The primary treatment of oral cavity cancer is still surgery. By discussing the surgical treatment of oral cavity cancer, the basic concept of head and neck surgery could be thoroughly reviewed. The oral cavity is defined as the hard palate and the anterior 2/3 of the tongue. With appropriate reconstruction, most defects can be repaired without a significant change in quality of life, unlike in the oropharynx or hypopharynx, where aspiration problems frequently occur. The selection of a surgical approach that can provide an appropriate field of view to obtain a resection margin of 5 mm or more has become the core of head and neck surgery. The role of prophylactic neck dissection is also well established in oral cavity cancer patients. Mandibulotomy for access to the oral cavity or mandibulectomy due to cancer invasion requires bony surgical techniques, and reconstruction also requires bone tissue reconstruction techniques as well as soft tissue. Therefore, oral cancer surgery is the most important primary area where all techniques of head and neck surgery are mobilized.
Background and Objectives : Panax ginseng C.A. Meyer is a medical plant that has been widely utilized as a tonic and nutritional agent since ancient times in Korea. Ginseng has anti-metastatic property of cancer and immunomodulating activity. The novel acidic polysaccharide compound(MB40) was isolated from the leaves of Panax ginseng C.A. Meyer. To determine immunomodulating activities of MB40, we evaluate anti-cancer and anti-metastatic effects of MB40 in tumor bearing immune competent mice. Material and Methods : C3H mice were divided into three equal groups(Cisplatin treatment group, MB40 treat-ment group, Cisplatin and MB40 treatment group) and were transplanted SCC(Squamous Cell Carcinoma) cells(2${\times}$106) to the lateral side of abdomen. From day 4 after transplantation, MB40 was administrated at dose of 10mg/kg, respectively, every other day by intratumoral injection. Cisplatin was systemically administrated at doses of 1mg/kg, respectively, every week by intraperitoneal injection. Results : 5 days after administration, tumors can be palpated in every mice group. After 13 days of administration, the mice group to which MB40 were administrated exhibited reduction in tumor size respectively, compared to cisplatin group. Overall status of mice such as body weight and activity were superior in MB40 group than cisplatin group. Conclusion : The result of this study indicates MB40 may have significant therapeutic effect and decreases complications induced by systemic chemotheraphy. MB40 may be developed as a novel and potent immunotropics to improve the cell immune system and anti-cancer drug for the treatment of cancer patients in head and neck squamous cell carcinoma.
Primary squamous cell carcinoma of the thyroid gland is a very rare event, representing much less than 1% of all malignant tumors of the thyroid gland. The cancer is characterized by rapidly progressive clinical course in spite of its differentiated morphologic features. In most cases, a squamous epithelium is believed to be a result of metaplasia of a follicular epithelium, although in rare exceptions, it can originate from a remnant of the thyroglossal duct or ultimobranchial body. Squamous cell carcinoma of the thyroid gland can occur in a pure form or mixed with adenocarcinoma. Because their clinical behavior is more aggressive than that of other malignant neoplasm of thyroid gland, the tumor should be treated more vigorously at its initial stage. Recently, authors experienced one case of primary squamous cell carcinoma of the thyroid gland. We report our case with a brief review of literature.

Lymphomas of the nasal sinuses are relatively uncommon and represent less than 1% of all head and neck malignancies. Most common symptoms are nasal obstruction, epistaxis, rhinorrhea. We report a case of primary extranodal diffuse large B-cell non-Hodgkin's lymphoma in nasal sinuses accompanying with exophthalmos. A 70-year-old male patient was referred to our hospital with exophthalmos and severe septal deviation. We conducted endoscopic biopsy with septoplasty and biopsy result was diffuse large B-cell lymphoma. He received chemotherapy(R-CHOP) and radiotherapy. At follow-up, he remained free of disease.

Dermoid cysts are benign neoplasm that is derived from both ectoderm and mesoderm. They may be found at various sites of the body, but are extremely rare to occur in the lower neck. We present the case of a 22-year old woman with a midline cystic mass anterior to the cervical trachea. The mass was excised under local anesthesia. Histopathologically the diagnosis was confirmed as a dermoid cyst.
Although liposarcoma is the second most common soft tissue sarcoma in adults, the incidence of liposarcoma in the head and neck is low. The histologic nature of liposarcoma is correlated clinically with treatment outcome, but histologic classification of liposarcoma is controversial. Well-differentiated liposarcoma and atypical lipoma are pathologic synonyms because they are identical lesions both morphologically and karyotypically. They represent the lowest grade lesions in the spectrum of liposarcoma. The terms "atypical lipoma" were introduced specifically to describe well-differentiated liposarcomas occurring in the extremities, because of their better prognosis in comparison with their retroperitoneal counterparts. We present 1 case of atypical lipomatous tumor of the neck.

A myoepithelial carcinoma, a rare malignant salivary gland neoplasm, occurs mostly in the parotid gland. The incidence of myoepithelioma is less than 1% of all salivary gland tumors, and malignant myoepithelioma is even more rare. Biological behavior of myoepithelial carcinoma has not been fully clarified. Malignant myoepithelioma shows clinicopathologic diversity and presents with various stages of myoepithelial differentiation. The definite treatment for malignant myoepithelioma is surgical excision, and the role of radiation therapy and chemotherapy is not yet established. In this study, we have experienced two patients with malignant myoepithelioma of the parotid gland who were treated with surgical excision.

Standard therapy for advanced head and neck cancer consists of a combination of surgery and radiation. However, survival of this patient population has not improved during the past 20 years. Many different multimodality treatment schedules have been proposed, and chemotherapy is often used with the intent of organ preservation. The present study was intended to establish the efficacy of concomitant chemoradiation with a single agent carboplatin in advanced head and neck cancers.The objectives were to investigate the feasibility of concomitant administration of carboplatin, monitor acute toxicity during radiotherapy, and determine subacute side effects, such as wound healing following surgery after chemoradiotherapy. A prospective study was conducted wherein a total of 40 patients with stage III and IV squamous cell carcinomas of oral cavity, oropharynx, hypopharynx and larynx were enrolled. All patients were treated with external beam radiotherapy and weekly carboplatin area under curve (AUC of 5). Radiotherapy was given in single daily fractions of 1.8-2 grays (Gy) to a total dose of 66-72 Gy. Salvage surgery was performed for any residual or recurrent locoregional disease. Neck dissection was recommended for all patients with neck disease showing less than a complete response after chemoradiation. A total of 40 patients were enrolled of whom 32 were males and 8 were females. Highest incidence of cancer was seen in the 5th-6th decades of life with a median age of 47.7 years. Oropharyngeal tumours constituted a maximum of 21 patients followed by hypopharynx in 10, larynx in 7 and oral cavity in 2. 80% of the patients had a neck node on presentation of which 40% had N2-N3 nodal status. TNM staging revealed that 58% of patients were in stage III and 43% in stage IV. Evaluation of acute toxicity revealed that 50% had grade II mucositis, 25% grade III mucositis, 2.5% grade IV mucositis. 50% of patients had grade I skin reactions, 65% of patients had grade I thrombocytopenia, and 24% of patients had grade I anaemia. After completion of treatment 65% of patients had complete response at the primary and regional sites, and 35% of patients had a partial response of whom 23% underwent neck dissection and 5% of them underwent salvage surgery at the primary site. At the end of one year there were six deaths and four recurrences and 70% were free of disease. Concurrent chemoradiation with carboplatin provided good locoregional control for locally advanced head and neck cancers. This regimen, although toxic, is tolerable with appropriate supportive intervention. Primary site conservation is possible in many patients. Chemoradiotherapy appears to have an emerging role in the primary management of head and neck cancers.
Angioimmunoblastic T-cell lymphoma (AITL) is a rare subtype of malignant lymphoma (ML), accounting for only 1 to 2% of all non-Hodgkin's lymphoma (NHL). Although ML of the parotid gland is rare, the majority are B-cell types. The AITL occurring synchronously in the parotid gland and lateral neck has not been reported earlier. It is classified as a high-grade malignancy with aggressive clinical features, and the prognosis is worse than any other type of NHL. We recently encountered a 72-year-old man with multiple mass on the ipsilateral parotid tail and lateral neck, and he was finally diagnosed as AITL. We report the unique and rare disease entity with a brief literature review.

Neck masses can have various origins and diverse presentations, making accurate diagnosis challenging. Schwannomas and lymphoepithelial cysts are commonly encountered neck tumors, and it is hard to get differential diagnosis. A 50-year-old woman presented with a progressively enlarging right neck mass discovered five years ago. Upon examination, a 3 cm firm, non-tender, and mobile tumor was found in the right neck level II region. Imaging studies suggested a tumor originating from the cervical sympathetic chain. The patient underwent a resection of the tumor under general anesthesia. Postoperative follow-up showed no complications or signs of recurrence. However, contrary to the initial suspicion of a schwannoma, the final pathological examination revealed a lymphoepithelial cyst. In this paper, we present a case of a cervical lymphoepithelial cyst misdiagnosed as a schwannoma, aiming to compare the clinical and histological characteristics of these two tumors and provide insights into appropriate diagnosis and management.
