Development and Analysis of the Effects of Caregiver Training Program on Aggressive Behavior m Elders with Cognitive Impairment

  • Purpose. The purposes of this study were to 1) describe the type and frequency of aggressive behavior of cognitively impaired nursing home resident, 2) develop a caregiver training program on prevention and management of aggressive behavior, 3) examine the effects of caregiver training program on the incidence of aggressive behavior of cognitively impaired nursing home resident, and 4) examine the effects of caregiver training program on nursing staff's aggressive behavior management skills. Methods. One-group, time series, quasi-experimental design with a pre-test and two post- tests was used. Data were collected from cognitively impaired home residents (N = 32) and nursing staff (N = 36) in a proprietary nursing home using Ryden Aggression Scale I, II, and Aggressive Behavior Management Scale. Data were entered and analyzed by descriptive statistics and repeated measures ANOVA. Results. Incidence of aggressive behavior was high with a mean score of 3.09 (SD = 3.11) at baseline. Caregiver training program was developed based on Progressively Lowered Stress Threshold (PLST) model and gerontological and psychiatric literature. The mean scores of aggressive behavior at baseline, Post I, and II did not differ significantly although the difference approached to the significant level (F = 2.925, p = .066). Nursing staff's aggressive behavior management skills increased at Post I, and at Post II when compared to baseline, and the difference was significant (F=12.736, p=<.00l). Conclusion. Caregiver training program showed potential impact on reduction of aggressive behavior in elders with cognitive impairment and was effective in increasing nursing staff's aggressive behavior management skills.

Health Status, Reproductive Health Problems, and the Degree of Prenatal Management in Married Working Women (기혼 취업여성의 건강상태, 생식건강상태 및 산전관리상태)

  • Purpose: This study was to examine health status, reproductive health problems and the degree of prenatal management in working women. Method: The subjects were 902 married working women among 2,000 women selected by cluster sampling. The work area was classified to product factory, school, office, etc. After an Informed consent was obtained, participants were asked to fill out a self-administered questionnaire. The instruments included a questionnaire, parity check list, menstrual problems and gynecologic problem check list. Result: Many women have experienced menstrual cycle change during the past year. There was a significant difference in general health, menstrual regularity, reproductive health and prenatal management by occupation type. 40.6 percent of the subjects have gynecological problems such as menstrual cycle change, perineal inflammation, irregular vaginal bleeding, amenorrhea, ovarian/uterine disease, infertility, or abortion. Prenatal care was received in only 28.5% of the total subjects. In addition, 16.3% answered they had experienced low birth weight babies. Conclusion: We can conclude that working woman have changes in menstruation cycle and in reproductive health status. Therefore, we suggest that some occupational characteristics may affect subject's reproductive health, these should be clear and avoided as much as possible.

Development and Effects of a Coping Skill Training Program for Caregivers in Feeding Difficulty of Older Adults with Dementia in Long-Term Care Facilities (요양시설 요양보호사를 위한 치매노인의 식사행동장애 대처기술훈련 프로그램 개발 및 효과)

  • Purpose: We developed and tested the effects of a coping skill training program for caregivers in feeding difficulty among older adults with dementia in long-term care facilities. Methods: A non-equivalent control group pretest-posttest design was used. The subjects comprised 34 caregivers (experimental group: 17, control group: 17) and 40 older adults with dementia (experimental group: 20, control group: 20). The developed program was delivered in 4-hour sessions over 6 weeks (including 2 weeks of lectures and lab practice on feeding difficulty coping skills, and 4 weeks of field practice). Data were collected before, immediately after, and 4 weeks after the program (January 3 to April 6, 2016). The data were analyzed using t-test and repeated measures ANOVA using SPSS/WIN 20.0. Results: Compared to their counterparts in the control group, caregivers in the experimental group showed a significantly greater improvement in feeding knowledge and feeding behavior, while older adults with dementia showed greater improvements in feeding difficulty and Body Mass Index. Conclusion: The study findings indicate that this coping skill training program for caregivers in feeding difficulty is an effective intervention for older adults with dementia in long-term care facilities.

Effects of Interprofessional Nurse-led Rehabilitation Intervention Program for Subacute Stroke Patients and their Caregivers: a non-randomized study

  • Purpose: Rehabilitation nursing requires an interdisciplinary comprehensive team approach that allows for enough time to promote patients' functional recovery. We identified the effects of a nurse-led rehabilitation intervention program on subacute stroke patients' activities of daily living, anger, and muscle strength, as well as their chief caregivers' satisfaction. Methods: Intervention group participants (n = 20) completed a rehabilitation intervention program, which integrated physical activities with psychological and social approaches. Results: The program did not significantly improve patients' activities of daily living or anger management; however, it significantly affected both anger-in (t = 3.384, p = .002) and anger-out (t = 2.167, p = .037) attitudes. Caregivers' satisfaction among the intervention group (t = 6.935, p < .001) decreased more significantly than that of the comparison group (t = 0.224, p = .825). Conclusion: Rehabilitation intervention program should enlist nurses' participation and promote a team-based approach during the rehabilitation program. Further, patients and their caregivers should be encouraged to express their emotions during counseling.

A Study on Gerontological Nursing: The Effects of Group Reminiscence on Depression of the Elderly (노인간호에 관한 연구 : 집단회상요법이 노인의 우울에 미치는 효과)

  • This study was to examine the effects of group reminiscence on depression of the elderly and to identify the possible use of the independent nursing intervention for reminiscence. The subjects, consisting of 46 elderly residents at D-ku, in Seoul, were divided into experimental and control group. The research was done by non-equivalent control group quasi-experimental design. Data were gathered from Oct. 4th to Dec. 2nd, 1989 through interview by questionnaire and measure the degree of depression by Zung's scale. Group reminiscence, was defined as independent nursing intervention belong to cognitive intervention by nurse. The schedule and content of reminiscence were developed by investigator. Analysis of data were done by frequency, mean, t-test, and Pearsons's correlations. The results were as follows: $\cdot$ There was no significant difference between experimental and control group after pre & post group reminiscence. $\cdot$ The mean depression score was 43.4 all subjects. In addition to, the result of added analysis were as follows; $\cdot$ There was significant difference in the depression score between healthy and unhealthy group according to perception by subjects (P<05). $\cdot$ There was significant difference between above average economic status and poor status according to perception by subjects (p<.05). $\cdot$ Depression of the subjects was related to the duration of widowhood(r=.3980, p=.015). Further research needs to be done on the effects of group reminiscence as independent nursing intervention. This study needs to be replicated in different settings, studied on the assess of psychological well being for the elderly with the exception of depression and to be identified psychological chacaracteristics of the elderly.

A Study of the Relationships among Health Promoting Behaviors, Health Locus of Control(HLOC), and Response Patterns to HLOC in Korean Elderly (일지역 노인의 건강증진 행위, 건강통제위 및 건강통제위 반응유형)

  • The purpose of this study was to identify the relationships among health promoting behaviors, Health Locus of Control(HLOC), and response patterns to HLOC in Korean elderly. The sample was composed of 162 healthy elderly ranging in age from sixties to eighties. The instruments for this study were modified Health Promoting Lifestyle Profile (45 items, 4 scales), and Multidimensional Health Locus of Control (18 items). For the in data analysis, SPSS PC$^{+}$ program was uilized for descriptive statistics, Pearson correlation, t-test, ANOVA and Stepwise multiple regression. The results of the study are as follows : 1. The total mean score for the HPLP was 2.411 (range 1-4), and the mean scores on the subscales were 3.324 for nutition, 2.709 for interpersonal support, 2.495 for exercise,2.225 for health responsibility, 2.205 for self actualization, 2.026 for stress management. 2. The mean scores for the HLOC in the elderly were HLOC-I : 23.531, HLOC-P : 21.914, HLOC-C : 18.667. 3. The response patterns of the HLOC identified eight types, pure internal, pure powerful others, pure chance, double external, believers in control, yea sayer, nay sayer and complex control. The “believers in control” was the largest group (22.8%), and “yea sayer” was the next largest group(17.9%). The “nay sayer”(5%) was the smallest group. 4. Demographic variables, such as education, economic status, religion and the perception of the health status showed strong connections to health promoting behaviors. 5. The HLOC-I and health promoting behavior were correlated positively(r=.165, P=.017), but the HLOC-P, the HLOC-C and the health promoting behavior were not correlated at the level of statistical significance. 6. There was not significant difference in health promoting behavior depending on the response patterns of the HLOC(F=1.171, P=.108). But, there were significant differences in two subscales of health promoting behavior exercise (F=3.279, P=.002), and stress management (F=3.165, P=.003). 7. Education, the perception of the health and economic status explained 21.0% of the variance for health promoting behaviors. These results suggest that several demographic factors are important factors in predicting the level of health promoting behaviors in elderly. So to enhance the health promoting behavior, nurses should use the different nursing strategies depending on the demographic characters of the elderly.

Implementation and evaluation of gerontological nursing education program: consist of knowledge about nursing care for elderly and elderly simulation experience (노인간호 교육과 노인유사체험으로 구성된 노인간호 교육프로그램의 적용 및 평가)

  • This study was conducted to test whether the education program consisted with knowledge about professional care for elderly and content of elderly simulation experience to improve empathy had positive effects on attitude and intention to practicing nursing care toward elderly. According to the study results, It was appeared that the experimental group provided both knowledge and elderly simulation experience showed more positive attitude toward elderly(t=2.56, p=.026) and intention to practicing nursing care for elderly(t=6.73, p=.003), compared with the group provided knowledge only. However, after experiencing elderly simulation program, the attitude toward elderly(t=1.22, p=.170) and intention to practice nursing care for elderly(t=-0.08, p=.470) in experimental group improved very little or so, whereas the attitude toward elderly(t=-2.33, p=.012) and intention to practice nursing care for elderly(t=-1.08, p=.150) in comparison group became even worse.

Analysis of the Effectiveness of Tai Chi Exercise for improving Balance (타이치 운동 중재 후 균형 증진에 관한 논문분석)

  • Purpose: This study aims to analyze the effectiveness of Tai chi exercise for improving balance and its tendency and helps to identify directions for future research. Method: 19 articles from Medline search of foreign journals(1981-2003) and 5 from Korean nursing journals (1981-2003) were surveyed. The contents analyses were focused on outcome measures and relative factors regarding balance. Result: Variable measures for balance were used in Tai Chi studies relating to balance. They included the functional measures such as 14 single-leg stance, 7 walking in physiological measures and 7 laboratory-based balance measures were done with platform stability test. The measure of effective Tai Chi exercise depended on the health status of subjects and the methods. Health status of subjects were only on physically inactive older adults in 17 articles and arthritis patients in 7 articles in the Tai Chi study. The significant effects of Tai Chi exercise on balance was revealed after 10weeks duration. Conclusion: Considering the results, Tai Chi exercise is more effective than any other studies for walking balance. But the reports on the outcome are inconsistent with wide variations in the choice of balance measures. It is suggested that future studies analyze systemic change through meta analysis and have a wide variety of subjects who need postural control and balance control. The methods of measures should be distinct from the balance state of subjects for the effectiveness of Tai chi exercise.

Factors related to Family Caregiver Satisfaction with Elderly in Nursing Facilities (시설입소 노인환자 부양가족의 만족도 영향요인)

  • This study aims at providing an upgraded suggestion to improve satisfaction for the family caregiver. This study was conducted to find the relationship between satisfaction and family support and outcome indicator to investigate the factors that have influence on the satisfaction. The study population was 126 families of 5 elderly residing in institutionalized elderly homes in seoul and kyunggi. The data collection was measured satisfaction, family support, and outcome indicator instrument. The satisfaction showed meaning relationship with outcome indicator and family support. Explainary power of independent variables of product index was 23%. Factors influencing satisfaction for the family was economic level, family support and outcome indicator. This study was done to give suggestions to improve family caregiver satisfaction and to serve as a basis for policy strategies by examining the current conditions of the nursing facilities.

The Effect of Aromatherapy on Stress of Nurses Working in Operating Room (향기흡입법이 수술실 간호사의 스트레스에 미치는 효과)

  • Purpose: To identify the effects of the aromatherapy on stress of nurses working in operating room. Methods: The study design was nonequivalent control-group pretest-posttest design. The subjects were 45 nurses working in the operating room. Experimental group were 24 nurses in G Hospital and control group were 21 nurses in U Hospital. All of the subjects were measured of the subjective stress, stress responses and the job stress. For aromatherapy, lavender, bergamot, and ylang were mixed in the ratio of 2 : 2 : 1. Results: 1) "The subjective stress of experimental group is lower than that of the control group" was supported (t=-2.70, p=.01). 2) "The stress responses of experi-mental group is lower than that of the control group" was supported (t=-2.49, p=.01). 3) "The job stress of experimental group is lower than that of the control group" was supported (t=-7.97, p=.00). Conclusion: This study suggested that such aroma inhalation method could be effective on stress of nurses working in operating room.
