골성숙도와 심박 변이도의 상관성에 대한 연구 (A Study on Relations between Skeletal Maturity and Heart Rate Variability)

  • Objectives The purpose of this study is to examine the relationship between skeletal maturity and heart rate variability (HRV) based on the bone age and HRV parameters. Methods 103 children from 6 years to 17 years of age, who do not have any disease-related symptom, and visited ${\bigcirc}{\bigcirc}$ oriental medicine hospital, are measured based on their bone age and short-term spectral analysis of HRV. Results 1. Skeletal maturity was significantly correlated with HRV indices: mean HRT and SDNN. As the skeletal maturity increases, the mean HRT was decreased and the SDNN was increased. 2. When classifying according to the skeletal maturity score, the mean HRT was higher in the 'below -0.4' group compare to '-0.3~0.7' group and '0.8~1.9' group. SDNN was higher in the '0.8~1.9' group compare to '-0.3~0.7' group, 'below -0.4' group 3. When classifying according to the sex, age and secondary sexual characteristics, as the skeletal maturity was increased, the mean HRT was significantly decreased and the SDNN was significantly increased only in the boys who did not develop secondary sexual characteristics. Conclusions Skeletal maturity could be statistically significant with HRV indices, especially to the boys and the children than the girls and the teenagers.

  • Objectives: The purpose of this study is to find the effect of Far Infrared Radiation (FIR) about improvement of chief complaints and health state. Methods: For this study we evaluated thermographic images, heart rate variability (HRV), visual analogue scale (VAS) score of 34 patients with various diseases. Tests were done two times, before and after treatments and we compared the test results. We used the temperature difference between CV12 and CV4, both sides of PC8, LR3 for recognizing improvement of blood circulation. We analyzed the change of complexity, stability of autonomic nervous system (ANS) by HRV test. The patients were asked to fill out questionnaire about the severity of symptoms by VAS score. Analyses were undertaken using SPSS ver.12.0.1 and p-value of < 0.05 was considered significant. Results: Statistical analysis shows that Far Infrared Radiation (FIR) had significant efficiency in increasing surface temperature and reducing VAS pain scores. In heart rate variability (HRV) test, LF/HF ratio showed tends to improve. Conclusions: The application of an FIR to whole body appears to alleviate various complaints of patients.

Sweet Bee Venom과 Bee Venom이 심박변이도(HRV)에 미치는 영향 (Effects of Sweet Bee Venom and Bee Venom on the Heart Rate Variability)

  • Objective : In this study, we investigated the effects of Sweet Bee Venom(SBV) and Bee Venom(BV) at a acupoint, HT7(Shinmun) on the Heart Rate Variability(HRV) in the healthy man. And we tried to observe how Sweet Bee Venom and Bee Venom affects on the balance of the autonomic nervous system. Methods : We investigated on 22 heathy volunteers consisted of 10 subjects in SBV group and 12 subjects in BV group. Study form was a randomized, placebo-controlled, double-blind clinical trial. 22 subjects of each group were injected SBV and BV at HT7(Shinmun). And we measured HRV by QECG-3:LXC3203 (LAXTHA Inc. Korea) on 7 times : before and after injection per 5minutes during 30minutes. Results : 1. After SBV injection, Mean-RR was significantly high from 0 to 10 minutes, Mean-HRV was significantly low from to 10 minutes, SDNN was significantly high after 25minutes, Complexity was significantly high from 5 to 10 minutes and RMSSD was significantly high from 5 to 10minutes. 2. Complexity of SBV Group significantly decreased from 20 to 25minutes, RMSSD of SBV Group significantly increased from 10 to 15minute and from $20{\sim}25$minutes, SDSD of SBV Group significantly increased from 10 to 15 minute and from $20{\sim}25$minutes compared with that of BV group. 3. After SBV injection, Ln(VLF) was significantly from 25 to 30minutes. Conclusions : The results suggest that SBV in heathy adult man tend to activate the autonomic nervous system compared to BV within normal range.

중국 기공 및 쿤달리니 요가 명상이 숙련자의 심박변이율(HRV) 변화에 미치는 영향에 관한 연구 (A Study on the Effects of Chinese Qigong and Kundalini Yoga Meditations on the Heart Rate Variability of Skilled Students)

  • In this paper, we have investigated effects of two specific meditations (Chinese qigong meditation and Kundalini yoga meditation) on the heart rate variability (HRV), which is a well-known quantitative measure of autonomic balance, of skilled students. To analyze the effects, the MIT/BIH physionet database was utilized. The database includes RR intervals of eight skilled Chinese qigong meditators (5 women and 3 men; age range 26-35) and four skilled Kundalini yoga meditators (2 women and 2 men; age range 20-52). RR intervals of each subject were measured before and during the meditations. For HRV analysis, we have used typical four HRV parameters - the low frequency to high frequency power ratio (LF/HF ratio), SD2/SD1 ratio, sample entropy, and fractal dimension. The LF/HF ratio was calculated by the autoregressive spectrum and the SD2/SD1 ratio was derived from the Poincar$\grave{e}$ plot. The sample entropy was computed from the phase space plot and the fractal dimension was estimated by the Higuchi's algorithm. In the experiments, the Wilcoxon signed rank test was employed because we used small datasets and compared HRV parameters before and during the meditations. As a result, we have found increment of the LF/HF and SD2/SD1 ratios in both meditations; whereas the sample entropy is decreased during the meditations. In addition, the fractal dimension is increased during the Chinese qigong meditation; whereas it is decreased during the Kundalini yoga meditation. The results show that the sympathetic nervous system is generally more activated in skilled Chinese qigong and Kundalini yoga meditators, but the activation of the parasympathetic nervous tone is suppressed.

임상시험 중 수면행위가 심박변이도에 미치는 영향 분석 (Assessment of covariate-effect of sleeping behavior on heart rate variability during a clinical trial)

  • The object of this study is to evaluate confounding effect of sleeping behavior on heart rate variability (HRV) in healthy volunteers during a randomized clinical trial. From 6 healthy volunteer, we acquired circadian electrocardiography(ECG) with a Holter monitoring device(DigiTrak Plus, Philips). The compatible analytical program, Zymed, was used for calculation of RR intervals of 24h ECG signal. Then, we generated HRV signals and calculated 9 HRV parameters in time domain and frequency domain for every 30 minute with Matlab 6.x software. We also encoded participants' activities as follows; stable condition(1), sleeping behavior(2), eating behavior(3), and undefined condition(4). Using SPSS 12.0, we tested if there was any difference between HRV parameters of day-time and night-time and between those of a subject in stable condition and in sleeping behavior. As a result, one of the most important parameter-LF/HF ratio, which indicates the autonomic balance between sympathetic and parasympathetic nervous systems, as same the other parameters, showed large inter-individual variations in circadian profiles. And, the mean LF/HF ratio of each participant was different between in stable condition and in sleeping behavior (p<.05). Consequently, the effect of sleeping behavior must be considered when one or more of HRV parameters, especially LF/HF ratio, are used as assessment index in clinical studies.

한방음악치료가 정신적 스트레스를 가한 정상 성인의 심박변이도에 미치는 영향 (Effects of Oriental Medicine Music Therapy for Mental Stress on Short-term Analysis of Heart Rate Variability)

  • Objectives : This study was conducted to assess effects of oriental medicine music therapy on heart rate variability(HRV) in normal subjects under mental stress. Methods : 34 healthy volunteers(control group:17, experimental group:17) participate in this study. The volunteers were randomly allocated into two groups. After 10-minutes rest period and instrumentation, both groups performed a mental stress test for 12-minutes. HRV was recorded before and after the mental stress. And then the experimental group applied oriental medicine music therapy for 10-minutes. In the control group, they rested for 10-minutes. And HRV was recorded. Results : After giving mental stress stimulation to both groups, LF and LF/HF ratio were significantly increased and HF, mean HRV were decreased. In experimental group, LF, LF/HF ratio were significantly decreased and HF, SDNN were significantly increased after applying oriental medicine music therapy. In control group, LF was significantly decreased. And LF/HF ratio was decreased and HF, SDNN were increased, but it was not significant. Conclusions : This results show that oriental medicine music therapy could be relatively useful to decrease sympathetic activity and prevent the alteration of autonomic nervous system due to mental stress.

근골격계 통증 환자에서 침 치료 후 심박수 변이도(Heart rate variability) 변화에 관한 연구 (Study on the Change of the Autonomic Nervous System by HRV Test in Musculoskeletal Pain Patients Treated with Acupuncture)

  • Objectives : This study was designed to evaluate the changes of autonomic nervous system by heart rate variability(HRV) test on the musculoskeletal pain patients treated with acupuncture on the Back-su points and the unfixed points, and untreated healthy adults. Methods : This clinical study have been carried out with 64 cases of musculoskeletal pain patients and 26 healthy adults. 35 patients were treated with acupunctureon on the Back-su points(Group I) and 26 patients were treated on the Unfixed points(GroupII), and two groups were tested by HRV at pre and post acupuncture treatment. 26 healthy adults(GroupIII) were tested twice by HRV without any stimulation. Results : In Group I, VAS was decresased $5.83{\pm}1.62$ to $5.17{\pm}1.89$ significantly(p<0.05) after acupuncture treatment, and LF/HF ratio was decreased significantly(p<0.05) as compared with healthy control Group. In GroupII, SDNN & RMSSD were increased singnificantly(p<0.05) as compared with other Groups. in Group I & II, MHRT was decreased significantly(p<0.05) as compared with healthy control Group. Conclusions : These results suggest that acupuncture treatment on the Back-su points was able to reduce the pain, suppress the sympathetic activities and enhance the parasympathetic activities in musculoskeletal patients.

  • Objectives : The purpose of this study is to assess the effect of laser acupuncture at $HT_7$ for reducing mental stress using power spectrum analysis of the heart rate variability. Methods : 36 healthy volunteers(control group: 18, treatment group: 18) participated in this study. After instrumentation and 5-minute rest period, 5-minute metal stress by operation was provided. HRV was recorded before and after the mental stress(1st, 2nd HRV). After 2nd HRV recording, the control group rested for 15 minutes without any treatments. Participants in the treatment group received laser acupuncture to $HT_7$ unilaterally and rested for 15minutes. Points were irradiated for 60seconds, and the intensity was 1.8J(output 20%). And then 3rd HRV was recorded. Results : In simple rest group, LF norm showed a significant change after mental stress. In laser acupuncture group, LF norm, HF norm, LF, HF and LF/HF ratio showed a significant change after mental stress. But there were no significant difference between two groups(ANCOVA test, p>0.05). In laser acupuncture group, LF norm, HF norm, HF and LF/HF ratio showed a significant change after laser acupuncture treatment. And there were significant differences between two groups(p<0.05). Conclusions : This study suggests that laser acupuncture at $HT_7$ can regulate and prevent the alternation of autonomic nervous system due to mental stress.

맥솔(脈率)과 심박변이도(心搏變移度)의 상관성(相關性) 연구(硏究) (Correlation between Pulse-respiration Ratio and Heart Rate Variability)

  • Background : Pulse-respiration ratio has been used for estimating subject's status in oriental medicine. Pulse and respiration is strongly associated with autonomic nerve system. But there is no research about correlation between pulse-respiration ratio and autonomic nerve system. Objectives : We performed this study to know correlation between pulse-respiration ratio and HRV(heart rate variability) that shows autonomic nerve system status well and to clarify clinical meaning of pulse-respiration ratio. Methods : After subject's 10 minutes rest, we measured subject's ECG, respiration pattern and HRV. In this research, subject's number is 95(Male 50/Female 45). We calculated pulse-respiration ratio from ECG and respiration pattern. Then, we analyzed correlation between pulse-respiration ratio and HRV parameters in all subjects, 2 group divided by Wan-Maek(P-R ratio 4.28). We tried to compare HRV parameters among Wan-Maek, Sak-Maek and Ji-Maek group. Correlation analysis between pulse-respiration Ratio and Pulse rate, respiration rate is performed. Finally correlation analysis between Respiration and HRV parameters in all subjects, 2 group divided by Wan-Maek(4.28) is studied. Results : 1. Mean pulse-respiration is 4.10${\pm}$0.67, Mean pulse rate is 68.06${\pm}$7.82bpm, Mean respiration rate is 16.81${\pm}$2.72 times per minute in all subjects. 2. Correlation analysis between pulse-respiration ratio and HRV parameters of high pulse-respiration ratio group is not significant. But, in low pulse-respiration ratio group, HFnorm(correlation coefficient 0.306, p= 0.018), lnHF (0.308, p=0.002) is significantly correlated with pulse-respiration ratio. 3. Comparison of HRV parameters among Wan-Maek, Sak-Maek and Ji-Maek Group is not significant. 4. Pulse-respiration ratio is more affected by respiration rate(correlation coefficient-0.17, p=0.000) than pulse rate (correlation coefficient 0.396, p=0.000). 5. Correlation analysis between respiration rate and HRV parameters of high pulse-respiration ratio group is not significant. But, in low pulse-respiration ratio group, HFnorm (correlation coefficient -0.327, p=0.011), LF/HF(0.346, p=0.007), lnHF (-0.355, p=0.006) are significantly correlated with respiration rate. Conclusion : Pulse-respiration ratio and parasympathetic index has positive correlation. The closer Wan-Maek, The higher parasympathetic index in low pulse-respiration ratio group. Respiration rate is more related with pulse-respiration ratio than pulse rate. Respiration is negatively correlated with autonomic parameters. And the slower respiration, the higher parasympathetic index in low pulse-respiration ratio group.

Development of a Classification Model for Driver's Drowsiness and Waking Status Using Heart Rate Variability and Respiratory Features

  • Objective:This study aims to evaluate the features of heart rate variability (HRV) and respiratory signals as indices for a driver's drowsiness and waking status in order to develop the classification model for a driver's drowsiness and waking status using those features. Background: Driver's drowsiness is one of the major causal factors for traffic accidents. This study hypothesized that the application of combined bio-signals to monitor the alertness level of drivers would improve the effectiveness of the classification techniques of driver's drowsiness. Method: The features of three heart rate variability (HRV) measurements including low frequency (LF), high frequency (HF), and LF/HF ratio and two respiratory measurements including peak and rate were acquired by the monotonous car driving simulation experiments using the photoplethysmogram (PPG) and respiration sensors. The experiments were repeated a total of 50 times on five healthy male participants in their 20s to 50s. The classification model was developed by selecting the optimal measurements, applying a binary logistic regression method and performing 3-fold cross validation. Results: The power of LF, HF, and LF/HF ratio, and the respiration peak of drowsiness status were reduced by 38%, 22%, 31%, and 7%, compared to those of waking status, while respiration rate was increased by 3%. The classification sensitivity of the model using both HRV and respiratory features (91.4%) was improved, compared to that of the model using only HRV feature (89.8%) and that using only respiratory feature (83.6%). Conclusion: This study suggests that the classification of driver's drowsiness and waking status may be improved by utilizing a combination of HRV and respiratory features. Application: The results of this study can be applied to the development of driver's drowsiness prevention systems.
