A hepatoid adenocarcinoma of stomach, a subtype of gastric cancr, is characterized by a histologic resemblance to a hepatocellular carcinoma and $\alpha$-feto protein production. Another feature is early metastasis to the liver and lymph nodes, thus revealing a poor prognosis. We report a case of a hepatoid adenocarcinoma of the stomach with liver metastasis. A 52-year-old male visited our hospital with a chief complaint of indigestion. Gastroscopic examination showed a Borrmann type-II lesion on the lesser curvature of the antrum. The CT scan showed a suspected advanced gastric cancer with liver metastasis. The serum AFP level was 123 ng/ml. A radical subtotal gastrectomy and a right hemihepatectomy were performed simultaneously. Pathologic examination confirmed the lesion to be confined to the submucosa. The gastric lesion was a hepatoid adenocarcinoma, and the hepatic lesion was a metastatic adenocarcinoma from the stomach. Therefore, he was classified as having stage IV (T1N1M1) gastric cancer. In cases of a hepatoid adenocarcinoma of the stomach, even patients with early gastric cancer can be staged into the poor prognostic group.
Gastric duplication is a rare anomaly which account for only 3.8% of all gastrointestinal duplication. Gastric duplications are usually cystic lesion without communication with lumen. Most frequent presentation is an abdominal mass with vomiting, mainly diagnosed within the first year of life. Surgical removal is necessary in all cases, and optimal timing for surgery is the time that diagnosis is made. However, prenatally diagnosed gastric duplication is getting more common, and determining timing for surgery is not easy due to absent or minimal symptoms just after birth. We experienced prenatally diagnosed gastric duplication in a female newborn baby that gastric duplication was suggested in $24^{th}$ week of gestational age through prenatal ultrasonogram. Surgical removal was done at 3 months after birth, and showed good results. We think that natural history of gastric duplication and prevalent age of surgical disease which is similar to gastric duplication such infantile hypertrophic pyloric stenosis should be considered when timing of surgery on prenatally gastric duplication is decided.
Adenosquamous carcinoma of stomach is a mixed glandular-epidermoid tumor where both components are neoplastic. Its incidence is extremely rare. The five theories on the origin of squamous components are 1) island of ectopic squamous epithelium in the gastric mucosa, 2) squamous metaplasia of gastric epithelium, 3) squamous differentiation in a preexisting adenocarcinoma, 4) endothelial cell differentiated toward squamous elements, and 5) totipotential undifferentiated cells of the gastric mucosa. We experienced three cases of adenosquamous carcinoma. Case 1 was a 71-year-old female patient. ; an ulcerative lesion was present in the pylorus, measuring 5cm in diameter. Case 2 was a 57-year-old male patient. ; an ulcerative lesion is present in the pylorus, measuring 6 em in diameter. Case 3 was a 58-year-old female patient. ; an ulcerative lesion was present in the body and fundus, measuring 10cm in diameter. Microscopic examination revealed a mixed malignant squamous and adenomatous component.

The protective effect of ginsenoside Re, isolated from ginseng berry, against acute gastric mucosal lesions was examined in rats with a single intraperitoneal injection of compound 48/80 (C48/80). Ginsenoside Re (20 mg/kg or 100 mg/kg) was orally administered 0.5 h prior to C48/80 treatment. Ginsenoside Re dose-dependently prevented gastric mucosal lesion development 3 h after C48/80 treatment. Increases in the activities of myeloperoxidase (MPO; an index of neutrophil infiltration) and xanthine oxidase (XO) and the content of thiobarbituric acid reactive substances (TBARS; an index of lipid peroxidation) and decreases in the contents of hexosamine (a marker of gastric mucus) and adherent mucus, which occurred in gastric mucosal tissues after C48/80 treatment, were significantly attenuated by ginsenoside Re. The elevation of Bax expression and the decrease in Bcl2 expression after C48/80 treatment were also attenuated by ginsenoside Re. Ginsenoside Re significantly attenuated all these changes 3 h after C48/80 treatment. These results indicate that orally administered ginsenoside Re protects against C48/80-induced acute gastric mucosal lesions in rats, possibly through its stimulatory action on gastric mucus synthesis and secretion, its inhibitory action on neutrophil infiltration, and enhanced lipid peroxidation in the gastric mucosal tissue.
A 69-year-old woman was presented with progressed dysphagia, gastric soreness and weight loss during 2 months. She was performed abdomen x-ray, EGDS and abdomen CT. Abdomen x-ray demonstrated punctuate calcification on LUQ. EGDS showed an ulceroinfiltrative mass with bleeding on cardia to antrum of stomach. And CT showed diffuse gastric wall thickness with multiple calcifications. Biopsy of the stomach and esophagus during EGDS examination revealed an adenocarcinoma, with signet ring cell type, infiltrating the wall of the stomach and the distal esophagus. Then acne scan was performed a few days later. It revealed intense uptake in LUQ, corresponding to the calcium containing neoplasm seen on the abdomen x-ray, EGDS and abdomen CT. And there was no evidence of any metastatic lesion and thyroid uptake on the bone scan. There are many reports about accumulation of the tracer in extraosseous lesion, but only a few literatures were reported about gastric calcification in stomach cancer. More over, no reports showed CT images. We are performed many diagnostic examinations and found well correlation between them. The reason of gastric calcification is considered with calcium deposition within extracellular space due to hemorrhage or necrosis. Other possibility offered to explain gastric calcification have been increased blood flow and/or increased neovascularity with capillary leaks of tracer, and specific enzymatic (phosphatases) receptor binding of tracer. So, it was happened ion exchange between intracellular calcium and phosphate groups of tracer.
Background: The aim of this study was to investigate the value of serum gastric markers to differentiate between patients with precancerous lesions and nonatrophic chronic gastritis. Materials and Methods: Serum samples of 128 patients with dyspepsia who were candidates for endoscopic examination were tested for pepsinogen (PG I and PG II), PG I/II ratio, gastrin 17(G-17), anti-Helicobacter pylori (anti-H pylori ) and anti-CagA antibodies. Two sample t-tests, chi-square tests and Pearson's correlation analyses were used for analysis using SPSS (version 20). Results: PGI, PG I/II ratio values were decreased significantly in the precancerous lesion group (0.05, 0.001 respectively). The frequency of H pylori infection was significantly (p=0.03) different between the two groups ofthe study. Conclusions: We suggest PGI and the PG I/II ratio as valuable markers for screening of premalignant gastric lesions.
Gymnosporia rothiana (walp) Lawson (celastraceae), commonly known as Maytenus rothiana, is used in Indian folk medicine as an antiulcerogenic agent. However, there have been no scientific reports regarding its antiulcer activity. Therefore, this study was intended to evaluate the antiulcer property of petroleum ether, chloroform, and methanolic extract of leaves of Gymnosporia rothiana at different dose levels in ethanol induced and indomethacin induced gastric ulcer models. It was observed that oral administration of all the extract of Gymnosporia rothiana produces significant reduction in ulcer lesion index as well as increase in volume and pH of gastric content in both experimental models, being petroleum ether extract the most effective at dose of 250 mg/kg; it significantly reduced gastric lesion index (70.06%), in comparison to omeprazole (71.20%) and methanolic extract at a dose of 500 mg/kg (67.22%). Increased gastric mucosal defense mechanism by petroleum ether extract is probably due to its high levels of terpenoids like $\beta$ amyrin, lupeol acetate. The present results clearly shows antiulcer effect of Gymnosporia rothiana against various irritants has been mainly due to cytoprotective effect mediated through prostaglandin and partly due to free radical scavenging activity.
We report a case of gastric adenoma which was found incidentally on $^{18}F$-FDG PET/CT study for cancer screening in asymptomatic patient. It showed focal and intensely increased. FDG uptake in the antrum of stomach. On the gastroduodenoscopy, it showed flat elevated lesion with irregular margin. Histologically, the lesion was confirmed gastric adenoma with high grade dysplasia and removed by endoscopic mucosal resection.
Based on the gross lesion, a survey of ulcer prevalence was carried out on pigs (3,161 heads) at a slaughterhouse in seoul from April to December in 1989. The pigs stomachs were observed anatomically and classified, according to the presence or absence of foreign bodies. Ulcer prevalence is usually classified according to size of pig farm, ages, and supplied month. The results in the studies were as follows, 1. Fundus followed the highest rate with 28.2%, esophagus with 24.5% each of cardiac and pyloric part with 7.8% and 4.1% in cases of gastric mucosa pathological studies examed total 3,161. In the acute ulcer prevalence, fundus follwed with 5.4%, esophagus with 2.6%, each of cardiac and pyloric part with 0.8% and 0.3%. When prevalent frequencies of fundus are compared with cardiac and pyloric part, they have meaning.(P <0.05). 2. In the survey pig of total 3,161, pathological changes of epithelium tissue foliowed 44.0%, erosion(11.5%), chronic ulcer(14.8%), acute ulcer(1.6%), scar formation (1.7%) and perforation (0.9%). 3. In general monthly prevalence rates of gastric lesion are higher in spring and fall than in summer. 4. In the size of pig farms, ulcer showed higher rate at professional farms than raising on the side. 5. When there are more fluid condition in stomach and they are old, the ulcer appeared for more. 6. Foreign bodies in stomach were sand, hairballs, and intestinal worms were detected (1%).
Objectives : The purpose of this study is to evaluate whether or not a pretreatment with Portulaca oleracea has an antioxidant effect in HCl-ethanol induced gastric mucosal damage. Methods : We elucidated the level of reactive oxygen species (ROS), lipid peroxidation, and two important constituents of antioxidant defense such as superoxide dismutase (SOD), glutathione (GSH) in these effects. Results : The oral administration of crude extract from P. oleracea attenuated the gastritic lesion area, submucosal edema and hemorrhage, and mucosal necrosis induced by HCl-ethanol. The MDA levels of control group were higher than those in the rats given the P. oleracea pretreatment. While the GSH levels of control were decreased, the GSH activity on the gastric mucosal layer maintain normal level in rats given the Portulaca oleracea pretreatment before HCl-ethanol induced gastritis significantly increased. However, the SOD activites were not altered by P. oleracea. Conclusions : The administration of Portulaca oleracea have a protective antioxidant effect against the gastric lesion induced by HCl-ethanol and may therefore be a promising drug for gastritis and gastric ulcer.
