Purpose: This study aimed to investigate sex differences in the effect of body mass index (BMI) and stress on high-risk diabetes mellitus (DM). Methods: Secondary analysis of data from 4,271 male and female adults participating in the Sixth Korea National Health and Nutrition Examination Survey 2015 was performed. The participants were evaluated using questionnaires and blood tests. Data were analyzed using descriptive statistics, t-test, ${\chi}^2-test$, and multiple logistic regression analysis (SPSS 24.0). Results: To identify sexspecific effects, interaction variables were included. Hemoglobin A1c (HbA1c) level was higher in men than in women, and the risk of DM decreased 0.31 times in women compared to that in men. As age increased, the odds of risk DM increased 1.03. The risk of DM increased 1.99 times in overweight individuals and 2.79 times for obese individuals compared to that in individuals with normal weight. Stress levels were higher in women than in men, but stress is not an influential factor in high-risk DM. In age-sex interaction, the odds of risk DM increased 1.02 in women compared to that in men as age increased. Conclusion: HbA1c level was affected by age-sex interaction, and age and sex should be considered in the application of HbA1c in the diagnosis of DM.

The prevalence of diabetes mellitus continues to rise alarmingly because of industrialization of society. The aim of this study was to investigate the antidiabetic effects of black ginseng extract (BGE) which was performed panax ginseng. First we examined the inhibitory effects of BGE on ${\alpha}$-glucosidase. But there was no practical effects in our observations. We, also, investigate the effects of BGE on glucose uptake of skeletal muscle using 2-NBDG in $C_2C_{12}$ myotube. BGE significantly improved the glucose uptake considered as a lowered blood glucose level. Effects of GBG05-FF on fasting blood glucose and glycated hemoglobin (HbA1c) were investigated in streptozotocin (STZ)-induced diabetic mouse. After injection of STZ, fasting blood glucose and glycated hemoglobin rapidly increased. But STZ-induced diabetic mouse treated with GBG05-FF significantly reduced the level of fasting blood glucose and HbA1c. This results showed that supplementation of BGE improve the diabetic parameters and BGE have a potentiality as a functional food for Diabetes mellitus.

Fire patterns have been used to determine the origin and cause of fires in every setting imaginable. However, it is very difficult to identify fire patterns from the fire-damaged remains of a devastated structure. If someone was killed by the fire, it is possible to identify fire patterns by analyzing the concentration of carbon monoxide-hemoglobin in the body of deceased as well as the pace of the fire. For example, a low level of carbon monoxide-hemoglobin in the body of the dead indicates a rapid fire with accelerants and the death was caused by severe heat and thick toxic fumes. However, a high level of carbon monoxide-hemoglobin in the body of the dead demonstrates that the fire was slow and/or there was a flameless form of combustion. Thus, this study identifies fire patterns through analyzing the level of carbon monoxide-hemoglobin concentration on the dead from the fire.
Background Some patients who need surgery refuse a blood transfusion because of their religious beliefs or concerns about blood-borne infections. In recent years, bloodless surgery has been performed successfully in many procedures, and is therefore of increasing interest in orthognathic surgery. Methods Ten Jehovah's Witnesses who visited our bloodless surgery center for orthognathic surgery participated in this study. To maintain hemoglobin (Hb) levels above 10 g/dL before surgery, recombinant erythropoietin (rEPO) was subcutaneously administered and iron supplements were intravenously administered. During surgery, acute normovolemic hemodilution (ANH) and induced hypotensive anesthesia were used. To elevate the Hb levels to >10 g/dL after surgery, a similar method to the preoperative approach was used. Results The 10 patients comprised three men and seven women. Their average Hb level at the first visit was 11.1 g/dL. With treatment according to our protocol, the average preoperative Hb level rose to 12.01 g/dL, and the average Hb level on postoperative day 1 was 10.01 g/dL. No patients needed a blood transfusion, and all patients were discharged without any complications. Conclusions This study presents a way to manage patients who refuse blood transfusions while undergoing orthognathic surgery. rEPO and iron supplementation were used to maintain Hb levels above 10 g/dL. During surgery, blood loss was minimized by a meticulous procedure and induced hypotensive anesthesia, and intravascular volume was maintained by ANH. Our practical approach to orthognathic surgery for Jehovah's Witnesses can be applied to the management of all patients who refuse blood transfusions.
Objectives: The American Diabetes Association (ADA) has recently recommended the HbA1c assay as one of four options for making the diagnosis of diabetes mellitus, with a cut-point of $\geq$ 6.5%. We compared the HbA1c assay and the fasting plasma glucose level for making the diagnosis of diabetes among Korean adults. Methods: We analyzed 8710 adults (age 45-74 years), who were not diagnosed as having diabetes mellitus, from the Namwon study population. A fasting plasma glucose level of $\geq$126 mg/dL and an A1c of $\geq$ 6.5% were used for the diagnosis of diabetes. The kappa index of agreement was calculated to measure the agreement between the diagnosis based on the fasting plasma glucose level and the HbA1c. Results: The kappa index of agreement between the fasting plasma glucose level and HbA1c was 0.50. Conclusions: The agreement between the fasting plasma glucose and HbA1c for the diagnosis of diabetes was moderate for Korean adults.
An intravenous pharmacokinetics for a new red cell substitute, PEG-hemoglobin SB1, was studied in SD rats. Total-hemoglobin and its metabolite methemoglobin in the plasma were determined using a spectrophotometer. The limit of quantitation was 0.01g/dL and the C.V for interday assay reproducibility was less than 6%. Upon intravenous administration of anticipated clinical dose, 10 ml(0.7 gHb)/kg, plasma concentration curve of total-hemoglobin was well described by one-compartment model. The $t_{\frac{1}{2}},{\;}CL_{t}$, Vd and $AUC^{0-48hr}$ were $8.23{\pm}0.96{\;}hr,{\;}0.06{\;}{\pm}{\;}0.01 {\;}dL/hr/kg,{\;}0.66{\pm}0.05{\;}dL/lg{\;}and{\;}13.6{\;}{\pm}{\;}1.01g{\cdot}hr/dL$, respectively, in male rats(n=5, $mean{\;}{\pm}{\;}SD$). Those parameters in female rats were $9.21{\;}\pm{\;}2.31{\;}hr,{\;}0.06{\;}{\pm}{\;}0.01{\;}dL/hr/kg,{\;}0.79{\pm}{\;}0.08{\;}dL/kg{\;}and{\;}13.0{\;}{\pm}{\;}2.36{\;}g{\cdot}hr/dL$, respectively. Similar kinetic profiles between males and females were also obtained from other parameters. Small amount of methemoglobin, an oxidative metabolite of SB1, was detected in the plasma of both sexes, where the $AUC^{0-48{\;}hr,m}$ and $t_{{\frac{1}{2}},m}$ were approximately $1.5{\;}g{\cdot}hr/dL$ and 20 hr, respectively. The present work provides a critical kinetic data for the effective clinical applications of PEG-hemoglobin SB1.
Ethylene oxide is a genotoxic carcinogen with widespread uses as industrial chemical intermediate and gaseous sterilant. 2-hydroxyethylated N-terminal valine in Hb is a good biomarker for biological monitoring of ethylene oxide exposure, because of its stability. For measuring the hemoglobin adduct formed by exposure of ethylene oxide, we studied the determination of (N-2-hydroxy-ethyl)valine(HEV) in hemoglobin adduct by using GC/MS. Firstly we synthesized HEV with 2-amino-ethanol and bromoisovaleric acid(BIVA) and confirmed it with GC/MS-FID. Its fragmentations were m/z 116(base ion, M+-45) and m/z 130(M+-31). For measuring HEV with higher sensitivity, we use derivatives which were PFPITH(pentafluorophenylisothiocianate) and TBDMS (tributyldimethylsilylation) by using Edman procedure. Its fragmentation were m/z 425(M+-57), m/z 383(M+-99) and m/z 172(M+-310) by using GC/MS. We did biological monitoring for mice inhalation exposure with 400 ppm ethylene oxide. The concentrations of hemoglobin adduct were $168{\pm}3.8\;and\;512{\pm}04$(nmol g-1 globin) at 0.5 hr/day 400 ppm ethylene oxide inhalation exposure group, and $631{\pm}17\;and\;2265{\pm}9.4$(nmol g-1 globin) at 1.0 hr/day 400 ppm ethylene oxide inhalation exposure for 1 and 4 weeks, respectively. We confirmed that (N-2-hydroxy-ethyl)valine(HEV) of hemoglobin was a good biomarker for biomonitoring of ethylene oxide exposure, and can measured with derivatives such as PFPITH(pentafluorophenylisothiocianate) and TBDMS(tributyldimethylsilylation) by using GC/MS.
Objectives: The purpose of this study is to investigate the impact of health behaviors of male workers without diabetes such as smoking, drinking and physical activities on HbA1c using 2015 and 2016 data from Korea National Health and Nutrition Examination Survey (KNHANES). Methods: The final study subjects of KNHANES were a total of 1,703 male workers in their 30s-50s who had normal levels of fasting blood sugar level, HbA1c, and hemoglobin. For the study variables, age, household income, educational level and occupation were included as socio-demographic characteristics; smoking behavior, drinking behavior, and physical activity behavior as characteristics of health behavior; and waist circumference, body mass index, systolic and diastolic blood pressures, total cholesterol, triglyceride and HDL cholesterol as medical examination characteristics were included. The analysis was conducted using independent variables t-test, one-way ANOVA, Pearson's correlation analysis and multiple regression analysis. Results: Socio-demographic factors that affect HbA1c were age, educational level, and occupation. As for HbA1c according to the status of current smoking, it was higher in order of the present, past and nonsmoking and HbA1c according to smoking amount pack-year increased linearly in between less than 1 pack-year and over 30 pack-year (p<0.001). Muscular exercises affected the reduction of HbA1c (p<0.05). In the multiple regression analysis with independent variables of the socio-demographic characteristics and health behavior, factors affecting HbA1c were age, occupation and smoking amount pack-year (p<0.05). In the multiple regression analysis where the characteristics of physical examination were added, factors affecting HbA1c were age, occupation, smoking amount pack-year, waist circumference and total cholesterol (p<0.001). Conclusions: As a result, the health behavior that had the most effect on HbA1c management for male workers without diabetes was the total lifetime smoking amount. Therefore, it is essential to prevent smoking as well as control, cholesterol to prevent diabetes for male workers.
A newly derived O$_2$ Saturation Model which can be adapted to the design and study of Artificial Lung and Blood Gas Calculator etc. is introduced on the basis of Electrical Equivalent Circuits. The presented 4 stage and 2 stage RC circuits have good correlatons with actual chemical reactions of Hemoglobin and Oxygen. However, from results of computer simulations, 2 stage equivalent model is more accurate than 4 stage and conventional O$_2$ saturation models.
