수술적 방법으로 유도된 3단계 발목염좌에 대한 전침의 진통기전 연구 (The Mechanism for Analgesic Effects of Electroacupuncture on Surgical Ankle Sprain Model Classified as Grade 3 in Rats)

  • Objectives : Electroacupuncture(EA) has been used effectively in producing analgesia on ankle sprain pain of humans and animals. Currently to examine the underlying mechanisms of the EA-induced analgesia, the effects of EA on weight-bearing forces(WBR) were examined at ankle sprain classified as grade 3 in rats. Methods : The severe ankle sprain classified as grade 3 was induced surgically by ankle ligament injury in the Sprague-Dawley rats. WBR of the affected foot were examined to evaluate effects and mechanism of EA(2 Hz, 1 ms pulse width, 2 mA intensity, for 15 min) which was applied to either SI6, GB34, or GB39 acupoints. The rats were pretreated with naltrexone(10 mg/kg, i.p.) as an opioid receptor antagonist or phentolamine(5 mg/kg, i.p) as an ${\alpha}$-adrenoceptor antagonist at 30 min before EA. Results : The daily repeat EA at either SI6, GB34, or GB39 showed significant analgesic effects on the severe ankle sprain. Particularly, daily EA at GB34 showed more potent analgesic effect than the others. In addition, the naltrexone pretreatment completely blocked the analgesic effect of EA at GB34, indicating the involvement of the endogenous opioid system in mediating the effect of EA at GB34. However, the phentolamine pretreament blocked analgesic effects of EA at either SI6 or GB39, indicating the involvement of ${\alpha}$-adrenoceptors in mediating the effect of EA at either SI6 or GB39. Conclusions : These data suggest that EA-induced analgesia on ankle sprain pain is mediated through either endogenous opioids or ${\alpha}$-adrenoceptors dependant on acupoint specific pattern.

부산·경남 지역 성인의 담낭용종 위험인자 및 초음파 영상의 형태학적 분석 (Analysis of Risk factors & Morphological Ultrasound Image for Gallbladder Polyp in Adults Living in Busan and Gyeongnam Provinces)

  • 본 연구는 부산 경남지역에서 담낭용종의 위험인자 및 초음파영상의 형태학적 분포를 알아보고자 하였다. 실험 대상은 2016년 1월~5월까지 부산 P병원 내원환자의 복부초음파 영상을 대상으로 하였다. 그 중 복부초음파와 혈청학적 검사를 동시에 실시한 399명을 대상으로 위험인자를 분석하였다. 담낭용종 위험인자들의 통계분석은 독립표본 t검정(independent t-test)과 카이제곱 검정(chi-square test)을 시행하였다. 차이검정 결과를 고려하여 독립변수에 대한 상대 위험비(odds ratio, OR) 산출을 위해 다중 로지스틱 회귀분석(multiple logistic regression analysis)을 시행하여 변수들로부터 예측모형을 산정하여 타당성을 검정하였다. 그 결과 담낭용종 위험인자로 남성, HBsAg 양성, 중성지방이 관련이 있음을 알 수 있었다. 담낭용종의 위험인자로 확인된 남성, HBsAg 양성, 중성지방으로 예측모형 및 예측 확률값을 산정하였다. 예측확률의 민감도 61.0%, 특이도 76.8%를 보였으며, ROC 곡선의 AUC 결과는 0.735를 보여 예측모형의 타당성을 확인할 수 있었다. 복부 초음파검사 상 관찰되는 담낭용종의 형태학적 분석 결과는 고 에코, 유경, 균질한 형태가 가장 많은 분포(27.5%)를 나타내었으며, 용종 개수는 2개(38%), 크기는 5~10 mm (53%)로 가장 많았다. 담낭용종과 관련된 간질환으로는 mild fatty liver (23%), diffuse hepatopathy (21%)로 나타났다.

사암침법(舍岩鍼法) 중 육양경(六陽經)의 승격(勝格) 혈위(穴位)에 대한 염전보사(捻轉補瀉) 침자(鍼刺)가 고지방식이(高脂肪食餌)로 유발된 고지혈증(高脂血症) 백서(白鼠)에 미치는 영향(影響) (Effects of five element constitutional acupuncture(Sa-am acupuncture) using needle manipulation to sedate Six Fu-Organ on serum lipid and liver function of hyperlipidemic rats induced by high fat diet)

  • Objective : The aim of this research was to investigate the effect on five element constitutional acupuncture(Sa-am acupuncture) at 4 acupoints by needle manipulation to sedate Six Fu-Organ on hyperlipidemia induced by high fat diet in rats. Method : We recorded data of weight gain, food intake, food efficiency, weight of liver, heart, spleen, lung, kidney, levels of total cholesterol, triglyceride, HDL-cholesterol, AST, ALT in serum of rats fed high fat diet for 5 weeks. Spraque Dawley rats were divided into eight groups(normal; normal diet and non-acupuncture group, control; non-acupuncture group, Acu-LI; acupuncture at $SI_5{\cdot}LI_5{\cdot}BL_{66}{\cdot}LI_2$ group, Acu-ST; acupuncture at $GB_{41}{\cdot}ST_{43}{\cdot}LI_1{\cdot}ST_{45}$, Acu-SI; acupuncture at $BL_{66}{\cdot}SI_2{\cdot}ST_{36}{\cdot}SI_8$, Acu-BL; acupuncture therapy at $ST_{36}{\cdot}BL_{40}{\cdot}GB_{41}{\cdot}BL_{65}$, Acu-TE; acupuncture at $BL_{66}{\cdot}TE_2{\cdot}ST_{36}{\cdot}TE_{10}$, Acu-GB; acupuncture at $LI_1{\cdot}GB_{44}{\cdot}SI_5{\cdot}GB_{38}$. After needles inserted, it was rotated clockwise and anticlockwise 12 times on rats fed high fat diet. Results : The body weight was decreased in Acu-SI, Acu-TE, Acu-GB group, /the food intake was decreased in Acu-TE, / the food efficiency was decreased in Acu-SI, /serum triglyceride was decreased in Acu-LI, Acu-ST, Acu-BL, Acu-GB, /serum total cholesterol was decreased in Acu-LI, Acu-BL, serum ALP was decreased in Acu-LI, Acu-ST, Acu-BL, Acu-TE, Acu-GB. While the relative weight of heart was increased in Acu-LI, Acu-SI, the relative weight of kidney was increased in Acu-SI, Acu-GB. Conclusion : These results suggest that five element constitutional acupuncture(Sa-am acupuncture) may be having an therapeutic influence on hyperlipidemia induced by high fat diet. Further researches of acupuncture manipulation are needed in the future based on our study.

접착시스템의 종류에 따른 유동성 레진과 법랑질의 미세전단 결합강도 (Microshear bond strength of a flowable resin to enamel according to the different adhesive systems)

  • 연구목적: 이 연구는 2종의 전부식 접착제와 4종의 자가부식 접착제를 이용하여 유동성 레진을 법랑질에 접착할 경우, 접착제에 따른 미세전단 결합강도와 파절양상을 비교하기 위하여 시행하였다. 연구 재료 및 방법: 발거된 60개의 대구치 치관의 법랑질을 이용하였고, 사용된 접착시스템의 종류에 따라 6개의 군; OS 군(One-Step), SB 군(Single Bond), SE 군(Clearfil SE Bond), TY 군(Tyrian SPE와 One-Step Plus), AP 군(Adper prompt L-Pop), GB 군(G-Bond)으로 분류하였다. 각 군의 법랑질 표면에 배정된 접착시스템을 적용한 후, Tygon tube를 이용하여 법랑질 표면에 유동성 레진(Filtek Z 350 flowable resin)을 접착하였다. 각 군의 미세전단 결합강도를 측정한 후 주사전자현미경하에서 파절양상을 비교하였다. 결과: 1. SB 군의 결합강도는 다른 모든 군보다 통계학적으로 높게 나타났고, GB 군의 결합강도는 다른 모든 군의 결합강도 보다 통계학적으로 낮게 나타났다(p < 0.05). 2. OS 군과 SE 군 및 AP 군의 결합강도는 SB 군보다 낮게 나타났지만, TY 군과 GB 군보다 통계학적으로 높게 나타났다(p < 0.05). 3. TY 군의 결합강도는 GB 군보다 통계학적으로 높게 나타났다(p < 0.05). 4. 접착성 파절은 TY 군과 GB 군에서 가장 많이 나타났고, 혼합파절은 SB 군과 SE 군에서 가장 많이 나타났으며, 응집성 파절은 OS 군, SB 군, SE 군 및 AP 군에서 각각 1개씩 나타났다. 결론: 접착 단계가 같은 접착시스템이라 해도 법랑질에 대한 유동성 레진의 미세전단 결합강도는 서로 다르게 나타났다.

골절에 침 치료를 적용한 2013년 이후 증례보고 문헌고찰 (A Review of Case Reports on the Application of Acupuncture as a Treatment for Fracture since 2013)

  • Objectives This study aimed to investigate the trend in treating fracture with acupuncture and to evaluate the quality of case reports. Methods All case reports of fractures treated with acupuncture were extracted from four Korean web databases. We classified these studies by five fracture sites and investigated frequently used meridian and acupoint, outcome measurements, treatment period. And we assessed the quality of the case reports by the STandards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA) guidelines. Results A total of 33 case reports were included. The outcome measurements were divided into six categories, and the outcome measurements used in more than three studies showed improvement in patient symptoms. The mainly used meridians for each fracture site were as follows: chest and abdomen (GB, LR, CV), back (BL, GB), upper limb (LI, TE), lower Limb (GB, ST, BL, SP). The most commonly used acupoints for each fracture site were as follows: chest and abdomen (Ashi points, GB24, GB25, GB26, LR13, LR14, CV16, CV17, CV18, CV19), back (BL23, BL24, BL25, BL26, BL40, BL51, BL52, BL60, GB34), upper limb (LI4, LI10, LI11, TE3), lower limb (GB34, GB40). According to the STRICTA guidelines, more than 54.54% of the reports were found to be 'not reported' or 'not sufficient' in the following categories: 'response sought', 'description of participating acupuncturist', and 'number of needle insertions per subject per session'. Conclusions The meridians and acupuncture points frequently used for acupuncture treatment of fractures were near the fracture site. Future clinical studies involving acupuncture must be reported in accordance with the STRICTA guidelines to improve transparency and uniformity.

Comparative Study of Implicit and Explicit Solvation Models for Probing Tryptophan Side Chain Packing in Proteins

  • We performed replica exchange molecular dynamics (REMD) simulations of the tripzip2 peptide (betahairpin) using the GB implicit and TI3P explicit solvation models. By comparing the resulting free energy surfaces of these two solvation model, we found that the GB solvation model produced a distorted free energy map, but the explicit solvation model yielded a reasonable free energy landscape with a precise location of the native structure in its global free energy minimum state. Our result showed that in particular, the GB solvation model failed to describe the tryptophan packing of trpzip2, leading to a distorted free energy landscape. When the GB solvation model is replaced with the explicit solvation model, the distortion of free energy shape disappears with the native-like structure in the lowest free energy minimum state and the experimentally observed tryptophan packing is precisely recovered. This finding indicates that the main source of this problem is due to artifact of the GB solvation model. Therefore, further efforts to refine this model are needed for better predictions of various aromatic side chain packing forms in proteins.

광PCB용 CMOS 광수신기 설계 (A CMOS Optical Receiver Design for Optical Printed Circuit Board)

  • 5Gb/s대역 크로스커플 구조의 트랜스임피던스 증폭기 및 제한증폭기가 연결된 광 수신기를 광 PCB에 응용하기 위해 설계 하였다. 회로는 0.18um CMOS 공정으로 구현되었다. 광 수신기는 0.5pF 광 다이오드 기생 캐퍼시턴스에서 $92.8db{\Omega}$ 임피던스 이득과 5Gbps의 주파수 대역을 갖는다. 그리고 1.8V, 2.4V 공급전압에서 9.74mV의 전력소모를 보인다. 입력단의 임피던스는 $50{\Omega}$ 이다. 회로를 광 PCB기판에 올려 광신호 송신 실험하여 5Gb/s 데이터의 수신을 확인하였다.

자하거(紫河車) 약침(藥鐵)이 수면장애(睡眠障碍)에 미치는 효과(效果) (The Effects of the Hominis Placenta Herbal acupuncture on Sleep pattern disturbance)

  • Objectives : This study has been designed and performed to identify the effect of Hominis Placenta herbal acupuncture which is usually used in reducing sleep pattern disturbances. Methods : The study subjects studied included 48 patients who were admitted in hospital located in Pusan, and they were classified into 2 groups : 25 patients in the experimental group who injected Hominis Placenta herbal acupuncture and 23 patients in the control group who were treated by acupuncture. The both group injected on $GB_{20}$, $GB_{12}$ and $GB_{20}$ for 5 days without medicine. The sleep pattern disturbance score was measured by using 15 questions according to Korean Sleep Scale A(Oh, Jin Joo. Song, Mi. Soon. Kim, Shin Mi. 1998). Results & conclusions : The sleep pattern disturbance score of the experimental group who injected Hominis Placenta herbal acupuncture was significantly lower than that of the control group(t=7.00 p=.00). These results provided that Hominis Placenta herbal acupuncture of $GB_{20}$, $GB_{12}$ and HT, was effective for relieving sleep pattern disturbances, it is need mere sample's number and more treatment's duration.

10 Gb/s XFP Transceiver용 Transmitter Optical Sub-assembly(TOSA)의 RF 설계/제작 및 주파수 특성 해석 (Design, Fabrication and Frequency Analysis of Transmitter Optical Sub-assembly for a 10 Gb/s XFP Transceiver)

  • XFP(10 Gb/s Small Form Factor Pluggable) 트랜시버 모듈에 사용되는 광송신 서브어셈블리(TOSA: Transmitter Optical Sub-assembly)를 열적, 전기적 측면에서 설계, 제작 및 평가하였다. 저가격이며 소형인 TOSA를 제작하기 위해서 바이어스 티 및 정합저항등을 AlN서브마운트 위에 집적하였다. 10 Gb/s의 초고속 전기 신호의 입력을 위해 CPW형 마이크로파 전송로를 스템 구조체내에 도입하였다. 소자의 전기적 특성 분석을 위해 서브마운트내에 실장되는 각종 전기광학 부품들과 스템 구조체들을 고속회로 모델링 하였다. 제작한 TOSA의 특성평가를 통해, 85$^{\circ}C$에서 11 GHz 이상의 -3 dB 대역폭을 얻을 수 있었다.

가스절연모선(GIB)에서 전자파 방전신호의 모드별 군속도 차이를 이용한 방전위치 산정기법 (Partial Discharge Location Method using Group Velocity Difference of Modes in a Electromagnetic Partial Discharge Signal in Gas Insulated Bus)

  • We developed a novel method of partial discharge(PD) location based on the fact that the waveform of PD signal propagate along the GIB (Gas Insulated Bus) is composed of several modes of electromagnetic wave with different group velocities and cut-off frequencies. From the PD waveform, measured at a broadband PD sensor attached on the GIB, we could derive arrival time and frequency components of different modes using the short term Fourier transform or etc. After the group velocities of different modes are calculated, the location of the PD source could be estimated. To show the effectiveness of this new locating method in a real on site application, we used this method to locate the position of a PD source at a 76 m long 345 kV GIB substation. The estimated location of the PD source using the method proposed above was in good agreement with the actual location found from the inspection result of internal component in the GIB with 2.4% of the estimation error.
