동작관찰 과제지향훈련이 만성 뇌졸중환자의 균형 및 보행능력에 미치는 효과 (Effects of Action-observational training Program on Gait and Balance of Patients with Hemiplegia)

  • 본 연구는 만성뇌졸중 환자에게 적용한 동작관찰 과제지향 훈련이 균형 및 보행능력에 미치는 영향을 알아보고자 하였다. 연구에 동의한 만성 뇌졸중환자 30명중 동작관찰 과제지향훈련을 시행한 실험군 15명과 일반 과제지향훈련을 시행한 대조군 15명에게 총 6주간 하루 30분씩 훈련하였다. 훈련효과를 알아보기 위해 훈련전 균형은 BT4로 검사를 시행하였고 보행능력은 10M검사를 실시하였다. 이에 동작관찰 과제지향 훈련군이 일반과제 지향훈련군에 비해 균형과 보행능력에서 유의미한 차이를 보였다. 이에 동작관찰 과제지향훈련이 만성뇌졸중환자의 균형 및 보행능력에 효과적인 프로그램임을 알 수 있다.

  • 본 연구는 만성 뇌졸중환자의 상지 기능과 일상생활동작에 대한 동작관찰훈련과 과제지향훈련의 효과를 비교하기 위하여 시행되었다. 연구대상자들은 총 12 명으로 1 집단과 2 집단에 6 명씩 무작위로 할당되었으며, 각 집단의 대상자들에게 동작관찰훈련(A)과 과제지향훈련(B)을 순차적으로 적용하였다. 본 연구는 교차연구 설계를 사용하여 1 집단은 A-B, 2 집단은 B-A의 순서로 중재가 적용되었다. 각 훈련은 2 주 동안 주 5회, 매일 30 분씩 시행되었으며, 총 실험기간은 4 주였다. 측정은 울프 운동 기능검사(Wolf motor function test, WMFT)와 수정된 바델 지수(modified Barthel index, MBI)를 사용하여 기초선, 2 주후, 4 주후에 시행되었다. 본 연구의 결과는 과제지향훈련과 동작관찰훈련 모두 만성 뇌졸중환자의 WMFT와 MBI 점수를 유의하게 향상시키는 것으로 나타났으나(p<.05), 두 훈련 방법의 차이는 없었다(p>.05). 훈련 전후의 효과크기는 두 집단 모두 동작관찰훈련을 시행하였을 때 더 높은 것으로 나타났다. 이러한 결과는 동작관찰훈련과 과제지향훈련 모두 뇌졸중 환자의 상지기능과 일상생활동작을 향상시키는데 도움이 된다는 것을 의미하는 것이며, 동작관찰훈련의 임상적용 가능성을 지지하는 것이다.

청주지역 일부 대학생의 식생활지침 실천여부에 따른 당류 섭취실태 및 당류 저감화 인식 (Sugar Intake and Sugar Reduction Perception by Practice of Dietary Action Guides among the College Students in Cheongju)

  • 본 논문은 청주지역 대학생을 대상을 식생활지침의 실천여부와 당류 섭취 실태 및 당류 저감화 인식에 미치는 영향을 파악하여 당류 섭취 줄이기를 위한 기초자료로 활용하고자 실시하였다. 연구대상자는 건강한 대학생 343명을 대상으로 식생활지침 실천여부를 점수화한 후 중앙값을 기준으로 식생활지침 실천도가 높은군과 실천도가 낮은군으로 성별에 따라 구분하였다. 연령, 신장, 체중, 체질량지수는 식생활지침 실천여부에 따른 남학생과 여학생 모두 군간 차이가 없었고, 여학생의 실천도가 낮은 군에서 주당 간식 횟수, 흡연율, 객관적인 단맛 인지도 평가가 유의적으로 높았다. 남학생과 여학생 모두 실천도가 낮은군에서 탄산음료, 가공우유, 집 밖에서의 간식 섭취가 높은 식행동을 보였고, 당류 간식으로부터 당류 섭취량이 실천도가 높은군에 비해 높았다. 여학생의 실천도가 낮은군에서 당류 섭취 권고기준을 초과하는 비율이 36.59%로 실천도가 낮은군(13.64%)에 비해 높게 나타났다. 당류 섭취 저감화 필요성은 남학생과 여학생 모두 차이를 보이지 않았고, 간식 구입 시 영양표시를 확인한다는 응답율은 남학생과 여학생 모두 실천도가 낮은군에서 낮았다. 영양표시 교육의 필요성은 남학생과 여학생 모두 80%이상으로 높게 나타났다. 이러한 결과를 종합하여 볼 때, 식생활지침 실천을 장려하는 것은 대학생의 당류 섭취를 줄이는데 도움이 될 것으로 보여진다.

The Effect of Action Observation Training on Balance and Cognition on the Elderly with Dementia

  • Purpose: The purpose of this study was to investigate the impact of action observation training (AOT) on the balance and cognition of the elderly with dementia. Methods: Twenty-four participants were randomly assigned to experimental and control groups (12 participants each). The 5-week intervention involved 45 minutes sessions, three times a week. Both groups did 30 minutes of general physical exercises, followed by the experimental group watching a 5 minutes video featuring functional movements and imitating them for 10 minutes. The control group watched a scenic video and did functional training for 10 minutes, mimicking the experimental group. All participants were evaluated using the Timed Up and Go (TUG) test, Functional Reaching Test (FRT), Berg's Balance Scale (BBS), and Korean version of Mini-Mental State Examination (K-MMSE) before and after the intervention. A paired t-test was conducted to compare the within-group change before and after the intervention. Two-way repeated measures ANOVA was performed to compare the between-group difference. The statistical significance level was set to p=0.05 for all variables. Results: The experimental group showed significant within-group changes in the TUG test, FRT, BBS, and K-MMSE (p<0.05). The control group showed a significant change in FRT and K-MMSE (p<0.05). A significant difference was observed between the experimental group and the control group regarding the change in the TUG test, BBS, and K-MMSE after the interventions (p<0.05). Conclusion: The findings of this study suggest that the AOT and repetition of actual movements, led to more significant improvements in balance and cognitive abilities compared to the control group that observed scenic landscapes.

지역사회복지관 한부모 자조집단 실천에 대한 사회복지사의 성찰 연구 -참여실행연구 자료를 중심으로 (A Study of Social Workers' Reflections on the Supportive Practice for Community Welfare Center Single Parent Self-Help Groups based on Participatory Action Research)

  • 본 연구는 지역사회복지관 한부모 자조집단 실천방안 모색을 위한 참여실행연구 자료를 활용하여, 연구를 수행한 사회복지사들이 참여실행연구과정에서 성찰한 내용이 무엇인지를 구체적으로 탐색하는 데 목적을 두고 있다. 참여실행연구에는 지역사회복지관 한부모 자조집단 실천의 어려움을 공유한 사회복지사와 교수자 9명이 참여했고, 10회기의 연구모임을 통해 동료 수퍼비전을 비롯한 다양한 활동을 하면서 사회복지사들이 소속된 기관의 4개 자조집단 실천에대한 계획-실행-성찰-재계획을 반복하는 순환적 연구를 수행하였다. 그 결과 현장에 밀착된 다양한 실천방안이 도출되었는데, 그 중 핵심은 사회복지사의 인식이 변화해야 한다는 것이다. 본 연구에서는 참여실행연구에서의 성찰을 통해 사회복지사의 인식이 변화된 것에 주목하고, 연구모임 녹취록 등 참여실행연구에서 생성된 자료를 활용하여 성찰의 구체적 내용을 질적으로 분석하였다. 그 결과, '자조집단에 적합한 실천이 미흡했음을 깨닫고 고민함', '회원-사회복지사 관계에 대한 변화된 관점을 바탕으로 사회복지사 역할을 다시 알아감', '당사자 주도 실천의 가능성을 확인함', '지역사회복지관 한부모 자조집단을 더 세밀하게 이해함', '지역사회복지관 한부모 자조집단의 발전을 새롭게 인식함' 등 5개 주제가 도출되었다. 이러한 결과를 토대로 지역사회복지관 한부모 자조집단을 중심으로 역량강화 실천과 성찰?반성적 실천에 대해논의하였다.

  • Cheonghunhwadam-tang(CHT) have been used in oriental medicine for many centuries as a therapeutic agent of vertigo by wind, fire and phlegm. CHTGR was CHT adding Gastrodae Rhizoma. The effects of CHTGR on the regional cerebral blood flow(rCBF) and mean arterial blood pressure(MABP) is not known. A comparative Study of action-mechanism of CHT and CHTGR on the cerebral hemodynamics is not known too. Therefore, purpose of this Study was to investigate effects of CHT and CHTGR on the rCBF and MABP, compare action-mechanism of CHT and CHTGR on the rCBF and MABP. The changes of rCBF and BP was determinated by Laser-Doppler Flowmetry(LDF). The results were as follows ; CHT extract was increased rCBF in a dose-dependent, but was not changed MABP compared with CHT non-treated group. CHTGR extract was decreased rCBF and MABP compared with CHTGR non-treated group in a dose-dependent. Action of CHT is not related with adrenergic β-receptor, cyclooxygenase and guanylate cyclase, but action of CHTGR is related with guanylate cyclase.

An EEG-based Brain Mapping to Determine Mirror Neuron System in Patients with Chronic Stroke during Action Observation

  • Purpose: The aim of this study was to compare EEG topographical maps in patients with chronic stroke after action observation physical training. Methods: Ten subjects were recruited from a medical hospital. Participants observed the action of transferring a small block from one box to another for 6 sessions of 1 minute each, and then performed the observed action for 3 minutes, 6 times. An EEG-based brain mapping system with 32 scalp sites was used to determine cortical reorganization in the regions of interest (ROIs) during observation of movement. The EEG-based brain mapping was comparison in within-group before and after training. ROIs included the primary sensorimotor cortex, premotor cortex, superior parietal lobule, inferior parietal lobule, superior temporal lobe, and visual cortex. EEG data were analyzed with an average log ratio in order to control the variability of the absolute mu power. The mu power log ratio was in within-group comparison with paired t-tests. Results: Participants showed activation prior to the intervention in all of the cerebral cortex, whereas the inferior frontal gyrus, superior frontal gyrus, precentral gyrus, and inferior parietal cortex were selectively activated after the training. There were no differences in mu power between each session. Conclusion: These findings suggest that action observation physical training contributes to attaining brain reorganization and improving brain functionality, as part of rehabilitation and intervention programs.

Utilizing the Health Belief Model to predicting female middle school students' behavioral intention of weight reduction by weight status

  • Weight reduction behavior is common among adolescent girls. The present study examined the status of weight reduction behavior and factors affecting the behavioral intention of weight reduction using the Health Belief Model (HBM) for female middle school students by weight category. Survey data was collected from three girl's middle schools in Gyeongju, Korea. A total of 299 female middle school students participated in this study. The questionnaire had questions about general characteristics, weight reduction behavior, and variables of HBM (perceived threat, perceived benefits, perceived barriers, cues to action, self-efficacy in dietary life and exercise, and behavioral intention of weight reduction). Descriptive statistics, Chi-square tests, ANOVA, and multiple regression analysis were applied to analyze the variables. A higher percentage of students in the overweight group attempted weight reduction than those in the underweight and the normal weight groups (P < 0.001). Among students who had attempted weight reduction, 73% tried diet therapy, while 78% tried exercise. Students in the normal and overweight groups showed significantly higher threat (P < 0.01) and cues to action (P < 0.001) than those in the underweight group. As for perceived benefits, barriers, and self-efficacy in dietary life and exercise, there were no significant differences among weight groups. Students in the overweight group showed the strongest intention of weight reduction and there were significant differences among the three weight groups (P < 0.001). Perceive threat (P < 0.01), cues to action (P < 0.001), and perceived self-efficacy (P < 0.01) were significantly associated to behavioral intention of weight reduction for all respondents. For the underweight group, perceived threat (P < 0.05) and perceived self-efficacy (P < 0.01) were the significant variables. For the overweight group, cue to action was the significant variable (P < 0.05).

홍삼 성분의 혈당강하작용 연구 (ll) :쥐의 배양 간세포의 당대사 관련 효소 활성에 미치는 홍삼 지용성 분획의 영향 조사 (Hypoglycemic Action of Red Ginseng Components (II). Investigation of the Effect of Fat Soluble Fraction from Red Ginseng on Enzymes Related to Glucose Metabolism in Cultured Rat Hapatocytes)

  • In this study, rat hepatocytes known to have active glucose metabolism were obtained to investigate the hypoglycemic action of fat soluble fraction of red ginseng by using the liver perfusion technique and incubated in two different media-one containing insulin and glucagon (control group), and the other containing glucagon only The activities of main regulating enzymes, such as glucokinase, glucose 6-phosphate dehydrogenase, 6-phosphogluconate dehydrogenate, and glucose 6-phosphatase, related to metabolic pathways of glucose in these two kinds of hepatocytes were compared between these two groups and the effects of addition of fat soluble fraction ($10^1$~$10^4$%) from red ginseng to these two groups on these enzymes were also detected. The results were as follows. The specific activity of enzymes such as glucokinase, flucorse 6-phosphate dehydrogenase, and 6-phosphogluconate dehydrogenase related to glucose-consuming pathways of insulin-deficient group was much less than control one. However, their decreased activity was recovered after the addition of fat-soluble fraction at all range of concentrations. The specific activity of these enzymes after the addition of ginseng components to the control group was also increased. On the other hand, the specific activity of glucose 6-phosphatase related to glucose-producing pathway of insulin-deficient group was much higher than control one, but their increased activity was decreased obviously after the addition of fat soluble fraction at all range of concentrations. The same results were observed after the addition of fat-soluble fraction to the control group. These results suggest that the red ginseng saponin components might be effective on diabetic hyperglycemia by regulating the activity of enzymes related to glucose metabolism directly and/or indirectly. The effects of fat-soluble fraction ($10^2$%) and ginsenosides (mixture, $Rb_1$ and $Rg_1$, $10^4$%) on hypoglycemic action were compared. As a result, they showed considerable effect on hyperglycemia, but the best eff ect on the activities of glucokinase and glucose 6-phosphate dehydrogenase was appeared by ginsenoside $Rb_1$ and that of 6-phosphogluconate dehydrogenase and glucose 6-phosphatase was by ginsenoside mixture.
