The Effect of Mild Cognitive Impairment Management Program on Traditional Korean Medicine in a Public Health Center (일개 보건소 한의약 경도인지장애 관리프로그램의 효과분석)

  • Objectives: The aim of this study was to evaluate the effect of a mild cognitive impairment (MCI) management program on traditional Korean medicine in a public health center. Methods: In order to examine the effect of the MCI management program, we investigated MCI patients who participated in the program. MCI patients were treated with Jodeung-san (JDS) or Dangguijagyag-san (DGJYS) for 6 weeks. Mini Mental State Examination-Dementia Screening (MMSE-DS), Geriatric Quality of Life-Dementia (GQOL-D), Geriatric Depression Scale in Korea (GDS-K), and biochemical examination of blood were conducted at intervals of 6 weeks. A satisfaction survey was collected after this program. Results: MMSE-DS mean score increased significantly, by 4.09 (p=0.000) in the JDS group and by 4.15 (p=0.000) in the DGJYS group. GQOL-D mean score also increased significantly, by 5.90 (p=0.000) in the JDS group and by 5.07 (p=0.000) in the DGJYS group. GDS-K mean score decreased significantly, by 10.63 (p=0.000) in the JDS group and by 9.69 (p=0.000) in the DGJYS group. MMSE-DS, GDS-K and GQOL-D scores were not significantly different between the JDS and DGJYS groups. All of the participants were satisfied with the program, and 90% of the participants expressed a high intention to participate in the program again. Conclusions: Both Jodeung-san and Dangguijagyag-san treatment showed improvement on cognitive function, depression and quality of life in this program. Additional studies with more subjects will be required to prove credibility and validity more clearly.

The Effect of Combined Cognitive-Motor Learning Program with Mild Cognitive Impairment Elderly Patients (경도인지장애노인 대상 융복합 운동 프로그램의 효과 : 신체 인지 기반 복합 인지-운동 중심)

  • The purpose of this study was to demonstrate exploring the field application of combined cognitive-motor learning program based on proprioceptive neuromuscular facilitation and Bartenieff Fundamental program. The combined cognitive-motor learning program(CC) was applied to the 10 MCI elder while 10 elder in occupational therapy(OT) took cognitive learning class. MMSE-K, Time up & go test(TUG), Tandem gait test(TA), GQOL-D were measured and analyzed. The collected data were analyzed by Independent & Paired T-test. The results were as follows: Both groups showed similar learning effect in MMSE-K test. However, in TA & GQOL-D test, CC group showed significant learning effect than OT group. From these result, we conclude that combined cognitive-motor learning program is valuable as alternative program for cognitive development and social development as well as physical development of MCI elder.

Effects of Occupation based Reminiscence Therapy on Early Dementia Patients' Cognitive function, Depression and quality of life (작업 중심 회상치료가 경도 치매 노인 환자의 인지기능과 우울, 삶의 질에 미치는 영향)

  • This study was performed to investigate the effects of Occupation based Reminiscence therapy on cognitive function, depression, and quality of life in elderly patients with mild dementia. Thirty-one elderly patients with mild dementia underwent Occupation based Reminiscence therapy for 40 minutes per session, once weekly, for 8 weeks. Montreal Cognitive Assessment Korean Version (MoCA-K), Short Form of Geriatric Depression Scale-Korean Version (SGDS-K), and Geriatric Quality of Life-Dementia (GQOL-D) were used to measure cognitive function, depression level, and quality of life, respectively. Mean MoCA-K score increased from $14.62{\pm}5.07$ before intervention to $16.88{\pm}4.55$ after intervention; mean SGDS-K score decreased from $7.28{\pm}4.28$ to $6.10{\pm}4.09$, and mean GQOL-D score increased from $26.05{\pm}5.45$ to $28.55{\pm}5.74$. Furthermore, these positive changes were all statistically significant. Based on the results of this study, it was concluded Occupation based Reminiscence therapy may positively affect cognitive function, depression, and quality of life in elderly patients with mild dementia.

The Effects 12 Weeks of Combined Exercise Programs on Activities of Daily Living and Quality of Living Index in the Vascular Dementia Elders (12주간 복합운동프로그램이 혈관성 치매노인의 일상생활 수행능력과 삶의 질 지수에 미치는 효과)

  • Purpose : This study was to investigate vascular dementia elders's activities of daily living, quality of life index and to determine how much vascular dementia elders is influenced by the 12 weeks combined exercise program with aerobic training and resistance exercise. Methods : Thirty patients with vascular dementia over 65 in B geriatrics hospital were recruited this study. Activities of daily living were measured by I-ADL(instrumental-activities of daily living) and B-ADL(basic-Activities of daily living) and quality of life index was measured by GQOL-D(geriatric quality of life scaledementia). Statistical analysis was used repeated one-way ANOVA to test mean difference by using SPSS 12.0 for windows. Results : After comparing the activities of daily living of experimental group that of control group according to the period of exercise, there were statistically significant differences in I-ADL, B-ADL score test and GQOL-D index test of both experimental and control groups. There was also a significant difference in comparing the results of 12 weeks exercise of the groups. Conclusion : 12 weeks combined exercise program had a good influence on vascular dementia elders's activities of daily living and quality of life index.

The Effect of Alternative Medicine on Quality-of-Life Outcomes for Dementia Patients: A Systematic Review and Meta-Analysis

  • Objectives: As effective treatments for dementia are lacking in Western medicine, complementary and alternative medicine (CAM) is considered a useful option. While the quality of life (QoL) is a vital outcome for patients with dementia, the QoL of patients receiving CAM for dementia remains ambiguous. This study aimed to determine the effect of CAM on QoL outcomes in dementia patients. Methods: A search was performed using the keywords "dementia," "Alzheimer's," "cognitive impairment," "Chinese," "Korean," "oriental," "herbal," "acupuncture," and "quality of life". All quantitative data were synthesized using R version 4.1.1. Results: Twenty-five randomized controlled trials (RCTs), 16 pre-post trials, and two cohort studies were selected for the systematic review. QoL in Alzheimer's disease (QOL-AD) (n=11, 25.6%) and geriatric QoL in dementia (GQOL-D, n=9, 20.9%) were the most utilized QoL instruments. Significant benefits in QoL were observed after receiving mind, body, combined mind and body, nursing, oriental medicine, and acupuncture therapies. In the meta-analysis, the combined effect was shown to significantly increase QOL-AD compared to before CAM interventions (standardized mean difference, SMD: 0.507; 95% confidence interval (CI), 0.191~0.824; p<0.01). The overall synthesized estimates in the GQOL-D showed a significantly improved QoL (SMD: 0.537, 95% CI: 0.238~0.837 p<0.01; one group; SMD: 1.465, 95% CI: 0.934~1.996, p<0.01). The seven studies assessing the cost-effectiveness of CAM reported uncertain outcomes. Conclusions: This study showed that CAM interventions benefited patients with dementia by improving their QoL. While additional standardized research is required, CAMs are suggested as effective clinical management for patients with dementia. They are also suggested as complementing therapies for these patients.

Effect of Implementing a Community-Care Occupational Therapy Program for Home Health Management (가정건강관리를 위한 커뮤니티케어 작업치료프로그램 적용 효과)

  • Purpose : Community care occupational therapy is an integrated mediation method such as evaluating and mediating daily life activities, evaluating and modifying the environment, and providing information by visiting the home where the client resides. In this study, we examine the effect of implementing a community-care occupational therapy program on the levels of depression, quality of sleep, and quality of life of elderly individuals living alone. Methods : Among the users of organizations C and D located in City B, province A, South Korea, a total of 32 elderly citizens aged above 65 were chosen as the study subjects and randomly divided into an experimental group and a control group. For the experimental group, a total of 24 sessions, each lasting 60 minutes, comprising a community-care occupational therapy program and each organization's own program, were conducted, with each session conducted twice a week for a total of 12 weeks. On the other hand, those in the control group participated in the existing programs of their organizations. Changes in the levels of depression, quality of sleep, and quality of life before and after the intervention were compared using the GDSSF-K, GQOL, and GQOL. Results : Overall, the levels of depression, quality of sleep, and quality of life were found to improve among the experimental group after the intervention. In a between-group comparison of the changes before and after the intervention, the experimental group was found to exhibit a more statistically significant difference than that of the control group. Conclusion : The results obtained in this study confirm that community-care occupational therapy programs have a positive effect on the levels of depression, quality of sleep, and quality of life of elderly individuals living alone. This implies that such programs may represent an effective solution for the problems of these individuals.

The effect of the multi intervention program applying to dementia elderly (치매노인에게 적용한 복합 중재프로그램의 효과 비교 연구)

  • Objective : We tried to know the effect on the ability of work performance, cognitive, balance, depression, ability of daily living, the quality of life, and time usage after applying the group cognitive therapy that based on lifestyle redesign to dementia elderly. Methods : We performanced the group cognitive therapy that based on lifestyle redesign to 15 dementia elderly for 12weeks by total 5 steps. We evaluated the ability of work performance(COPM), cognitive(MMSE-DS), balance(BBS), depression(GDS-K), ability of daily living(MBI), the quality of life(GQOL-D), and time usage(OQ) of before and after the programme. Results : There was a significant difference of the ability of work performance, balance, the quality of life, and locomotive area of time usage after applying the group cognitive therapy that based on lifestyle redesign to dementia elderly. Conclusion : The group cognitive therapy that based on lifestyle redesign programme is effective for satisfaction of work performance and improvement of the quality of life and balance ability. The programme is also effective for locomotive area of time usage. We found out the effect of the programme in variety of the life of dementia elderly.

Observational Study of the Treatment of 116 Cases of High Risk Group for Dementia and Depression in Multiple Korean Medicine Clinic (치매 및 우울증 고위험군 116례의 한의 치료에 대한 관찰 연구)

  • In order to reduce the social and economic costs due to the increase in dementia patients, which is a representative senile disease in an aging society, it will be important to prevent dementia and to detect and treat early in high-risk groups. This study reported the results of treatment for the elderly with high risk of dementia and depression who received Korean medicine (KM) treatment in 9 KM clinics. Medical charts were surveyed on 116 patients with high risk of dementia and depression who received KM treatment at 9 KM clinics in Gangseo-gu from September 1, 2020 to December 31, 2020. The majority of the patients were female(76.72%), the average age was 71.66±7.18 years old, and the average education level was 8.96±3.91 years. The average treatment period was 61.47±10.30 days, the average number of treatments was 15.38±1.06 times, and both acupuncture and herbal medicine were administered. After receiving KM treatment, cognitive assessment scores such as MMSE-DS and MoCA-K, depression-related GDSSF-K score, dementia knowledge, attitude, and preventive behavior, blood debilitation scale, and GQOL-D score were improved compared to before. There was no specific adverse reaction except that one patient with elevated AST and ALT levels was observed. This study showed improvement in cognitive function, depression, dementia-related scale, and quality of life in high-risk groups for dementia and depression over 60 years old who received KM treatment. More systematic and large-scale planned clinical studies will be needed.

A Study on the Quality of Life of Elderly People with Dementia and the Environmental Factor of Facilities (치매노인의 삶의 질과 시설 환경 요인에 관한 연구)

  • There have lately been a variety of social issues in our society due to rapid social changes. Specifically, how to approach elderly people who suffer from dementia is never an easy task, and few in-depth studies have ever focused on their quality of life due to that. The purpose of this study was to examine the quality of life of elderly people with dementia and the relationship between their quality of life and the environments of facilities for them in an attempt to lay the foundation for the development of compatible programs tailored to the environments of the facilities and for relevant policy setting. It's ultimately meant to improve the quality of life of the elderly with dementia and the environments of facilities for them. The subjects in this study were elderly people with dementia who were housed in senior residential and medical welfare facilities in Daegu and Gyeongsangbukdo. The collected data were analyzed with a SPSS 12.0 program, and frequency analysis, cross-tabs and multiple logistic regression analysis were utilized. As a result, facility environments were identified as one of the variables that had a significant impact on the quality of life of the elderly people with dementia. There are some suggestions about how to boost their quality of life: First, good environments should be prepared in consideration of the characteristics of elderly people with dementia in order for themto be satisfied with their own quality of life, and the way of looking at their potentials should be changed. Second, it's found that main caregivers affected the quality of life of the elderly people with dementia, and the kind of programs that focus on the improvement of the relationship between elderly people with dementia and their main caregivers is required. Third, there should be a change in the environments of the facilities. The facilities should be well equipped to successfully respond to the symptoms of elderly people with dementia. To redress their poor accessibility to the facilities, infrastructure involving nursing homes and professional personnels should be built by utilizing the Internet, and the facilities and local community should make concerted efforts to provide quality care to elderly people in want of it.
