긴장형 두통을 주소로 입원한 환자의 별갑약침을 포함한 한방치료에 대한 임상 보고 1례 (A Case Report of a Patient with a Tension-Type Headache Treated by Korean Medical Treatment with ByeolGab Pharmacopuncture)

  • Objectives: The aim of this study is to report the effects of Korean medical treatment with ByeolGab pharmacopuncture on a patient with a tension-type headache. Methods: We treated a tension-type headache using Korean medical treatment including ByeolGab pharmacopuncture for 6 days a week for 2 weeks. As a measurement, the numerical rating scale (NRS), the Henry Ford Headache Disability Inventory (HDI), and a six point Likert scale were used to confirm pharmacopuncture therapy efficacy. Results: After treatment, symptoms related to tension-type headache and other complaints were decreased and evaluation using the NRS, HDI, and six-point scale showed improvement in the patient. Conclusions: This study shows that Korean medical treatment with ByeolGab pharmacopuncture has beneficial effects on control of tension-type headache and improvement in quality of life for patients.

경근자법(經筋刺法)을 이용한 두통치료에 대한 임상적 고찰 (Clinical Study about Meridian Tendino-musculature Acupuncture on Headache)

  • Objectives : To broaden understanding about meridian tendino-musculature acupuncture on headache and to evaluate the effect of meridian tendino-musculature acupuncture on headache. Methods and Results : From Feb. 2012 to Jan. 2013, the 64 patients were outpatients for treatment of headache in Department of Acupuncture and Moxibustion Medicine, Traditional Korean Medical Hospital, Semyung University and treated with meridian tendino-musculature acupuncture. To evaluate the effect of meridian tendino-musculature acupuncture, we devide 3groups(21 occipital-headache patients, 27 temporal-headache patients, 16 frontal-headache patients) upon pain area and search pain intensity(VAS were used), different frequency, duration time. As a result, headache are improved remarkably within two weeks. Conclusions : Meridian tendino-musculature acupuncture was found to be helpful to patients who wish to recover from headache. In order to make this meridian tendino-musculature acupuncture more available, we should pay more attention to improving treatment appliance and acupuncture techniwue.

만성 긴장성 두통 환자와 정상 대조군의 압력통각 역치 변화에 대한 비교연구 (Analysis of the Change of the Pressure Pain Threshold in Chronic Tension-Type Headache and Control)

  • Although Chronic tension-type headache(CTTH) is one of the most common symptom in primary headache, there is no definite mechanisms. But muscular factors and psychological factors is supposed to be related with CTTH according to many other studies and pressure pain threshold(PPT) is used to measure muscular factors. Methods 1. We performed this study with 63 patients fulfilling the International Headache Societ criteria for chronic tension-type headache and 20 healthy control group and measured the PPT of temporal muscle and trapezius muscle. 2. We investigated the correlation between clinical characteristic and PPT. 3. Each of the CTTH groups and Control group is divided to four group again - HNP, Spondylosis, Sprain, Normal according to Cervical spine X-ray. Results 1. The PPT of temporal muscle and trapezius muscle in the CTTH is significantly lower than that of Control. 2. In CTTH group, the PPT has significant positive relation with duration of headache and continued time of headache. And the PPT has significant inverse relation with Frequency of headache and Level of headache. 3. In CTTH group, spondylosis group has the highest PPT and normal group is second. And there are significant difference between spondylosis group and the others. Conclusion : We found that PPT is strongly significant to measure muscular factor in CTTH.

군발 두통 환자에게 한의치료를 적용했던 1례 (A case of Cluster Headache treated with Traditional Korean Medicine)

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine(TKM) on a patient with Cluster Headache. ■ Methods A patient diagnosed as Cluster Headache was treated with herbal medication(Sihogyejitang plus Oryungsan), acupuncture, electro-acupuncture. Then we evaluated the improvement with NRS(Numeral Rating Scale) of headache, number of headache attack and O2 inhalation. ■ Results We could observe decrease NRS of headache and number of headache and O2 inhalation after 10 days of the TKM treatment and there was no reattack after treatment for 43days follow up period. ■ Conclusion This case showed the effect of TKM treatment on Cluster headache.

침도 치료를 중심으로 시행한 한의복합치료 후 호전을 보인 일차성 두통 환자 4례에 대한 증례보고 (Effects of Korean Medicine Treatment focusing on Acupotomy of headache: A Case Report of Four Patients)

  • Background: The aim of this study was to report the effect of acupotomy for patients with headache. Methods: Four patients with headache were treated with acupotomy. The improvement of symptoms was evaluated by numeric rating scale(NRS), Headache impact test-6(HIT-6) and headache attack frequency. Results: The numeric rating scale score and headache attack frequency decreased throughout the treatment period. No side effects were observed during treatment. Conclusion: Acupotomy had shown a positive clinical effect in the treatment of a headache in this case series. Further studies are required for its application on various headache.

측두하악장애 환자의 두통 양상의 분류 : 일차성 두통 vs 측두하악장애로 인한 두통 (Clinical Characteristics of Headaches in Temporomandibular Disorder Patients : Primary Headache vs Headache Attributed to TMD)

  • 본 연구는 측두하악장애와 연관된 두통의 유병율을 평가하고 측두하악장애와 연관된 두통의 양상을 평가함으로써 두통과 측두하악장애와의 관계를 평가하기 위하여 시행되었다. 2008년 1월부터 2008년 6월까지 측두하악장애 관련 증상을 주소로 조선대학교 치과병원 구강내과에 내원한 환자 중 일차성 두통을 가지고 있는 환자 66명을 대상으로 하여, 측두하악장애, 두통에 관한 평가를 시행하였다. 연구결과는 다음과 같다. 1. 측두하악장애을 기준으로 분류하였을 경우, 측두하악관절 및 저작근이 동시에 이환된 경우가 66명 중 36명(54.5%)로 가장 많았다. 2. 두통을 기준으로 분류하였을 경우, 긴장성 두통이 66명중 59명(89.39%)로 가장 많았다. 3. 66명 중 36명(54.5%)이 측두하악장애와 연관된 두통을 호소하였다. 4. 측두하악장애와 연관된 두통을 호소한 36명 중 19명(52.78%)이 본래 존재한 일차성 두통과는 다른 양상이 측두하악장애와 연관되어 나타난다고 하였다. 즉 측두하악장애와 일차성 두통을 동시에 가지고 있는 경우, 측두하악장애와 연관된 두통이 상대적으로 흔하게 발생된다고 볼 수 있을 것이다. 또한 그 중 과반수 이상이 기존 두통양상과는 다른 증상을 호소하는 것으로 보아, 측두하악장애와 두통의 연관성을 연구하는데 있어 새로운 두통양상에 대한 연구가 추가적으로 이루어져야 할 것이다. 또한 측두하악장애와 두통과의 연관성을 평가하기 위해서는 보다 많은 모집단을 대상으로, 보다 세분화된 기준을 가지고 대조군을 설정하여 평가하는 것이 추후에 필요할 것으로 사료된다.

근긴장상 두통에 관한 연구 (Studies in Muscle Contraction Headache)

  • The patient with muscle contraction headache usually have one or more specific trigger points. These trigger points have been treated with various treatment modalities including "stretch and spray" and regional infiltration with local anesthetics with or without corticosteroids. I treated 36 patients with muscle contraction headache with regional infiltration of local anesthetics and steroid into trigger points and the results were as follows 1) The diagnosis of muscle contraction headache was possible by confirming specific trigger points by palpation. 2) Patients relieved rapidly from headache by regional infiltration of local anesthetics and steroid into the tender point. 3) Single injection was effective in relieving headache. But the curability of the single injection could not be assessed because of difficulty in follow-up study. 4) Active trigger points could be occasionally inactive, which also made difficult in assessing the effectiveness of the treatment.

Intracranial Chronic Subdural Hematoma Presenting with Intractable Headache after Cervical Epidural Steroid Injection

  • Postdural punctural headache (PDPH) following spinal anesthesia is due to intracranial hypotension caused by cerebrospinal fluid (CSF) leakage, and it is occasionally accompanied by an intracranial hematoma. To the best of our knowledge, an intracranial chronic subdural hematoma (CSDH) presenting with an intractable headache after a cervical epidural steroid injection (ESI) has not been reported. A 39-year-old woman without any history of trauma underwent a cervical ESI for a herniated nucleus pulposus at the C5-6 level. One month later, she presented with a severe headache that was not relieved by analgesic medication, which changed in character from being positional to non-positional during the preceding month. Brain magnetic resonance imaging revealed a CSDH along the left convexity. Emergency burr-hole drainage was performed and the headache abated. This report indicates that an intracranial CSDH should be considered a possible complication after ESI. In addition, the event of an intractable and changing PDPH after ESI suggests further evaluation for diagnosis of an intracranial hematoma.

  • Headaches are one of the most common conditions associated with temporomandibular disorder (TMD). In the present paper, we evaluated the relationship between headache and TMD, determined whether headache influences the symptoms of TMD, and reported two cases of TMD accompanied by headache. Our practical experience and a review of the literature suggested that headache increases the frequency and intensity of pain parameters, thus complicating dysfunctional diseases in both diagnostic and treatment phases. Therefore, early and multidisciplinary treatment of TMD is necessary to avoid the overlap of painful events that could result in pain chronicity.

요부 경막외 차단후 발생한 두통 및 기뇌증 -증례 보고- (Headache and Pneumocephalus after Lumbar Epidural Block -A case report-)

  • Headache after epidural block is the most frequent result of an unintentional dural puncture. This form of headache is usually caused by cerebrospinal fluid leakage through the dural puncture site. Another proposed cause of postdural puncture headache is the unintentional injection of air into the subarachnoid space. We experienced a case of severe headache with a patient after lumbar epidural block and discovered air in the intracranial subarachnoid space(pneumocephalus) with the aid of brain computerized tomography.
