백색 가토 좌골 신경의 신경 결손부위에서 자가 골격근 충진 도관을 이용한 신경재생 연구 (Experimental Study for Nerve Regeneration Using Tubes Filled with Autogenous Skeletal Muscle in a Gap of Rabbit Sciatic Nerves)

  • The bridging of nerve gaps is still one of the major problems in peripheral nerve surgery. To evaluate the role of silicon tube in nerve regeneration, gaps were made by resection of tibial components of sciatic nerves of twenty-five New Zealand rabbits. The gaps were divided into five groups. In group I, the tibial components of sciatic nerves were isolated and the incision immediately closed. In group II, 1-cm segments of the nerve were removed and the silicon tubes filled with autogenous skeletal muscle were sutured in place. In group III, 1-cm segments of the nerve were removed and the silicon tubes filled without muscle were sutured in place. In group IV, 2-cm segments of the nerve were removed and the silicon tubes filled with autogenous skeletal muscle were sutured in place. In group V, 2-cm segments of the nerve were removed and the silicon tubes filled without muscle were sutured in place. At 16th week, the eletromyography, the light and transmission electron microscopy were performed. Nerve conduction study stimulating sciatic nerve proximal to the lesion and recording at gastrocnemius muscle showed that the compound muscle action potentials of the group II with 1 cm nerve defect filled with muscle were higher amplitudes than the group III without muscle. Compound muscle action potentials of the group IV with 2 cm defect filled with muscle showed similar results in comparison with the group V. The light and transmission electron microscpy showed that a good morphological pattern of nerve regeneration in 1 cm gap than 2 cm and in gap with muscle than gap without muscle.

A verification on the physical effectiveness of therapeutic horseback riding exercise: Focused on the EMG analysis

  • Various studies related to therapeutic horseback riding have been reported to be positive for the therapeutic effect of patients with cerebral palsy; however, most of the previous studies focused on to muscle development with training period related to the physical effects of therapeutic horseback riding. To identify the causes and phenomena of muscular activation of the body through actual therapeutic horseback riding exercise and to promote the excellence of physical effects of therapeutic horseback riding. This study was a nonrandomized prospective positive-controlled trial design. Twelve teenaged males with cerebral palsy were selected who had experienced riding exercise for 8-12 months. This study measured 8 muscle activities of the pectoralis major muscle (PM), biceps brachii (BB), rectus abdominis muscle (RA), latissimus dorsi muscle (LD), spinal erector muscle (SE), rectus femoris muscle (RF), anterior tibial muscle (AT), and external gastrocnemius muscle (EG) by using electromyography (EMG). Muscle activity was significantly higher in horse riding position than sitting on the common chair in all muscles (PM, BB, RA, LD, SE, RF, AT, and EG). The activity of the body muscles according to the difference of horse walking method (walk: WA; sitting trot: ST; and riding trot: RT) of therapeutic horse riding showed the highest muscle activity in the PM muscle at ST, and the highest activity at BB, RA, LD, SE, and AT muscles at ST and RT, and showed the highest muscle activity in RF and EG muscle at RT. The results of this study suggest that intervention for the treatment of cerebral palsy patients can use therapeutic riding exercise as a rehabilitation method.

Effect of Walking with Combat Boots on the Muscle Tone and Stiffness of Lower Extremity

  • Background: Shoes cover the feet and ankle joints and come into contact with the ground directly during walking, and the shape of shoes is related to the muscle tone of the lower extremity muscles. However, no study has been conducted on the muscle tone of the lower extremity after wearing combat boots. Objectives: To compare and analyze the effects of walking in combat boots and in athletic shoes on muscle tone and stiffness, to identifying the effect of the characteristics of shoes on the muscle tone. Design: Randomized controlled trial. Methods: Thirty subjects were randomly divided into a combat boots group and an athletic shoes group, and interventions were implemented. Both groups walked for 30 minutes on a treadmill at 4.2 km/h. MyotonPRO was used to measure the muscle tone and stiffness of the lower extremity. The measuring sites were set to five muscles on both legs. Results: In the combat boots group, muscle tone and stiffness of the medial gastrocnemius on the dominant side, the muscle tone and stiffness of rectus femoris, and the muscle stiffness of hamstring on the non-dominant side significantly decreased after walking. In the athletic shoes group, there was no significant change in the muscle tone and stiffness. Conclusion: The results of this study can be used to inform the wearing combat boots while walking on a treadmill reduces the muscle tone and stiffness of the lower extremity compared to athletic shoes. It indicates that the restriction of joint movement occurring when wearing combat boots influences reducing muscle tone and stiffness.

Effect of Static Recovery and Dynamic Recovery on the Cardiopulmonary Variables, Lower Extremity Muscle Activity after Progressive Resistance Exercise to Maximal Point

  • This study was to examine on the respiratory variables, heart rate and muscle activity between the static recovery and dynamic recovery after progressive resistance exercise to maximal point. Subjects were 15 students enrolled in N University. All were tested two times (static recovery and dynamic recovery) and were requested to perform a walking on a treadmill after progressive resistance exercise to maximal point. Electromyography(EMG) was used to monitor the muscle activity(TA: Tibialis Anterior, MG: Medial Gastrocnemius) during gait. CPEX-1 was used to measure the respiratory variables and heart rate. The dynamic recovery group was shown the significant lower heart rate than that of static recovery group at during gait. Respiratory rate showed statistically a significant difference. Electromyography(RMS, root mean square) showed a non-significant difference. But the dynamic recovery group of muscle activity was found highly in TA and MG. This study indicated that the dynamic recovery method evidenced more faster than the static recovery method. And this type of dynamic rest by walking can be a help of recovery after exercise.

PNF 팔·다리 패턴에 따른 반대측 다리의 근활성도 비교 (Comparison of Muscle Activity in the Contralateral Lower Extremity from the PNF Arm Pattern and Leg Pattern)

  • Purpose: This study compared and analyzed the effect of the proprioceptive neuromuscular facilitation (PNF) arm extension pattern and leg flexion pattern on the contralateral lower extremity muscles when the patterns were applied to the same subject. Methods: In the study, 35 healthy men and women who understood the PNF patterns were selected as participants. The participants completed the PNF arm extension-abduction-internal rotation pattern and leg flexion-adduction-external rotation with knee flexion pattern in the supine position. While the patients' completed each pattern, the contralateral leg muscle activity was measured to examine the irradiation effect. The maximum isometric contraction time of the muscles to be measured was kept for 5 seconds, and the mean value was obtained by repeating the pattern three times. Results: When the leg flexion-adduction-external rotation with knee flexion pattern was completed, the muscle activity in the vastus lateralis, vastus medialis, biceps femoris, tibialis anterior, and gastrocnemius of the contralateral lower extremity was significantly greater than that found in the PNF arm extension-abduction-internal rotation pattern. Conclusion: The PNF leg flexion pattern showed greater muscle activity on the contralateral lower extremity than the arm extension pattern. Thus, the PNF leg extension pattern is more effective in the activation of the muscles associated with weight-bearing activity.

Carrageenan으로 유도된 염증성 근통증 흰쥐 모델에서 냉치료에 의한 척수의 c-fos의 발현 (Expression of spinal cord c-fos with cold therapy in rats of carrageenan-induced inflammatory muscle pain)

  • Expression of c-fos, an immediate early gene, has accepted to be a marker of functional activity in neurons. This study was aimed to investigate the effects of cold therapy on the expression of spinal cord c-fos in rats of carrageenan-induced muscle pain. Muscle pain was induced in male Sprague-Dawley rats by intra-muscular injection of gastrocnemius with $2\%$ carrageenan. The paw withdrawal latency (PWL) and tail flick test (TFT) responses to heat stimuli were used to detect secondary hyperalgesia produced by the muscle pain and measured to assess the effects of cold. The expression of c-fos was determined in the lumbar regions of the spinal cord by reverse transcription-polymerase chain reaction (RT-PCR) and immunohistochemistry assays. The secondary hyperalgesia to heat simuli (PWL and TFT) were significantly reduced in cold therapy compared with that in the controls. In RT-PCR assays the expression of c-fos mRNA was down-regulated in the lumbar spinal cord in cold group. In addition, Fos immunoreactivity in the dorsal horn of the lumbar spinal cord was decreased in cold group. These results suggested that application of cold attributed to increase PWL and TFT responses and to decrease expression of the c-fos produced by muscle pain.

정상 성인에서 착지 시 착지 높이가 근활성도와 지면반발력에 미치는 영향 (Effect of Landing Heights on Muscle Activities and Ground Reaction Force during Drop Landing in Healthy Adults)

  • Purpose: The study was designed to investigate the effects of landing heights on muscle activities and ground reaction force during drop landing. Methods: Sixteen healthy adults were recruited along with their written informed consent. They performed a drop-landing task at the height of 20, 40, and 60cm. They completed three trials in each condition and biomechanical changes were measured. The data collected by each way of landing task and analyzed by One-way ANOVA. Ground reaction forces were measured by force flate, muscle activities measured by MP150 system. Results: There were significant differences in ground reaction forces, and significant increases in muscle activities of tibialis anterior, medial gastrocnemius and biceps femoris with landing heights. Conclusion: These findings revealed that heights of landing increases risk factors of body damage because of biomechanical mechanism and future studies should focus on prevention from damage of external conditions.

좌식 작업에 있어서 슬관절 각도 변화에 따른 하지 근력 및 압력분포 분석에 관한 연구 (An Analysis on Muscle Strength of Lower-extremity and Pressure Distribution in Sitting Posture)

  • The purpose of this study is to provide basic data, such as exerting muscle power of the lower-extremity, EMG test and pressure distribution for designing ergonomic workstation in sitting posture. The exerting muscle power of the lower-extremity was measured by PRIMUS in 4 postures of 90$^{\circ}$, 120$^{\circ}$, 150$^{\circ}$ and 180$^{\circ}$. And performed ANOVA test on Max. and Mean 100%MVC. In EMG test for surveying muscle mobiligation, 5 muscles(Rectus Femoris, Vastus Lateralis, Gastrocnemius, Soleus, Tibialis Anterior) were employed. Additional experiment in pressure distribution in sitting posture by Pliance(16$\time$16 poles), Max. pressure was measured and performed ANOVA test on the results. Concludingly, sitting posture with 120$^{\circ}$ lower-extremity is the best design criterion for ergonomic workstation in sitting posture.

경근(頸筋)의 동통(疼痛) 및 근(筋) 탄력상태(彈力狀態)에 대한 허실(虛實)의 동서의학적 고찰 (Pain and Muscle Elasticity for Deficiency-Excessiveness Discussed by the View of Oriental and Western Medicine)

  • Prognosis in oriental medicine gathers information by four examination methods. It provides important information to understand the degree of deficiency - excessiveness of a patient to treat properly. To generalize the degree of deficiency - excessiveness can be found by seeing the patient's muscle response and pain perception to the palpitations.The theoretical basis to generalize deficiency - excessiveness, oriental and western medical understanding of pain perception and the elasticity of muscle were discussed.The usual symptoms for the excessiveness could include Pain (dislikeness to the palpitation), Stiffness of nape and limbs, Contracture of the limbs, Clonic convulsion and Fast pain. The symptoms for the deficiency could include Pain (likeness to the palpitation, Gastrocnemius muscle spasm, Flaccid paralysis of limbs and Slow pain. More theoretical bases for generalization of deficiency - excessiveness are needed along with the simplifying the complex clinical symptoms. In this way, we can discuss about deficiency - excessiveness with the regard to western medicine to help its generalization.

뒷다리 현수 유도 위축근에 대한 저빈도 전기자극의 VEGF 발현에 미치는 영향 (Effect of Low Frequency Electrical Stimulation on VEGF Expression of Suspension Induced Atrophic Muscle)

  • Purpose : The purpose of this experiment was to evaluate the pre-application effect of low frequency electrical stimulation(LFES) on VEGF expression of atrophic muscle and to determine the optimal pre-application period of LFES for prevent muscle atropy Methods : Twenty-five adult sprague-dawley rats were randomly assigned to weight bearing group, hindlimb suspension for 14 days group, hindlimb suspension with pre-application of LFES for 14 days group, hindlimb suspension with pre-application of LFES for 11 dsys group and, hindlimb suspension with pre-application LFES for 7 dsys group. 16Hz of Biphasic pulse current was applied to gastrocnemius for 15min per days. Results : VEGF were decreased expression in HSG groups, whereas VEGF were significantly increased in HS+ES14G, HS+ES11G, HS+ES7G groups Conclusion : LFES during the hindlimb suspension showed a positive effect in VEGF induction and early application is strongly encourage VEGF induction. This indicated that pre-application of LFES could prevent muscle atrophy.
