Gentamicin is a broad-spectrum aminoglycoside antibiotic used in the treatment of bacterial infection. Although side effects of gentamicin such as nephrotoxicity and ototoxicity have been investigated, the information on the hepatic effects of gentamicin is still limited. In the present study, gene expression profiles were analyzed in the liver of gentamicin treated mice using Affymetrix GeneChip$^{(R)}$ Mouse Expression 430A 2.0 Array. Totally, 400 genes were identified as being either up- or down-regulated over 1.5-fold changes (P<0.01) in the liver of gentamicin treated mice. Among these deregulated genes, 16 up-regulated genes mainly involved in transport (Kif5b, Pex14, Rab14, Clcn3, and Necap1) and 20 down-regulated genes involved in lipid and other metabolisms (Hdlbp, Gm2a, Uroc1, and Dak) were selected using k-means clustering algorithm. The functional classification of differentially expressed genes represented that several stress-related genes were regulated in the liver by gentamicin treatment. This data may contribute in understanding the molecular mechanism in the liver of gentamicin treated mice.
To elucidate the mechanism of gentamicin induced renal dysfunction, renal functions and activities of various proximal tubular transport systems were studied in gentamicin-treated rats (Fisher 344). Gentamicin nephrotoxicity was induced by injecting gentamicin sulfate subcutaneously at a dose of 100 $mg/kg{\cdot}day$ for 7 days. The gentamicin injection resulted in a marked polyuria, hyposthenuria, proteinuria, glycosuria, aminoaciduria, phosphaturia, natriuresis, and kaliuresis, characteristics of aminoglycoside nephropathy. Such renal functional changes occurred in the face of reduced GFR, thus tubular transport functions appeared to be impaired. The polyuria and hyposthenuria were partly associated with a mild osmotic diuresis, but mostly attributed to a reduction in free water reabsorption. In renal cortical brush-border membrane vesicles isolated from gentamicin-treated rats, the $Na^+$ gradient dependent transport of glucose, alanine, phosphate and succinate was significantly attenuated with no changes in $Na^+-independent$ transport and the membrane permeability to $Na^+$. These results indicate that gentamicin treatment induces a defect in free water reabsorption in the distal nephron and impairs various $Na^+-cotransport$ systems in the proximal tubular brush-border membranes, leading to polyuria, hyposthenuria, and increased urinary excretion of $Na^+$ and other solutes.
Objectives: Bacterial resistant infections have become a global health challenge and threaten the society's health. Thus, an urgent need exists to find ways to combat resistant pathogens. One promising approach to overcoming bacterial resistance is the use of herbal products. Green tea catechins, the major green tea polyphenols, show antimicrobial activity against resistant pathogens. The present study aimed to investigate the effect of catechins, green tea extract, and methylxanthines in combination with gentamicin against standard and clinical isolates of Staphylococcus aureus (S. aureus) and the standard strain of Pseudomonas aeruginosa (P. aeruginosa). Methods: The minimum inhibitory concentration (MIC) and the minimum bactericidal concentration (MBC) values of different agents against bacterial strains were determined. The interactions of green tea extract, epigallate catechin, epigallocatechin gallate, two types of methylxanthine, caffeine, and theophylline with gentamicin were studied in vitro by using a checkerboard method and calculating the fraction inhibitory concentration index (FICI). Results: The MICs of gentamicin against bacterial strains were in the range of $0.312-320{\mu}g/mL$. The MIC values of both types of catechins were $62.5-250{\mu}g/mL$. Green tea extract showed insufficient antibacterial activity when used alone. Methylxanthines had no intrinsic inhibitory activity against any of the bacterial strains tested. When green tea extract and catechins were combined with gentamicin, the MIC values of gentamicin against the standard strains and a clinical isolate were reduced, and synergistic activities were observed (FICI < 1). A combination of caffeine with gentamicin did not alter the MIC values of gentamicin. Conclusion: The results of the present study revealed that green tea extract and catechins potentiated the antimicrobial action of gentamicin against some clinical isolates of S. aureus and standard P. aeruginosa strains. Therefore, combinations of gentamicin with these natural compounds might be a promising approach to combat microbial resistance.
Gentamicin에 저항성을 나타내는 세균을 병원 하수로부터 분리하여 aminoglycoside-(3)- N-acetyltransferase를 암호화하는 유전자인 aac(3)II의 존재여부를 dot-blot hybridization으로 조사하였다. aac(3)II유전자의 일부를 탐침으로 사용한 결과 gentamicin저항성 세균의 41% (39/95)가 이 유전자를 가지고 있었다. aac(3)II와 Tn3에서 각각 설계된 primer를 사용한 PCR 결과 aac(3)II를 가지고 있는 39개 균주 중 13개 균주가 TnS-aac(3)II구조를 가지고 있음을 알 수 있었다. aac(3)II의 상류에 Tn3를 가지고 있는 13개 균주의 gentamicin에 대한 최소억제농도는 가지고 있지 않은 균주에 비해 상대적으로 높았다. 13개 균주를 동정한 결과 5개는 Eschelichia coli, 3개는 Acinetobacter johnsonii, Enterobacter agglomerans와 Micrococcus luteus가 각각 2개, 그리고 1개의 Pseudomonas facilis로 동정되었다. 이러한 결과들은 Tn3-aac(3)II구조가 gentamicin 저항성 세균들에서 널리 분포하고 있음을 말해준다.
The treatment with gentamicin in the presence of pretreatment with bismuth nitrate significantly reduced blood urea nitrogen compared with given gentamicin alone. But the amelioration of gentamicin-induced nephrotoxicity by bismuth nitrate was abolished by pretreatment with indomethacin that is cyclooxygenase inhibitor, which significantly decreased renal glutathione S-transferase activity and thiobarbituric acid reactive substance compared with mice of given gentamicin and bismuth nitrate. On the other hand, treatment with bismuth nitrate significantly increased prostaglandin $E_2$ production in rat kidney slice. These results suggest that bismuth nitrate might ameliorate the nephrotoxicity of gentamicin via prostaglandin $E_2$ production.
To evaluate the influence of ochratoxin A on the pharmarcokinetics of gentamicin, gentamicin concentrations in the serum, renal cortex and medulla together with parameters of the renal function and histological changes were compared between ochratoxin A-treated rats (0.1 mg of ochratoxin A/kg of body weight, ip, daily for 14 days) and normal rats. Gentamicin was given with a single intramuscular injection (10mg/kg of body weight). Ochratoxin A resulted in an increase of the half-life, the area under the concentration-time curve, the apparent volume of distribution and a decrease of the total body clearance of gentamicin, and accumulated significantly (p<0.01) more gentamicin in the kidneys.
Micromonosporas purpurea로부터 효율적 gentamicin 생산을 위해 protoplast fusion와 protoplast mutagenesis 방법이 검토 되었다. $CO^{60}\;irradiation\;(2.3{\times}10^5$ units, UV 3 min) 방법에 의해서 MP3-112, MP3-141, MP3-143을 분리 했다. 특히 MP3-143균주는 최대 gentamicin생산량이 얻어졌다. 개량된 MP3-143균주를 이용해서 탄소원 소비, 균체성장, 그리고 gentamicin 생산량이 batch culture에서 비교되었다. MP3-413와 parent 균주의 glucose 소비는 배양 2일과 3일 후에 각각 완전히 이루어졌다. 그러나 균체성장과 Soybean oil 소비는 비슷한 결과 얻어졌다. Gentamicin최대 생산량은 배양 5일 후 29756 U/ml였다. 이 결과는 parent 균주에 비해 생산량이 5.6배 증가했다.
The effect of circadian rhythm on gentamicin pharmacokinetics was studied in rabbits who took a single intravenous 2 mg/kg dose of gentamicin at 09:00 in the morning (a.m.) and 22:00 in the evening (p.m.). A significant circadian rhythm of pharmacokinetic parameters as a function of time of day was noted in rabbits, showing lower total body clearance $CL_t$ and higher serum area under the curve (AUC) when given in the evening. The half-life $t_{1/2}$ was shorter in the morning $(3.88\pm0.62h)$ when compared to the evening $(4.76\pm0.75\;h)$. The AUC was greater in the evening $(25.92\pm3.49\;{\mu}g/ml{\cdot}hr)$ than that in the morning $(22.42\pm3.42\;{\mu}g/ml{\cdot}hr)$, most likely because the CLt was significantly higher when gentamicin was given in the morning $(0.18\pm0.28\;ml/hr)$ versus in the evening $(0.15\pm0.26\;ml/hr)$. The $t_{1/2}$ of gentamicin in the evening was increased significantly(p<0.05) compared to those of gentamicin in the morning. It is reasonable to consider individual circadian rhythm for effective dosage regimen of gentamicin in clinical chronotherapeutics.
Gentamicin is useful to the Gram negative bacterial infection, but its nephrotoxicity is a serious problem and the incidence is probably increasing. The toxicity of gentamicin to the kidney is site-specific to the proximal tubule. In this study, we measured daily peak and trough level of gentamicin, serum creatinine, serum $Beta_2$-microglobulin and 24-hr urine $Beta_2$-microglobulin in 10 gentamicin treated patients. All the patients had their peak levels of gentamicin in the safe therapeutic range, and their trough level showed no evidence of gentamicin accumulation. There was no patient who showed his daily serum creatinine and $Beta_2$-microglobulin rise significantly. But 24-hour urine $Beta_2$-microglobulin showed significant rise from basal level (mean $5.8{\pm}1.62{\times}$) on the 5 th day of gentamicin treatment. Thus, serial monitoring of proximal tubular function with urinary $Beta_2$-microglobulin excretion has potential value in the assessment of insults of gentamicin to this site. But clinical significance of raised urinary $Beta_2$-microglobulin excretion in relation to the serum creatinine should be further studied.
Gentamicin은 임상에서 많이 사용되는 aminoglycoside계 항생물질로서 세균의 세포막 단백질 합성을 억제하여 살균작용을 나타낸다. 최근 Gentamicin이 동물사료에 포함되거나 동물약품으로 많이 사용되어, 이를 복용한 식용가축에서의 잔류 량에 대한 인체유해성이 WHO/FAO 식품첨가물 전문가 협의회에서 논의되고 있다. Gentamicin의 육가공류의 잔류허용량 기준설정을 위한 독성 재평가의 일환으로 in vivo. in vitro 염색체이상시험을 실시하여 다음과 같은 결론을 얻었다. 1. 체외 염색체이상시험에서는 포유동물 배양세포인 chinese hamster lung cell을 배양하여 gentamicin sulfate 및 gentamicin을 최고 처리농도 5mg/$m\ell$부터 세포독성시험을 실시한 결과, 세포독성을 나타내지 않았다. 본 시험에서는 5mg/$m\ell$를 최고농도로 2.5, 1.25mg/$m\ell$의 3농도를 직접법 및 대사활성화법으로 각 농도당 2매의 플레이트씩 슬라이드를 제작, 결과를 판독한 결과, 직접법 및 대사활성화법 모두에서 전 농도 군에서 음성대조군과 같은 정도의 염색체이상을 유발하여 유전독성이 없음을 나타내었다. 2. 체내 염색체 이상시험에서는 ddY마우스를 이용하여 gentamicin sulfate의 LD$_{50}$의 1/2에 해당하는 200mg/kg을 최고농도로 gentamicin 과 gentamicin sulfate를 암수 각각 3마리씩 공비 2의 3농도로 투여한 후, 24시간째 골수세포의 염색체 표본을 제작하여 관찰한 결과, 세포독성 및 염색체 이상을 유발하지 않았다. 또한 동물약품으로 사용되는 치료용량 및 투약방법에 근거하여 10mg/kg 및 5, 2.5mg/kg을 1일 1회씩 4회 투여한 군에서도 암수에 상관없이 전 농도 군에서 염색체이상을 나타내지 않아 유전독성을 나타내지 않음을 관찰하였다.
