정상인의 한냉자극에 대한 심박변이도 및 도플러 초음파 혈류계를 이용한 말초 혈류관찰 (Clinical Observation on Heart Rate Variability and Peripheral Blood Flow by Doppler Ultrasound in Healthy Subjects Stimulated by Cold Stress)

  • Objectives: This study was performed to investigate the effect of cold stress on heart rate variability and peripheral blood flow by Doppler ultrasound in healthy subjects. Methods: We classified the subjects by their Yin or Yang tendency using a Yin-Yang questionnaire. Cold stress was taken on the left hand of subjects for 10 seconds. Heart rate variability and peripheral blood flow were measured on the right radial artery before and after cold stress. Results: The Yang tendency group showed a significant decrease of heart rate after cold stress which was not seen in the Yin tendency group. The Yang tendency group showed a decrease of LF, total power, and increase of HF after cold stress, while the Yin tendency group showed the opposite. However, there were no statistically significant differences of heart rate variability analysis between the results of the two groups before and after cold stress. Recovery time of blood flow velocity by ultrasound Doppler was significantly different between the Yin and Yang tendency groups after cold stress. Conclusion: These results suggest that people with Yin or Yang tendency may show different responses in the autonomic nervous system. Further modified studies may include the responses to various manipulation techniques in acupuncture treatment and the individual responses according to pattern identification in traditional Korean medicine.

출혈성 쇼크 흰쥐 모델에서 경혈 지압이 혈압과 심박수 변화에 미치는 영향 (A Study for Changes of Blood Pressure and Heart Rate by Acupressure to Acupoints in Rat Model of Hemorrhagic Shock)

  • Objectives : In order to find a possible non-invasive manipulation tool for maintenance of the cardiovascular functions in hemorrhagic shock, this study was aimed at evaluating effects of acupoints acupressure on the changes in blood pressure and heart rate from an animal model of hemorrhagic shock. Methods : In adult Sprague-Dawley rats, hemorrhagic shock was induced by a withdrawal of arterial blood from the femoral artery with volume of 0.8 ml per 100 g of body weight using peristaltic syringe pump. We applied the acupressure with a pressure oscillator to tail as a control and 2 different acupoints of sobu(HT8), youngchun(KI1) under 3 different conditions : 1) normal arterial blood pressure without bleeding, 2) at the beginning of bleeding, and finally 3) hemorrhagic shock. Results : Under normal arterial blood pressure without hemorrhage, there was a significant increase in systolic and diastolic blood pressures by the acupressure to the tail, HT8 and especially KI1 for 30 sec compared with before acupressure. Under hemorrhagic shock condition, the tail acupressure had minimal changes in cardiovascular parameters. Either the HT8 or KI1 acupressure resulted in a significant increase in arterial pressure but did not heart rate. At the beginning of bleeding, tail acupressure failed to change the reduction of arterial pressure and heart rate. However, there was a significant increase in blood pressure and heart rate following either the HT8 or especially KI1 acupressure. Conclusions : HT8 and KI1 acupressure affected cardiovascular signs but tail acupressure did not in rat model of hemorrhagic shock. These experimental data suggest that a acupressure with a pressure oscillator to HT8 or KI1 can be one of alternative emergency manipulations to ameliorate compromised cardiovascular functions under hemorrhagic shock condition.

Using Shoulder Straps Decreases Heart Rate Variability and Salivary Cortisol Concentration in Swedish Ambulance Personnel

  • Background: Previous research has shown that paramedics are exposed to risks in the form of injuries to the musculoskeletal system. In addition, there are studies showing that they are also at increased risk of cardiovascular disease, cancer, and psychiatric diseases, which can partly be explained by their constant exposure to stress. The aim of this study is to evaluate whether the use of shoulder straps decreases physical effort in the form of decreased heart rate and cortisol concentration. Methods: A stretcher with a dummy was carried by 20 participants for 400 m on two occasions, one with and one without the shoulder straps. Heart rate was monitored continuously and cortisol samples were taken at intervals of 0 minutes, 15 minutes, 30 minutes, 45 minutes, and 60 minutes. Each participant was her or his own control. Results: A significant decrease in heart rate and cortisol concentration was seen when shoulder straps were used. The median values for men (with shoulder straps) at 0 minutes was 78 bpm/21.1 nmol/L (heart rate/cortisol concentration), at 15 minutes was 85 bpm/16.9 nmol/L, and at 60 minutes was 76 bpm/15.7 nmol/L; for men without shoulder straps, these values were 78 bpm/21.9 nmol/L, 93 bpm/21.9 nmol/L, and 73 bpm/20.5 nmol/L. For women, the values were 85 bpm/23.3 nmol/L, 92 bpm/20.8 nmol/L, and 70 bpm/18.4 nmol/L and 84 bpm/32.4 nmol/L, 100 bpm/32.5 nmol/L, and 75 bpm/25.2 nmol/L, respectively. Conclusion: The use of shoulder straps decreases measurable physical stress and should therefore be implemented when heavy equipment or a stretcher needs to be carried. An easy way to ensure that staff use these or similar lifting aids is to provide them with personalized, well-adapted shoulder straps. Another better option would be to routinely sewn these straps into the staff's personal alarm jackets so they are always in place and ready to be used.

불면증이 동반된 여성 갱년기 환자의 심박변이도 특성 분석 : 후향적 차트리뷰 (Analysis on Heart Rate Variability (HRV) Characteristics of Patients with Insomnia during Perimenopause and Postmenopause: A Retrospective Chart Review)

  • Objectives: The purpose of this study is to demonstrate Heart Rate Variability characteristics of menopausal patients with insomnia. Methods: From March 1, 2014 to June 20, 2017, Heart Rate Variability was measured in 102 menopausal patients who visited Cheonan Korean Medicine Hospital of Daejeon University. We compared accompanying symptoms and Heart Rate Variability values depending on sleep quality in menopausal women. Results: The accompanying symptoms of menopausal patients were as follows: hot flushes (45.1%), tiredness (25.49%), chest discomfort and palpitations (23.53%), headache (17.65%), arthralgia and muscular pain (17.65%), cold sensitivity of hands and feet (15.69%), urinary frequency (14.71%) and anxiety (10.78%). The frequency of chest discomfort and palpitation was significantly higher in the menopausal insomnia group than in normal sleep group. Comparing Heart Rate Variability between two groups, Standard deviation of the NN interval (SDNN), Total Power (TP), and Low Frequency (LF) values were significantly lower in insomnia group. Conclusions: Chest discomfort and palpitations were more frequent in insomnia patients in menopausal women than normal sleep group, and Standard deviation of the NN interval (SDNN), Total Power (TP), Low Frequency (LF) were significantly lower in HRV values.

심박수 측정을 위한 안면 얼굴 영상 데이터 수집 시스템 설계 (Design of Facial Image Data Collection System for Heart Rate Measurement)

  • 본 논문은 심박수 측정을 위한 안면 얼굴 영상 데이터 수집 시스템을 설계한다. 본 논문의 설계 내용은 웹 카메라를 이용하여 사용자 얼굴 영상 정보들을 수집하고, 수집된 사용자 얼굴 영상 정보들을 이용하여 심박수를 측정하는 기능이다. 웹 카메라를 이용한 비접촉식 심박수 측정으로 인하여 오차가 발생할 가능성이 있다. 따라서 본 논문에서는 심박수 측정시 얼굴 영상 데이터 분류를 통해서 오차가 발생한 경우와 정상적인 경우를 구별하여 심박수 프로그램 오차 수정에 이용할 수 있도록 하고자 한다. 오차가 발생된 경우의 자료를 이용하여 오차를 줄이기 위한 목적으로 사용할 수 있도록 한다. 본 논문에서 제안하고 설계한 내용에 대해서 실험을 수행하였다. 실험 결과 본 논문에서 설계한 내용이 정상적으로 동작됨을 확인할 수 있었다.

Cardiac Activation Associated with Non-Periodic Leg Movements in Comparison to Periodic Leg Movements during Sleep in Patients with Restless Legs Syndrome and Healthy Subjects

  • Objectives: Periodic leg movements in sleep (PLMS) are associated with arousals and autonomic activation, which may contribute to higher cardiovascular disease risk in patients with restless legs syndrome (RLS). Non-periodic leg movements in sleep (NPLM) are leg jerks in sleep that does not satisfy standard criteria of PLMS. The aim of this study was to evaluate impact of short-interval leg movements in sleep (SILMS) and isolated leg movements in sleep (ILMS) in comparison to PLMS on heart rate in both patients with RLS and healthy controls. Methods: Seven idiopathic RLS patients and 9 controls were enrolled in this study. Polysomnographic studies were analyzed and leg movements (LM) were automatically detected. NPLM can be classified as SILMS and ILMS. SILMS are LM separated by an inter-movement interval (IMI) shorter than 10 s, and ILMS are LM with IMI longer than 90 s. Frequency and heart rate associated with SILMS, ILMS, and PLMS in RLS patients were compared to those in controls. Heart rate change associated with LM were determined for a fixed time window. Results: Frequencies of SILMS and ILMS of patients with RLS were not significantly different to those of controls. RLS patients presented higher heart rate change associated with SILMS than PLMS before movement onset, while heart rate change associated with SILMS, ILMS, and PLMS were not different in the controls. Conclusions: Although the number of SILMS is not higher than PLMS, SILMS may have closely associated with higher cardiac activation of RLS than PLMS. Therefore, SILMS might be an important treatment target for patients with RLS to reduce long-term cardiovascular risk. Long-term prospective studies are needed to evaluate the relationship between NPLM and cardiovascular disease in patients with RLS.

  • 딥러닝의 발전은 의료 분야에서도 다양한 응용을 가능하게 하고 있으며 이러한 애플리케이션 중에 심박수 측정은 개인의 건강을 관리하기 위한 필수적인 아이템이라 할 수 있다. 광혈류 측정을 이용한 기존 방법의 경우 스마트워치 같은 장비의 착용이 필수적이다. 그러나 최근 딥러닝 기술의 발전은 비침습식으로 원격에서 사용자의 얼굴 이미지를 분석하여 심박수를 높은 성능으로 측정가능하게 한다. 본 연구에서는 모바일 환경에서 사용 가능한 경량화된 심박수 추정 방법론을 제안한다. 이 방법론은 2D 컨볼루션에 기반한 특화된 2채널 네트워크 구조를 사용하여, 혈류와 근육 수축으로 인한 얼굴의 미세한 움직임과 색상 변화를 고려한다. 제안하는 네트워크 구조는 이미지 특성을 분석하는 인코더와 혈류량 파동을 예측하는 회귀 레이어로 구성되어있다. 이러한 복합적인 특성을 동시에 분석함으로써, 제한된 컴퓨팅 리소스를 가진 환경에서도 심박수를 정확하게 추정할 수 있다. 이 연구의 접근 방식은 침습적인 기술 없이도 심박수를 효과적으로 모니터링 할 수 있는 새로운 경로를 제공할 것으로 예상한다.

Optical Stimulation and Pacing of the Embryonic Chicken Heart via Thulium Laser Irradiation

  • Optical stimulation provides a promising alternative to electrical stimulation to selectively modulate tissue. However, developing noninvasive techniques to directly stimulate excitable tissue without introducing genetic modifications and minimizing cellular stress remains an ongoing challenge. Infrared (IR) light has been used to achieve optical pacing for electrophysiological studies in embryonic quail and mammalian hearts. Here, we demonstrate optical stimulation and pacing of the embryonic chicken heart using a pulsed infrared thulium laser with a wavelength of 1927 nm. By recording stereomicroscope outputs and quantifying heart rates and movements through video processing, we found that heart rate increases instantly following irradiation with a large spot size and high radiant exposure. Targeting the atrium using a smaller spot size and lower radiant exposure achieved pacing, as the heart rate synchronized with the laser to 2 Hz. This study demonstrates the viability of using the 1927 nm thulium laser for cardiac stimulation and optical pacing, expanding the optical parameters and IR lasers that can be used to modulate cardiac dynamics.

The Influence of Cardiovascular system caused by warming effect of Far-infrared radiation

  • As a result of using a heat generator to experiment the physiological influence of the human body due to the warming effect of far-infrared radiation (FIR), the blood pressure of the subjects lowered and stabilized due the expansion of capillary vessels and salt discharge during perspiration as the temperature of the generator elevated($30{\sim}65^{\circ}C$). In case of heart rate, it decreased and stabilized when the temperature of the ‘far-infrared radiation heat generator’ was at a low temperature below $40^{\circ}C$. At a high temperature above $44^{\circ}C$, there was a slow elevation in the heart rate. However, the elevation of the heart rate is not a sudden elevation, therefore, does not give much stress to the heart.

유니버설 패션에 기반한 시니어 심박측정 의류 디자인 연구 (Research on Heart Rate Sensing Clothing Design for Seniors Based on Universal Fashion)

  • The number of elderly citizens has risen in Korea and resulted in an aging society. Correspondingly, the social interest in the aging population has escalated immensely; however, research or product development on the quality of life for seniors has shortcomings. Healthcare smart clothing is required to help the elderly with changes and weaknesses that follow aging; however, there is unfortunately insufficient amounts available. This study explores the feasibilities of smart clothing for seniors based on a universal design. Based on previous research, we analyzed the universal design theory, body shape characteristics and design requirements for seniors, and heart rate measurement method. The design is different according to body shape and body shape is different between sex, age, and body race; therefore, subjects were limited to 70-74 year old Korean males in this study. This study proposes a guideline for heart rate sensing clothing that satisfies the 'universal design' aspects as well as the functionality of heart sensing, senior's physical characteristics and needs. It has broadened the range of smart clothing, which was once limited to the younger generation and provided a foundation for the development of specialized smart clothing for seniors.
