온열요법을 결합한 산삼비만약침의 폐경기 복부비만 개선효과: 증례보고 (The Effects of Wild Ginseng Complex Pharmacopuncture Combined with Hyperthermia on Abdominal Obesity in Post-Menopause Women: Case Report)

  • 폐경기 복부비만 환자 2명에게 2주 이내에 총 5회의 온열요법을 결합한 산삼비만약침 치료를 시행한 결과, 복부둘레의 감소를 확인할 수 있었다.

  • The objective of this study is to propose Korean Medicine treatment for recurrent limb weakness after Guillain-Barre syndrome (GBS) improvement by intraveinous immunoglobulin, and to report its effectiveness. Manual muscle test (MMT), Korean modified Bathel index (K-MBI), and tendon reflex were used to evaluate the patient. The patient was improved hip joint, knee joint, ankle joint MMT from grade 3-/3- to grade 5/5 and in the upper limb the patient can do big joint exercise but cannot do micromovement like writing or using cell phone. When discharge date the patient's wrist joint MMT grade is improved grade 5-/5- to grade 5/5. The K-MBI score is improved from 71 to 86 and there was a big change in walking and chair/bed transfer, there was no change in tendon reflex. This study suggests that Korean Medicine can be effective for patients who have recurrent limb weakness after GBS improvement.

Changes In Mechanical Strength of Compression HIP Screws in Relation to Design Variations - A Biomechanical Analysis

  • Compression Hip Screw (CHS) is one of the most widely-used prostheses for the treatment of intertrochanteric fractures because of its strong fixation capability. Fractures at the neck and screw holes are frequently noted as some of its clinical drawbacks, which warrant more in-depth biomechanical analysis on its design variables. The purpose of this study was to evaluate changes in the strength with respect to the changes in design such as the plate thickness and the number of screw holes. Both mechanical test and FEM analysis were used to systematically investigate the sensitivities of the above-mentioned design variables. For the first part of the mechanical test, CHS (n=20) were tested until failure. The CHS specimens were classified into four groups: Group Ⅰ was the control group with the neck thickness of 6-㎜ and 5 screw holes on the side plate, Group Ⅱ 6-㎜ thick and 8 holes, Group Ⅲ 7.5-㎜ thick and 5 holes, and Group Ⅳ 7.5-㎜ thick and 8 holes. Then, the fatigue test was done for each group by imparting 50% and 75% of the failure loads for one million cycles. For the FEM analysis, FE models were made for each group. Appropriate loading and boundary conditions were applied based on the failure test results. Stresses were assessed. Mechanical test results indicated that the failure strength increased dramatically by 80% with thicker plate. However, the strength remained unchanged or decreased slightly despite the increase in number of holes. These results indicated the higher sensitivity of plate thickness to the implant strength. No fatigue failures were observed which suggested the implant could withstand at least one million cycles of fatigue load regardless of the design changes. Our FEM results also supported the above results by showing a similar trend in stress as those of mechanical test. In summary, our biomechanical results were able to show that plate thickness could be a more important variable in design for reinforcing the strength of CHS than the number of screw holes.

대퇴골두 무혈성괴사의 치료에서 전방 회전 절골술의 조기 추시 결과 (The Early Result of Anterior Rotational Osteotomy in the Treatment of Osteonecrosis of the Femoral Head)

  • 1992년 12월부터 1994년 6월까지 영남대학교 의과대학 부속병원 정형외과에 내원한 대퇴골두무혈성 괴사 환자 28명, 31 고관절에 대해 시행한 경전자간 회전 절골술의 결과는 다음과 같다. 1. 대상 환자는 한명을 제외하고는 모두 남자였으며, 평균 연령은 44.3세로 삼, 사십대가 대부분이었다. 2. 무혈성 괴사의 원인은 과다한 음주에 의한 것이 18례로 가장 많았고, 괴사의 정도는 Ficat와 Arlet의 분류에 의하면 II a가 16례로 가장 많았다. 3. 절골술 후 고정 방법으로는 screw가 15례, DHS가 16례였고, 고정 방법에 따른 대퇴경간각의 변화로는, screw를 사용한 경우는 술전에 비해 내반고 되는 경향을 보였고, DHS를 사용한 경우는 외반고 되는 경향을 보였다. 4. 술전 대퇴골두 측면 방사선 사진에서 측정한 대퇴골두 전체 관절면에 대한 건전한 관절면의 비는 1/3이하가 8례, 1/3 이상이 23례(74%)이었다. 5. 술후 고관절의 전후면 방사선 사진에서 측정한 비구의 체중 부하면에 대한 전위된 건전한 대퇴골두의 관절면 비는 36% 이상이 24례(78%), 21-35%가 6례, 20% 이하가 1례였다.

재활원의 물리치료대상 뇌성마비아의 임상적 고찰 (Clinical studies of 38c.p. children in rehabilitation center)

  • The prognosis and result of treatment are influenced by associated cerebral handicaps. 38 cerebral palsied children in rehabilitation center were analized and neuromotor type severity associated defects, and their correlation were studied. The results were follows: 1. neuromotor type distribution was 65.78% of spasticity, 21.05% of athetoid, 2.63% of Ataxia, Tremor, and Rigidity, 5.26% of mixed type. 2. Mean values of I.Q, S.Q and M.Q were 84.3, 63.0 and 26.6 respectively, and percentages of strabismus, speech disturbance, shoulder abnormality, spine abnormality and hip abnormality were 26.3%, 81.5%, 5.2%, 52.6% and 15.9% respectively. 3. Significant differences were noted between each group of mild, moderate, and severe in S. Q, M. Q, and spine abnormality and between moderate and severe group in speech disturbance.

  • 본 연구는 앞서 발표된 '교합이 악구강계 및 전신에 미치는 영향 (I), (II), (III)'에 이어지는 네 번째 논문으로 교합과 악구강계 및 전신 증상과의 관계를 다루고 있는 다섯 가지 주요 이론 중 Myodontics theory 에 관한 내용이다. 이번 논문에서는 Myodontics theory에 대한 기본 개념과 치료방법 및 그 한계에 대하여 정리하였다.

  • Purpose: Obesity is already a major clinical problem in Korea, so obesity prevention and treatment practices are gradually interested in Korean Medicine doctors. Moreover subjective body shape image entered as a important medical topic recently. To make a graph system for clinical tool which based on the Korean national standard physical scale. Methods: Physical data of the girth of upper arm, abdomen, hip, buttock, flank, calf, and body weight were statistically analyzed, which collected through the fifth national standard physical scale research. And then reviewed those statistical validation. Results: Relationship of the each body scale data and those graphs were fit and right statistically. Conclusion: These new achieved graph system of Korean standard body scales could be adopt as a useful clinical tool for body image related patients such as obesity, PCOD and postpartum patient.

  • Synovial chondromatosis is an uncommon condition and involvement of the glenohumeral joint is rare comparing its involvement of knee and hip. We report a case of synovial chondromatosis with its radiography, MRI and microscopic findings and treatment by arthroscopic synovectomy & loose body removal which have been developed in right shoulder of a 46 year-old-female and presented as impingement syndrome.

  • 대퇴골두의 연골 용해증은 관절 연골의 진행성 파괴로 인해, 이차적으로 발생하는 관절 간격의 감소와 강직이 특징이며, 이는 특발성으로 발생하는 경우가 가장 많고, 이차적 원인으로는 감염이나 외상 또는 장기간 고정 등이 거론되고 있다. 저자들은 45세 남자에서 화농성 고관절염 후 발생한 대퇴골두 연골 용해증을 경험하고 관절경하에서 치험하였기에 보고하는 바이다.

초미립 WC-10%Co 초경합금의 물성에 미치는 타탄화물 첨가의 영향 (Effects of the Addition of Other Carbides on the Properties of Submicron WC-10% Co Cemented Carbides)

  • The effects of added VC, Cr$_3$C$_2$ and TaC on the microstructures and properties of submicron WC-10%Co cemented carbides. The relative sintered density of compact was increased by addition of Cr$_3$C$_2$ but decreased oppositely by addition of VC or TaC. The growth of WC grains was significantly suppressed by addition of these carbides. The hardness of these alloys was increased by addition of other carbides and showed a maximum value by simultaneously added VC and Cr$_3$C$_2$. The transverse rupture strength(T.R.S.) was in- creased by addition of Cr$_3$C$_2$, while it was decreased by addition of VC or TaC. The relative sintered density and T.R.S. of these alloys were improved by HIP-treatment. The maximum T.R.S. was 328kg/mm$^2$ in the Wc-10%Co cemented carbide with addition of 0.5%VC.
