  • Sparganosis is a parasitic infection caused by the plerocercoid tapeworm larva of the genus Spirometra. Although the destination of the larva is often a tissue or muscle in the chest, abdominal wall, extremities, eyes, brain, urinary tract, spinal canal, and scrotum, intramuscular sparganosis is uncommon and therefore is difficult to distinguish from a soft tissue tumor. We report a case of intramuscular sparganosis involving the gastrocnemius muscle in an elderly patient who was diagnosed using ultrasonography and MRI and treated by surgical excision. At approximately 1 cm near the schwannoma at the right distal sciatic nerve, several spargana worms were detected and removed.

  • Purpose: A hyperextended knee is described as knee pain associated with an impaired knee extensor mechanism. Additionally, a hyperextended knee may involve reduced position sense of the knee joint that decreases the individual's ability to control end-range knee extension movement. The purpose of this study was to investigate the effects of visual biofeedback information for plantar pressure distribution on knee joint angle and lower extremity muscle activities in participants with hyperextended knees. Methods: Twenty-three participants with hyperextended knees were recruited for the study. Surface electromyography signals were recorded for the biceps femoris, rectus femoris, gastrocnemius, and tibialis anterior muscle activities. The plantar pressure distribution was displayed and measured using a pressure distribution measuring plate. Knee joint angle kinematic parameters were recorded using a motion analysis system. The visual biofeedback condition was the point at which the difference between the forefoot and backfoot plantar foot pressure on the monitor was minimized. The Wilcoxon signed-rank test was used to determine the significance between the visual biofeedback condition and the preferred condition. Results: The knee joint angle was significantly decreased in the visual biofeedback condition compared to that in the preferred condition (p<0.05). The rectus femoris and gastrocnemius muscle activities were significantly different between the visual biofeedback and preferred conditions (p<0.05). Conclusion: The results of this study showed that visual biofeedback of information about plantar pressure distribution is effective for correcting hyperextended knees.

  • With comparison of maximum vertical reaction force and lower limb on drop landing between normal and flat foot group, this study is to provide fundamental data of the prevention of injury and the treatment of exercise which are frequently occurred on flat foot group's drop landing. The surface electrodes were sticked on lateral gastrocnemius muscle, medial gastrocnemius muscle, tibialis anterior and the drop landing on a force plate of 40cm was performed with a normal group who had no musculoskeletal disease and a flat foot group of 9 people who had feet examinations. Vertical reaction force were significantly statistically different between two groups(p<.001). Muscle activity of lower limbs in all three parts were not statistically different but showed high tendency on average in the flat foot group. The flat foot group had difficulties in diversification of impact burden and high muscle activity. Therefore, it was suggested that muscular strengthening of knee joints and plantar flexions of foot joints which were highly affected in impact absorption will be required.

  • The purpose of this study was to investigate the muscle activity of the lower extremity during driver swing in three-foot positions (Feet Open Stance, Feet Straight Stance, Lead Foot Open Stance). The electromyograms of gastrocnemius, tibialis anterior, and vastus lateralis during swing were measured and analyzed in three sections (take away - back swing, back swing - down swing, and down swing - follow swing). There was no significant difference in muscle activity according to foot position. Muscle activity according to phase was significantly higher in right gastrocnemius and tibialis anterior, and the left and right vastus lateralis in down swing - follow swing. In conclusion, the difference in muscle activity according to foot position is insignificant, and it is considered that the muscle activity to maintain the balance of the body increases toward the end of swing.

  • Background : Although plank exercises is reported to the changes in muscle activity of the deep muscles and superficial muscles among the core muscles. However, no study has examined the effects of forearm plank exercise on tone and stiffness in the superficial back line muscle. Objective: To compare the effects of sling forearm plank exercises and mat forearm plank exercises on the superficial back line muscle tone and stiffness. Design: Randomized controlled clinical trial (single blind) Methods: The subjects were randomized to sling forearm plank exercise group (N = 8) or mat forearm plank exercise group (N = 8). The measurements were taken for each research group following exercises: the muscle tone and stiffness of upper lumbar muscles, lower lumbar muscles, long head of biceps femoris, and medial part of gastrocnemius among the superficial back line muscles. Results: Sling forearm plank exercise group Indicated statistically significant increases in stiffness of medial part of gastrocnemius (p<.05). However, mat forearm plank exercise group reported no statistically significant in muscle tone and stiffness of all measured muscles. No significant differences in measured variables were found between the groups. Conclusions: These results suggest that the forearm plank exercise performed with an unstable surface in the defined sling can increase the stiffness of calf muscle, but it is unlikely to achieve increases in muscle tone and stiffness of the overall superficial back line muscles.

  • Objective: This study was aimed to compare the thickness and pennation angle of gastrocnemius through ultrasonography during the heel-drop exercise on ankle dorsiflexion angle. Design: Cross-sectional study. Methods: Nineteen normal adults in their 20s had voluntarily participated in this study. All subjects performed the ankle heel-drop exercise with ankle dorsiflexed to $0^{\circ}$, $10^{\circ}$, and $20^{\circ}$: heel-drop exercise with ankle dorsiflexed to $0^{\circ}$ was executed on floor-level, heel-drop exercise with ankle dorsiflexed to $10^{\circ}$ on a wooden-block of 2.3 cm in height, and heel-drop exercise with ankle dorsiflexed to $20^{\circ}$ on a wooden-block of 5.5 cm in height. In each regimen, the subjects completed a session of 100 heel-drop exercises (10 repetitions${\times}$10 sets; with 30 seconds rest following each set; with 24 hours rest following each exercise). Before and immediately after each heel-drop exercise, the thickness and pennation angle of gastrocnemius were measured using an ultrasonography. Results: After the performance of the heel drop exercises with ankle dorsiflexed to $0^{\circ}$, $10^{\circ}$, and $20^{\circ}$, the thickness of the gastrocnemius was significantly higher than pre-exercise (p<0.05), and furthermore heel-drop exercise with ankle dorsiflexed to $10^{\circ}$ was significantly higher than exercise with the ankle dorsiflexed to $0^{\circ}$ (p<0.05). However, as for the pennation angle of the gastrocnemius, there were no significant changes after each heel-drop exercise. Conclusions: This finding suggest that the heel-drop exercise with ankle dorsiflexed to $0^{\circ}$, $10^{\circ}$, and $20^{\circ}$ is effective on the strengthening of the gastrocnemius. Furthermore, the heel-drop exercise with the ankle dorsiflexed to $10^{\circ}$ is more effective than with the ankle dorsiflexed to $0^{\circ}$.
