Concordance in Cervical HPV Detection between Hybrid Capture 2 and HPV GenoArray Tests

  • HPV type-specific detection may promote cervical screening program and vaccination development worldwide. We conduct a study comparing HPV Hybrid capture II (HC II) Test and Hybribio GenoArray test, a newly developed HPV type-specific assay, in patients with cervical epithelial neoplasm. Results showed a good concordance in cervical HPV detection between two tests (kappa value 0.80, p<0.05, McNemar test). Our study may promote utilization of type-specific HPV detection that is helpful for cervical cancer screening and vaccination.

일부 여대생의 인유두종 바이러스 지식에 관한 연구 (Study on the Knowledge of Human Papilloma Virus in Female University Students)

  • Purpose: This study was performed to examine the knowledge of HPV in female university students. Methods: Participants consisted of 285 female university students(nursing major;153, others;132). A dichotomy with 20 items of HPV knowledge was developed by a researcher. Results: Reliability of the HPV knowledge tool was Cronbach' alpha .87, and Split-half Guttman coefficient .85. Correction rates of HPV knowledge by item ranged from 19.6 to 76.8%. There were no significant differences in HPV knowledge scores by major group or sex related characteristics. Conclusions: Accurate, university wide education regarding HPV knowledge for female university students should be done regardless of the major; nursing or not. Replication studies with different ages or sex groups and application of HPV education in relation to STI prevention and cervical cancer prevention are recommended.

HPV Vaccine Knowledge and Perceived Risk of Cervical Cancer among Female College Students in Taiwan

  • Aims: The study targeted the HPV vaccine knowledge and perceived risk of cervical cancer among female college students in Taiwan as well as the relationship between knowledge of the HPV vaccine and perceived risk of cervical cancer. Materials and Method: The results of this study on female college students are described using descriptive and correlation designs. A convenience sampling approach was employed with a self-filling structured questionnaire. Results: A total of 150 students completed the questionnaire. Values were 7.49 for the mean HPV vaccine knowledge scale and 18.0 for their mean perceived risk of cervical cancer scale. HPV vaccine knowledge was positively correlated with perceived risk of cervical cancer. Conclusions: The findings of this study can serve as a reference for future HPV prevention in Taiwan.

중국과 한국여성에서 나이에 따른 인유두종바이러스의 유병률과 분포 (Human Papillomavirus Prevalence and Distribution according to Age among Korean and Chinese Women)

  • 자궁경부암은 전세계 여성에서 세 번째로 많이 발생하는 암이다. 인유두종 바이러스(HPV) 감염이 자궁경부암 발병에 주요한 역할을 한다. HPV 감염은 지리적 위치나 면역학적 상태, 성적 병력, 유전적 요인 등에 영향을 받는다. 예를 들어, HPV 감염률이 스페인에서는 1.5%인 반면, 온두라스와 케냐에서는 39%에 육박한다. 그러나 HPV 감염률과 나이와의 연관성에 대해서는 알려진 바가 적다. 나이에 대한 HPV 감염률 분석은 어느 나이대가 자궁경부암에 고위험군인지를 알아낼 수 있다. 또한 특정 HPV 유전형과 나이 간의 연관성을 입증할 수 있다. 게다가 백신을 주사하는 데에 최적의 나이대를 명확히 알 수 있을 것이다. 이 연구에서는 한국과 중국 여성에서의 HPV 감염률과 유전형 분포를 나이에 따라 분석하였다. 한국과 중국 여성의 자궁 탈락 세포로 REBA HPV-ID$^{(R)}$ 검사법(YD 진단법, 용인, 대한민국)을 통해 HPV 유전형을 파악하였다. 편평세포암과 고등급 병변의 환자군에서는 40대와 50대가 주를 이룬 반면, 중간병변의 환자군에서는 30대와 40대가 주를 이뤘다. HPV 16, 18형 감염은 주로 40대에 속했다. 이러한 결과는 지속적인 감염이 자궁경부암을 일으킨다는 주장을 뒷받침한다.

  • 인유두종바이러스(human papillomavirus: HPV)는 감염되었을 때 각종 악성 종양을 유발할 수 있는 작은 DNA 바이러스이다. 고위험군에 속하는 HPV의 감염은 암으로 진행될 수 있는 가능성이 크다. 본 논문은 HPV를 분류할 수 있는 기계 학습 기법을 제안하고자 한다. 제안된 학습 기법은 단백질 서열을 효과적으로 분류할 수 있는 커널(kernel) 방법에 기반을 두고 있다. 위험군 분류는 감염의 메커니즘의 이해와 유전자칩과 같은 새로운 의학 도구의 개발 등에 있어서 중요한 정보를 제공해 줄 수 있다. 실험 결과는 중요한 부위의 탐색에 의한 커널 기반의 학습 방법이 우수한 성능을 보이는 것으로 나타났다.

No Detection of 'High-risk' Human Papillomaviruses in a Group of Iranian Women with Breast Cancer

  • The presence of viral DNA in breast cancer cells is controversial. However, some studies have revealed a possible role for the human papillomavirus in the pathogenesis of breast cancer. The aim of the present study was to investigate the presence of HPV-DNA in breast tissue in a group of Iranian women with and without breast cancer and identification of the detected HPV types. Paraffin-embedded specimens from 65 malignant breast cancer cases and 65 cases with benign breast lesions were investigated for presence of HPV-DNA by nested polymerase chain reaction. We found HPV-DNA in 22 (33.8%) of the breast cancer specimens. All non-cancerous specimens were negative. Low and high-risk HPV types, including HPV-6 (26.2%), HPV-16 (1.5%), HPV-35 (1.5%), HPV-52 (1.5%), and HPV-11 (1.5%) were detected in our study. HPV-6 was the most prevalent type in the breast cancer specimens. Although high-risk HPV types have been shown to have a major role in cervix cancer, there have been no data that support the same relevance for other types of malignancies. Furthermore, presence of low-risk HPV types in malignancies still is a matter of debate. The data presented in this study indicates a strong need for epidemiological studies correlating different HPV types in human breast cancer.

Human Papillomavirus Distribution among Women in Western Shandong Province, East China using Reverse Blot Hybridization Assay

  • Cervical cancer is the third most common cancer in women worldwide and there is a significant association between human papillomavirus (HPV) infection and cervical cancer. Certain HPV groups, labeled high-risk (HR) HPV groups, are strongly associated with malignancies of the human cervix. HPV prevalence and genotype distribution were analyzed using the REBA $HPV-ID^{(R)}$ (YD Diagnostics, Yongin, Korea) assay based on the reverse blot hybridization assay (REBA) with a total of 324 liquid-based cytology samples from women in Western Shandong Province, East China and results were compared with cytological diagnosis. Most of the HPV genotypes that were detected in high-grade cervical lesions were HR-HPV genotypes such as HPV 16, 18, 33, 53, and 58. The prevalence of these HR-HPV genotypes increased in high-grade cervical lesions. However, from low- to high-grade cervical lesions, the ability to detect LR-HPV genotypes decreased. Additionally, in general, the single HPV genotype infection rate increases in proportion to the severity of the lesion. The study findings suggest that a currently available preventive vaccine against HPV 16 and 18 may have limited effectiveness for prevention of all HPV infection in this province. Finally, based on these findings, these data could guide national or regional vaccination programs in the Western Shandong Province of East China to substantially reduce the burden of cervical lesions.

여중생 딸을 둔 어머니의 인유두종 바이러스 백신 접종 의도 및 관련 요인 (Factors associated with the Intention of Human Papillomavirus Vaccination among Mothers of Junior High School Daughters)

  • 본 연구는 계획된 행위이론을 적용하여 여중생 딸을 둔 어머니의 HPV(human papillomavirus) 백신 접종 의도를 파악하고, 관련 요인을 알아보기 위함이며, G광역시에 소재한 여중학교 4개교의 어머니 198명을 대상으로 하였다. 연구 도구로 HPV 백신 접종에 대한 태도, 주관적 규범, 지각된 행위 통제는 Askelson 등이 개발한 도구를 사용하였으며 자료 수집 기간은 2012년 9월 17일부터 24일까지였고, 수집된 자료는 SPSS 18.0 Program을 사용하였다. 본 연구 결과 여중생 어머니들의 HPV 백신 접종 의도는 비교적 높았지만, 6개월 이내에 접종하겠다고 한 어머니는 29.8% 뿐이었고, 어머니들이 생각하는 HPV 백신 최적 접종 연령이 평균 17.74세였다. 대상자의 HPV 백신 접종 의도는 태도, 지각된 행위 통제, 주관적 규범과 통계적으로 유의한 상관관계가 있었고, HPV 백신 접종에 대한 태도, 주관적 규범, 지각된 행위 통제가 접종의도의 중요한 영향 요인임이 확인되었으며, 대상자의 HPV 백신 접종 의도를 84.3%를 설명하였다. 그러므로 어머니들이 HPV 백신 접종에 대해 긍정적으로 생각하고, 백신 접종을 쉽고, 용이하게 지각하도록 격려하는 홍보 및 사회적 분위기 조성이 필요하리라 사료된다.

Pap Smear Combined with HPV Testing: A Reasonable Tool for Women with High-grade Cervical Intraepithelial Neoplasia Treated by LEEP

  • Background: To evaluate HPV testing by Hybrid Capture II (HCII) in conjunction with cytology in detecting the residual/recurrence disease after treatment of high-grade cervical intraepithelial neoplasia (CIN II-III) with loop electrosurgical excision procedure (LEEP). Materials and Methods: A retrospective review of 158 patients with histologically confirmed CIN II-III who underwent LEEP between January 2011 and October 2012 was conducted. Post-treatment control was scheduled at the 3rd, 6th, 12th and 18th month. All patients were followed up by Pap smear and HR-HPV genotype and viral load testing. Results: Pre-treatment, HR-HPV DNA, was detected in all specimens of the patients. At follow-up, 25 patients were diagnosed as the residual/recurrent disease during the FU visit, among whom, 16 patients with positive margin: 13 patients (52%) with HR-HPV DNA+/cytology+, 2 patients (8%) with HR-HPV DNA+/cytology-, 1 patient (4%) with cytology+/HR-HPV DNA-; 9 patients with clean margin - 5 patients (55.6%) with HR-HPV DNA+/cytology+; 2 patients (22.2%) with HRHPV DNA+/cytology-, 2 patients (22.2%) with cytology+/HR-HPV DNA-. None of them persisting HR-HPV DNA-/cytology-with positive or negative margin was identified as the residual/recurrent disease. The majority of residual/recurrent disease was detected at the 12th and 18th month FU, and there was almost no difference in the sensitivity and negative predictive value (NPV) between at the 3rd month and the 6th month FU visits. 14 residual/recurrence disease (14/46:30.4%) had pre-treatment high viral load (>5 000 RUL/PC) and 11 (11/112, 9.8%) with pre-treatment low viral load, P<0.05. Conclusions: (1) The persistence HR-HPV DNA is the root cause of the residual/recurrent disease for the women treated for high-grade CIN; the pre-treatment viral load and margin can be seen as the predictor. (2) The FU visit beginning at the 6th month post-treatment and lasting at least 24 months with the combination of cytology and HPV testing. (3) Patients with high pre-treatment HPV load, which is considered as one risk of developing the residual/recurrent disease, should be paid more attention (especially above 500RUL/PC) to by clinicians.

Detection of Human Papillomavirus in Male and Female Urine by Electrochemical DNA Chip and PCR Sequencing

  • Background: Cervical cancer is the second most common cancer in Thai women after breast cancer. Currently, the Papanicolaou (Pap) smear is the recommended procedure for cervical cancer screening in Thailand, but only a relatively small percentage of women follow this screening program. An alternative method to detect HPV genotypes associated with cervical cancer is self-sampling of urine, which is a more widely accepted method. Our study aimed to evaluate the prevalence of HPV in Thai women using urine and cervical swabs and prevalence of HPV in Thai men using urine samples. Materials and Methods: Tumorigenic HPV detection was accomplished by electrochemical DNA chip and PCR/direct sequencing. In addition to HPV prevalence, we report the concordance between different methods and sample types. One-hundred and sixteen women and 100 men were recruited. Histological examination revealed normal cytology in 52 women, atypical squamous cells of undetermined significance (ASCUS) in 9, low-grade squamous intraepithelial lesions (LSIL) in 24, and high-grade squamous intraepithelial lesions (HSIL) in 31. One-hundred men were classified as heterosexuals (n=45) and homosexuals (n=55). Results: The most prevalent HPV genotype in our study was HPV16. The HPV detection rate was generally lower in urine samples compared with cervical samples. Overall, there was good agreement for the detection of carcinogenic HPV from female cervical samples between the DNA chip and PCR/sequencing, with 88.8% total agreement and a kappa value of 0.76. In male urine samples, the level of agreement was higher in heterosexuals compared with homosexuals. Conclusions: Further improvement is required to increase an overall yield of HPV DNA detection in urine samples before clinical application of a urine-based HPV screening program. The electrochemical DNA chip test is a promising technique for carcinogenic HPV detection.
