Antioxidants Effects and Roles of Endogenous Sulfhydryls in the Gastric Mucosal Protection of Puerariae Radix (갈근(葛根)의 항산화작용에 의한 위점막 보호효과와 내인성 Sulfhydryl 화합물의 영향)

  • Ethanol induces compoundhemorrhagic gastric lesions and causes a dose-dependent decrease in the concentration of endogenous nonprotein sulfhydryls in rat gastric mucosa. Sulfhydryl-containing drugs protect rats from ethanol - induced gastric lesions. Based on this findings, we investigated the involvement of sulfhydryl compounds in the antioxidant effects of Puerariae radix, a traditional herbal medicine, against ethanol - induced gastric lesions in the absence and presence of iodoacetamide(IDA. sulfhydryl blocking agent) in rats. respectively. Because of the known role of sulfhydryls in gastric cytoprotection, its role in gastric antioxidation was of intrest. In vitro, Puerariae radix extract(PRE) reduced linoleic acid autooxidation and exert DPPH radical scavenging effect. In vivo. PRE increased antioxidants(SOD, catalase. GSH) and reduced lipid peroxide level in ethanol-induced gastric mucosal lesions. But treatment with PRE plus IDA significantly inhibit the antioxidant effects such as SOD and GSH but did not affect catalase levels. These results suggest that Puerariae radix may play roles in the gastric cytoprotection through antioxidant effects and increase of SOD activity and GSH level are dependent of endogenous sulfhydryls.

Protective Effect of Taurine on Indomethacin-induced Gastric Mucosal Injury

  • It has been suggested that oxygen-derived free radicals play an important role in the pathophysiology of acute gastric ulceration induced by NSAIDs and ischemia-reperfusion. Taurine is hypothetized to exert its protective effect on NSAIDs-induced gastric injury by its antioxidant properties. Protective effect of taurine on indomethacin-induced gastric mucosal lesion and its protection mechanism were investigated. Intragastric administration of 25 mg/kg of indomethacin induced hemorrhagic lesions on the glandular stomach in rats. Pretreatment with 0.25 or 0.5 g/kg of taurine one day before or for 3 days significantly reduced the gastric lesion formation and inhibited the elevation of lipid peroxide level in gastric mucosa. The luminol-dependent chemiluminescence of rat peritoneal neutrophils increased immediately after treatment of FMLP or indomethacin. Taurine (5-20 mM) inhibited chemiluminescence of neutrophils activated by FMLP. Human neutrophils (polymorphonuclear leukocytes) significantly adhered to the confluent monolayer of human umbilical vein endothelial cells (HUVEC) after coincubation with indomethacin. This neutrophil adhesion induced by indomethacin to HUVEC was prevented by taurine in a dose-dependent manner. These results indicate that the protective effect of taurine against NSAIDs-induced gastric mucosal injury is due to its antioxidant effect, which inhibits lipid peroxidation and neutrophil activation.

Expression of CDX2 and Villin in Gastric Cardiac Intestinal Metaplasia and the Relation with Gastric Cardiac Carcinogenesis

  • Objective: To determine whether CDX2 and villin protein expression are associated with intestinal metaplasia (IM) in gastric cardiac mucosa and to explore the relationship with evolution of gastric cardiac adenocarcinoma (GCA). Methods: We studied 143 gastric cardiac biopsy or resection specimens from Henan province China, including 25 cardiac gastritis specimens with IM, 65 dysplasia specimens with IM and 35 gastric cardiac adenocarcinoma specimens and stained them for CDX2 and villin by the immunohistochemical SP method. 15 normal gastric cardiac biopsy specimens were also collected as control. Results: (1) Normal gastric mucosa presented no CDX2 and villin expression. The positive rates of CDX2 protein in cardiac gastritis with IM, dysplasia with IM, and carcinoma tissues were 84.0% (21/25), 66.7% (32/48) and 36.4% (20/55), respectively. While the positive rates of villin protein in cardiac gastritis with IM, dysplasia with IM, and carcinoma tissues were 76.0% (19/25), 70.8% (34/48) and 45.5% (25/55), respectively. There were significant differences among the three groups for both CDX2 and villin (P<0.01). Spearman's rank correlation coefficient(rho) showed a close correlation between the two proteins (r=0.843, P<0.01) and both were positively related with tumor differentiation (both P<0.05), but not associated with age, sex, invasion and metastasis of lymph node (P>0.05). Conclusion: Our results suggest that ectopic expression of CDX2 and villin may be involved in early-stage IM and tumorigenesis in gastric cardia and the expression of villin may be regulated by CDX2.

Early Gastric Mucosal Cancer Associated with Synchronous Liver Metastasis (위 점막암에 동반된 간전이 1예)

  • Early gastric cancer (EGC) is defined as a carcinoma confined to the mucosa or submucosa of the stomach, with or without lymph-node metastasis. Synchronous liver metastasis is 5. $12.8\%$ in advanced gastric cancer, but is very low in EGC. A 64-year-old woman was admitted to St. Vincent's Hospital with a complaint of epigastric pain. Gastrofiberscopic examination showed a polypoid mass on the gastric antrum. Abdominal computed tomography demonstrated an intraluminal polypoid mass in the gastric antrum, but no tumor mass in the liver. A laparotomy revealed a solitary liver metastasis, we performed a distal partial gastrectomy with a group-2 lymph-node dissection and resection of metastatic liver tumor. Histologic examination showed a tubular adenoma with a focal carcinomatous change, Which was confined to the gastric mucosa and to the metastatic adenocarcinoma in the liver. We present a case of early gastric mucosal cancer associated with synchronous liver metastasis, along with a review of the literature.

  • This paper presents a study for a development of the oriental medical treatment on the gastric disease caused by Helicobacter Pylori. Helicobacter Pylori is observed in stomach mucosa and it is caused gastric disease via various routes. Helicobacter Pylori is spread generally mouth to mouth by men. So it is related to personal health condition or life style. The treatment of Helicobacter Pylori is an antibiotic combination therapies, but it caused a problem of side effect and drug resistant. In the oriental medical treatment, an immunopotentiation reduces an infective rate of Helicobacter Pylori and prevents a progressive gastric disease.

The Effects of Sasammaickmoondong-tang against Gastric Mucosal Lesions (Mouse의 위점막 염증에 대한 사삼맥문동탕의 면역반응 연구)

  • Objectives: This study was carried out to investigate the effects of Sasammaickmoondong-tang (SME) on gastric mucosal lesions induced by indomethacin in mice. Methods: The normal group was no inflammation-induced mice. The control group was gastro-inflammation-induced mice. The sample group was mice administered SME after gastro-inflammation elicitation. Results: In the common morphology and histochemical change, the control group was observed with various injury-mucous surface cell, micro-villi, paneth cell, surface epithelial cell, goblet cell - by hemorrhagic erosion, while the sample group was as same as the normal group. In the immunohistochemical change, the distributions of COX-1, Bcl-2, and BrdU treated with SME were noticeably higher than in the control group (p<0.05). The distributions of TUNEL, NF-B, COX-2, IL-2R-, NK-1.1, ICAM-1, and CD11b/18 in those treated with SME were noticeably lower than in the control group (p<0.05). Finally, the distribution of SBA was the same as in the normal group. Conclusions: According to the above results, it is supposed that Sasammaickmoondong-tang is applicable to gastric mucosal lesions.

  • We examined the stomaches of slaughtered pigs to evaluate the gastric lesions. Gastric ulcer accompanying parakeratosis and hyperkeratosis were most frequently observed in the mucosa layer of pars oesophagea. Diffuse congestion and focal necrosis was also frequently seen on the fundus. Pierce wounds by toothpick were seen from 3 stomaches.

  • Numerous persons complained dyspepsia even though no specific objective findings are revealed by imaging study, laboratory examinations and other clinical research. To find out of so many symptoms what they are, I would to approach by two ways. One way is oriental medical literatural study and the other is Helicobacter pylori infection that is accepted as one of most important causal factors of many gastric diseases. Background/Aims: Recently, the role of Helicobacter pylori as a causal factor in the etiology of gastric cancer, peptic ulcer, gastritis and low-grade gastric mucosa-associated lymphoid tissue(MALT) lymphoma is well known. Using endoscopy, biopsy urease testing and histology are recommanded as the tests of choice. Serological test is not recommanded at the moment because of its low sensitivity and espicially low specificity. The urea breath test is more sensitive and specific noninvasive test than serologic test, but it is not widely available yet. Methods/Results: We studied 90 cases by diagnostic endoscopy as a screening test for the persons complaining gastrointestinal symptoms. As a result eighteen persons are revealed to be Helicoacter pylori infected histologicaly. Conclusion: More specific literatural studies are requied.

Clinical Role of Magnifying Endoscopy with Narrow-band Imaging in the Diagnosis of Early Gastric Cancer (조기 위암의 진단에 있어서 확대 내시경을 동반한 협대역 내시경의 역할)

  • Narrow-band imaging (NBI) is the most widely used image-enhanced endoscopic technique. The superficial microanatomy of gastric mucosa can be visualized when used with a magnifying endoscopy with narrow-band imaging (ME-NBI). The diagnostic criteria for early gastric cancer (EGC), using the classification system for microvascular and microsurface pattern of ME-NBI, have been developed, and their usefulness has been proven in the differential diagnosis of small depressed cancer from focal gastritis and in lateral extent delineation of EGC. Some studies reported on the prediction of histologic differentiation and invasion depth of gastric cancer using ME-NBI; however, its application is limited in clinical practice, and further well-designed studies are necessary. Clinicians should understand the ME-NBI classification system and acquire appropriate diagnostic skills through various experiences and training to improve the quality of endoscopy for EGC diagnosis.
