A Clinical Survey Study on Clinical Symptoms of Dementia Patients (치매 환자의 임상증상에 대한 조사 연구)

  • Objectives : The aim of this survey study is to analysis clinical symptoms and patterns of dementia patients. The results of this study will be used to support development of the oriental herbal medicine for dementia. Methods : Clinical data were collected from 41 dementia patients and 20 aged persons who have no cognitive impairment in 3 sites including 2 hospitals and 1 sanatorium. 'The instrument of oriental medical evaluation for dementia' is used to evaluate clinical symptoms and patterns of all subjects. Results : 1. Subjects of deficiency syndrome patterns were nearly three times more than subjects of excess syndrome patterns. 2. In dementia patient group, the average rate for 4 clinical symptoms, related with treatment principle of clear heat, was 26.9%. 3. The average rate for 4 clinical symptoms, related with condition of urine and feces, was 15.6%. 4. The average rate for 5 clinical symptoms, related with anger, irritation, anxiety and restlessness, was 40.0%. Conclusions : 1. It is needed to develop new herbal medicine for dementia focuing on clear heat, anger, irritation, anxiety, restlessness, and condition of urine and feces. 2. 'The instrument of oriental medical evaluation for dementia' has strong tendency that excessively reflects general geriatric symptoms, related to deficiency syndrome patterns. so, more clinical symptoms of excess syndrome should be added in this tool.

Illness Experience of People with Chronic Hepatitis B in Korea (한국 만성 B형 간염 환자들의 질병 경험)

  • Purpose: The purpose of this study was to explore the experiences of people with chronic hepatitis B (CHB) in Korea. The specific aim was to identify major problems that people with CHB face and strategies that they are dealing with. Methods: A grounded theory method was utilized. The data were collected by individual in-depth interviews from 12 CHB patients from one of the major hospitals in Korea. Results: After constant comparative analysis, a core category emerged as "illness management with self-reliance and will." Seven major strategies that were identified in dealing with the illness were maintaining receptive and positive attitudes; restraining excessive work and greed; searching for information; controlling illness information; adhering to practices for not spreading the viral disease; abstaining from alcohol and smoking and maintaining healthy eating habits; nd using alternative therapies. The outcomes that result from employing these strategies were identified as burden, depression and helplessness, stress for maintaining compliance, and dispirited interpersonal relationships. Conclusion: The results of this study suggest that most people with CHB in Korea have problems in psychosocial area. Thus health professionals need to provide not only informational support but also emotional one to improve quality of life of the people with CHB.

Post-Surgical Recovery Patterns of the Elderly (노인환자의 수술후 회복패턴에 관한 연구)

  • This study examined two differences in physical and psychological recovery patterns after surgery in the elderly. The sample consisted of 40 patients with abdominal surgery In five large hospitals in Seoul. The data for this study were collected from Apr. 20 to Nov. 26 by structured questionnaire, chart review and call. Physical recovery was assessed by ADL, a Cantril Ladder Scale and a Visual Analogue Scale. Psychological recovery was measured by the Geriatric depression Scale and a Cantril Ladder Scale. The data were analyzed using frequency, percentage, Pearson Correlation Coefficient, and MANOVA by SPSS/WIN. The result are as follows : 1. Physical recovery indicated significant improvement over time with the exception of ADL(F=.812 p=.449). Perceived physical health were significantly improved(F=6.189 p=.004). Pain & discomfort was significantly decreased(F=3.927 p=.025). 2. Perceived psychological health was significantly improved over time(F=20.648 p=.000), but depression showed no statistical significance improvement over time(F=1.393 p=.256). 3. There were no significant effects of sex, age, complication and combined chronic diseases on physical and psychological recovery patterns. 4. There were significant correlations between operation time and pain(r=-.331 p=.020), recovery time and perceived psychological health(r=-.320 p=.024), recovery time and pain(r=.404 p=.005). There were significant correlations between admision period and ADL(r=-.418 p=.004), perceived physical health(r=-.354 p=.014), depression(r=.280 p=.042), and perceived psychological health(r=-.447 p=.002). BRAS showed significant correlation with ADL(r=-.458 p=.002). 5. With an increase in the degree of perceived health(physical and psychological), ADL was significantly increased. With an increased in the degree of depression and pain, ADL and perceived health(physical and psychological) were significantly decreased. In conclusion, the elderly patient recovered significantly over time with the exception of ADL and depression. It these we suggested to considered when planning care for elderly patients.

Factors Associated with Vancomycin-Resistant Enterococcus Colonization in Patients Transferred to Emergency Departments in Korea

  • Background: Vancomycin-resistant enterococci (VRE) infections have become a major healthcare-associated pathogen problem worldwide. Nosocomial VRE infections could be effectively controlled by screening patients at high risk of harboring VRE and thereby lowering the influx of VRE into healthcare centers. In this study, we evaluated factors associated with VRE colonization in patients transferred to emergency departments, to detect patients at risk for VRE carriage. Methods: This study was conducted in the emergency department of a medical college-affiliated hospital in Korea. Every patient transferred to the emergency department and admitted to the hospital from January to December 2016 was screened for VRE using rectal cultures. In this cross-sectional study, the dependent variable was VRE colonization and the independent variables were demographic and clinical factors of the patients and factors related to the transferring hospital. Patients were divided into two groups, VRE and non-VRE, and previously collected patient data were analyzed. Then we performed logistic regression analyses of characteristics that differed significantly between groups. Results: Out of 650 patients, 106 (16.3%) had positive VRE culture results. Significant variables in the logistic analysis were transfer from geriatric long-term care hospital (adjusted odds ration [aOR]: 8.017; 95% confidence interval [CI]: 1.378-46.651), hospital days (4-7 days; aOR: 7.246; 95% CI: 3.229-16.261), duration of antimicrobial exposure (1-3 days; aOR: 1.976; 95% CI: 1.137-3.436), and age (aOR: 1.025; 95% CI: 1.007-1.043). Conclusion: VRE colonization in patients transferred to the emergency department is associated primarily with factors related to the transferred hospitals rather than demographic and clinical characteristics.

The Effect of Communication Competency and Job Stress of Long Term Care Hospital Nurses on Turnover Intention focused on control effect of emotional intelligence (요양병원 간호사의 의사소통능력, 직무스트레스가 이직의도에 미치는 영향 : 감성지능 조절효과 중심으로)

  • This study examined the effect of communication competency and job stress of nurses working in long term care hospitals on turnover intention and to verify the effect of emotional intelligence control on the relationship among communication competency, job stress and turnover intention. The subjects consisted of 240 nursing students, and data were collected during from February 1 to February 28, 2018. The data were analyzed with descriptive statistics and Pearson's correlations that used IBM SPSS WIN 20.0 program and the analysis of control effect was made by using Hierarchical Multiple Regression Analysis. The result of this study can be summarized as follows. The turnover intention was positively correlated with job stress. The factors which had effect on turnover intention were pay satisfaction, job stress and age. Based on the results of this study, it is necessary to develop a systematic program that reflects clinical characteristics and education in order to reduce the turnover intention of the nurse.

Nurses' Knowledge, Attitude and Education Needs for Sexuality of the Elderly (간호사의 노인의 성에 대한 지식, 태도 및 성교육 요구도)

  • The purpose of this study was to investigate nurses' sexual knowledge, attitude and sex education needs of the elderly. The participants were 235 nurses who were working at hospitals, geriatric hospitals and nursing homes at I city, K city and J city in Jeollabukdo province. A survey was conducted using self questionnaire scale from December 1, 2012 to December 10, 2012. First, the mean score of sexual knowledge was $15.91{\pm}6.43$. It was statistically significant according to marital status(p=.024), workplace(p<.001), duration of work experience(p=.006), sex education needs(p<.001), sex education acceptance(p<.001). Second, the mean score of attitude was $38.26{\pm}4.41$. It was statistically significant according to workplace(p=.033), living with the elderly(p=.009), sex education needs(p<.001) and sex education acceptance(p<.001). Third, the mean score of sex education needs was $35.97{\pm}6.28$. It was statistically significant according to age(p=.023), living with the elderly(p=.014), sex education needs(p<.001) and sex education acceptance(p<.001). Nurses' knowledge on the elderly's sexuality showed a significantly positive correlation with attitude(p<.001) and sex education needs(p<.001). This study indicated that general education on sexuality of the elderly for undergraduate nursing students and registered nurses is essential. More active nursing care on the elderly's sexuality is required when providing them with health assessment and nursing interventions.

A Survey of Needs and Types of Home Physical Therapy, Visiting Physical Therapy and School Physical Therapy (가정.방문물리치료 및 학교물리치료의 필요성 및 유형실태에 대한 조사연구)

  • The purpose of this survey was to give data and information about type and needs of Home Physical Therapy, Visiting Physical Therapy and School Physical Therapy for physical and nurse. The subjects were 154(99 physical therapists and 55 nurses) who were working at geriatric rehabilitation hospitals and children hospitals. The period of questionary collection was from the 15 of August to the 15th of September 2011. And data was analysis from 99 articles such as journals related to physical therapy, and searched with keyword 'home and visiting physical therapy' by web site and Korea National Assembly Library from 1991 to 2011. The data was analysis with percentage, mean, standard deviation and ANOVA by SPSS PC 12.0. The results were as follows; 1. The definition of 'Home Physical Therapy' has been community based on physical therapy service for the patient who had diagnosis by medical doctor, has been based on medical law. The definition of 'Visiting Physical Therapy' has been community based on physical therapy service at home for the patient who had diagnosis by medical doctor, for the national basic living security, and senior citizen over 65 years who lives alone, has been based on law for community health and law of long term health insurance. The definition of 'School Physical Therapy' has been school based on physical therapy service at school after class for the disabled children who are studying at school, has been based on special education law article 28. 2. As for the knowledge of the Home and Visiting and School Physical Therapy, both groups PT and nurse were 'I do not know'125(81.3%) of the difference the concept of 3 definitions, so it means to need education and information about the different concept of three physical therapy. As for the needs of home and visiting physical therapy, both groups of PT and Nurse were 'needs' 151(98.1%). Physical therapist showed of 'Needs' on visiting physical therapy 35(35.4%), home physical therapy 32(32.3%), and schole physical therapy 32(32.3%). Nurse showed of 'Needs' on home physical therapy 23(41.8%). visiting physical therapy 19(34.5%), school physical therapy 13(23.6%). Therefore it is necessary to have home and visiting physical therapy as for the elderly and disabled person. 3. As for the qualification of Home and Visiting physical therapist, both PT and nurse groups showed as follows; take post graduation education program for home and visiting therapy after became PT : home physical therapist 108(70.1%), visiting physical therapist 106(68.8%). So it means education center or university can be developed post graduation program for home and visiting physical therapist. 4. As for the 'Needs' of school physical therapy, both groups of PT and nurse showed as follows; 'Needs' 142(92.2%), 'Needs superviser education program' 148(96.1%), in PT group showed 'I will participate of education program' 92(92.9%). 5. As for the present states of research papers or report of home, visiting, and school physical therapy was as follows; the 103 papers for 8 fields about' the needs of home and visiting physical therapy' from 1991 to 2011, the 13 papers for 2 fields about school physical therapy from 2001 to 2011, so total papers were 114 articles.

A Study on the Major Factors Affecting Health-Related Quality of Life of Elderly Stroke Survivors (뇌졸중 생존 노인들의 건강관련 삶의 질 영향 요인)

  • The main purposes of the study were to identify factors affecting health-related quality of life of elderly stroke survivors, and to suggest social work interventions for improving their quality of life. The participants were 328 elderly stroke survivors over 55 years old recruited from 18 long-term care hospitals and 13 nursing care facilities in Chung-Nam Province, Korea. The data was collected through a direct survey method from Dec. 16, 2008 to Jan. 17, 2009. Statistical analysis revealed that the overall health-related quality of life among participants was relatively low, especially in the social relationships domain. As a result of hierarchical multiple regression analysis, depression emerged as the most significant predictor of elderly stroke survivors' health-related quality of life, followed by financial burden in future, activities of daily living, acceptance of disability, comorbidity, and major financial supporter(spouse). Health-related quality of life of elderly stroke survivors in long-term care facilities was affected by a variety of variables from disease-related factors to environmental ones. Particularly psychosocial variables were the most powerful factors to predict HRQOL of elderly stroke survivors. Based on the results, social work implications were discussed in the context of developing clinical strategies to alleviate elderly stroke survivors' health-related quality of life in long-term care facilities. Living with chronic illness and disability is a great challenge to the elderly. Most of all long-term residence in care facilities can cause deterioration of quality of life in old age. From now on, studies on health-related quality of life of elderly with chronic illness and disability should be activated in social work field.

An identification of determinants to the development of intrapreneurial intention in small & medium sized local hospital in South Korea

  • Purpose: The present study identified the determinants in the development of intrapreneurial intention in small and medium-sized local hospitals. A careful literature review led to the development of a conceptual model which identified two types of employee competence-individual competence and managerial competence-to influence intrapreneurial orientation positively. It was hypothesized that intrapreneurial orientation predicts intrapreneurial intention and is mediated by intrapreneurial commitment. Methodology/Approach: The target population was chosen from two medical institutions of 'D' Hospital and 'E' Geriatric Hospital in Changwon City, South Korea. Samples were collected from 299 respondents who completed a structured questionnaire. Findings: The results from a structural equation modeling statistical analysis indicated that (1) individual competence and managerial competence positively and significantly predict intrapreneurial orientation, (2) intrapreneurial orientation positively and significantly influences intrapreneurial intention, (3) intrapreneurial commitment partially mediates the relation of intrapreneurial orientation to intrapreneurial intention, and (4) the mediation effect of intrapreneurial commitment was significant in the medical-personnel group, but not in the non-medical group. Practical Implications: Overall findings from the present work provide vital insights into understanding the preconditions for developing employee intrapreneurship in small and medium-sized local hospitals.

Job importance, job performance, and job satisfaction in dietitians at geriatric hospitals or elderly healthcare facilities in Jeju (제주지역 요양 (병)원 영양사의 직무중요도, 직무수행도 및 직무만족도 분석)

  • Purpose: This study analyzed job importance, job performance, and job satisfaction in 38 dietitians working at geriatric hospitals and elderly healthcare facilities in Jeju surveyed from September 15-24, 2014 with the aim of providing basic data for improving the quality of meals and nutrition management for elderly patients. Methods: Data were analyzed using descriptive analysis, ${\chi}^2$-test, t-test, ANOVA, and Pearson's correlation coefficients using the SPSS Win program (version 12.0). Results: Regarding job importance, the average score was 4.29 (out of 5), indicating that hygiene and safety management scored the highest at 4.77 (out of 5), and nutrition management scored the lowest at3.86. In terms of job performance of subjects, the average score was 2.87 (out of 5), indicating that cooking operation management scored the highest at 4.42 (out of 5). Regarding the Importance-Performance Analysis (IPA) of job importance and job performance, hygiene and safety management and cooking operation management scored high for importance and performance (B quadrant) menu management, human resource management, and nutrition management scored low for importance and performance (C quadrant) and purchasing management and financial management were included inD quadrant and A quadrant, respectively. For the level of job satisfaction of subjects, the average score was 3.37 (out of 5), indicating that relationships with colleagues scored the highest at 3.72, and improving professionalism scored the lowest at 2.95. Additionally, job importance and performance of subjects were positively correlated withjob satisfaction (r = 0.395, r = 0.386, both p < 0.05). Conclusion: In conclusion, scores for job importance and job performance of nutrition management were low, and job satisfaction of improving professionalism scored low. Therefore, continuous training and education programs of nutrition management should be provided to improve professionalism of dietitians at geriatric hospitals and elderly healthcare facilities.
