%. Willingness to use contraception in the future was as high as 86% (Table 17). Tuberculosis control: Number of registration patients at the health center currently was 25. The number indicates one eighth of estimate number of tuberculosis in the area. Number of discharged cases in the past accounted for 79 which showed 50% of active cases when discharged time. Rate of complete treatment among reasons of discharge in the past as low as 28%. There needs to be a follow up observation of the discharged cases (Table 18). Dental problems: More than 50% of the total population have at least one or more dental problems. (Table 19) B. Medical care problems Incidence rate: 1. In one month Incidence rate of medical care problems during one month was 19.6 percent. Among these health problems which required rest at home were 11.8 percent. The estimated number of patients in the total population is 1,206. The health problems reported most frequently in interviews during one month are: GI trouble, respiratory disease, neuralgia, skin disease, and communicable disease-in that order, The rate of health problems by age groups was highest in the 1-4 age group and in the 60 years or over age group, the lowest rate was the 10-14 year age group. In general, 0-29 year age group except the 1-4 year age group was low incidence rate. After 30 years old the rate of health problems increases gradually with aging. Eighty-three percent of health problems that occured during one month were solved by primary medical care procedures. Seventeen percent of health problems needed secondary care. Days rested at home because of illness during one month were 0.7 days per interviewee and 8days per patient and it accounts for 2,161 days for the total productive population in the area. (Table 20) 2. In a year The incidence rate of medical care problems during a year was 74.8%, among them health problems which required rest at home was 37 percent. Estimated number of patients in the total population during a year was 4,600. The health problems that occured most frequently among the interviewees during a year were: Cold (30%), GI trouble (18), respiratory disease (11), anemia (10), diarrhea (10), neuralgia (10), parasite disease (9), ENT (7), skin (7), headache (7), trauma (4), communicable disease (3), and circulatory disease (3) -in that order. The rate of health problems by age groups was highest in the infants group, thereafter the rate decreased gradually until the age 15-19 year age group which showed the lowest, and then the rate increased gradually with aging. Eighty-seven percent of health problems during a year were solved by primary medical care. Thirteen percent of them needed secondary medical care procedures. Days rested at home because of illness during a year were 16 days per interviewee and 44 days per patient and it accounted for 57,335 days lost among productive age group in the area (Table 21). Among those given medical examination, the conditions observed most frequently were respiratory disease, GI trouble, parasite disease, neuralgia, skin disease, trauma, tuberculosis, anemia, chronic obstructive lung disease, eye disorders-in that order (Table 22). The main health problems required secondary medical care are as fellows: (previous page). Utilization of medical care (treatment) The rate of treatment by various medical facilities for all health problems during one month was 73 percent. The rate of receiving of medical care of those who have health problems which required rest at home was 52% while the rate of those who have health problems which did not required rest was 61 percent (Table 23). The rate of receiving of medical care for all health problems during a year was 67 percent. The rate of receiving of medical care of those who have health problems which required rest at home was 82 percent while the rate of those who have health problems which did not required rest was as low as 53 percent (Table 24). Types of medical facilitied used were as follows: Hospital and clinics: 32-35% Herb clinics: 9-10% Drugstore: 53-58% Hospitalization Rate of hospitalization was 1.7% and the estimate number of hospitalizations among the total population during a year will be 107 persons (Table 25). Medical cost: Average medical cost per person during one month and a year were 171 and 2,800 won respectively. Average medical cost per patient during one month and a year were 1,109 and 3,740 won respectively. Average cost per household during a year was 15,800 won (Table 26, 27). Solution measures for health and medical care problems in rural area: A. Health problems which could be solved by paramedical workers such as nurses, midwives and aid nurses etc. are as follows: 1. Improvement of environmental sanitation 2. MCH except medical care problems 3. Family planning except surgical intervention 4. Tuberculosis control except diagnosis and prescription 5. Dental care except operational intervention 6. Health education for residents for improvement of utilization of medical facilities and early diagnosis etc. B. Medical care problems 1. Eighty-five percent of health problems could be solved by primary care procedures by general practitioners. 2. Fifteen percent of health problems need secondary medical procedures by a specialist. C. Medical cost Concidering the economic situation in rural area the amount of 2,062 won per residents during a year will be burdensome, so financial assistance is needed gorvernment to solve health and medical care problems for rural people.
