Motor Function Recovery in Stroke Patients with Corona Radiata Infarct: 4 Case Studies

  • Purpose: The aim of this study was to use fMRI and clinical prognosis criteria to evaluate therapeutic interventions in stroke patients with corona radiata infarct and acquire fundamental information about recovery mechanisms. Methods: Four subjects (2 men, 2 women) who had strokes with corona radiata infarct were recruited. For all subjects, motor functions such as motricity index (MI), modified brunnstrom classification (MBC), functional ambulatory category (FAC), and bathel index (BI) were evaluated. Evaluations were done at least 4 times over a period of approximately 6~7 months from stroke onset. We compared the final evaluation with the first. Results: All patients with corona radiata infarct showed improvement in motor outcomes with the passing of time. The strength of all patients improved from zero or trace levels to normal or good levels in the MI (Motricity Index) test. Other motor outcomes including the modified brunnstrom classification (MBC), the functional ambulatory category (FAC), and the bathel index (BI) also improved with the passing of time. Conclusion: Stroke patients with corona radiata infarcts change for the better over time. Therefore, one can introduce clinical interventions by the aspect of progress in functional motor recovery.

지역사회 뇌졸중 환자를 위한 순환식 집단 운동프로그램의 효과 (Effect of a Weekly Circuit-Group Exercise Program on Community-Living Individuals With Chronic Stroke)

  • This study aimed to evaluate the effect of a weekly circuit-group exercise program on functional performance, flexibility of the trunk and lower extremities, and balance in individuals with chronic stroke who are living in a community. Thirteen community-living individuals (eleven males and two females) with chronic stroke participated in this study. The group exercise program was set up as a weekly circuit application with four stations aiming to strengthen the muscles, increase endurance, improve flexibility, and enhance functional capacity. The average duration of a session was an hour, and the sessions were conducted once a week for six months for a total of 24 sessions. Assessments for functional performance, flexibility of the trunk and lower extremities and balance were conducted four times: before treatment, after one month of treatment, after four months of treatment, and after six months of treatment. There were significant improvements in functional performance and flexibility of the trunk and lower extremities in the given time frame. No significant improvement was observed in balance ability. The findings suggest that a weekly circuit-group exercise program has some benefits in terms of managing the physical symptoms of individuals with mobility problems after stroke. Therefore, this program can be adapted and employed as a community-based rehabilitation program for such patients. Further studies with various community-based treatments will be conducted to validate these findings.

Physical Activities and Health-related Quality of Life of Individuals Post Stroke

  • PURPOSE: The purpose of this study is to examine the relationship between the physical activities of individuals post-stroke and their HRQL, as well as to determine whether their functional abilities contribute to their amounts of physical activity. METHODS: The study's subjects included 90 individuals post-stroke. Their amounts of physical activity were measured using the International Physical Activity Questionnaire (IPAQ), and their HRQL was measured using the Medical Outcomes Study 36-Item Short-form Health Survey (SF-36). In addition, the functional abilities of the subjects were measured. For the measures of physical activities and the HRQL, Pearson's correlation coefficients were used to identify the strengths of the associations between the measures. A hierarchical linear regression model was used to determine whether physical activities had independent impacts on the HRQL. RESULTS: This study found that the physical activities performed by the subjects affected the SF-36 physical component score (PCS) (12%). However, the physical activities and the SF-36 mental component score (MCS) showed no statistically significant relationship, whereas functional abilities and physical activities had a statistically significant relationship (r = .57~.86, p<.001). CONCLUSION: The present study identified a correlation between physical activity and the PCS. Therefore, individuals post-stroke should be encouraged to carry out more physical activities, including more frequent walking activities.

  • Purpose : In this study, stroke patients' limits of stability and functional reach test and tibialis anterior, gastrocnemius muscle of lower extremity muscle activities to evaluate the correlation. Methods : 30 adult stroke patients to participate in this study. Limits of stability were measured using biorescue, tibialis anterior and gastrocnemius muscle of muscles activities were measured with functional reach test when there was movement. Results : Limits of stability and functional reach test (r=0.753, p<0.01), RMS value of the limits of stability and tibialis anterior muscle (r=0.706, p<0.01), RMS value of the limits of stability and gastrocnemius muscle (r=0.766, p<0.01), RMS value of the functional reach test and tibialis anterior muscle (r=0.835, p<0.01), RMS value of the functional reach test and gastrocnemius muscle (r=0.663, p<0.01), RMS value of the tibialis anterior and gastrocnemius muscle (r=0.816, p<0.01) correlations are shown as statistically significant. Conclusion : The balance and lower extremity muscle activities of stroke patients were studied, and were positively correlated with the RMS value of the limits of stability and functional reach test, tibialis anterior muscle, and gastrocnemius muscle. This study has shown that stroke patients' ankle joint muscle activity can greatly.

일부 뇌졸중 환자의 기능변화 및 관련요인 (Difference of Functional Outcome and Related Factors in Patients With Stroke)

  • The purposes of the present study were to determine the difference of functional outcome, and to identify factors associated with functional difference in patients with stroke. The sample consisted of 56 stroke patients who had received physical therapy at the physical therapy unit of the Dongeui Medical Center in Busan city between January 2000 and June 2002. Stroke patients were evaluated by physical therapists 3 times; The first day in physical therapy (PT) (T1), one month after the first day in PT (T2), and two months after the first day in PT (T3). Functional status was assessed with the Functional Independence Measure (FIM) instrument, a validated instrument for documenting the severity of disability and assessing the outcome of rehabilitation treatment. Functional gain was calculated over T2-T1, T3-T1, and T3-T2. SAS statistical software was used for the analysis. The Student's t-test, paired t-test, analysis of variance (ANOVA/Tukey and Scheffe), and analysis of covariance (ANCOVA) were used to examine the functional difference in variables. Repeated measures ANOVA was also used to analyze the functional difference by time (T1, T2, and T3). Multiple regression analysis was performed to determine the effects of independent variables on the difference of functional outcome as defined by the FIM score. A total of 56 stroke patients were evaluated, their average age${\pm}$standard deviation was $61.6{\pm}9.3$ years (range: 40~81 yr). The functional status of patients who received physical therapy for about 2~3 months was significantly improved (mean FIM scores, $20.5{\pm}1.8$, $28.9{\pm}1.9$, and $8.41{\pm}1.1$ points for each time period, respectively) (p<.0001). Diabetes was significantly associated with the FIM score for T2-T1 (p<.05). The type of diagnosis was significantly associated with the FIM score for T3-T1 (p<.05). Gender, smoking, and the FIM score on admission were significantly associated with the FIM score for T3-T2 (p<.05). In conclusion, gender, smoking, diabetes, the type of diagnosis, and the FIM score on admission were significantly associated with improved FIM scores. We recommend that further research should explore the functional outcome by using larger sample sizes, longer follow-up periods, and more sensitive assessment instruments.

  • The Functional Independence Measure (FIM) is widely used to determine the dependency of activity of daily living in rehabilitation patients. The purposes of this study were to evaluate the unidimentionality of the FIM physical items and to analyze the validity of cross-functional levels in stroke survivors in Korea. Thirteen physical items of FIM were rated according to an ordinal scale of a 7-level classification. Two hundred and seventy-nine patients participated in the study (age range 18~92 years and 57% male). Six items-eating, bladder control, bowel control, transfer to and from the bed/wheelchair, transfer to and from the toilet, and bathing-showed misfits with the Rasch model. The most difficult item was 'bathing', the easiest item was 'bowel control'. Although there were several differences within functional levels, the hierarchical order of item measures was rather similar. 'Bathing' was the most difficult in high level patients (above 60), however 'stairs' was most difficult in the middle level (41~60) group. In the low level group (below 40), 'toileting' was the most difficult. In conclusion, the present study has shown several differences of item difficulty among functional levels. This result will be useful in planning interventions, and developing rehabilitation programs for stroke survivors.

  • 본 연구의 목적은 뇌졸중 동물 실험 모델에서 환경 강화는 기능 회복을 향상 시키고 이로 인하여 뇌에 형태학적, 생화학과 행동에서 특징적인 변화와 그 효과들을 소개하고, 뇌졸중 환자에 환경 강화의 적용은 재활치료 및 작업치료를 포함한 다양 치료와 전 임상 실험의 중요성을 설명하고자 한다. 이 종설 논문에서는 주로 뇌졸중 동물 실험 모델에서 환경 강화로 인한 기능적 회복에 대한 효과와 신경 친화성 물질, 특정 단백질의 발현 및 임상 적용 사례 등의 연구 결과들을 소개하였다. 마지막으로 임상에서 뇌졸중 환자에게 환경 강화의 적용 가능성과 작업치료 및 재활치료의 관점에서 설명하였다.

  • Stroke is a leading cause of chronic physical disability. The recent randomized controlled trials have that motor function of chronic stroke survivors could be improved through physical or pharmacologic intervention in the stroke rehabilitation setting. In addition, several functional neuroimaging techniques have recently developed, it is available to study the functional topography of sensorimotor area of the brain. However, the mechanisms involved in motor recovery after stroke, are still poorly understood. Four motor recovery mechanisms have been suggested, such as reorganization into areas adjacent to the injured primary motor cortex (M1), unmasking of the motor pathway from the unaffected motor cortex to the affected hand, attribution of secondary motor areas, and recovery of the damaged contralateral corticospinal tract. Understanding the motor recovery mechanisms would provide neurorehabilitation specialists with more information to allow for precise prognosis and therapeutic strategies based on the scientific evidence; this may help promote recovery of motor function. This review introduces several methodologies for neuroimaging techniques and discusses theoretical issues that impact interpretation of functional imaging studies of motor recovery after stroke. Perspectives, for future research are presented.

The effect of gait training with an elastic ankle-foot orthosis on balance and walking ability of persons with chronic stroke: a randomized controlled trial

  • Objective: The purpose of this study was to investigate the effect of treadmill exercise on the posture and walking speed of chronic stroke survivors with an ankle-foot orthosis. Design: Randomized controlled trial. Methods: Twenty-four chronic persons with chronic stroke admitted to Bobath Memorial Hospital in Seongnam city were divided into two groups by random blind method. Treadmill exercise with an elastic ankle-foot orthosis was performed in the experimental group and treadmill exercise was performed in the control group. The experiment was carried out for 6 weeks, and the experiment was carried out three times a week for 20 minutes per session. To measure the effect, static balance was measured using the MTD system before and after training, and the Berg Balance Scale (BBS) was used to measure functional balance. Results: There was a statistically significant difference between the 2 groups in the BBS measurement results for confirming the functional balance (p<0.05). Also, there was a significant difference between the 2 groups in single limb support time, step time and step length (p<0.05). Conclusions: In this study, it was found that treadmill exercise with an elastic ankle-foot orthosis in persons with chronic stroke was effective in maintaining functional balance, walking ability, step length, and step time. Therefore, it is necessary to use a flexible ankle-foot orthosis with proper treadmill exercise as a method of improving balance and walking speed of chronic stroke survivors.

  • Objectives : We studied the aspects of headache in the patients with stroke. We compared character of headache on stroke with functional one. Methods : The subject of this study was based on 63 patients with headache who were admitted to the oriental hospital after stroke. We analyzed patients into sex & age, month, severity of headache on stroke type & lesion, site & character of headache. associated symptoms & signs on headache state.Results : The age of headache with stroke is higher than general headache. There was significant differences between general headache and headache with stroke. The cerebral hemorrhage is severer than cerebral infaction in th pain of headache. The most frequently appeared site of headache is temporal area and associated symptoms is vertigo & nausea.Conclusions : The headache of stroke patient is different form functional headache. because of cerebral vascular accident influence on inducing headache. Therefore we should study different methods between the headache of stroke patient and the others in medical treatment.
