역류성 식도염에 대한 천연 미네랄 워터의 효과 (Effects of Natural Mineral Water on Reflux Esophagitis)

  • 본 연구는 다량 미네랄 6종 및 미량 미네랄 8종이 함유된 천연 미네랄 워터의 기능성을 확인하기 위해 역류성 식도염 유발 동물모델을 이용하여 개선 효과를 분석하였다. RAW264.7에서의 세포 생존율 측정 결과를 통해 미네랄 워터는 세포에 대한 독성이 없음을 확인하였으며, 역류성 식도염 동물모델에서 위 내용물 역류에 의한 식도 손상을 감소시켰으며, 식도 점막 보호 작용으로 인해 세포 연접 단백질 발현을 억제하였다. 따라서, 미네랄 워터의 식도 보호 작용은 조직 내 염증성 단백질 및 점막 재생 관련 단백질을 조절함으로써 나타난 것으로 사료된다. 이러한 연구 결과를 통해, 미네랄 워터의 기능성을 확인함으로써 음용수, 식품 생산 등의 다양한 산업 분야에서의 활용성과 가축 생산에 있어 면역증진 및 염증 관련 질병에 대처할 수 있는 친환경 기능성 사료첨가제로서의 미네랄 워터의 이용 가치를 확인할 수 있었다.

Analysis of the Occurrence of Diseases Following Gastrectomy for Early Gastric Cancer: a Nationwide Claims Study

  • Purpose: Various changes in nutrition, metabolism, immunity, and psychological status occur through multiple mechanisms after gastrectomy. The purpose of this study was to predict disease status after gastrectomy by analyzing diseases pattern that occur or change after gastrectomy. Materials and Methods: A retrospective cohort study was conducted using nationwide claims data. Patients with gastric cancer who underwent gastrectomy or endoscopic resection were included in the study. Eighteen target diseases were selected and categorized based on their underlying mechanism. The incidence of each target disease was compared by dividing the study sample into those who underwent gastrectomy (cases) and those who underwent endoscopic resection for early gastric cancer (controls). The cases were matched with controls using propensity score matching. Thereafter, Cox proportional hazard models were used to evaluate intergroup differences in disease incidence after gastrectomy. Results: A total of 97,634 patients who underwent gastrectomy (84,830) or endoscopic resection (12,804) were included. The incidence of cholecystitis (P<0.0001), pancreatitis (P=0.034), acute kidney injury (P=0.0083), anemia (P<0.0001), and inguinal hernia (P=0.0007) were higher after gastrectomy, while incidence of dyslipidemia (P<0.0001), vascular diseases (ischemic heart disease, stroke, and atherosclerosis; P<0.0001, P<0.0001, and P=0.0005), and Parkinson's disease (P=0.0093) were lower after gastrectomy. Conclusions: This study identifies diseases that may occur after gastrectomy in patients with gastric cancer.

Anti-microbial and anti-inflammatory effects of Cheonwangbosim-dan against Helicobacter pylori-induced gastritis

  • Background: There are various Helicobacter species colonizing the stomachs of animals. Although Helicobacter species usually cause asymptomatic infection in the hosts, clinical signs can occur due to gastritis associated with Helicobacter in animals. Among them, Helicobacter pylori is strongly associated with chronic gastritis, gastric ulcers, and gastric cancers. As the standard therapies used to treat H. pylori have proven insufficient, alternative options are needed to prevent and eradicate the diseases associated with this bacterium. Cheonwangbosim-dan (CBD), a traditional herbal formula that is popular in East Asia, has been commonly used for arterial or auricular flutter, neurosis, insomnia, and cardiac malfunction-induced disease. Objectives: The present study investigated the antimicrobial effect of CBD on H. pylori-infected human gastric carcinoma AGS cells and model mice. Methods: AGS cells were infected with H. pylori and treated with a variety of concentrations of CBD or antibiotics. Mice were given 3 oral inoculations with H. pylori and then dosed with CBD (100 or 500 mg/kg) for 4 weeks or with standard antibiotics for 1 week. One week after the last treatment, gastric samples were collected and examined by histopathological analysis, real-time quantitative polymerase chain reaction, and immunoblotting. Results: Our results showed that CBD treatment of AGS cells significantly reduced the H. pylori-induced elevations of interleukin-8, inducible nitric oxide synthase (iNOS), and cyclooxygenase-2 (COX-2). In the animal model, CBD treatment inhibited the colonization of H. pylori and the levels of malondialdehyde, inflammation, proinflammatory cytokines, iNOS, and COX-2 in gastric tissues. CBD also decreased the phosphorylation levels of p38 mitogen-activated protein kinase family. Conclusions: This study suggests that CBD might be a prospective candidate for treating H. pylori-induced gastric injury.

위궤양의 진행에 있어 MAPKs의 세포특이적 활성 (Cell-type Specific Activation of MAPKs in the Progression of Gastric Ulcer in Rats)

  • MAPKs는 다양한 세포자극에 반응하는 중요한 세포신호전달경로이며, 특히 세포의 생존과 사멸과정에 관여한다고 알려져 있다. 그러나 위궤양의 진행에 있어 MAPKs의 세포특이적 활성에 관한 연구결과들에 대해서는 잘 알려져 있지 않다. 본 연구에서는 이부프로펜에 의해 유도된 위궤양의 진행에 있어 활성화된 MAPKs가 다양한 세포들에 어떻게 분포하는지를 실험하였다. 위궤양 유발은 200 mg/kg 이부프로펜을 하루에 8시간간격으로 3번 투여하였다. 동물부검은 이부프로펜을 투여한 후 24, 48, 72시간에 실시하였고, 위조직은 면역조직화학 및 웨스턴블랏에 사용되었다. 활성화된 p-ERK는 정상 랫드 위점막상피의 위상피증식층에서만 주로 발현되었으나, 이부프로펜을 투여한 후 24시간째에는 위기저부의 벽세포들에서 강하게 발현되었다. 이부프로펜을 투여 후 48시간 경과한 군에 있어서는 위궤양에 인접한 위점막상피 또는 위궤양 기저부의 결합조직에 나타난 신생혈관, 염증세포 및 육아조직에서 p-ERK가 강하게 발현되었다. 반면에 p-JNK는 초기 위점막손상을 나타내는 위표면점막상피와 샘위의 점막상피세포의 핵에서 주로 발현되었다. 점차적으로 p-JNK는 위궤양 기저부의 결합조직내에 침윤된 염증세포, 섬유모세포에서 특히 강하게 염색되었다. P-p38 양성세포는 위점막의 결합조직내에서 분산되어 관찰되었으며, 특히 위궤양 기저부의 결합조직내로 침윤된 대식 세포에 강한 염색성을 나타내었다. 이상의 연구결과는 각각의 MAPK들이 위궤양진행에 있어 특정세포들의 활성화에 관여하는 것으로 보여진다.

급성 출혈 환자에서 N-Butyl Cyanoacrylate를 이용한 경도관 동맥색전술의 유용성 (Efficacy of Transcatheter Arterial Embolization with N-Butyl Cyanoacrylate in Acute Bleeding Patient)

  • Background: N-Butyl Cyanoacrylate (NBCA) is a liquid embolic material that can be useful for transcatheter arterial embolization (TAE) of acute bleeding especially in patients with coagulopathy, because it does not depend on coagulation for its therapeutic effect. The aims of this study were to evaluate the clinical efficacy and safety of TAE with NBCA in acute bleeding patients. Methods: Between August 2003 and September 2004, TAE using NBCA for acute bleeding was performed in 23 patients (16 men, 7 women; mean age, 56.5years). The causes of bleeding were gastric ulcer (n=5), postoperative bleeding (n=4), post-biopsy bleeding (n=3), postpartum bleeding (n=3), duodenal ulcer (n=2), angiodysplasia (n=2), gastric lymphoma (n=1), iatrogenic injury (n=1), CMV gastritis (n=1), stab injury of the liver (n=1). TAE was performed using 1:3 mixtures of NBCA and iodized oil. The angiographic and clinical success rate, recurrent bleeding rate, procedure-related complication and clinical outcomes were evaluated. Results: The angiographic and clinical success rate was 100% and 91.3% (21/23), respectively. There was no serious ischemic complication. Recurrent bleeding occurred in 2 patients (8.7%) and they were managed with successful second TAE (n=1) and endoscopic treatment (n=1). Nine patients (39.1%) had coagulopathy at the time of TAE and clinical success rate in this group of patients was 88.9% (8/9). Conclusions: TAE with NBCA is highly effective and safe treatment modality for acute bleeding patients, especially when the patient has a coagulopathy.

급성 위염 유발 마우스 동물 모델에서 구기자(枸杞子) 에탄올 추출물의 위점막 손상 보호 효과 (Protective Effects of a Lycium chinense Ethanol Extract through Anti-oxidative Stress on Acute gastric lesion mice)

  • Objectives : Gastric lesions affect many people around the world and their development are results of the imbalance between destructive and protective factors in the gastric mucosa. Lycium chinense has been widely used as a traditional Korean medicine, it was recently reported that they have potent anti-inflammatory effects in chronic hepatitis models. Therefore, this study aimed to investigate the anti-inflammatory activity of Lycium chinense extract (LCE) on HCl-Ethanol induced gastric lesion mice.Methods : The ICR mice were divided randomly into five groups of six animals each. Group A was normal mice, and group B was treated orally with 0.5 ml 150 mM HCl-60% Ethanol. Mice in group C and D were pre-treatment of LCE (100 mg/kg and 200 mg/kg bodyweight, p.o before HCl/ethanol treatment) and group E was orally administered sucralfate (10 mg/kg).Results : 150mM HCl/60% ethanol-induced gastric mucosal injury mice were ameliorated mucosal damage upon histological evaluation by treatment of LCE. Pre-treatment of LCE attenuated reactive oxidative species (ROS) and produces peroxynitrite (ONOO-) in stomach tissues. As results of stomach protein analyses, LCE effectively reduce inflammatory-related factors such as cyclooxygenase-2 (COX-2), tumor necrosis factor alpha (TNF-α), inducible nitric oxide synthase (iNOS) and interleukin-6 (IL-6) in gastric lesion mice. In addition, nuclear factor kappa B (NF-κB) and inhibitor of phosphorylation of nuclear factor kappa B (p-IκB) were down-regulated in LCE-administrated gastric lesion mice.Conclusions : Our discovery supports that the therapeutic activity of LCE ameliorate the development of gastric lesion via suppressing the oxidative stress and gastric partial inflammation induced by 150 mM HCl/60% ethanol.

Anti-ulcer and antioxidant activity of leaves of Madhuca indica in rats

  • The leaves of Madhuca (M.) indica J.f.Gmel. (Sapotaceae) have been used traditionally in folk medicine due to its astringent properties and are effective in treatment of eczema and snake bites. Methanolic extract of M. indica is rich in tannins and has been proven experimentally to possess antibacterial activity. The present study was undertaken to evaluate the anti-ulcer and antioxidant activity of M. indica in rats. The methanolic extract of leaves of M. indica was tested at various doses (75, 150 and 300 mg/kg, p.o.) for its effect on gastric secretion and gastric ulcers in pylorus-ligation and on ethanol- induced gastric mucosal injury in rats. The significant reduction in ulcer index in both the models along with an increase in the pH of the gastric fluid and mucin content of stomach, and the acid secretory parameters such as total acidity and volume of gastric fluid were also significantly reduced along with reduction in the pepsin activity in pylorusligated rats proved the anti-ulcer activity of M. indica. The increase in the levels of superoxide dismutase, catalase and reduced glutathione and decrease in lipid peroxidation in both the models proved the antioxidant activity of M. indica. Thus it can be concluded that M. indica possesses anti-ulcer activity, which can be attributed to its antioxidant mechanism of action.

The Effect of $Luteolin-7-O-{\beta}-D-Glucuronopyranoside$ on Gastritis and Esophagitis in Rats

  • This Study evaluated the inhibitory action of $luteolin-7-O-{\beta}-D-glucuronopyranoside$, luteolin which was isolated from Salix gilgiana leaves, and omeprazole on reflux esophagitis and gastritis in rats. Reflux esophagitis and gastritis were induced surgically and by the administration of indomethacin, respectively. The intraduodenal administration of $luteolin-7-O-{\beta}-D-glucuronopyranoside$ decreased the ulcer index, injury area, gastric volume and acid output, and increased the gastric pH compared with luteolin. $Luteolin-7-O-{\beta}-D-glucuronopyranoside$ significantly decreased the size of the gastric lesions that had been induced by exposing the gastric mucosa to indomethacin. The malondialdehyde content, which is the end product of lipid peroxidation, was increased significantly after inducing of reflux esophagitis. The malondialdehyde content was decreased by $Luteolin-7-O-{\beta}-D-glucuronopyranoside$ but not luteolin or omeprazole. $Luteolin-7-O-{\beta}-D-glucuronopyranoside$ has a more potent antioxidative effect than luteolin. $Luteolin-7-O-{\beta}-D-glucuronopyranoside$ is a promising drug for the treatment of reflux esophagitis and gastritis.

부식제에 의한 상부 위장관 손상의 임상양상과 위험인자 (The Clinical Characteristics and Risk Factors of Upper Digestive Lesions that are due to Ingestion of Caustic Material)

  • Purpose: Though caustic injury of the upper digestive tract can lead to severe sequelae, there are few clinical studies on this subject. This study was undertaken to evaluate the clinical characteristics, the endoscopic findings and the risk factors of the upper digestive lesions in patient with caustic ingestion injury. Methods: We retrospectively reviewed the medical records of 137 patients who ingested caustic materials and who visited to our emergency room from January, 2000 to June, 2009. Results: The most common ingested agent was sodium hypochlorite (44.5%), followed by acetic acid (19.7%), hydrochloric acid (11.7%) and lye (8.0%). Ingestion for suicidal attempt (62.0%) was more frequent than accidental ingestion (30.7%). Grade IIa injury was the most frequent finding on endoscopy of the esophagus and Grade 1 injury was the most frequent finding on endoscopy of the stomach. For the late sequelae, there were 9 cases (6.6%) of esophageal stricture and 2 cases (1.5%) of gastric outlet obstruction. The initial signs and symptoms did not correlate with the development of stricture, but leukocytosis, and grade III injury were related to the risk of developing stricture. Conclusion: Caustic injury of the upper gastrointestinal tract is frequently observed on early endoscopy and it can cause significant late sequelae such as stricture. Therefore, it is necessary to evaluate these patients with regular follow up endoscopic examinations for the management of late sequelae.

Antigastritic and anti-ulcerative constituent from Panax ginseng head and its pharmacological activity

  • Head of Panax ginseng C. A. Meyer indicates its growth number of years and has been widely used for supplying energy to weaklings or used as vomit. Butanol fraction of Panax ginseng head was significantly effective on gastritis and ulcer models in rats. and also had anti-oxidative properties in the previous study. It has been well established that gastric ulcer is induced by imbalance between aggressive factors and protective factors. and the oxidative reaction makes the lesions on gastric mucosal injury severer.
