임플란트 경부형상이 주위골 응력에 미치는 영향에 관한 유한요소법적 분석 (Finite element analysis of peri-implant bone stress influenced by cervical module configuration of endosseous implant)

  • 연구목적: 임플란트 경부의 치은관통부 형상이 주위골 응력분포에 미치는 영향에 대해 조사하고자 한다. 연구재료 및 방법: 높이 2.8 mm, 상부 직경 4 mm, 하부 직경 2.7 mm 인 직선형 치은관통부를 가지는 ITI의 일체형(one piece) 임플란트 (Straumann, Waldenburg, Switzerland)를 Base Model로 사용하여, 치은관통부 외형에 함몰부를 부여하여 곡선형으로 수정한 4개의 해석 모델 (Model-1, -2, -3, -4)을 설정하였다. Base Model을 포함, 모두 5개의 경우에 대해 축대칭 유한요소모델링을 통해 임플란트 장축에 평행인 수직 방향과 임플란트 장축에 $30^{\circ}$ 경사진 방향으로 각각 50 N의 힘이 작용할 때 발생되는 임플란트 주위골의 응력을 해석하여 비교하였다. 체계적인 응력비교를 위해 임플란트 주위에 19개의 절점을 응력 관찰점으로 선정하였으며, 경부 치밀골에 설정된 5개 관찰점의 응력으로부터 회귀분석법으로 임플란트/골 사이에서 생기는 최대응력값을 추정하여 정량적인 비교를 실행하였다. 결과: 최대 골응력은 치은관통부가 직선인 기본모델에서 가장 컸으며, 치은 관통부를 곡선으로 설계한 경우 응력이 감소되었다. 치은 함몰부가 클수록 응력감소 정도가 커졌으며 함몰부의 수직위치가 몸체부에 가장 가까운 Model-4에서 응력감소 정도가 전체의 약5%로 가장 컸다. 결론: 임플란트의 경부 형상은 골응력에 영향을 미치며, 이를 곡선형으로 함으로써 또한 그 함몰부를 몸체부에 근접하게 함으로써 경부골 응력감소를 효과적으로 도모할 수 있다.

객체 형상 및 속성정보 지침에 따른 수목 BIM 라이브러리 개발 (Development of Plant BIM Library according to Object Geometry and Attribute Information Guidelines)

  • 건설분야의 전면 BIM 도입을 위한 정부정책이 실행 중인 가운데, 조경 BIM 모델의 구축 및 활용은 아직까지 저작도구의 한계, 자연소재 모델링의 어려움, 라이브러리 등 BIM 콘텐츠 부족 등의 문제로 어려움을 겪고 있다. 특히 수목은 조경분야의 전문성을 대표하는 설계요소로서 BIM 모델 제작시 반드시 포함되어야 하지만 모델링 과정에서 종종 생략되거나 필요한 정보가 포함되지 않아 BIM 데이터 품질을 저하시킨다. 이에 본 연구에서는 BIM 표준을 준수하고 조경실무에서 상용화할 수 있는 수목 라이브러리를 개발함으로써 조경 BIM 모델의 구축과 활용에 도움을 주고자 하였다. 이를 위해 Revit을 기반으로 수목의 3D 형상과 속성정보 항목을 선정하여 교목과 관목의 라이브러리를 개발하였다. 형상정보는 수종 고유의 특징을 표현하되 단순화하여 LOD200, LOD300, LOD350 수준으로 작성하였고, 속성정보는 수목의 품명, 규격, 물량산출에 필요한 정보 외에도 생태적 속성정보와 환경성능 정보를 포함하여 총 24개의 항목을 포함시켰다. 파일은 객체명만으로도 조경분야의 객체로서 위계가 드러나고 수목의 구분이 이루어지도록 명칭을 부여하였다. 이렇게 개발한 수목 라이브러리를 활용하여 공동주택단지의 조경 BIM 모델을 작성하고 사용성을 검토하였다. 그 결과, 수목 라이브러리를 사용하여 조경 BIM 모델을 효과적으로 구축할 수 있었으며 도면 작성, 물량 산출, 디자인 검토 등 BIM 모델의 기본적인 활용방안에서 적절히 운용되는 것을 확인할 수 있었다. 본 연구에서 개발한 라이브러리에는 지피초화류가 제외되었으며, 수목의 환경성능 등 다양한 데이터베이스가 아직 구축되지 않아 속성정보에 변수들이 탑재되지 않았다는 한계가 있다. 앞으로 자연소재에 대한 BIM 모델링 도구 및 기술 개발, 다양한 데이터베이스 구축에 대한 노력이 필요하며, 라이브러리의 제작, 관리, 보급 및 유통을 담당할 주체와 시스템을 마련해 나가야 한다.

Co-60에 의한 전신조사시 선량분포 (Dose Distribution of Co-60 Photon Beam in Total Body Irradiation)

  • 골수이식을 받게 될 환자의 이상 골수를 완전히 죽이기 위해 MV 정도의 선질의 광자선에 의한 전신방사선요법이 시행되고 있다. 국소방사선요법에 이용되고 있는 방사선치료장치에 의한 일상적인 방법으로 환자의 전선에 걸쳐 방사선을 조사하기에는 조사면의 크기가 훨씬 미치지 못한다. 그래서 환자의 전선에 걸쳐 방사선을 조사할 수 있는 방법이 개발되어야 한다. 방사선 전신조사를 위한 여건이 병원에 따라 다를 것이기 때문에 병원에 따라 독특한 방법이 개발될 수 있다. 서울대학교병원에서는 코발트치료기 만이 두부를 기울일 수 있어서 전신조사에 이용될 수 있다. 코발트치료기의 두부를 밖으로 90$^{\circ}$ 기울일 때 선축은 수평이고 또한 맞은편 벽과 직각이 된다. 이 때 선원에서 맞은편 벽가지 거리는 319cm 이였다. 벽에서 환자의 중앙시상면까지 간격을 40cm라고 가정할 때, 중앙시상면에서 명목상 최대 조사면 크기가 122cm$\times$122em 이였고, 조사선량 분포를 측정한 결과로는 130cm$\times$129cm 이였으며 상하방향에서는 대칭이 아니였다. 환자가 쭈그리고 앉은 자세를 취한다면 조사면의 크기는 전신조사를 시행할 수 있을 정도로 충분히 크다. 환자 좌우폭의 평균을 30cm 라고 가정하고, 중앙시상면에서 선원쪽 15cm 위치에 기준표면 (SSD는 264cm, 명목상 조사면 크기 115.5cm$\times$155.5cm)을 두고 단면의 크기가 25cm$\times$25cm이고 두께가 30cm 인 폴리스티렌 팬톰에서 평판형 전리함으로 PDD를 측정하였다. 최대선량점의 깊이는 0.3cm 이였고 표면선량율은 82%, 50% 깊이는 16.9cm였다. 대향조사시 선축상 선량분포는 중점의 선량에서 10%이내로 일치하였다. SCD를 279cm. 최대 조사면, 기준깊이 15cm 에 대한 TPR 을 폴리스티렌 팬톰에서 깊이 10cm 에서 20cm 에 걸쳐 측정하였다 . 측정범위에서 TPR 은 직선성을 보였다. 인체팬톰의 최대 전단면(coronal plane) 에 있는 각 구멍에 TLD 조각을 넣고, 코발트 선원에서 팬톰의 시상면까지 거리를 279cm 되게 하고 선축은 팬톰의 27번 절편과 28번 절편의 접변과 최대 전단면의 교차선과 일치시켜 양방향에서 15분씩 조사하여 전단면에서 선량을 측정하였다. 팬톰내 선축상 중앙점의 선량을 기준으로 다른 부위의 선량을 비교하였다. 두경부와 복부, 폐의 하반에서 선량의 차이는 $\pm$ 10% 이내였고, 폐의 상반과 어깨와 골반 부위에서 선량은 10%이상 저선량을 보였다. 특히 어깨부위에는 30%이상 저선량을 보였다. 이로부터 서울대병원과 유사한 조건에서 코발트로 전신조사하는 경우에는 폐나 두경부에 대응하는 조직보상체를 이용하기보다는 어깨부위에 선량을 추가하는 것이 바람직할 것이라고 생각한다.

토모테라피 치료 시 MVCT Image의 Slice Thickness 차이에 따른 선량 비교 (Extra Dose Measurement of Differential Slice Thickness of MVCT Image with Helical Tomotherapy)

  • 의료용 선형가속기 (linear accelerator)와 나선형 컴퓨터 단층 촬영 장치 (helical computed tomography scanner)의 결합 장치인 토모테라피는 세기 변조 방사선 치료 (intensity modulated radiation therapy(IMRT))의 큰 혁신을 이끌었다. 토모테라피 치료 과정에서, Megavoltage computed tomography (MVCT) 영상획득은 치료 환자의 정확한 자세 정렬을 위해 치료 전 또는 후에 이용된다. 그러나 이는 환자의 총 선량을 증가시키는 결과를 만들며, 본 연구는 이처럼 MVCT 영상 획득 시 증가되는 선량을 Cylindrical "Cheese" Phantom을 이용하여 측정, 비교하였다. 각각의 pitch 별로 (1, 2, 3 mm) 동일한 개수의 slice (10 slice), 그리고 동일한 length (약 9 cm)를 scanning하여, 이때의 선량 (MVCT Scanning Dose)을 A1SL ion chamber를 이용하여 측정하였다. 측정 결과 동일한 Slice 개수 (각각의 pitch 당 10개)일 때, MVCT scanning dose의 평균값은 각각 (pitch 1, 2, 3mm) 2.24 cGy, 1.02 cGy, 0.81 cGy가 측정되었다. 동일한 length에서 MVCT scanning dose의 평균값은 각각 (pitch 1, 2, 3mm) 2.47 cGy, 1.28 cGy, 0.88 cGy가 측정되었다. 이는 할당된 pitch와 scanning length가 MVCT scanning dose에 큰 영향을 미치는 가장 중요한 매개 변수임을 말하며, pitch는 MVCT scanning dose와 역비례 관계를 나타냈다. 때문에 적절한 pitch와 scanning length의 선택으로 치료 선량 외의 추가 선량을 최소로 줄여야 하겠다.

온도자극이 충전된 치질에 미치는 영향 (THE EFFECTS OF THERMAL STIMULI TO THE FILLED TOOTH STRUCTURE)

  • 본 연구에서는 수복에 따른 치질 내부 구조물의 열적변화를 평가하기 위하여 와동이 형성된 인공 치아모형에 아말감충전, 아말감과 스테인레스 스틸관 수복, 금인레이와 금관수복, ZOE 이장 및 아말감충전 등으로 수복한 후 온도변화를 관찰하였다. 각 실험군에 $4^{\circ}C$$60^{\circ}C$로 2초와 4초 동안 자극을 가한 후 치아내부의 온도분포를 2차원적으로 분석하였고, 치수 중앙부에서 외측으로 층을 형성하여 3차원적인 해석을 시행한 결과, 다음과 같은 결론을 얻었다. 1. $4^{\circ}C$의 자극이 2초간 가해진 아말감 충전 경우, 아말감 충전과 스테인레스 스틸관을 장착한 경우에서는 $1^{\circ}C$ 내외의 차이를 보였으며, 치수와 상아질 경계면에서 3초 경과시에 $29^{\circ}C$까지 급격한 온도 하강을 보였으며 9초 후에 $25^{\circ}C$에 도달하였다. 또한 금으로 수복한 경우에서는 3초 후 $25^{\circ}C$까지 하강하여 그 온도를 유지하였으며, ZOE 이장 경우에서는 최저온도가 $4^{\circ}C$ 이상 높게 나타났다. 2. $4^{\circ}C$ 자극을 4초간 가한 경우에서는 9초 후에서 2초 자극시보다 $2-3^{\circ}C$ 낮았으며, 금 수복 경우에는 5초 후에 $21^{\circ}C$의 최저온도를 보인 후 점차 회복되었다. 3. 아말감 충전 및 스테인레스 스틸관을 같이 장착한 경우에 $60^{\circ}C$, 2초간 자극에는 3초 후 $40^{\circ}C$의 온도로서 상승하였으나 9초 후에 $30^{\circ}C$로 하강 하였으며, 금 수복에서는 2초 후 $41^{\circ}C$까지 온도가 상승하였고, 9초 후 $28^{\circ}C$를 보여 $13^{\circ}C$의 차이가 있었다. ZOE 이장 경우에는 온도차이가 $5^{\circ}C$로서 안정된 양상을 보였다. 4. $60^{\circ}C$, 4초 자극에 대한 아말감 충전 및 스테인레스 스틸관을 같이 장착한 경우에서 5초 후 $42^{\circ}C$, 9초 후 $35^{\circ}C$를 보였으나 금 수복 경우에서는 3초 후 $49^{\circ}C$, 9초 후 $31^{\circ}C$의 온도변화를 나타내었다. 5. 3차원 분석에서 치수 중앙부에서 멀어질수록 온도변화가 심하였다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
