PET/CT 검사에서 SharpIR 재구성 방법의 평가 (Evaluation of SharpIR Reconstruction Method in PET/CT)

  • 서론: 종래의 PET 영상 재구성에 있어서 FBP 등에 비해 3차원 반복 재구성 방법이 일반적으로 대체하고 있으며, 이것은 검출기 기하학적 특성과 완벽한 3차원 산란 평가 및 저잡음 randoms 평가 등의 더 진보된 재구성 알고리즘을 제공하고 활용되고 있다. 최근에 SharpIR알고리즘은 3차원 반복 재구성 알고리즘으로 PET 검출기 응답 정보를 통합하여 PET 영상의 잡음을 효과적으로 감소시켜 대조도를 향상 시키기 위한 것으로 알려지고 있다. 본 연구에서는 새로운 반복 시스템 모델인 SharpIR에 대한 성능 평가와 임상에서의 적용 가능성에 대해 알아보고자 한다. 실험재료 및 방법: 검출기 응답에 대한 분해능을 측정하기 위해 유리관(내경 1.1 mm, 두께 0.2 mm)에 $^{18}F$-FDG (250 MBq/mL)을 주입하여 축 방향 시야의 중심과 축 방향으로 5, 10, 15, 20 cm만큼 떨어진 지점에서 획득하였고 VUE point HD와 VUE point HD-SharpIR로 재구성하여 각각의 영상에서 반치폭을 구하였다. 또한 영상품질평가로 image quality phantom (NU2-2001)을 이용하여, 여러 개의 각각 다른 반지름을 가지는 원형구에 cold (직경 28, 37 mm)와 ho (직경 10, 13, 17, 22 mm)부분을 나누어 배경잡음을 주고 영상의 대조도를 평가하였다. 획득된 영상은 VUE point HD와 VUE point HD-SharpIR로 재구성을 하였다. 임상실험에서는 전신검사를 시행받은 환자 중 병소가 있는 환자 10명을 대상으로 VUE point HD와 VUE point HD-SharpIR로 재구성하였다. 이때 iterations을 1~10까지 변경하여 병소 부위와 간 부위에 관심영역을 설정하여 대조도를 평가하였다. 결과: VUE point HD로 재구성한 영상에서는 시야 중심으로부터 축방향 거리 증가와 함께 반치폭이 함께 증가하였지만 VUE point HD-SharpIR로 재구성한 영상에서는 거리가 증가하여도 일정한 반치폭을 나타냈다. 대조도는 팬텀 실험과 임상 실험에서 VUE point HD-SharpIR이 VUE point HD보다 대조도의 향상을 나타냈다. 결론: 검출기 시스템 응답에 대한 더 많은 정보를 포함시킴으로써 SharpIR 알고리즘은 VUE point HD에서 사용되는 기본 모델의 정확성을 향상시켰다. 또한 SharpIR은 VUE point HD보다 각각의 복셀에 관련된 더 많은 측정 위치를 가지는 시스템 모델이기 때문에 더욱 정교한 재구성 모델의 결과를 나타내기 위해 더 많은 반복이 걸린다. 결론적으로 SharpIR은 PET 영상에서 대조도를 향상시켰고 임상에서 적용할 수 있는 최적화된 재구성 조건을 알아보기 위해 종단적 연구를 통해 적용한다면 임상에서 유용하게 사용될 것이다.

골 밀도 평가를 위한 뼈의 전기 전도도 재구성: 시뮬레이션 (An Electrical Conductivity Reconstruction for Evaluating Bone Mineral Density : Simulation)

  • 골다공증은 골 조직의 량이 감소하여 골절의 가능성이 증가하는 상태이다. 뼈의 전기적 특성은 골 밀도와 관련이 있으며, 특히 뼈의 절기 저항은 골 소실이 증가할수록 감소하는 것으로 알려져 있다. 만일 신뢰성 있는 측정이 가능하다면, 뼈의 전기적인 특성은 골다공증 진단을 위한 유용한 변수가 될 수 있음을 의미한다 본 연구에서는 EIT (electrical impedance tomography) 기술을 이용하여 뼈의 전기 전도도 평가를 시도하였다. 뼈와 주위의 연조직 간의 전기적인 특성의 차이가 크기 때문에 (100 정도), 일반적으로, 뼈의 EIT를 얻기는 쉽지 않다. 본 연구에서는 이상 경계 검출을 위해 개발된 adaptive mesh regeneration 기법을 이용하여, 주어진 경계면의 기하학적 조건하에서, 경계 내부의 전기전도도를 복원할 수 있도 록 하였다. 수치 해석은 균일한 타원형 (장경 17 mm, 단경 15 mm) 뼈와 주위의 연 조직으로 구성된 반경 40 mm인 원통형 팬텀으로 모의한 경골에 대해 수행하였다. 타원형 뼈는 팬텀의 원형 단면 중심에서 15 mm 위쪽에 위치하도록 하였다. 연 조직의 전기 전도도는 4 mS/cm로 고정하였고, 뼈에 대해서는 0.01 - 1 mS/cm에서 변하도록 하였다. 측정 오차에 의한 효과를 분석하기 위해 시뮬레이션은 측정 오차를 포함하도록 하였다. 시뮬레이션 결과, 만일 측정 오차가 5 % 미만으로 유지될 경우, 재구성된 뼈의 전기전도도는 10 % 이내의 오차를 갖는 것으로 나타났다. 예측할수 있듯이 복원의 정확도는 뼈의 전기 전도도가 증가함에 따라 증가하였다. 이것은 사용된 EIT가 골 소실이 진행된 뼈에 대한 좀더 정확한 정보를 제공할 수 있음을 시사한다. 본 연구는, 뼈의 해부학적인 정보를 알고 있을 때 뼈와 주위의 연조직에 대한 단순한 이상 영상에 한정된 결과를 제공하지만, EIT 기술이 골다공증 골절을 예측하는 새로운 방법으로 활용될 가능성을 시사한다.

재발한 Vulvar 종양의 근접치료 시 Modified Mupit Applicator의 적용 (Application of Modified Mupit for the Recurrent Vulva Cancer in Brachytherapy)

  • 목 적: 재발한 vulva 종양의 근접 치료 시 정상조직의 장애와 종양 체적 내의 선량 균등도를 개선하기 위해 modified MUPIT applicator를 자체 고안하였으며 이에 대한 유용성을 평가하고자 한다. 대상 및 방법: Modified MUPIT applicator는 template, cylinder, interstitial needle로 구성 되었으며, 종양 체적을 정하기 위하여 치료 전 CT를 시행하였다. CT 영상을 이용하여 Interstitial needle의 삽입 위치를 결정하고, 수술실에서 template를 치료 부위에 고정을 시키고, cylinder를 vaginal cavity에 삽입한 후, interstitial needle을 종양 체적 내에 삽입하였다. 삽입된 interstitial needle의 정확한 위치를 확인하기 위하여 확인용 CT를 시행하였으며, simulation을 통하여 얻어진 orthogonal film을 이용하여 치료 계획을 수립하였다. 일일 종양 선량은 600 cGy, BID로 3,000 cGy를 조사하였으며, 치료 시 직장 선량을 평가하기 위하여 TLD를 이용하여 anal verge를 기준으로 5개 지점에서의 직장 선량을 측정하였다. 결 과: Modified MUPIT applicator 대한 유용성을 평가하기 위하여 측정된 직장 선량은 34.1 cGy, 57.1 cGy, 103.8 cGy, 162.7 cGy, 165.7 cGy로 측정되었으며, 외부방사선치료(EBRT: External Beam Radiation Therapy), 강내방사선치료(ICR: Intracavitary radiotherapy)와 중첩되는 지점은 34.1 cGy, 57.1 cGy로 매우 우수하게 평가되었다. 결 론: 자체 제작한 modified MUPIT applicator 사용하여 조직 내 근접치료(Interstitial brachytherapy)를 시행함으로써, 외부방사선치료로 접근하기 어려운 종양에 대하여 선량 균등도를 효율적으로 개선할 수 있었고, 직장 선량을 최소화하여 직장의 부작용 발생 확률을 현저히 감소시킬 수 있었다.

콘빔 전산화단층촬영(CBCT) 시스템에서 기계적 오류에 관한 연구 (A Study on Mechanical Errors in Cone Beam Computed Tomography(CBCT) System)

  • 본 연구는 선형가속기의 회전불균형의 영향으로 발생 되는 CBCT 영상의 setup 오차 변화를 분석하여, 회전불균형에 대한 정도관리의 필요성을 연구 하였다. CBCT 시스템의 3차원 체적영상모드를 이용하여 $360^{\circ}$ 회전과 $180^{\circ}$ 회전으로 Catphan503 팬텀과 균질 팬텀의 3차원 영상을 획득하였고, setup 오차를 측정하기 위해 나선형 CT의 기준영상과 함께 비교 분석 하였다. 표준 정도관리의 절차를 시행하여 정상적인 상태를 확인하고, 임의적으로 갠트리의 회전균형을 조절한 후, 균형상태와 불균형상태의 각각 CBCT 영상에 대하여 X, Y, Z, Roll, Pitch, and Yaw의 6차원적 관점에서 setup 오차를 측정하고 분석하였다. Setup 오차의 변화율은 갠트리 회전균형의 조정 전 후를 확인한 결과, 직교 좌표계는 $360^{\circ}$의 회전에서 X축 방향으로 0.6 mm, Y축 방향으로 0.5 mm, Z축 방향으로 0.5 mm의 최대 변화율을 보였다. $180^{\circ}$의 회전은 X축 방향으로 0.9 mm, Y축 방향은 0.2 mm, Z축 방향은 0.3 mm의 최대 변화율을 보였다. 또한 회전 변환계는 회전 불균형이 커질수록 평균적인 값의 차이가 점점 증가하였고, 최대값은 $1.1^{\circ}$를 나타내었다. 영상의 분해능은 균형 조정 전 후 영상분석 툴에서 2 lp/cm 차이를 나타내었다. CBCT 시스템의 정도관리는 기준 권고안을 만족하였다. 갠트리의 회전 불균형이 클 때, setup 오차의 직교 좌표계의 변화는 크지 않았으나, 회전 변환계의 변화율은 기준 권고 값인 $1^{\circ}$에 해당하는 값과 초과하는 $1.1^{\circ}$를 나타내었다. 이는 갠트리의 불균형이 setup 오차에 영향을 미치는 것을 알 수 있었으며, 또한 회전 변환계의 setup 오차에 대한 6차원적인 보정이 더욱 요구됨을 알 수 있다. 그러므로 정확하고 정교한 영상유도 방사선치료를 수행하기 위해서는 갠트리의 균형도를 확인해야 하며, 정도관리 항목으로 추가되어야 한다고 사료되어진다.

한반도 식생에 대한 MODIS 250m 자료의 BRDF 효과에 대한 반사도 정규화 (A Reflectance Normalization Via BRDF Model for the Korean Vegetation using MODIS 250m Data)

  • 지표변수는 지면 근처의 기후변화에 중요한 역할을 하기 때문에, 충분히 높은 정확성을 가진 값이 산출되어야 한다 하지만 지표 반사도는 강한 이방성(non-Lambertian) 특징을 가지고 있기 때문에, 위성 천저각으로부터 멀어질수록 태양-지점-위성과의 기하학적 영향을 더욱 강하게 받는 효과를 가져온다. 또한 지표 각 영향을 포함하고 있는 지표 반사도는 노이즈를 가지게 된다. 따라서 본 연구의 목적은 한반도 지역의 MODIS 반사도 자료(250m)를 이용하여 각 영향이 제거된 보다 정확한 반사도 값에 대한 데이터베이스를 제공하는 것이다. 본 연구에서는 매일 2회씩 제공하는 MODIS(Moderate Resolution Imaging Spoctroradiometer) 센서의 가시영역과 근적외영역의 반사도(250m)자료를 이용하였다. 먼저 구름화소를 제거하기 위해서 연속적인 물리과정을 통하여 각각의 구름 화소를 제거하였다. 그리고 지리보정은 MODIS 센서에서 제공하는 지리정보자료를 이용하여 2차 다항회귀식을 통한 최근접 내삽법을 사용하였다. 본 연구에서는 지표 이방성 효과를 보정하기 위해서 반 경험적 양방향성분포함수(BRDF) 모델을 사용하였다. 이 알고리즘은 위성으로부터 관측된 위성천정각, 태양천정각, 위성방위각, 태양방위각과 같은 각 성분을 이용하여 Kernel-deriven 모델의 역변환을 통하여 지표 반사도를 재생산한다. 먼저 우리는 BRDF 모델을 수행하기 위해 총 31일 모델 관측 실행기간을 고려하였다. 다음 단계로 각각의 화소 및 밴드에 대해서 BRDF 모델을 통하여 분리된 각 성분들을 변조함으로써 위성 직하점 반사도 정규화가 수행되었다. 모델을 이용하여 산출된 반사도 값은 실제 위성 반사도 값과 잘 일치하였고, RMSE(Root Mean Square Error)값은 전체적으로 약 0.01(최고값=0.03)이였다. 마지막으로, 우리는 한반도 지역에 대해서 2001년 동안 총 36개로 구성된 정규화 지표반사도 값의 데이터베이스를 구축하였다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter. At the first stage, background noise is removed and at the next stage, only spectrum of QRS complex components is passed. Two adaptive filters can afford to keep track of the changes of both noise and QRS complex.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
