Background: Dental caries is mainly composed of various cellular components and is deposited around the tooth surface and gums, causing a number of periodontal diseases. Streptococcus mutans is commonly found in the human oral cavity and is a significant contributor to tooth decay. The use of antibacterial ingredients in oral hygiene products has demonstrated usefulness in the management of dental caries. This study investigated the anticaries effect of the ethanol extract of Terminalia chebula (EETC) against S. mutans and their cytotoxicity to gingival epithelial cells. Methods: The EETC was prepared from T. chebula fruit using ethanol extraction. Disk diffusion, minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), and colony forming unit (CFU) were analyzed to investigate the antimicrobial activity of the EETC. Glucan formation was measured using the filtrate of the bacterial culture medium and sucrose. Gene expression was analyzed using reverse transcription-polymerase chain reaction (RT-PCR). Cytotoxicity was analyzed via the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide assay. Results: The antibacterial activity of the EETC was explored using disc diffusion and CFU measurements. The MIC and MBC of the EETC were 10 and 20 ㎍/ml, respectively. EETC treatment decreased insoluble glucan formation by S. mutans enzymes and also resulted in reduced glycosyltransferase B (gtf B), gtf C, gtf D, and fructosyltransferase (ftf), expressions on RT-PCR. In addition, at effective antibacterial concentrations, EETC treatment was not cytotoxic to gingival epithelial cells. Conclusion: These results demonstrate that the EETC is an effective anticaries ingredient with low cytotoxicity to gingival epithelial cells. The EETC may be useful in antibacterial oral hygiene products for the management of dental caries.
Purpose: With the increasing prevalence of orthodontic treatment in adults, clear aligner treatments are becoming more popular. The aim of this study was to evaluate the effect of orthodontic treatment on periodontal tissue and to compare orthodontic treatment with fixed appliances (FA) to clear aligner treatment (CAT) in periodontitis patients. Methods: A total of 35 patients who underwent orthodontic treatment in the Department of Periodontology were included in this study. After periodontal treatment with meticulous oral hygiene education, patients underwent treatment with FA or CAT, and this study analyzed patient outcomes depending on the treatment strategy. Clinical parameters were assessed at baseline and after orthodontic treatment, and the duration of treatment was compared between these two groups. Results: The overall plaque index, the gingival index, and probing depth improved after orthodontic treatment (P<0.01). The overall bone level also improved (P=0.045). However, the bone level changes in the FA and CAT groups were not significantly different. Significant differences were found between the FA and CAT groups in probing depth, change in probing depth, and duration of treatment (P<0.05). However, no significant differences were found between the FA and CAT groups regarding the plaque index, changes in the plaque index, the gingival index, changes in the gingival index, or changes in the alveolar bone level. The percentage of females in the CAT group (88%) was significantly greater than in the FA group (37%) (P<0.01). Conclusions: After orthodontic treatment, clinical parameters were improved in the FA and CAT groups with meticulous oral hygiene education and plaque control. Regarding plaque index and gingival index, no significant differences were found between these two groups. We suggest that combined periodontal and orthodontic treatment can improve patients' periodontal health irrespective of orthodontic techniques.
Cyclosporin A(CsA) is now widely used to treat organ transplant recipients. But CsA has various short-and long-term side effects. Especially, gingival hyperplasia is not easy to resolve since its nature is still unknown. This study discusses the pathogenesis of CsA-induced gingival hyperplasia on the basis of data obtained from light and electron microscopic studies of biopsis from patients on CsA treatment after kidney transplantation. Light microscopically, the multilayered squamous epithelium showed an irregular surface of parakeratosis and deep invaginations in the subepithelial tissue. At lamina propria, we observed bundles of irregularly arranged collagen fiber, some fibroblasts, numerous capillary vessels and a large diffuse infiltration of plasma cells. Ultrastructurally, many fibroblasts, collagen fibers, collagen fibrils were present in lamina propria. On the basis of the data collected, we propose that the morphological features of the dimensional increase in gingival tissue associated with CsA treatment in kidney transplant patients may be considered proliferative fibroblasts, collagen fibers, collagen fibrils in lamina propria.
Objectives : This study mean to confirm the antibacterial activity of a garlic extract widely culturing in our region and was to determine the effect of dentifrice containing 0.1% extracts of garlic on dental plaque and gingivitis in a double blind and clinical studies in 50 healthy adults aged from 20 to 22 years who provided a consent for their participation. Methods : The antibacterial activity was evaluated using triple distilled water and the Kirby-Bauer disk diffusion method and the minimal inhibitory concentration(MIC) against various pathogens for periodontal disease, such as P. gingivalis 381(ATCC33277), was estimated. The experimental groups classified according to the concentration of garlic extract used: 10,000ppm(A), 5,000ppm(B), 2,500ppm(C), 1,000ppm(D). Oral examination of subjects was performed through clinical periods and on day of baseline, 6, 12, 19, 25 days plaque index and gingival index were scored by Turesky' modified index and Loe & Silness index. After 12, 19, 25 days use of their respective dentifrices, statistically decreases of plaque index, gingival index were shown in both the experimental and the control group, respectively. Results : There was significant antibacterial activity in the "2,500ppm(C)" group against P. gingivalis 381. Experimental group exhibited significantly the lower plaque levels and the higher levels of gingival health by the use of the dentifrices contained extract of garlic from 6 days compare with control group(p<0.05). The degree of decrease was more significant on gingivitis level of the experimental group than the control group(p<0.05). Conclusions : This findings indicated that the oral products containing a garlic extract is effective in preventing and treating periodontal diseases, and has potential value in inhibiting periopathogens.
The keratinized mucosa around the implant is an important key in health of soft tissue and hard tissue. The purpose of this study is showed that the keratinized mucosa is associated with the keratinized mucosa index, plaque index, gingival index, probing depth. which is investigated to observing the peri-implant mucosa of mandibular partial edentulous patuent using periodontal parameter by previously published paper. It was estimated 6 site with regard to 80 fixture for 28 person, and the average age is 46.8. Each estimation is the order of less trauma, that is, plaque index, keratinized mucosa index, gingival index and probing depth. In this study, statstically analyzed treatment is used for Spss V 7.0 for Windows(Spss Inc, USA). The Kruskal Walis Test is used to compare the amount of the keratinized mucosa is into the $0{\sim}3$ index, with plaque index, gingival index and probing depth. Mann-whitney Test is used to interpreate the relation of plaque index and probing depth, which is showed significant difference. The Result are as follows 1. The kertinized mucosa index 3 amounts to 47.7%, which is much higher than the other indices and the index order is followed 3, 1, 2 and O. 2. The plaque index 1 amounts to 61.7%, which is much higher than the other indices and the index order is followed 1, 2, 3 and O. The plaque index 0 is significant to each of index(P<0.05). The plaque index is decrease as the keratinized mucosa index is increased. 3. The probing depth for 2mm, 1mm, 3mm is 48.9%, 23.5%, 16.8% respectively, which is most occupied. The probing depth 2mm and 3mm for the keratinized mucosa index is significant(P<0.05). The probing index is decreased as the keratinized mucosa index is increased. 4. The gingival index 0 amounts to 58.0%, which is much higher than the other indices and the index order is followed 0, 1, 2 and 3.
This study was conducted to investigate the optimal management interval in 33 healthy adults by observing changes in oral health on the basis of the plaque control index and gingival index. When plaque control score was compared according to the period, it was found that oral hygiene management was the best in the fourth visit (p<0.05). Improved oral hygiene status was observed during the 3- and 6-month follow-up (p<0.05). Regarding gingival index, the fourth visit showed the healthiest gingival status (p<0.05). At the 3-month follow-up, the gingival index worsened, but a healthy gingival status was attained by 6-month follow-up (p<0.05). The plaque control score according to interest in dental health showed that the 'interested' group had good oral hygiene management (p<0.05). As a motivation for oral hygiene status and gingival health, examination with a 'phase contrast microscope' in the first visit and calculation of the 'evaluation index' in the follow-up visit tended to improve the patients' ability for oral hygiene management (p>0.05). The questionnaire survey showed, that the optimal management interval was 1, 3, and 6 months. As a result, with the effective management interval for the preventive management program focused on professional mechanical tooth cleaning, which was administered weekly, the maximum ability for oral hygiene management was attained at the fourth visit. The effective management period was 1 month. The use of a phase contrast microscope and the calculation of the evaluation index for oral hygiene management could influence the motivation to improve oral hygiene management.
All contours of the restoration not directly related to occlusion are related to the gingival tissues only. And proper contour of restoration is essential for the health of the periodontal tissues. But there are so many controversies about the contour of the restoration, and there is no uniform agreement in the literature as to which contour of restoration is best for periodontium. In general, the contour of restoration means the supragingival contour only but in the case of the intracrevicular restorative procedure the subgingival contour of restoration must be considered. Because a portion of the restoration is placed in a gingival sulcus which is extremely vulnerable to periodontal disease. In this article the concepts or theories of the supragingival contour, the subgingival contour, and the emergence profile were discussed. The contour of the restoration and the biotype of the periodontium must be considered in intracrevicular restorative procedure. And sufficient tooth preparation is important factor to develop the proper contour of restoration which is kind to periodontium.
Dental plaque has been considered as one of the major causes of inflammation. Therefore, removal of dental plaque by tooth brushing is the most important to prevent gingival and periodontal diseases and to maintain postoperative gingival health. The author executed clinical examination on 100 outpatients who were admitted to infirmary of dental college, S.N.U. The patients were examined on their tooth brushing method, frequency, habit and admitted experience to dental clinic. The obtained results were as follows; 1. The patient group who brushed with the method of Stillman, showed the lowest plaque index. 2. The examined group who brushed more frequently per a day, showed the tendency of reduced plaque index. 3. Plaque index was lowest in the group who brushed after meals and before bed. 4. The patient group who visited more frequently to dental clinic, showed the lower tendency of plague index. 5. Among the total examined group, those who recognized the importance of oral hygiene were less than 10%.
The author applied the attachment fixation overdenture on the patient whose residual alveolar ridge height was poor to increase mechanical denture retention, and observed the periodontal condition of the abutment roots after insertion of attachment fixation overdenture. The author obtained the conclusions as follows; 1. Attachment fixation overdenture showed better mechanical retention than conventional overdenture did, but it resulted unfavorable crown-root ratio. 2. Within one year after insertion, there were not significant changes in periodontal health, which was indicated by plaque index, gingival index, pocket depth, tooth mobility gingival hyperplasia and alveolar bone change. 3. Mild periodontal thickening was observed. 4. This study emphasized the importance of adequate follow-up care and home care instructions.
