Autonomic Neurocardiac Function in Patients with Major Depressive Disorder - Using a Heart Rate Variability Test Battery - (주요우울장애 환자에서의 자율신경심장기능 - Heart Rate Variability 검사를 사용하여 -)

  • Objectives: Major depression is associated with an increased risk of cardiovascular mortality. One possible explanation for this association is that major depression influences autonomic neurocardiac regulation. However, previous studies on the relationship between heart rate variability (HRV) and major depression have revealed conflicting results. The purpose of this study is to clarify that major depressive patients compared to healthy controls show a reduction in HRV as an expression of reduced modulation of vagal activity to the heart. Methods: According to DSM-IV, the time and frequency domain HRV indices (5-min resting study) of 30 patients with major depressive disorder were compared with those of 30 healthy controls. Standardized HRV tests enable quantitative estimation of autonomic nervous system function. Results: After controlling for age and gender, subjects with major depression showed a higher heart rate and significantly lower modulation of cardiovagal activity compared to controls. The total power (TP) band, very low frequency (VLF: 0.003-0.04 Hz) band, low frequency (LF: 0.04-0.15 Hz) band, and high frequency (HF: 0.15-0.4 Hz) band were significantly reduced in subjects with major depression compared to control subjects. Conclusion: Patients with major depression may suffer from functional disturbances in the interaction between the sympathetic and parasympathetic autonomic systems.

The Application of the Measurement of Heart Rate and Velocity during Training to Assess Racing Performance in Thoroughbred Horses (더러브렛 경주마에서 운동능력 평가를 위한 훈련 중 심박수 및 속도측정 수치 활용방안 연구)

  • This study was performed to apply the measurement of heart rate and velocity in training horses for assessing race performance. Additionally, we aimed to identify parameters that can be used to evaluate the training level and exercise capacity. Eleven healthy 2- to 6-year-old Thoroughbreds were trained by the standard training program and heart rate and velocity were measured by using heart monitoring system and GPS. Regression analysis in heart rate and velocity data was performed to calculate velocity parameters. The mean maximal heart rate in gallop was $214{\pm}11bpm$. The mean $V_{140}$, $V_{180}$, $V_{200}$ and $VHR_{max}$ were $13.8{\pm}4.3km/h$, $37.5{\pm}3.8km/h$, $49.3{\pm}4.3km/h$ and $57.4{\pm}7.1km/h$ respectively. The mean $V_{140}$ of high performance racehorses was significantly higher than that of low performance racehorses (P < 0.05). Moreover, analyzing the correlation between velocity parameters and racing ability-related categories showed that $V_{140}$ was positively correlated with rating (P < 0.05), $V_{180}$ and $VHR_{max}$ were positively correlated with prize money per race (P < 0.05). Also, $V_{140}$ was significantly correlated with G1F (P < 0.05). The results of this study have shown that the measurement of heart rate and velocity during training could be useful methods to assess fitness for races or performance potential. Especially, $V_{140}$ is a good parameter to evaluate a performance of racehorses in Korea.

The Clinical Study of 1500 Cases in Open Heart Surgery (개심술1,500례의 임상적 고찰)

  • From May 1977 to November 1993, 1, 500 cases of open heart surgery were performed under the cardiopulmonary bypass. In 1500 cases of open heart surgery 975 cases[65%] were congenital heart disease and 525 cases[35%] were acquired heart disease. In 975 cases of congenital heart disease, there were 812 cases[83%] of acyanotic congenital heart anomaly and 163 cases of cyanotic congenital anomaly. In 525 cases of acquired heart disease, most cases were valvular heart disease in which 557 valves were implanted. The each operative mortality of congenital and acquired disease was 7.7% and 7.4% respectively and then overall mortality rate was 7.6%. There has been increasing incidence of open heart surgery in infants in recent years. After 1990, 28 cases of infant open heart surgery below the 12month were performed. Most lowest weight was 2, 200g[15 days]. Operative mortality of open heart surgery in infant with weight below the 10kg was very high as a 25.7%.

  • The purpose of this study was to examine theory about indoor thermal comfort-environment as well as to determine thermal sensation and physical responses for men in summer and winter indoor environment, under various air temperature and relative humidity, with male university students. Subjective Evaluation, Heart rate(Electrocardiogram, ECG), Electroencephalo gram(EEG) were examined. We found that comfort of people was achieved at SET*. 24.7$^{\circ}C$, -0.82${\alpha}$-wave) both in summer and in winter.

  • This experiment was performed to investigate the electrocardiographic changes in experimentally induced hypocalcemia and hypercalcemia in Korean black goats by dosing with 5% disodiumethylene diamine tetraacetic acid at 0.07$m\ell$/kg body weight/min and 10% Ca-borog-luconate at 0.075 $m\ell$/kg body weight/min, respectively. the result were summarized as follows: Heart rate, S-T segment and Q-Tc interval at 3.23 ${\pm}$ 0.10mEq/L plasma calcium level(hypocalcemia) were increased to 100${\pm}$10.5 rate/min, 132 ${\pm}$10msec and 510${\pm}$40msec, respectively. Heart rate, S-T segment and Q-T interval at 6.89${\pm}$0.23mEq/L plasma calcium level(hypercalcemia) were decreased to 73.2${\pm}$5.16 rate/min, 87${\pm}$10msec and 372${\pm}$30msec, respectively. The degree of changes of the heart rate, S-T segment and Q-Tc interval at low plasma calcium level was higher than those at high plasma calcium level.

Effect of Static Recovery and Dynamic Recovery on the Cardiopulmonary Variables, Lower Extremity Muscle Activity after Progressive Resistance Exercise to Maximal Point

  • This study was to examine on the respiratory variables, heart rate and muscle activity between the static recovery and dynamic recovery after progressive resistance exercise to maximal point. Subjects were 15 students enrolled in N University. All were tested two times (static recovery and dynamic recovery) and were requested to perform a walking on a treadmill after progressive resistance exercise to maximal point. Electromyography(EMG) was used to monitor the muscle activity(TA: Tibialis Anterior, MG: Medial Gastrocnemius) during gait. CPEX-1 was used to measure the respiratory variables and heart rate. The dynamic recovery group was shown the significant lower heart rate than that of static recovery group at during gait. Respiratory rate showed statistically a significant difference. Electromyography(RMS, root mean square) showed a non-significant difference. But the dynamic recovery group of muscle activity was found highly in TA and MG. This study indicated that the dynamic recovery method evidenced more faster than the static recovery method. And this type of dynamic rest by walking can be a help of recovery after exercise.

Palliative surgery for cyanotic heart disease: 102 cases (선천성 청색증 심장병에 대한 고식적 수술: 102례 보)

  • From Jan. 1962 to Aug. 1983, one hundred patients with cyanotic heart disease underwent various palliative operations at the department of thoracic and cardiovascular surgery, S.N.U.H. In the period from Jan. 1962 to Dec. 1973, in which the open heart surgery was not routinely performed, sixty-two operations including 2 cases of second shunt operation were performed in sixty patients, and all of them were tetralogy of Fallot except three cases. Various palliative procedures such as Glenn, Brock, Waterston and Blalock-Taussing operation were used in this period with overall mortality rate of 16%. In the period from Jan. 1980 to Aug. 1983, forty patients with cyanotic heart disease were operated and majority of them was complex anomalies. Only Blalock-Taussing operation was used in this period with a mortality rate of 20%. These two groups of patients were compared according to age, diagnosis and results of operations, and it appears that Blalock-Taussing operation is effective palliation for patients with cyanotic heart disease, especially with complex anomaly, with an acceptable mortality.

  • Delayed cardiac tamponade in an uncommon and frequently fatal complication after open-heart surgery. We had been experienced two cases of delayed cardiac tamponade as a complication of open-heart surgery and treated successfully by reinsertion of pericardial drain through subxiphoid route. First case was 60 years old female patient and underwent MVR under impression of MSi + Ti Second case was 19 years old male patient and underwent total correction of T.O.F.with Blalock shunt [Lt]. Both cases had Initial symptoms, which were epigastric pain, chest tightness, dropped blood pressure, and increased pulse rate and respiratory rate, mimic as low cardiac output syndrome after open-heart surgery. Roentgenogram of the chest showed a rapid increased cardiothoracic ratio. It is important to realize the presence of late cardiac tamponade for proper diagnosis of complication after open-heart surgery.

Clinical Analysis of Open Heart Surgery - Review of 477 cases - (개심술 477예에 대한 임상적 고찰)

  • Between April, 1986 and July, 1991, 477 patients underwent open heart surgery with hypothermic cardiopulmonary bypass. There were 242 patients [50.7%] of acyanotic congenital anomalies, 34 patients [7.1%] of cyanotic congenital anomalies, and 187 patients [39.2%] of acquired heart diseases, 8 patients [1.7%] of coronary artery diseases, and 6 patients [1.3%] of mixed anomalies. Among the 276 congenital cardiac anomalies, 147 patients [53.3%] were male and 129 patients [46.7%] were female, ranged in age from 2 years to 58 years. Among the 187 acquired heart diseases, 72 patients [38.5%] were male and 115 patients [61.5%] were female, ranged in age from 10 years to 68 years. The common congenital defect were VSD and ASD in acyanotic cardiac patients, and TOF in cyanotic cardiac patients. Among the 187 acquired heart diseases, 180 patients underwent operation for cardiac valvular diseases, 4 patients were resected left atrial myxoma, and 3 patients underwent operation for aortic regurgitation with ascending aortic aneurysm. The operative mortality rate was 1.2% in acyanotic cardiac patients, 11.8% in cyanotic cardiac patients, and 6.9% in acquired cardiac patients, with overall mortality rate 4.2%.

  • Of all pesticides, carbamates are known to be most common, since alternatives such as organophosphates have long lifetime and are extremely toxic to produce a delayed neurotoxic effect. Although a number of studies about toxicity of carbofuran, a most widely used carbamate, have been reported, its cardiovascular toxicity has not yet been studied. In the present study, we investigated its cardiovascular toxic effect in anesthetized rat in vivo and in isolated Langendorff rat heart, In anesthetized rat model, carbofuran (10 mg/kg) significantly reduced heart rate, and transiently increased blood pressure. In isolated rat heart, carbofuran (10${\mu}{\textrm}{m}$) caused a significant depression in the left ventricular developed pressure (LVDP), indicating contractile dysfunction by carbofuran. Carbofuran (10${\mu}{\textrm}{m}$) also decreased coronary flow rate (CFR) in isolated heart, indicating carbofuran-induced coronary dysfunction. These results suggest that carbofuran can cause cardiac dysfunction in rat in vivo and vitro.
