Application of General QA Tools to Quality Improvement Activities in a Hospital's Emergency Room (응급실에서의 질 향상 기법 적용)

  • Background : Although a number of studies are related to QA improvement, there are few studies applied various method of QA tools. This study reviewed the availability of general quality assurance(QA) tools according to ten steps in performing quality improvement activities at emergency room of a tertiary teaching hospital which has more than 1,000 beds. Methods : All patients in emergency room from 15th Oct. 1997 to 5th Sep. 1998 were surveyed. The survey data based on the patient's records of emergency room were evaluated according to length of stay, and we tried to identify problems with management of emergency room. To solve the problems, our team applied general QA tools(brainstorming, flow chart, nominal group technique, benchmarking, cause-and-effect diagram, run chart, control chart) to quality improvement activities and discussed the availability of the tools. Results : After the activities such as changes of staffing, the establishment of local area network and chest pain clinic, application of triage and so on, the percentage of patient who had stayed more than six hours was reduced from 56.0% to 46.8%. The mean number of patients per day in emergency room was increased from 49 to 62. But the reporting time for laboratory test was not changed after these activities. Conclusion : Each QA tool has unique benefit and limitation, but we can implement and evaluate the quality improvement activities more scientifically and systematically by applying these tools to practice according to QA ten steps.

Clinical Analysis of Musculoskeletal Patients Who Visited the Emergency Room of an Oriental Medicine Hospital (한방병원 응급실에 내원한 근골격계 환자에 대한 임상적 분석)

  • Objectives : By recording basic data of musculoskeletal patients visiting the emergency room of an oriental medicine hospital. we can understand their characteristics and gain better insight about them. Method : We performed a retrospective study of 551 patients who visited the emergency room of an oriental medicine hospital, from November 1,2008 to October 31, 2009. Result : 1. Of 551 total patients, the male to female ratio was almost same and the 3rd decade of life was the peak age group. 2. The majority of patients visited emergency room between 10:00 and 24:00(midnight). 3. Most patients visited on a Saturday or Sunday. 4. By monthly distribution, the patients increased slightly during January, May and October. 5. The majority of patients visited the emergency room within 24 hours of onset. 6. The patients were categorized as follows : lumbar sprain 66.06%, cervical sprain 16.25%, ankle sprain 9.04%. 7. Admission rate was 21.96%. Conclusion : Assuming that this research will continue adding patient's data continuously, this work will help up to understand the features of musculoskeletal patients who visiting the emergency room of the oriental medicine hospital.

The Effects of Leg Elevation and Stretching Exercise on Monitored Anesthesia Care (MAC) with Dexmedetomidine (Dexmedetomidine 감시마취관리 환자의 회복 시 적용한 하지거상 및 말초운동의 효과)

  • Purpose: The purpose of this study was to examine the effect of leg raising and peripheral excercise on recovery of the patients who were applied with dexmedetomidine for their dental surgery. Methods: There were two groups, the experimental group (n=35) and control group (n=35), in this study. We checked blood pressure, pulse rate, oxygen saturation, sedation level and symptoms such as dizziness and somnolence every 30 minutes. These parameters were assessed throughout the participants' recovery room stay. Leg raising and peripheral excercise were conducted in the experimental group in the recovery room. We have conducted chi-square test, Fisher's exact test, t-test, ANOVA, and ANCOVA to compare the measured parameters in both groups. Results: The experimental group showed a significant elevation of mean arterial blood pressure, and mitigation of somnolence, sedation and dizziness compared to the control group. Conclusion: Leg raising and peripheral exercise is effective to expedite recovery in the patients who were applied with dexmedetomidine for their dental surgery.

The Self Concept of Adolescent Consumers and the Evaluation and Experience of Adolescent Entry into Harmful Place (청소년 소비자의 자아개념과 유해업소 출입에 대한 평가 및 경험)

  • This study examined the subcategories of the self concept of adolescent consumers, and investigated the influence of this self concept and the demographic characteristics on the evaluation and experience of adolescent entry into harmful places. The adolescent group was divided into 4 subgroups: (group I with negative evaluation and without entry experience, group II with negative evaluation but with experience;, group III with positive evaluation but without experience, and group IV with positive evaluation and with experience). The differences of demographic characteristic and self concept between these. subgroups were inspected. The results of this study were as follows. First, the self concept of the adolescent consumer consists of body self, social self, achievement self, scholarship self and family self. Among these self concepts, scholarship self was the highest, and family self was the lowest. Second, from the result of regression analysis using the demographic element as an independent variable to inspect the factors influencing self concept, sex, school year, living with parents, and economic status were statistically significant. Third, in case of entry evaluation, singing room was evaluated most positively among 10 places with a score of 4.20 and pub/drunkenness was evaluated most negatively. In case of entry experience, singing room was the most frequently experienced place and discotheque was the least. Fourth, with respect to demographic characteristics and influences of self concept, the influencing factors on entry evaluation were sex, school year, social self, and family self, and the influencing factors on entry experience were sex, school year, living with parents, social self, and family self. Finally, examination of the differences of demographic characteristics and self concept according to entry evaluation and experience, revealed the distinctive variables among the 10 harmful places to be sex, school year, living with parents, body self, social self, achievement self, scholarship self, and family self.

The Effect of a Decrease Indwelling Catheter Size and Preoperative Education on Bladder Discomfort and Patient's Nursing Needs (유치도뇨관 크기 감소 및 정보제공이 수술 후 유치도뇨관으로 인한 불편감과 간호요구도에 미치는 효과: 회복실에 입실한 성인남자 환자를 중심으로)

  • Purpose: This study was carried out to investigate the effect of a decrease in indwelling catheter size and preoperative education on bladder discomfort and nursing needs. The study was conducted on patients in the recovery room after their surgery. Methods: This study is a randomized control trial. Data were collected from September 2011 to February 2012 at a university hospital in Bundang, Korea. The sample consisted of 75 patients, excluding one patient from the experimental group 2. From among the patients that were sent to the ICU, 24 were in experimental group 1, 24 were in experimental group 2, and 27 were in the control group. Data were collected and analyzed using a chi-square test, t-test, and one-way ANOVA. Results: There were significant differences between the bladder discomfort experienced by patients in experimental group 1 and the control group, and experimental group 2 and the control group. Then nursing needs of the control group were higher than those of the experimental groups 1 and 2. Conclusion: The decrease in indwelling catheter size and preoperative nursing education is an effective nursing intervention in perioperative care.

Numerical Analysis of Thermal Environment Control in High-Density Data Center (고밀도 데이터센터의 열환경제어를 위한 수치해석)

  • Increasing heat generation in CPUs can hamper effective recirculation and by-pass because of the large temperature difference between the exhaust and the intake air through a server room. This increases the overall temperature inside a data center and decreases the efficiency of the data center's cooling system. The purpose of the data center's cooling system is to separate the intake and exhaust air by controlling the computer room air-conditioner(CRAC). In this study, ICEPAK is used to conduct a numerical analysis of a data center's cooling system. The temperature distribution and the entire room are analyzed for different volumetric flow rates. The optimized volumetric flow rate is found for each CPU power. The heat removal and temperature distribution for CPU powers of 100, 120, and 140 W are found to be the best for a volumetric flow rate of $0.15m^3/s$. The numerical analysis is verified through RTI indicators, and the results appear to be the most reliable when the RTI value is 81.

The Effects of Pre-operative Visual Information and Parental Presence Intervention on Anxiety, Delirium, and Pain of Post-Operative Pediatric Patients in PACU (회복실에 대한 사전 정보제공과 보호자 상주 중재가 수술 직후 각성 시 소아 청소년 환아의 불안, 섬망 및 통증에 미치는 효과)

  • Purpose: The purpose of this study was to test whether pre-operative visual information and parental presence had positive effects on anxiety, delirium, and pain in pediatric patients who awoke from general anesthesia in a post-surgical stage. Methods: This study used a non equivalent control-group post test design (n=76). Independent variables were provision of pre-operative visual information and parental presence for post-surgical pediatric patients in PACU (post anesthesia care unit). Dependent variables were anxiety, delirium, and pain in the pediatric patients measured three times at 10 minute intervals after extubation in the PACU. Measurements included Numerical Rating Scale for assessing state anxiety, Pediatric Anesthesia Emergence Delirium Scale by Sikich & Lerman (2004) for delirium, and Objective Pain Scale by Broadman, Rice & Hannallah (1988) for pain. Results: Experimental group showed significantly decreased state anxiety at time points-10, 20, and 30 minutes after extubation. Delirium was significantly lower at 10 minutes and 30 minutes after extubation in the experimental group. Pain was significantly lower at 10 minutes after extubation in the experimental group. Conclusion: The results of this study suggest that this intervention can be a safe pre-operative nursing intervention for post-surgical pediatric patients at PACU.

A Study on the Application of Swedish Group Home for the Elderly with Dementia in Korea (치매노인을 위한 스웨덴 그룹홈의 우리나라 적용가능성 연구)

  • The purpose of this study was to determine the extent to which Swedish group home for elderly people with dementia coed be applied to the situation in Korea. One hundred and ninety six Korean subjects, responsible for supporting a member of their family afflicted with such an illness in their own homes, comprised the sample population for this study. Data was collected using questionnaires. frequency distribution, mean, and chi-square tests were used to analyze the data. The results of this study indicated a great possibility for adapting these kinds of group home in Korea. The respondents showed positive opinions with the basic concepts and space plans of Swedish group homes excepts for some details, like the number of residents per room. The respondents felt that a group home for dementia with the fellowing features was desirable : smart scale, single-family detached house,3-4 rooms, a total of 6-8 residents, managed by non-profit organizations or the government at a monthly cost of 530,000-1,000,000 won. In order to develope these group homes for dementia people in Korea, political assistance, combined with economic support, are strongly needed. The successful establishment of a group home will contribute not only to the welfare of elderly people with dementia but to their families as well.

  • This study analyzed the type of the teaming space of 84 elementary schools which were prized as excellent schools on architectural planning and design in Japan from 1985 to 2005. As a result of the study, more types of learning space appeared at different elementary schools, and there was a tendency to plan as one unit space which include class room, multipurpose space, and others, on the planning of learning space. They intended to be uses for the teaming and living activity as a group of a school year.

A comparison of resident's behavior of operating air conditioner according to the charge for electric (사용자의 전기요금 부담 여부에 따른 여름철 냉방기 사용 특성의 비교)

  • An electrical bill is one of the various factors that influence on energy use of residents. This study focused on electrical bills and searched that whether electrical bills affect resident behavior of operating air conditioner and their controlled indoor climate. We surveyed two groups for field study ; the one is residents who live in a studio apartment in Seoul and they are charged for electric as energy consumption they used, and the other is residents who live in a university dormitory and they are not charged for their energy consumption. We measured how long they used the air conditioner for cooling, on/off temperature of air conditioner and room temperature for these two groups. Residents were interviewed about their cooling needs, decisions about when to turn on their air conditioner and so on. We found that dormitory group has much amount of time used the air conditioner than studio apartment group and there are differences of room temperature between two groups when they turn on or off air conditioner. The result shows that perception of money affects resident behavior of operating air conditioner and their acceptable thermal range.
