전문간호사 양성을 위한 간호대학원 교과과정 및 운영방안 개발 (A Study for Curriculum Development for Advanced Nurse Practitioner Program)

  • The traditional nursing roles have become increasingly blurred. Nurses are now working in different ways and at higher levels of practice that enable nurses to adapt their roles and take on new responsibilities. The advanced role of nurses requires a different kind of master-level prepared education. Method & Result: This article describes an curriculum development process in preparing registered nurses for their advanced nurese' roles in the area of acute adult health nursing, geriatric nursing, pediatric nursing, neonatal intensive care nursing and oncology nursing. Several important issues to be solved regarding introduction of APN were also discussed. Conclusion: The curriculum that was proposed in the study will equip nurses to meet the challenges of future healthcare provision and will be a model to other areas of nursing practice and curriculum development.

노인유사체험이 간호사의 노인에 대한 태도에 미치는 영향 (The Effect of Senior Simulation on Nurses′ Attitude Toward the Elderly)

  • Purpose: This study was performed to explore the effect of senior simulation on nurses' attitudes toward the elderly. Method: Twenty-seven nurses working in various settings such as acute hospitals, community health centers, geriatric hospitals, and clinics were recruited. Among them, 25 subjects completed the whole experimental protocol. Aging Semantic Differential Scaling was utilized to evaluate attitudes toward the elderly and 'Suit for Experiencing Being Aged' from the Sakamoto Model was provided for the experiment. Before and after the experiment subjects filled out questionnaires. Result: Attitude score before experiment was 4.36, which indicates neutral attitude. Objective attitude scores were not different significantly after experiment. However subjective statements indicated attitude changes in a positive way. Conclusion: Senior simulation can affect nurses' attitude toward elderly in subjective way. That is, nurses became more empathetic and understanding to elderly's physical limitations and felt more initiative nursing approach were needed in caring elderly.

변혁적 리더십과 조직몰입의 관계에 미치는 임파워먼트의 매개효과 (The Mediator Effect of Empowerment in Relationship between Transformational Leadership and Organizational Commitment)

  • Purpose: This study was to determine the mediator or moderator role of empowerment in the relationship between the head nurse's transformational leadership and organizational commitment as perceived by staff nurses. Methods: Data was collected from 451 nurses in a tertiary hospital located in B city by means of structured questionnaires. Data was analyzed with descriptive statistics, Pearson's correlation coefficient and hierarchial multiple regression using the SPSS program. Results: Empowerment showed mediating effects between the head nurse's transformational leadership and organizational commitment as perceived by staff nurses. However, empowerment did not show moderating effects. Conclusion: Nurses' perception of empowerment, the head nurse's transformational leadership and organizational commitment have a strong relationship. This finding suggests the importance of empowering nurses to increase organizational commitment.

치매 환자를 돌보는 간호사의 역할과 업무 (Roles and Tasks of Nurses Caring People with Dementia)

  • Purpose: The research aims to identify the roles of nurses at dementia clinics or geriatric hospitals to set their tasks. Methods: This research has been conducted through literature review and focus group methodology. The field survey has been done for 195 nurses at 36 hospitals for the aged and dementia clinics from Oct. 9 to Oct. 28, 2006. Results: The nurses were identified to play eight roles as clinical specialists, educators, researchers, counselors, consultants, collaborators, leaders, managers, and advocators. They were also known to implement 27 nursing tasks and 104 nursing activities. Conclusion: The findings of this research could provide a foundation for the nursing care work as well as become a practice guideline for the nurses, at these hospitals, to implement versatile roles and tasks.

노인요양병원 간호사의 병원감염관리에 대한 인지도 및 실천도: 융복합적 접근 방안 모색 (Perception and practice of hospital infection control in nurses of geriatric hospital : for convergent approach)

  • 본 연구는 융복합적 접근을 위해 노인요양병원 간호사의 병원감염관리에 대한 인지도와 실천도를 파악하고, 제변수간의 관계를 조사하는 연구이다. Lee(2011)가 사용한 병원감염관리 설문지를 이용하여 2012년 10월 29일부터 11월 3일 까지 자료를 수집하였고, 수집된 자료는 t-test, ANOVA 및 Correlation 으로 분석하였다. 병원감염관리 실천도의 평균 점수는 인지도 평균 점수 보다 통계적으로 유의하게 낮았다. 영역별 인지도와 실천도는 손씻기 영역, 호흡기감염관리 영역, 소독 오염물품관리 영역, 감염성 폐기물관리 영역에서 통계적으로 유의한 차이가 있었으며 실천도가 인지도보다 낮았다. 인지도는 연령, 학력, 감염관리지침서 유무에 따라 차이가 있었으며, 실천도는 감염관리지침서 유무에서만 차이가 있었다. 병원감염관리 인지도와 실천도는 통계적으로 유의한 정적상관관계가 있었다. 따라서 본 연구결과는 노인요양병원 감염관리에 대한 인지도와 실천도를 높이기 위한 제도적 지원과 더불어, 지속적이고 다양한 교육방법을 통해 체계적이고 전문적이며 실제적인 교육을 제공하는 것이 필요하다는 것과 병원감염관리 능력을 증진시키기 위한 프로그램을 개발할 필요를 제기한다.

요양병원 간호사와 간호조무사의 직무만족과 자존감 및 간호업무성과에 관한 연구 (Job Satisfaction, Self-Esteem, and Nursing Task Performance among Registered Nurses and Nurse Assistants in Long-Term Care Hospitals)

  • Purpose: This study was conducted to compare job satisfaction, self-esteem, and nursing task performance between registered nurses (RN) and nurse assistants (NA) in long-term care hospitals. Method: The sample consisted of 203 nursing personnel (RN 99 and AN 104), who were working in one of 5 long-term care hospitals in 5 different areas. Data were collected by structured questionnaires from March 15 to April 30, 2010, and analyzed using the descriptive statistics, independent t-test, One-way ANOVA, and Pearson correlation coefficients with SPSS/Win 17.0 program. Result: There was no significant difference between nurses and nurse assistants in job satisfaction and self-esteem. But nurses had higher levels of nursing task performance than nurse assistants. Conclusions: These findings demonstrate the necessity of developing programs that will help to improve job satisfaction and self-esteem in nurses and nursing task performance in nurse assistants. Strategies need to be developed to maximize the value of the existing workforce without impacting on the quality of care delivered.

치매 노인환자와 간호사의 대화 분석: 대화의 구조와 연속체 형태를 중심으로 (Analysis of Conversation between Elderly Patients with Dementia and Nurses: Focusing on Structure and Sequential Patterns)

  • Purpose: The purpose of the study was to identify functional structure and patterns of dialogue sequence in conversations between elderly patients with dementia and nurses in a long-term care facility. Methods: Conversation analysis was used to analyze the data which were collected using video-camera to capture non-verbal as well as verbal behaviors. Data collection was done during February 2005. Results: Introduction, assessment, intervention, and closing phases were identified as functional structure. Essential parts of the conversation were the assessment and intervention phases. In the assessment phase three sequential patterns of nurse-initiated dialogue and four sequential patterns of patient-initiated dialogue were identified. Also four sequential patterns were identified in nurse-initiated and three in patient-initiated dialogues in the intervention phase. In general, "ask question", "advise", and "directive" were the most frequently used utterance by nurses in nurse-initiated dialogue, indicating nurses' domination of the conversation. At the same time, "ask back", "refute", "escape", or "false promise" were used often by nurses to discourage patients from talking when patients were raising questions or demanding. Conclusion: It is important for nurses to encourage patient-initiated dialogue to counterbalance nurse-dominated conversation which results from imbalance between nurses and patients in terms of knowledge and task in healthcare institutions for elders.

노인 요양병원 간호사의 환자 안전동기와 환자안전도가 환자안전관리 활동에 미치는 영향 (Effects of Patient Safety Motivation and Degree of Patient Safety on Patient Safety Management Activities of Geriatric Hospitals Nurses)

  • 본 연구는 노인 요양병원 간호사의 환자안전관리 활동 영향요인을 파악하여 노인 요양병원의 환자안전사고를 예방하기 위한 기초자료로 활용하기 위해 실시한 서술적 조사연구이다. 대상자는 노인 요양병원 간호사 220명이며, 자료수집은 2023년 2.1~2.28일까지 실시 하였으며, 수집된 자료는 SPSS 29.0 프로그램 이용하여 t-test, ANOVA, Scheffe's test, Person's correlation coefficients, Multiple linear regression으로 분석하였다. 환자 안전동기는 환자안전도(r=.41, p<.001), 환자안전도는 환자안전관리 활동(r=.18, p<.01)과 양의 상관관계를 보였으며, 환자안전관리 활동에 가장 유의한 영향요인은 환자안전도(𝛽=.21, p<.001)와 환자안전지침서(𝛽=.16, p<.001)로 나타났고, 설명력은 7.5%였다(Adj R2=.075, p<.001). 따라서 환자안전관리 활동 역량을 증진시키기 위해 안전사고 발생 이전에 사고를 미연에 방지하도록 위험예지 훈련과 함께 안전사고 후 효과적인 대처를 위한 실습교육을 강화하는 환자안전 교육 프로그램 개발과 적용을 제안한다.

요양병원 간호사의 감정노동과 직무몰입이 이직의도에 미치는 영향 (The Effects of Emotional Labor and Job Involvement on Turnover Intention of Nurses in Long-term Care Hospitals)

  • Purpose: The purpose of this study was to identify the influencing factors on turnover intention of nurses in long-term care hospitals. Methods: Data were collected from 210 nurses in 11 long-term care hospitals in B city. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and Stepwise multiple regression. Results: Turnover intention was significantly correlated with emotional labor(r=.35, p<.001) and job involvement (r=-.38, p<.001). In a multiple regression, emotional labor (${\beta}=.31$, p<.001), age (${\beta}=-.27$, p<.001), job satisfaction (${\beta}=.24$, p<.001), job involvement (${\beta}=-.23$, p<.001), and number of patients assigned(${\beta}=.14$, p=.009) were associated with turnover intention. These factors attributed to 41.2% of the total variance in turnover intention. Conclusion: Considering the results of this study, proactive educational and/or human resource management interventions need to be developed especially for those younger nurses in order to reduce emotional labor as well as to promote job satisfaction and job involvement of nurses in long-term care hospitals.

간호사의 노인간호 스트레스, 노화인식, 노화불안이 노후준비에 미치는 영향 (The Influence of Care Stress for Older Patients, Self-perceptions of Aging, Aging Anxiety on Retirement Preparation in Nurses)

  • Purpose: The purpose of this study was to explore the influence of care stress for older patients, self-perceptions of aging, and anxiety about aging on preparation for retirement in clinical nurses. Methods: This was a cross-sectional descriptive study in which participants were 222 nurses who had at least six months of work experience and were involved in caring for older patients in the last six months at the work site. Data were collected from August to September, 2020 using an online survey questionnaire. Data were analyzed using the IBM SPSS/WIN 25.0 program. Results: The mean level of preparation for retirement in the participants was moderate (3.21 out of 5 points). Among the sub-domains of preparation for retirement, financial preparation had the lowest score. Self-perceptions of aging and aging anxiety were significant predictors for retirement preparation in nurses, accounting for 16% of the variable's total variance. Conclusion: A greater level of preparation for retirement was associated with a positive perception of aging and a decreased level of aging anxiety in the clinical nurses. Further research should focus on exploration of specific determinants of financial preparation for retirement and development of intervention strategies for improving preparation for retirement in the nursing workforce.
