Protective Effect of Taurine on Indomethacin-induced Gastric Mucosal Injury

  • It has been suggested that oxygen-derived free radicals play an important role in the pathophysiology of acute gastric ulceration induced by NSAIDs and ischemia-reperfusion. Taurine is hypothetized to exert its protective effect on NSAIDs-induced gastric injury by its antioxidant properties. Protective effect of taurine on indomethacin-induced gastric mucosal lesion and its protection mechanism were investigated. Intragastric administration of 25 mg/kg of indomethacin induced hemorrhagic lesions on the glandular stomach in rats. Pretreatment with 0.25 or 0.5 g/kg of taurine one day before or for 3 days significantly reduced the gastric lesion formation and inhibited the elevation of lipid peroxide level in gastric mucosa. The luminol-dependent chemiluminescence of rat peritoneal neutrophils increased immediately after treatment of FMLP or indomethacin. Taurine (5-20 mM) inhibited chemiluminescence of neutrophils activated by FMLP. Human neutrophils (polymorphonuclear leukocytes) significantly adhered to the confluent monolayer of human umbilical vein endothelial cells (HUVEC) after coincubation with indomethacin. This neutrophil adhesion induced by indomethacin to HUVEC was prevented by taurine in a dose-dependent manner. These results indicate that the protective effect of taurine against NSAIDs-induced gastric mucosal injury is due to its antioxidant effect, which inhibits lipid peroxidation and neutrophil activation.

Gastric wall abscess after endoscopic submucosal dissection

  • Gastric wall abscess, a localized form of phlegmonous gastritis, is a rare complication of endoscopic resection. We report the first case of gastric wall abscess developing after endoscopic submucosal dissection in Korea. A 72-year-old woman visited our clinic to receive treatment for gastric adenoma. The patient successfully underwent endoscopic submucosal dissection with no complications. The final diagnosis was well-differentiated tubular adenocarcinoma. We performed follow-up endoscopy 10 weeks later and found a large subepithelial lesion on the posterior wall of the gastric antrum. Abdominal computed tomography revealed hypodense wall thickening and a 5 cm heterogenous multilobular mass in the submucosal layer of the gastric antrum. Submucosal invasion with mucin-producing adenocarcinomas could therefore not be excluded. The patient agreed to undergo additional gastrectomy due to the possibility of a highly malignant lesion. The final diagnosis was acute suppurative inflammation with the formation of multiple abscesses in the mural layers and omentum. The patient was discharged with no complications.

  • AU-164 was synthesized as a reversible gastric $H^+/K^+$ ATPase inhibitor, and its effects were tested in various systems. AU-164 inhibited rabbit gastric $H^+/K^+$ ATPase with an $IC_{50}$/ of 9 $\mu$M. On the other hand, AU-164 was a weak inhibitor for dog kidney $Na^+/K^+$ ATPasc, indicating the selectivity for gastric $H^+/K^+$ ATPase. The reversible property of the AU-164-induced inhibition of $H^+/K^+$ ATPase was confirmed by filtering the inhibition mixture through Sephadex G-25M column. In vivo basal acid secretion was also inhibited by AU-164 under the pylorus ligation of Sprague-Dawley rats. In addition, AU-164 protected dose dependently gastric lesion induced by ethanol in rats. The $ED_{50}$ value of 62 mg/kg p.o was estimated. These results suggest that AU-164 is a potent, selective and reversible gastric $H^+/K^+$ ATPase inhibitor, and that AU-164 has a potential use for the clinical therapeutics of peptic ulcer disease.

Human neutrophils (polymorphonuclear leukocytes) significantly adhered to confluent monolayer of human umbilical vein endothelial cells(HUVEC) after coincubation with aspirin or indomethacin. Also taurine prevented neutrophil adhesion induced by these drugs to HUVEC in dose-dependent manner. These results indicate that the protective effect of taurine against NSAIDS-induced gastric mucosal Injury is due to its antioxidant effect, which inhibits lipid peroxidation and neutrophil activation.

A Case Report on Korean Medicine Treatment for a Patient Diagnosed with Acute Gastric Mucosal Lesion on Ultrasound Imaging (초음파 영상에서 급성 위점막 병변으로 진단된 환자에 대한 한의치료 치험 1례)

  • Objectives: This study reports a clinical case that showed improvement in the symptoms of a patient with acute gastric mucosal lesion (AGML) through ultrasound. Methods: A 90-year-old woman with AGML was treated with Korean medicine treatments, including Hyangsapyeongwisan Soft Ext, acupuncture, and moxibustion. The responses before and after treatment were evaluated using ultrasound imaging and the numerical rating scale (NRS). Results: After treatment, ultrasound imaging showed improvement in gastric mucosa thickness, from 1.45 cm to 0.53 cm at the anterior wall and from 1.25 cm to 0.36 cm at the posterior wall. The NRS scores for epigastric pain, nausea, and dizziness all decreased, and the symptoms of diarrhea disappeared. Conclusion: This study suggests that Korean medicine treatment can be a therapeutic option in treating the symptoms of patients with AGML.

Gastroduodenal intussusception of a gastrointestinal stromal tumor: a rare cause of acute pancreatitis

  • Patients with symptomatic gastrointestinal stromal tumor (GIST) typically present with gastrointestinal bleeding and abdominal pain. This report presents an unusual case of fundic GIST complicated by gastroduodenal intussusception, manifesting as acute pancreatitis. The patient presented with epigastric pain and pancreatic enzyme elevation; thus, he was diagnosed with acute pancreatitis. Computed tomography showed evidence of pancreatitis and a 4×4.7 cm well-defined hyperdense lesion in the 2nd part of the duodenum, compressing the pancreatic head and pancreatic duct. Esophagogastroduodenoscopy revealed invagination of the gastric folds into the duodenum, causing pyloric canal blockage consistent with gastroduodenal intussusception. Spontaneous reduction of the lesion during endoscopy revealed a 4 cm pedunculated subepithelial mass with central ulceration originating from the gastric fundus. Endoscopic ultrasound demonstrated a heterogeneous hypoechoic lesion originating from the 4th layer of the gastric wall. Laparoscopic-endoscopic intragastric wedge resection of the fundic lesion was subsequently performed, and surgical histology confirmed GIST.

Antigastritic and Antiulcer Actions of the Extract of Head of Panax ginseng Radix (인삼노두 추출물의 위염 및 위궤양에 대한 효과)

  • In a preliminary screening of plant extracts for the antigastritic and antiulcer actions in rats, the extracts of head of Panax ginseng Radix showed positive activity in HCl ethanol-induced gastric lesion. Among the systematic fractions of hexane, chloroform, butanol and water, the most potent butanol fraction reduced significantly HCl ethanol-induced gastric lesion at the oral dose of 500 mg/kg. In pylorus ligated rats, hexane and butanol fraction showed decreases in the volume of gastric secretion and acid output, of which effects were stronger in butanol fraction. Further assays with butanol fraction disclosed that it significantly suppressed the aspirin-induced and Shay ulcer. The butanol fraction at the intraduodenal dose of 500 mg/kg showed significant stimulation of mucus secretion.

  • In order to investigate the effect of BODUOPAE-SAN(BOS), experiments were performed on analgesic effect induced by acetic acid, duration of hypnosis induced by pentobarbital-Na in mice and gastric ucler in Shay rats and indomethacin induced gastric ulcer, HCI-ethanol induced gastrci lesion in rats and gastric juice secretion in Shay rats. The results are as followings. 1. BOS showed significantly an analgesic effect induced by acetic acid. 2. BOS prolonged the duration of hypnosis induced by pentobarbital-Na in rats 3. BOS rised the spontaneous motility of isolatied ileum of mice temporarily. 4. BOS depressed the gastric motility of rats significantly. 5. BOS showed an anti-ulcer effect in Shay rats and indometacine-induced ulcer rats significantly. 6. BOS reduced the ulcer index of the HCl-ethanol induced gastric lesion in rats. 7. BOS reduced the gastric juice secretion in Shay rats. From above results think BOS is more effective in comparison to the OPAESAN alone on gastrointestinal disorder.

  • Objective : his study was carried out to investigate the effect of Dansameum (DS) on the gastrohemorrhagic lesion induced by ammonia in rats. Methods : Rats were pretreated with DS extract 25 mg/kg for 10 days and then were given amonia through gastric tube. The animals were killed 1 hr after amonia treatment. Results : DS extract siginificantly reduced the gastrohemorrhagic lesion score, the gastric lipid peroxide level, the gastric urease activity, the gastric myeloperoxidase activity, the gastric acid phosphatase activity, xanthine oxidase activity and type conversion, and increased the gastric glutathione level considerably. In photomicrographs of stomach tissue in rat, we could see the gastrohemorrhagic lesion induced clearly. conclusions : These results suggest that DS extract may be effective in peptic ulcer.

Effects of Coptidis Rhizoma Herbal Acupuncture Extract on the Acute Gastric Mucosal Lesion Progression Induced by Compound 48/80 in Rats

  • Objectives : Coptidis Rhizoma has been used for stomach disease. However, its property is so cold that it might be avoided to prescribe for the elderly and the infirm having indigestion or diarrhea. Accordingly, the present study was designed to investigate the protective effects of Coptidis Rhizoma herbal acupuncture extract against acute gastric mucosal lesions induced by compound 48/80 in rats. Methods : The Coptidis Rhizoma herbal acupuncture (CRHA) was injected in Choksamni and Chungwan 1 h before compound 48/80 treatment. The animals were sacrificed under anesthesia 3 h after compound 48/80 treatment. The stomachs were removed and the amount of gastric adherent mucus, gastric mucosal hexosamine, SOD, XO, TBARS and histological examination were performed. Results : The decline of gastric adherent mucus, gastric mucosal hexosamine and the histological defects of gastric mucus were significantly protected by CRHA treatment. Gastric adherent mucus in control group was reduced to $38.2{\pm}5.0%$. CRHA groups significantly protected the loss of mucus to $77.5{\pm}4.9%$. Mucosal hexosamine content showed similar patterns. Mucosal hexosamine content in control group was reduced to $45.2{\pm}6.2%$. CRHA groups significantly protected the loss of mucus to $83.0{\pm}7.0%$. The changes of gastric mucosal SOD and TBARS were recovered by CRHA treatment as well. Conclusions : CRHA showed the protective effects on the acute gastric mucosal lesions induced by compound 48/80 in rats. These results suggest that CRHA may have protective effects on the gastritis.
