GLOBAL STABILITY OF HIV INFECTION MODELS WITH INTRACELLULAR DELAYS

  • In this paper, we study the global stability of two mathematical models for human immunodeficiency virus (HIV) infection with intra-cellular delays. The first model is a 5-dimensional nonlinear delay ODEs that describes the interaction of the HIV with two classes of target cells, $CD4^+$ T cells and macrophages taking into account the saturation infection rate. The second model generalizes the first one by assuming that the infection rate is given by Beddington-DeAngelis functional response. Two time delays are used to describe the time periods between viral entry the two classes of target cells and the production of new virus particles. Lyapunov functionals are constructed and LaSalle-type theorem for delay differential equation is used to establish the global asymptotic stability of the uninfected and infected steady states of the HIV infection models. We have proven that if the basic reproduction number $R_0$ is less than unity, then the uninfected steady state is globally asymptotically stable, and if the infected steady state exists, then it is globally asymptotically stable for all time delays.

  • The purpose of this descriptive study was to investigate the general knowledge and attitude of physical therapist to patient with HIV/AIDS. Many physical therapists are afraid of AIDS patients because they have a little information of AIDS and fear of HIV infection from AIDS patients at work site. This study was carried out from June to October in 2003 for collecting data. The 271 physical therapists volunteered for a this study and completed questionnaires. The findings of this study can be summarized as follows. 1)The knowledge of prevention of HIV/AIDS influences the attitude of physical therapists to patients with HIV/AIDS. 2)The knowledge of symptoms of HIV/AIDS influences the attitude of physical therapists to patients with HIV/AIDS. 3)The knowledge of transmission routes of HIV/AIDS influences the attitude of physical therapists to patients with HIV/AIDS. 4)84.9 percentage of participant in survey suggested college should have curriculum about HIV/AIDS. In conclusion, physical therapists with more knowledge of HIV/ADIS could positively treat HIV/AIDS patients.

  • 후천성 면역 결핍 증후군(AIDS, Aquired Immune Deficiency Syndrome)과 사람 면역결핍 바이러스 (HIV, Human Immunodeficiency Virus)관련 질환은 1981년 처음 AIDS가 보고된 이래 전 세계적으로 인류의 건강을 위협하고 있는 공중보건학적 문제이다. 그동안 HIV/AIDS 관련 질환에 대한 치료들은 주로 감염 경로 차단 및 관련된 내과적 질환의 치료에 초점이 맞추어져 왔다. 그러나 AIDS 환자 본인뿐만 아니라 가족, 치료진들은 흔히 HIV 감염으로 인한 심리적인 어려움에서부터 HIV 전파 위험을 높이는 물질 오남용 문제, 우울증, 치매 같은 전형적인 정신과 질환 및 자살에 이르기 까지 다양한 종류의 정신건강 상의 문제들에 자주 직면한다. 이와 같은 신경정신과적 문제들은 치료 결과나 예후, 삶의 질 등에 영향을 미칠 수 있기 때문에 환자의 가족들은 AIDS로 인한 정신과적 질환이나 현상들에 대해 잘 이해하고 적절하게 대처할 필요가 있다.

Inhibitory effects of Korean plant resources on human immunodeficiency virus type 1 protease activity

  • Some Korean plants were found to inhibit HIV-1 protease activity. The extracts of Acanthopanax koreanum (stem bark), Berchemia berchemiaefolia (stem), Berchemia berchemiaefolia (bark), Distylium racemosum (leaves), Distylium racemosum (stem), Lindera erythrocarpa (leaves), Physalis alkekengi var. francheti (root), Platycarya strobilacea (stem), Rodiola rosea (root), Rosa davurica (stem), Syringa dilatata (leaves), Viburnum awabuki (stem) and Viburnum awabuki (leaves) showed significant inhibitory effect against HIV-1 protease. Camelliatannin H from Camellia japonica and uvaol from Cratagus pinatrifida were potent active inhibitors of HIV-1 protease with $IC_{50}$ values of $0.9\;{\mu}M$ and $5.5\;{\mu}M$, respectively. The cure and prevention of AIDS have been a global challenge since it was discovered in the ealy 1980s. However, the development of anti-HIV agent that can effectively treat or prevent this disease are still demanded.

  • Chronic inflammatory demyelinating polyneuropathy (CIDP) is an acquired immune-mediated polyneuropathy. Corticosteroids, intravenous immunoglobulin (IVIG) and plasmapheresis have been reported to be effective treatment. Rarely, CIDP can occur in the patients with HIV infection. The clinical features and electrophysiological findings of CIDP are known to be similar in patients with and without HIV infection. We report a 30-year-old male with HIV infection associated CIDP who improved after the administration of intravenous immunoglobulin and long term oral prednisone.

  • Spontaneous intraspinal and intracranial subdural hematomas (SDHs) are rare, potentially life-threatening complications. We present the case of a 38-yearold highly active antiretroviral therapy-naïve human immunodeficiency virus (HIV)-positive patient who developed simultaneous intraspinal and intracranial SDHs without hemostatic abnormalities. The patient presented with acute lower limb weakness and back pain, and later developed neurological symptoms, including diplopia and incontinence. This case highlights the importance of considering SDHs in HIV-positive patients presenting with neurological symptoms, and underscores the need for prompt diagnosis and multidisciplinary management.

Changing Patterns of Communicable Diseases in Korea (우리나라 전염성 질환의 변화 양상)

  • Before twentieth centuries and during early twentieth centuries, communicable diseases were the major cause of morbidity and mortality in Korea. But reliable data are not available. After 1975, the overall morbidity and mortality from communicable diseases, rapidly declined. Recently many new pathogenic microbes were recognized: L. monocytogenes, Hantaan virus, Y. pseudotuberculosis, P. multocida, L. pneumophilia, Human immunodeficiency virus (HIV), G. seoi, H. capsulatum, C. burnetii, V. cholerae O139, C. parvum, F. tularensis, E. coli O157:H7, B. burgdorferi, S. Typhimurium DT104, Rotavirus, hepatitis C virus and so on. Since the first HIV infection recognized in 1985, the reported cases of infection and deaths from HIV/AIDS have been steady increased each year. Legionnaire's disease, E. coli O157:H7 colitis, listeriosis and crytosporidiasis have been occurring just sporadically among immunocompromized cases. Many re-emerging communicable diseases were occurred in Korea: leptospirosis, malaria, endemic typhus, cholera, tsutsugamushi disease, salmonellosis, hepatitis A, shigellosis, mumps, measles, acute hemorrhagic conjunctivitis, brucellosis and so on. Leptospirosis and tsutsugamushi diseases have been noticed as major public health problems since 1980s. The malaria that had been virtually disappeared for a decade has reappeared from 1993 with striking increase of patients in recent 3-4 years. The distributions of salmonella and shigella serotypes have been changed a lot in recent few decades. Furthermore rapid emergence of antibiotic-resistant bacterial strains induces more difficult and complex problems in control of communicable diseases. We must recognize on the importance of environment and ecosystem conservation and careful prescription of anti-microbial agent in order to prevent communicable diseases.

A Study of Assessing Educational Needs on Program Planning for AIDS(Acquired Immunodeficiency Syndrome) Inservice Education for Clinical Nurses (임상간호사를 위한AIDS(후천성 면역 결핍증) 교육 프로그램 개발을 위한 기초 연구)

  • Nurses are at the forefront of providing hands-on care to patients infected with HIV(Human Immunodeficiency Virus). Therefore, appropriate and sensible infection control precaution should be taken at all times. The purpose of this study is to find out nurse's loaming needs about HIV/AIDS ; and nurse's willingness to perform nursing care to HIV/AIDS patients ; and the degree of fellowing universal precautions in nursing care ; and the degree of knowledge of HIV/AIDS to provide information on the guide for the further continuing education. The respondents of this study were 280 nurses in a university hospital in Kwang-Ju city. Data were collected using a self-administered questionnaire and analyzed by using percentages, means, Pearson's correlation, t-test, ANOVA, Duncan's grouping. The findings were as follows ; 1. The mean age of the nurses was 28.9 years, and the mean duration of their clinical experience was 6.8 years. Majority of the nurses(83.9%) had no experience in taking care of the AIDS patients. 2. The content s nurse's need to learn were showed in order such as clinical manifestations, infection control, patient care, and the mode of transmission of HIV/AIDS. 3. Even though the nurses had quite low knowledge about HIV/AIDS(mean score was 12.2 from the total of 26), they are willing to take care of the AIDS patients (mean score was 39.5 from the total of 60), and the degree of following universal precaution was also not so bad (mean score was 40.8 from the total of 50). 4. Nurses who are over 35 years old and had informal education regarding HIV/AIDS better following universal precautions. Nurses who had over 12 years of clinical experiences have more willingness to perform nursing care to the AIDS patients. Nurses who had experiences in taking care of the AIDS patients were more knowledgeable about HIV/AIDS. These findings suggest that some strategies should be develope to increase the willingness to perform the nursing care to the AIDS patients. And also continuing education program should be develope and run for the clinical nurses so that they can provide effective and appropriate nursing care to the AIDS patients.

Azasugar-Containing Phosphorothioate Oligonucleotide (AZPSON) DBM-2198 Inhibits Human Immunodeficiency Virus Type 1 (HIV-1) Replication by Blocking HIV-1 gp120 without Affecting the V3 Region

  • DBM-2198, a six-membered azasugar nucleotide (6-AZN)-containing phosphorothioate (P = S) oligonucleotide (AZPSON), was described in our previous publication [Lee et al. (2005)] with regard to its antiviral activity against a broad spectrum of HIV-1 variants. This report describes the mechanisms underlying the anti-HIV-1 properties of DBM-2198. The LTR-mediated reporter assay indicated that the anti-HIV-1 activity of DBM-2198 is attributed to an extracellular mode of action rather than intracellular sequence-specific antisense activity. Nevertheless, the antiviral properties of DBM-2198 and other AZPSONs were highly restricted to HIV-1. Unlike other P = S oligonucleotides, DBM-2198 caused no host cell activation upon administration to cultures. HIV-1 that was pre-incubated with DBM-2198 did not show any infectivity towards host cells whereas host cells pre-incubated with DBM-2198 remained susceptible to HIV-1 infection, suggesting that DBM-2198 acts on the virus particle rather than cell surface molecules in the inhibition of HIV-1 infection. Competition assays for binding to HIV-1 envelope protein with anti-gp120 and anti-V3 antibodies revealed that DBM-2198 acts on the viral attachment site of HIV-1 gp120, but not on the V3 region. This report provides a better understanding of the antiviral mechanism of DBM-2198 and may contribute to the development of a potential therapeutic drug against a broad spectrum of HIV-1 variants.

Development and Utilization of an Human Immunodeficiency Virus Type 1 (HIV-1)-Based Retroviral Vector System to Express HIV-1 Envelope Glycoprotein in $CD4^+$ T Cells (HIV-1 Retroviral Vector System의 개발 및 $CD4^+$ T 세포에서 HIV-1 Envelope 당단백질의 발현)

  • 본 연구에서는 $CD4^+$ T 세포를 일회성으로 감염시킬 수 있고 $CD4^+$ T 세포에 HIV-1 envelope 유전자를 전달할 수 있는 바이러스 입자를 생산하는 HIV-1 complementation system을 개발하였고 이 system을 이용하여 $CD4^+$ T 세포에 선택적으로 HIV-1 envelope 당단백질을 발현시켰다. 이 system은 Gag/Gag-Pol expressor와 Env expressor로 구성되어있다. Gag/Gag-Pol expressor는 바이러스 입자 생산에 필요한 구조단백질과 기능단백질을 발현시키지만 packaging signal이 결핍되어 바이러스 입자로 유전자가 들어가지 못하도록 제조되었다. Env expressor는 Tat, Rev와 envelope 당단백질을 발현시키고 packaging signal을 갖고 있어 바이러스 입자로 envelope 유전자가 들어가도록 제조되었다. Gag/Gag-Pol expressor로부터 Gag와 Gag-Pol의 발현은 Rev 단백질을 요구하였고 Env expressor로부터 Rev 단백질 이 제공될 때 Gag와 Gag-Pol 단백질은 효율적으로 발현되었다. Gag/Gag-Pol과 Env expressor로 cotransfection된 COS-1 세포에서 $CD4^+$ T 세포를 일회성으로 감염시킬 수 있는 바이러스 입자가 생산되었다. 생산된 바이러스 입자는 $CD4^+$ T 세포에 HIV-1 envelope 유전자를 전달하여 envelope 당단백질을 발현시켰고 복제 가능한 자손 바이러스의 생성을 유도하지 못하였다. 본 연구에서 개발된 방법은 $CD4^+$ T 세포에서 envelope 당단백질의 기능을 분석하고 관심 있는 유전자를 $CD4^+$ T 세포에 전달하는 바이러스 입자의 생산에 이용할 수 있다.
