Objective : To evaluate the role of lumbar drainage in the prevention of shunt-dependent hydrocephalus after treatment of ruptured intracranial aneurysms by coil embolization in good-grade patients. Methods : One-hundred-thirty consecutive patients with aneurysmal subarachnoid hemorrhage in good-grade patients (Hunt & Hess grades I-III), who were treated by coil embolization between August 2004 and April 2010 were retrospectively evaluated. Poor-grade patients (Hunt & Hess grades IV and V), a history of head trauma preceding the development of headache, negative angiograms, primary subarachnoid hemorrhage (SAH), and loss to follow-up were excluded from the study. We assessed the effects on lumbar drainage on the risk of shunt-dependent hydrocephalus related to coil embolization in patients with ruptured intracranial aneurysms. Results : One-hundred-twenty-six patients (96.9%) did not develop shunt-dependent hydrocephalus. The 2 patients (1.5%) who developed acute hydrocephalus treated with temporary external ventricular drainage did not require permanent shunt diversion. Overall, 4 patients (3.1%) required permanent shunt diversion; acute hydrocephalus developed in 2 patients (50%). There was no morbidity or mortality amongst the patients who underwent a permanent shunt procedure. Conclusion : Coil embolization of ruptured intracranial aneurysms may be associated with a lower risk for developing shunt-dependent hydrocephalus, possibly by active management of lumbar drainage, which may reflect less damage for cisternal anatomy than surgical clipping. Coil embolization might have an effect the long-term outcome and decision-making for ruptured intracranial aneurysms.
목적 : 고식적인 자기공명혈관조영술 (MR Angiography, MRA) 기법으로는 영상화가 어려웠던 대동맥궁의 주요 분지들의 평가에 있어서 새로운 MR기법인 조영증강 MRA의 임상적 유용성을 알아보고, 그 화질을 사용한 코일의 종류에 따라 비교해 보고자 한다. 대상 및 방법 : 뇌혈관 질환을 의심하여 고식적인 기법으로 뇌 및 경동맥 MRA를 시행한 29명에서 전향적으로 Gd-DAPA 15-20ml를 일시에 손으로 주입한 후 대동맥궁과 그의 주요 분지들에 대해 3시기의 고속 MRA를 시행하여 그 화질을 분석하였다. MRA는 1.0T MR기종에서 3D-FISP기법으로 얻었으며 총 영상 획득 시간은 40-60초였다. 영상 분석은 무명동맥, 양측 총경동맥, 양측 쇄골하동맥과 양측 척추 동맥들의 기시부로부터 전장에 걸쳐 화질을 주관적으로 3등급(good; 명백히 정상소견을 보이는 경우, fair; 약간 낮은 신호를 보이나 정상으로 진단하기에 비교적 만족할 만한 경우, poor; 협착이 모호하거나, 인공물이나 너무 낮은 신호로 혈관을 볼 수 없어 카테터 혈관조영술을 요하는 경우)으로 나누어 평가하였으며, 양측 총경동맥의 분기(bifurcation) 부위에서는 고식적인 기법의 영상과 그 화질을 비교평가하였다. 또한 세가지 사용한 코일의 종류(CP body array 12예, CP neck array 9예, head-and-neck 8예)에 따른 화질 차이를 정성적 및 정량적(신호대 잡음비)으로 분석하였다. 결과 : 대동맥궁 주요 분지들의 전반적인 화질은 55% (16/29)에서 'good', 34%(10/29)에서 'fair'로 평가되어 대부분 고식적인 카테터 혈과조영술이 피요치 않을 정도로 만족할 만한 화질을 보였다. 양측 총경 동맥분지 부위에서는 65%(17/26)에서 고식적인 3D-TOF기법과 같거나 나은 영상을 보였다. CP body array 코일을 사용한 경우가 CP neck array 코일이나 head-and-neck 코일을 사용한 경우보다 정성적 및 정량적으로 유의하게 나은 영상을 보였다(p<0.05). 결론 : 고속 조영증강 MRA기법은 단시간내 (40-60초)에 대동맥구의 주요 분지들을 그 기시부위부터 두개골 저부에 이르기까지 대부분에서 잘 나타내주므로 선별검사로서 임상적으로 유용하리라 생각되며 CP body array 코일을 사용하였을 경우에 CP neck array 코일이나 head-and-neck 코일을 사용한 경우보다 좀더 나은 화질을 얻을 수 있으리라 생각된다.
We have developed a whole-head MEG system for basic brain research and clinical application. The sensor system consists of a 152 SQUID gradiometer array oriented and located in a suitable way to cover a whole head of the human. The system measures magnetic fields generated by neuronal currents in the brain to get information on the brain activities. For this purpose, the field sensitivity determined by the position, orientation and geometry of the pickup coil as well as amplification factor of the electronic circuits should be known precisely. However, the position and orientation of the pickup coil might be changed from the designed specifications during cool down of the dewar and it is necessary to characterize the field sensitivity. In this study, we made calibration systems to determine the actual position and orientation of the 152 pickup coils and compared the localization results of the N100m source in the auditory cortex.
아몰퍼스 자성와이어를 센서헤드로 이용한 금속 구조체의 미세결함 검사에 대해 검토하였다. 측정 방법은 스파이럴 코일의 균일 자장범위에 직선 갭을 가진 동판을 놓고 갭 주위를 관통하는 자속량의 차이를 센서헤드의 유기전압으로 측정하는 것이다. 이 결과 두께 1mm, 갭폭 0.5mm의 동판에 대해 100kHz~600kHz의 주라수 범위에 있어서 갭 근방에서 약 2.5mV의 전압차가 얻어져 결함의 유무를 확인할 수 있었다.
목적 : 사람에 비하여 크기가 현저히 작은 동물의 자기공명영상 촬영을 위하여 상용으로 인체에 적용하는 Head 코일보다 원통 반지름이 작은 Low-Pass Type Birdcage 코일을 제작함으로써 보다 큰 신호대잡음비 (Signal-to-Noise Ratio. SNR)를 얻고자 하였다. 방법 : 자기공명영상을 얻기 위해 Spin Echo 펄스시퀀스와 Fast Spin Echo 펄스시퀀스를 사용하였다. T1 강조영상을 얻기 위한 Spin Echo 펄스시퀀스의 매개변수는 TR/TE=300/17 ㎳, Matrix=256$\times$256, Field Of View (FOV)=150 mm, Slice Thickness=2 mm 이었다. T2 강조영상을 얻기 위한 Fast Spin Echo 펄스시퀀스의 매개변수는 TR/TE=3000/96 ㎳, Matrix=256$\times$256, Field Of View (FOV)=150 mm, Slice Thickness=2 mm 이었다. 원통의 지름이 13 cm인 Birdcage 코일은 12개의 elements로 구성되어 있으며 길이는 22 cm로 제작되었다. 결과 : 코일 원통의 반지름의 크기에 따른 SNR을 비교하기 위하여 인체용 Knee 코일과 동물용으로 제작된 코일을 이용하여 각각의 팬톰 영상을 획득하였다. 팬톰 영상으로부터 측정된 SNR의 값을 통해 반지름이 작은 동물용 코일의 SNR이 더 크다는 걸 확인할 수 있었다. 토의 및 결론 : 본 연구를 통하여 같은 형태의 Birdcage 코일일 경우 원통의 반지름에 따라 SNR이 다르며, 특히 반지름이 작을 때 SNR이 더 크다는 것을 알 수 있었다. 따라서 코일의 크기에 비해 촬영하고자 하는 대상물의 부피가 작은 경우 대상물의 부피에 맞추어 코일을 제작하면 SNR이 보다 뛰어난 영상을 얻을 수 있을 것으로 사료된다.
광섬유전류센서에서 온도변화에 따른 감도변화를 제거하기 위한 연구를 하였다. 센서헤드로는 두개의 원형틀을 수직으로 결합하고 여기에 광섬유를 번갈아 감아서 사용하였다. 이 경우 수직한 원형틀은 광섬유의 고유편광축을 상호수직하게 하므로 감겨진 광섬유에 발생된 선형복굴절량을 상쇄하여 최소화할 수 있었다. 이렇게 제작된 전류센서는 온도에 대해 보다 안정된 성능을 보였다. 센서헤드부의 온도는 약 1시간 40분동안 20-45.deg.C의 느린 변화를 주었다. 이때의 전체적인 센서 오차는 .+-.1.2% 정도였고 500A에서 3시간동안 안정성 측정을 했을 경우 .+-.1%미만의 변화를 보였다. 센서의 헤드부 전후단의 광섬유에 의해서 신호의 요동이 생기는 것을 방지하기 위한 방법으로 센서헤드부에 편광기와 편광분할기를 놓고 리드부와 분리를 했다. 광원으로서는 두개의 레이저 다이오드를 편광이 수직이 되게 배열함으로써 무편광 광원으로서의 효과를 내었다. 신호처리는 각 채널별로 분리를 하여 기계적인 부분에 의한 광손실의 영향을 배제하였다.

This paper proposes a helmet-type whole-head brain stimulator system considering a realistic head geometry. For more accurate design and computer simulations, a realistic volume conductor model was adopted and the current evoked on human cerebral cortex was analyzed using the boundary element method (BEM). To obtain a more focalized evoked current around the target points, various coil configurations were tested and an average targeting error of about 10 mm was obtained.
This paper contributes to development of a new chip mounting head system for flip chip. Recently, the LDM(Linear DC Motor) has been widely used, because it has particular merits than the rotary type motors. In this paper, we proposed a macro/micro positioning system for force control of a chip mounting system. In the proposed macro/micro system, the macro actuator provide the system with a gross motion while the micro device yields fine tuned motion to reduce the harmful impact force that occurs between very small sized electronic parts and PCB surface. In order to prove the effectiveness of the proposed macro/micro chip mounting system, we compared the proposed chip mounting head with the conventional chip mounting head equipped with a macro actuator only. A series of experiments were executed under the mounting conditions of various access velocities and PCB stiffness. As a result of this study, a satisfactory voice coil actuator as the micro actuator has been developed, and its performance meet well the specifications desired for the design of the chip mounting head system and show good correspondence between theoretical analysis and experimental results.
