Spontaneous Thoracic Epidural Hematoma - Case Report - (흉추부에 발생한 자발성 척수 경막외 혈종 - 증 례 보 고 -)

  • Spontaneous spinal epidural hematoma, an idiopathic accumulation of blood in the vertebral epidural space without identifiable predisposing factors, is a rare condition. The diagnosis can be made from a careful history and neurological examination, but clinical diagnosis is often difficult because of its non-specific symptomatology and it can be confused with myocardial infarct, musculoskeletal pain, vasculitis and acute dissection of an aortic aneurysm. For a favourable outcome, early decompressive laminectomy and evacuation of hematoma are necessary. We report a 50-year-old female who presented with acute paraparesis and back pain on thoracic area. Diagnosis of spinal epidural hematoma which extended over $T_4-T_9$ was made by magnetic resonance imaging. We performed immediately decompressive laminectomy and evacuated the epidural hematoma. No cause for bleeding was evident. Postoperatively, neurological symptoms disappeared within six weeks.

A Case Report of Unspecified Tremor Treated by the Way of Reinforcing Weakness of the Spleen and Stomach(脾胃虛弱) (비위허약(脾胃虛弱)으로 변증한 상세불명의 진전 치험 1례)

  • Objectives : This study was designed to research the effect of oriental treatments for tremor by the way of reinforcing weakness of the spleen and stomach(脾胃虛弱). Methods : A 74 year-old female patient has been suffered by severe tremor and nausea for a month. We treated the patient by the way of reinforcing weakness of the spleen and stomach(脾胃虛弱) with Herbal medication and acupuncture. Results : As a result of oriental treatments, nausea was disappeared, and tremor was improved. Conclusions : These result suggest that oriental treatments have an effect on tremor.

  • A cutaneous adverse drug reaction (CADR) refers to an unexpected skin and mucosal reaction caused by drug administration. In the present case, a 65-year-old male presented with generalized itching and a maculopapular rash after taking Western medication, including anticonvulsants and a nonsteroidal anti-inflammatory drug (NSAID). He was treated with Gamiseungmagalgeun-tang, a traditional Korean herbal medicine. After treatment, the patient's symptoms improved, without recurrence. Based on this experience, traditional Korean herbal medicine, including Gamiseungmagalgeun-tang, may be beneficial for improving symptoms of CADRs.

A Study of the Correlation between Stroke Incidence by Day of the Week and Risk Factors (중풍(中風)의 요일별(曜日別) 발생(發生)과 위험요인(危險要因)과의 상관성(相關性)에 관(關)한 연구(硏究))

  • Objective : This study was aimed to investigated the correlation between the stroke incidence by day of the week and risk factors in acute stroke patients. Methods : From October in 2005 to October in 2007, 673 acute stroke patients wereincluded. Patients were hospitalized within 14 days after the onset of stroke in DongGuk University International Hospital, Kyungwon University In-cheon Oriental Medical Hospital or the Department of Cardiovascular and Neurologic Diseases (Stroke Center), Kyung Hee University Oriental hospital. We investigated general characteristics, stroke types, age group, educational backgrounds, medical history, lifestyle (the impact of stress, exercise, smoking and drinking) and Sasang constitution according to the stroke incidence by day of the week. Results : After examining each participants with day of the week, the order of days by incidence was Monday, Wednesday, Sunday, Saturday, Tuesday, Thursday and Friday respectively. Monday and Tuesday showed the highest ratio. As the aspect of demographic data of subjects, the group under 55 years and from 55 years to 70 years showed the highest ratio on Monday. In the group between 25 years to 55 years, the ratio, by the classification of actual productive age, was statistically higheron Monday. In addition, the ratio showed statistically higherby educational background, from elementary school to high school and over high school. In regard tothe medical aspect, incidence of cerebral hemorrhage showed higher ratio on Sunday, and incidence of brain infarction was higheron Monday. However, there was no difference of the past history between the two groups. With regard tothe aspect of lifestyle of subjects, the group with huge stress before the incidence of stroke showed a higher ratio of stroke incidence on Monday. Participants who didn't exercise regularly and participants who didn't smoke or drink showed higher ratio of stroke incidence on Monday. However, this was not important statistically. In regard to the aspect of Sasang constitution, Soeumin showed the highest ratio of stroke incidence on Wednesday, Taeuminon Monday, and Soyangin on Saturday and Monday. Conclusion : According to these results, several cardiovascular risk factors affect stroke incidences on Monday. Further studies will be needed to help understand the correlation between stroke incidence by day of the week and risk factors in acute stroke patients.

  • Below are the reports arranged from the research of ${\ll}$XiFangZiMingTangJiuJing(西方子明堂灸經)${\gg}$, which appears in the research of the main issue. ${\ll}$XiFangZiMingTangJiuJing${\gg}$ was written during the Nan-song period, approximately between 1142~1194, and was probably completed between 1142~1148. This book is the very first book specific on the moxibustion theory which exists these days and also thoroughly contains the moxibustion theory. So it contains many documents of the moxibustion theory which are missing these days. Also it's possible that the illustration presented in this book followed ${\ll}$MingTangJing(明堂經)${\gg}$ of SunSiMiao(孫思邈). The way of arranging the GuiJing(歸經) of YuZue(兪穴) and deciding the location of acupuncturing(穴位) differs from reports given by other documents. As seen above, ${\ll}$XiFangZiMingTangJiuJing${\gg}$ has a consulting value on the moxibustion theroy(灸法), both in clinic and the research of documents. So we are looking forward that there would be a more profound research continuing in the future.

Research of Yinyanmaisihou(陰陽脈死候) in medical books from Mawangdui Han dynasty tomb (마왕퇴(馬王堆) 출토(出土) 의서(醫書) 중(中) 《음양맥사후(陰陽脈死候)》에 관(關)한 연구(硏究))

  • Object : In this thesis we researched the transformation of five symptoms of final life stage from ${\ll}$Sumun(素問)${\gg}$, ${\ll}$Yingshu(靈樞)${\gg}$ and ${\ll}$Nanjing(難經)${\gg}$ and pulse diagnosis method. Method : We studied medical books from Mawangdui Han dynasty tomb Results and Conclusion : 1. The symptoms of final life stage according to Yinyanmaisihou(陰陽脈死候) is suggested by observation of symptoms introducting Yinyan five-elements theory. 2. The symptoms of final life stage according to Yinyanmaisihou(陰陽脈死候) and "Qijue(氣絶)" from ${\ll}$Lingshu(靈樞) Jingmai(經脈)${\gg}$ made use of twelve merdian bian(遍) diagnosis method and ${\ll}$Lingshu(靈樞) Zongshi(終始)${\gg}$, ${\ll}$Sumun(素問) Zhenyaojingzhonglun(診要經終論)${\gg}$ made use of twelve merdian biaoben(標本) diagnosis method. Afterward oriental medical biology which suggests agreement between outside and inside symptoms needs to be examined by use of science and medical theory to apply medical treatment .

A Case Report of Korean Medicine Treatment for a Patient with Functional Abdominal Pain (한의약 치료 후 호전된 기능성 복통 환자 치험 1례)

  • Objectives: The aim of this case report was to present the effects of Korean medicine treatment in a patient with functional abdominal pain. Methods: A 24-year-old female patient was treated with Sogeonjung-tang and Gammaekdaejo-tang extract granules with acupuncture therapy. Abdominal pain and soreness were assessed daily with a numeric rating scale (NRS) Results: With 7 days of treatment, abdominal pain was relieved from NRS 7 to NRS 3. By the 6th day of treatment, abdominal soreness had disappeared. Conclusion: These results show that functional abdominal pain can be relieved with Korean medicine treatment. However, further well-designed studies are required to confirm these findings.

  • Objective: The purpose of this study is to examine the relation between the Obesity and the prevalence of LBP. Methods: From February 2001 to April 2001 we studied body composition, Body Mass Index, Basal Metabolic Rate, Obesity Degree, Waist Hip Ratio, percent body fat and muscular form of 40 patients with LBP and 40 patients with Internal disease, who visited Dong-Guk University Bundang Hospital by using Inbody 3.0. Results: 1. Sexual ratio in this study is 1:4(male:female). 2. Fluid, protein, mineral mass and BMR in patients with LBP are lower than those in patients with Internal disease, but percent body, WHR and obesity degree with LBP are higher than those in patients with Internal disease. 3. LBP prevalence is shown to rise with increasing WHR. 4. LBP prevalence is shown to rise with decreasing BMR. Conclusions: Patients with LBP are obeser than patients with Internal disease. LBP prevalence is shown to rise with increasing WHR in statistics. But LBP prevalence is shown to rise with decreasing BMR in statistics.

A Case of Headache and Memory Disorder after Traumatic Brain Injury Treated with Modified Seogakjihwang-tang (외상성 뇌손상 후두통과 기억장애가 발생한 환자에 서각지황탕가감방(犀角地黃湯加減方)을 투여한 1례 보고)

  • The purpose of this study is to report the clinical effects of modified Seogakjihwang-tang on a patient who had headache and memory disorder after traumatic brain injury, which were considered as the result of blood stasis. The patient had headache and memory disorder after traumatic brain injury and treated with modified Seogakjihwang-tang three times a day for 60days. As a result, the numerical rating scale and pain rating score of headache decreased and the scores of mini-mental state examination-Korea, the Korean version of Montreal cognitive assessment and the mini-memory test increased during treatment with modified Seogakjihwang-tang. Therefore, this case supports a possibility that modified Seogakjihwang-tang might be an effective treatment to patients with headache and memory disorder after traumatic brain injury.

Korean Medical Treatment for Prolonged Central Hyperthermia Following Pontine Hemorrhage: A Case Report (뇌교 출혈 이후 발생한 지연된 중추성 고체온증에 대한 한방치료 1례)

  • A patient had central fever following pontine hemorrhage. Central hyperthermia caused by stroke is a rare case, and it is difficult to control. There are few case reports about central hyperthermia in Korean medical treatment (KMT). The patient suffered central hyperthermia accompanied by tachycardia, dyspnea, and irritability. However, there was no evidence of infection. Thus, hypnotics, sedatives, and a minor tranquilizer (Lorazepam and Midazolam) was prescribed. Despite a temperature peak of $39.9^{\circ}C$, most of the symptoms were alleviated. The patient's average body temperature was about $37^{\circ}C$, which is higher than most people at $36.5^{\circ}C$. His symptoms were diagnosed as ascendant hyperactivity of liver Yang (肝陽上亢), and the patient was prescribed Shihogayonggolmoryo-tang. During the 44 days of KMT, there was no change in his average body temperature and no central hyperthermia over $39^{\circ}C$. This case report demonstrates the possibility of controlling central hyperthermia caused by pontine hemorrhage using KMT.
