Modulation of Drug Resistance in Ovarian Cancer Cells by Inhibition of Protein Kinase C-alpha (PKC-α) with Small Interference RNA (siRNA) Agents

  • Objective: To determine whether silence of $PKC-{\alpha}$ expression by small interference RNA (siRNA) might regulate MDR1 expression and reverse chemoresistance of ovarian cancer. Methods: We measured gene and protein expression of MDR1 and $PKC-{\alpha}$ in ovarian cancer cells and assessed their correlation with cell drug resistance. We also examined whether blocking $PKC-{\alpha}$ by RNA interference (RNAi) affected MDR1 expression and reversed drug resistance in drug sensitivity tests. Results: The drug resistance cell lines, OV1228/DDP and OV1228/Taxol, had higher gene and protein expression of MDR1 and $PKC-{\alpha}$ than their counterpart sensitive cell line, OV1228. SiRNA depressed $PKC-{\alpha}$ gene protein expression, as well as MDR1 and protein expression and improved the drug sensitivity in OV1228/DDP and OV1228/Taxol cells. Conclusion: These results indicated that decreasing $PKC-{\alpha}$ expression with siRNA might be an effective method to improve drug sensitivity in drug resistant cells with elevated levels of $PKC-{\alpha}$ and MDR1. A new siRNA-based therapeutic strategy targeting $PKC-{\alpha}$ gene could be designed to overcome the chemoresistance of ovarian cancer.

Mechanistic Analysis of Taxol-induced Multidrug Resistance in an Ovarian Cancer Cell Line

  • Objectives: To establish a taxol-resistant cell line of human ovarian carcinoma (A2780/Taxol) and investigate its biological features. Methods: The drug-resistant cell line (A2780/Taxol) was established by continuous stepwise selection with increasing concentrations of Taxol. Cell morphology was assessed by microscopy and growth curves were generated with in vitro and in vivo tumor xenograft models. With rhodamine123 (Rh123) assays, cell cycle distribution and the apoptotic rate were analyzed by flow cytometry (FCM). Drug resistance-related and signal associated proteins, including P-gp, MRPs, caveolin-1, PKC-${\alpha}$, Akt, ERK1/2, were detected by Western blotting. Results: A2780/Taxol cells were established with stable resistance to taxol. The drug resistance index (RI) was 430.7. Cross-resistance to other drugs was also shown, but there was no significant change to radioresistance. Compared with parental cells, A2780/Taxol cells were significantly heteromorphous, with a significant delay in population doubling time and reduced uptake of Rh123 (p<0.01). In vivo, tumor take by A2780 cells was 80%, and tumor volume increased gradually. In contrast, with A2780/Taxol cells in xenograft models there was no tumor development. FCM analysis revealed that A2780/Taxol cells had a higher percentage of G0/G1 and lower S phase, but no changes of G2 phase and the apoptosis rate. Expression of P-gp, MRP1, MRP2, BCRP, LRP, caveolin-1, PKC-${\alpha}$, Phospho-ERK1/2 and Phospho-JNK protein was significantly up-regulated, while Akt and p38 MARK protein expression was not changed in A2780/Taxol cells. Conclusion: The A2780/Taxol cell line is an ideal model to investigate the mechanism of muti-drug resistance related to overexpression of drug-resistance associated proteins and activation of the PKC-${\alpha}/ERK$ (JNK) signaling pathway.

Clinical Effectiveness of Cefoperazone(Cefobid) in Obstetrics-Gynecological Infection (부인과감염증(婦人科感染症)에 대(對)한 Cefoperazone(Cefobid)의 임상효과(臨床效果))

  • A total of 30 cases of gynecological infection were treated with Cefoperazone at the Department of Ob-Gy, Seoul Baik Hospital, Seoul, Korea from Sept. 1979 to Aug. 1980. Cefoperazone sodium was administered in a dose of 2g/day intramusculary or intravenously for 6 to 13 days. The effective rates of Cefoperazone based on clinical and bacteriological response were 100% in 9 cases of acute non-gonorrheal PID, 5 cases of postpartum endometritis, 2 cases of recurrent non-gonorrheal PID and 80% in acute gonorrheal PID(8 out of 10 cases). A case of recurrent gonorrheal PID and 2 out of 3 cases of postoperative infection responded also satisfactorily to Cefoperazone treatment. There were no drug-related abnormal findings in urinalysis, hematology and blood chemistry. Any particular side effects, except for one case of fever and rash which disappeared spontaneously without any treatment or discontinuation of the drug, were not noted during or after the treatment.

Analysis of Nursing Interventions Performed by Gynecological Nursing Unit Nurses Using the Nursing Interventions Classification (간호중재분류 (NIC)에 근거한 부인과 간호단위의 간호중재 분석)

  • Purpose: The purpose of this study was to identify nursing intervention performed by nurses on gynecological nursing units. Methods: The instrument in this study is based on the fifth edition of Nursing Interventions Classification (NIC) (2008). Data was collected by Electronic Medical record from August, 2010 to October, 2010 at one hospital and analyzed by using frequencies in the Microsoft Excel 2010 program. Results: Of a total of 82 NIC, domains of the nursing interventions showed higher percentages for physiological: basic (36.3%) and physiological: complex (34.5%). The classes of nursing interventions showed higher percentage for health system medication (12.1%), perioperative care (10.0%), and drug management (8.6%). The most frequently used top interventions were Discharge Planning. The thirty least used interventions was environmental management. Top thirty most frequently used interventions belonged to the domain of physiological: basic (37.9%), physiological: complex (31.1%), and behavioral (5.4%). Conclusion: These findings will help in the establishment of a standardized language for gynecological nursing units and enhance the quality of nursing care.

A Study of Women's Health and Disease through Advertisements in Dong-a Newspaper between 1920 and 1945 (1920년~1945년까지의 동아일보 광고를 통해 본 여성의 건강과 질병)

  • In this study, we investigated advertisements in Dong-a newspaper about gynecological drugs between 1920 and 1945 and analyzed the awareness of women's health and disease during the Japanese Colonial Rule. The advertisements included leukorrhea, oligomenorrhea, dysmenorrhea, feeling of cold, sterility, hysteria, and sexually transmitted infection and identified inattentive menstruation, improper intercourse, poor pre- and post-natal care as the cause for illness. This study includes the following limitations; it only analysed advertisements from Dong-a newspaper, and most of the drugs were manufactured in Japan and might not accurately reflect the health and disease of Korean women. Suggested future studies may include analysis of institute magazine and Japanese news advertisements during this period.

  • Immunotherapy is an effective treatment option for gynecological malignancies. Radiologists dealing with gynecological patients undergoing treatment with immune checkpoint inhibitors should be aware of unconventional immune-related imaging features for the evaluation of tumor response and immune-related adverse events. In this paper, immune checkpoint inhibitors used for gynecological malignancies and their mechanisms of action are briefly presented. In the second part, patterns of pseudoprogression are illustrated, and different forms of immune-related adverse events are discussed.

Herbal Drugs through Chosun Maiyak Corporation 《朝鮮賣藥株式會社》in Japanese Colonial period (조선매약주식회사를 통해 본 일제강점기 한약의 모습)

  • Objectives l: This study is to investigate the emergence of herbal medicine during Japanese colonial period by reviewing newspaper advertisements and brochures about patient medicine of Chosun Maiyak Corporation. Methods : 69 kinds of patent medications and 584 newspaper advertisements are throughly reviewed to investigate dosage form, drug effect, sales unit and method, price, and advertisement pattern. Results : Lyungsinwhan represented more than half of the total advertisements, followed by Yungmihwan, Sahyangsohabwon, Yeollyeonggobondan, and Chiljehyangbuhwan. Lyungsinwhan was advertised mostly in spring and summer whereas Yungmihwan and Yeollyeonggobondan were advertised in spring and fall. Dosage form included pill(丸), mixture(膏), powder(散), and liquid(水, 液), while pill and mixture prevailed over other forms. Drug effect included dermatological, pediatric, digestive, and gynecological effects in the increasing order of drug numbers. Sales unit and price were found to vary significantly. Sales method included nationwide network distribution of medicine through postcards and stores. Conclusion : Herbal medicine during the colonial period was distributed in various kinds of forms with different effects and advertisement patterns through diverse sales unit and price.

A clinical analysis of patients that used herbal medicine in pregnancy and retrospective survey of patient's treatment satisfaction (임신 중 치료목적으로 한약을 복용한 환자에 대한 실태분석 및 치료에 대한 만족도 조사)

  • Purpose : This study was designed to establish safety and efficacy of using herbal medicine in pregnancy and to investigate patient's treatment satisfaction. Methods : We investigated general pregnancy outcomes of twenty-five gynecological outpatients who visited Dongguk univ. Kangnam Oriental Hospital, from July 1, 2002 through December 31, 2004 and have taken herbal medicine in pregnancy. Additionally we investigated newborn's growth status and patient's treatment satisfaction. Results : The rate of normal delivery of patients used herbal medicine for diseases that possibility of abortion is not high was 100% and there was no malformation in normally delivered 21 cases. Most of Newborn's growth status was normal and the average score of patient's satisfaction was 3.5(absolutely satisfied=5). Conclusion : The results of this study show safety and efficacy of using herbal medicine in pregnancy. And In the future, It is thought that more exactly constructed clinical survey need to be carried out.

Patient-Controlled Analgesia(PCA) for Pain Management after Gynecological Surgery (Baxter $Infusor^{(R)}$를 이용한 부인과 수술후의 통증 자가 조절)

  • The use of intravenous patient-controlled analgesia is an effective and increasingly used means of providing postoperative pain relief. Recently a non-electric, disposable and portable infusor, the Baxter $Infusor^{(R)}$, has developed. This delivers not only a continuous drug infusion but can also deliver extradoses of medication on demand. The present study examined the benefits of two kinds of analgesics for pain management in 28 patients undergoing gynecological surgery. One group, 14 patients, received i.v. meperidine 0.5 mg/kg as loading dose in the recovery room and PCA with meperidine 3 mg/kg/day for 3 days only(M group). In the other group, 14 patients, also received i.v. meperidine 0.5 mg/kg as loading dose in the recovery room and PCA meperidine 3 mg/kg/day for 3 days and droperidol 5 mg(MD group). The PCA device used was the Baxter $Infusor^{(R)}$. This unit was fitted with patient control module which had a flow rate 0.5 ml/hr and the lockout time was 15 min. Resulting from the study, the MD patients in the first and second days post-operatively, reported less pain compared with the M group. VAPS(Visual Analogue Pain Scales) values were $3.52{\pm}l.61$ vs. $2.22{\pm}0.69$, $2.38{\pm}1.12$ vs. $1.45{\pm}0.48$ and $1.93{\pm}1.65$ vs. $0.98{\pm}0.36$, respectively pertaining to M and MD groups. In conclusion, the MD group with meperidine and droperidol(mixed regimen) provided more effective postoperative analgesia than M group(meperidine only).

A Retrospective Study of Patients that Used Herbal Medicine During Pregnancy (임신 중 한약 복용 환자에 대한 후향적 연구)

  • Objectives: This study was to establish safety and efficacy of using herbal medicine during pregnancy and to investigate patient's satisfaction. Methods: We investigated general pregnancy outcome, birth history, newborn infant's physical condition and patient's satisfaction. Survey respondents were 54 gynecological outpatients who visited Daejeon University Dunsan Oriental hospital from January 1, 2011 through February 28, 2013. Results: A total of fifty one cases maintained pregnancy and 3 cases had miscarriages. Forty seven cases delivered normally, 4 cases gave birth prematurely. There was no congenital malformation due to using herbal medicine and most of newborn babies had on the average weight and height and were healthy condition. The average score of patient's satisfaction was 3.44 point (Excellent=5). Conclusions: This study presents safety and efficacy of using herbal medicine during pregnancy.
