Direction Reorganization of Public Health Center Functions through Analysis of Medical Service Status by Public Health Care Institutions (지역보건의료기관의 진료현황 분석을 통한 보건소 기능개편 방향)

  • Health centers provided intensive health care services for local residents according to changes in the times and environment. Public health centers were given various roles such as medical treatment, administration, and service, and the demand for functional reorganization has emerged. We analyzed the literature on the functional restructuring of public health care institutions. In addition, the current status of medical services, which is the main function of institutions, will be analyzed through health insurance statistical data, and detailed contents will be analyzed according to regional types and income levels. As a result of the analysis of medical services at institutions, the total number of patients was 2,238,000, and the number of visits was 11,806 times. Total medical expenses were 169.6 billion won, of which 132 billion won was found to be benefit. When analyzing the number of patients per institution, public health centers had the largest number of 4,326, and the share of benefit was also the highest at public health centers. It should focus on the function of providing local health and medical services related to health promotion and disease prevention in the community. This functional reorganization of public health centers can contribute to forming cooperative relationships with private medical institutions in the local community. For this, first, to establish the role, essential functions for public health centers for preventive health management are established. Secondly, regular manpower expansion and flexible manpower management are required in the human resources sector. Finally, in the organizational sector, it is necessary to establish a step-by-step organizational system according to environmental changes.

A Case Report of Complex Korean Medicine Treatment Application Including Chuna Manual Therapy for Functional Recovery After Sacroiliac Joint Fusion (천장관절융합술 후 기능 회복에 대한 추나요법을 포함한 한의복합치료 증례보고 1례)

  • Objectives This case study aimed to investigate the effect of complex Korean medicine treatment including Chuna manual therapy on sacroiliac joint (SIJ) pain status post SIJ fusion with sacroiliac screw fixation. Methods Complex Korean medicine treatments including Chuna manual therapy were provided to patients with SIJ widening due to a traffic accident trauma. Measurement of range of motion and manual muscle test to evaluate functional activities of daily living was conducted before and after treatment. Moreover, outcome estimates were performed using the numeric rating scale, pain disability index, and EuroQol 5-dimension five-level questionnaire. Results After complex treatment, functional activities of daily living improved. Sacroiliac joint pain decreased and the quality-of-life score improved. Conclusions This study suggests that treatment with complex Korean medicine treatment including Chuna manual therapy may improve traumatic SIJ widening status post SIJ fusion. A postoperative rehabilitation protocol based on accumulated research results considering a multidisciplinary approach should be prepared to ensure holistic treatment.

  • The study aimed to identify major factors related to global self-rated health of the community-dwelling elderly people in rural areas. Interviews were conducted with 433 persons over 65 years of age. The socio-economic characteristics, chronic disease status, measures of functional and mental health, life satisfaction, health-related behaviors including smoking, drinking, exercise, social activity, dietary habit, and food intakes were analyzed to determine their influence on self-rated health index. Data on food intake were obtained through the 24-hour recall method. The self-rated health of rural elderly was poor or very poor as reported by 42.6% and 52.4% of the men and women, respectively. Poor self-rated health was found to be related to elderly aged $65{\sim}74$, female, absence of work, more chronic diseases, dependence on Instrumental Activity Daily Living (IADL), higher mental unhealthy days, lower current life satisfaction, less social activity, lower dietary habit scores, lower intake of total food, fruit, eggs, fishes & shell fishes, vitamin C (%RDA), and Nutrient Adequacy Ratio (NAR). The results of the multiple regression analysis showed that poor self-rated health index is significantly associated with more chronic diseases, mental unhealthy days, gastrointestinal disease, musculoskeletal disease, less social activity, and lower intake of fruits. The results also suggested that improving the nutritional status and functional ability, and reducing the burden of chronic diseases are beneficial to the self-rated health index of the elderly.

The Continuum of Rehabilitation Care and the Rehabilitation Services that are Needed by People with Severe Disabilities (재가 중증 장애인의 지속적 재활 관리 실태와 재활 서비스 요구)

  • Objectives: As the number of people with disabilities is increasing and their needs for care are varied, the continuum and comprehensiveness of their rehabilitative care are getting more important. This study was performed to understand the utilization of rehabilitation services and requirements of care among the people with severe disabilities in Korea. Methods: We interviewed 578 disabled persons who had severe extremity and cerebral impairment in the urban and rural areas of Korea. The questionnaire included questions on their general characteristics, the type of disability, their physical function (ADL, IADL), their use of rehabilitation services after discharge and their requirements for rehabilitation care Results: Only 12.6% of people with disability in the community continuously used the available medical rehabilitation care. The associated factors for utilization of rehabilitation services were pain and admission for rehabilitative treatment in the acute phase. There was a great need for rehabilitation services in community and this varied according to gender, the socio-economic status, the functional status and the geographic region. The gap between utilization and need for rehabilitation services was largest in the economic support. The gap of primary health care was larger in the rural area than in the urban area. Conclusions: The needs for rehabilitation service were diverse according to the individual functional status, the regional characteristics and other general characteristics of people with disability. Strategies should be considered to eliminate the barriers to obtain rehabilitation services for the people with disability in the community.

Functional Movement Screen as a Predictor of Occupational Injury Among Denver Firefighters

  • Background: The Functional Movement Screen (FMSTM) is a screening tool used to assess an individual's ability to perform fundamental movements that are necessary to do physically active tasks. The purpose of this study was to assess the ability of FMS to predict occupational injury among Denver Fire Department firefighters. Method: FMS tests were administered from 2012 to 2016. Claim status was defined as any claim occurrence vs. no claim and an overexertion vs. no claim/other claim within 1 year of the FMS. To assess associations between FMS score and claim status, FMS scores were dichotomized into ≤ 14 and > 14. Age-adjusted odds ratios were calculated using logistic regression. Sensitivities and specificities of FMS predicting claims at various FMS score cut points, ranging from 10 to 20 were tested. Results: Of 581 firefighters (mean ± SD, age 38 ± 9.8 y) who completed FMS between February 2015 and March 2018, 188 (32.4%) filed a WC claim in the study time frame. Seventy-two of those (38.3%) were categorized as overexertion claims. There was no association between FMS score and claim status [odds ratio (OR) = 1.27, 95% confidence interval (CI): 0.88 - 1.83] and overexertion claim vs. no claim/other claim (OR = 1.33, 95% CI: 0.81 - 2.21). There was no optimal cutoff for FMS in predicting a WC claim. Conclusions: Although the FMS has been predictive of injuries in other populations, among this sample of firefighters, it was not predictive of a future WC claim.

Quality of Life in Colorectal Cancer Patients with Chemotherapy-Induced Peripheral Neuropathy (항암화학요법으로 인한 말초신경병증을 경험하는 대장암 환자의 삶의 질)

  • Purpose: The purpose of this study was to identify the quality of life in colorectal cancer patients with chemotherapy-induced peripheral neuropathy. Methods: A total of 93 patients were recruited in the cross-sectional survey design. Quality of life in colorectal cancer patients were measured by European Organization for Research and Treatment of Cancer (EORTC) QLQ C30 and CIPN20. Results: In the QLQ C30, the mean score of the global health status was 59.41, the functional scale was 73.29 and symptom scale was 26.72. In CIPN20, the mean score of sensory scale was 32.70, autonomic scale was 22.88 and motor scale was 16.12. In the QLQ C30, the global health status showed significant differences according to surgery (p=.027) and the functional scale, and the symptom scale showed significant differences according to gender (p=.046, p=.020) and nonpharmacologic intervention (p=.001, p=.009). The CIPN20, the sensory scale showed significant differences according to age (p=.006), DM (p=.005), grade of CIPN (p=<.001) the status of chemotherapy (p=.001) and nonpharmacologic intervention (p=.010). Conclusion: The level of quality of life in colorectal cancer patients with peripheral neuropathy was relatively low. There is a need for developing a nursing intervention for colorectal cancer patients to improve their quality of life and to decrease chemotherapy-induced peripheral neuropathy.

Effect of Eisenia Bicyclis and Its Pill on Serum Lipid Status in Rats Fed High Fat Diet (고지방식이 급여 흰쥐에서 대황 및 대황정제환의 혈청 지질 개선 효과)

  • In order to investigate whether Eisenia bicyclis (EB) as a functional food material improves serum lipid status, supplementation of EB powder or EB extract to the high fat/cholesterol diet was tested in 6-week old male Sprague-Dawley rats for four weeks. In the first experiment, four kinds of seaweed powder -Eisenia bicyclis (EB), Undaria pinnatifida (Up), Laminaria japonica (LJ), and Ecklonia stolonifera (ES) were compared with alginic acid (ALG) as well as control. In the second experiment, EB water extract and EB ethanol extract and EB pill containing EB and LJ powder were compared with the EB powder. Amounts of dietary fiber contained in experimental diets were adjusted to provide 5% of diets. Serum total cholesterol was significantly lower only in rats fed LJ or EB powder, but HDL cholesterol was higher in rats fed UP, LJ, ES or EB powder. Ratios of HDL-/total cholesterol of all seaweed groups including ALG were significantly higher than that of the control group. Serum triglyceride was lower in LJ, EB nd ES powder groups than the control group. In comparison of EB extracts, triglyceride level was decreased in rats fed only EB water extract as compared to control rats, but HDL cholesterol was elevated with both water and ethanol extracts. Serum levels of total cholesterol, triglyceride were decreased and HDL cholesterol was increased by the EB pill. Serum levels of thiobarbituric acid-reactive substances were lower in rats fed ES powder, and EB powder, EB water extract or EB pill, as compared to the control rats. Supplementation of EB powder and EB pill reduced serum level of GOT and GPT, respectively, as compared to the control group. In conclusion, EB and EB pill improve serum lipid status and may be utilized as ingredient of functional foods for the purpose of improving serum lipid profile and inhibiting peroxidation of lipids.

Influence of oral health status on oral health-related quality of life(OHIP-14) among elderly people in Busan (부산지역 65세 이상 노인의 구강건강상태에 따른 구강건강 관련 삶의 질(OHIP-14))

  • Objectives : The purpose of this study was to investigate the relationship between the oral health status of elderly people and their oral health-related quality of life. Methods : The subjects were the elderly people over 65 years old in Busan. A total of 479 elderly people participated in the study from general hospital, two research institutes, eight dental clinics, six welfare institutions, sixteen senior centers, and three nursing homes. The oral health impact profile-14 (OHIP-14) was used in the routine dental checkups of the elderly people. Results : Those who had no oral symptoms had a better oral health-related quality of life. Among the subfactors of the OHIP-14, significant differences were shown in functional limitation(p=0.001), physical pain(p<0.001), emotional discomfort(p<0.001), physical disability(p=0.001), emotional disability (p=0.001), social disability(p=0.005), physical handicap(p=0.003) and total OHIP-14(p<0.001). Those who had 18 sound teeth(natural teeth) or more had a better oral health-related quality of life. Among the subfactors of the OHIP-14, significant differences were shown in functional limitation(p<0.001), physical pain(p=0.007), emotional discomfort(p=0.019), physical disability(p=0.018), Emotional disability(p=.032) and total OHIP-14 (p=0.006). Conclusions : The results revealed a close relationship between oral health status and oral health-related quality of life. The number of sound teeth(natural teeth) and frequency of toothbrushing had a more positive influence. Therefore oral health programs for the elderly people can preserve remaining teeth. Toothbrushing is the best way to improve the quality of life in the elderly people.

Determinants of Sense of Control in Later Life (노년기 자기 통제감에 영향을 주는 요인)

  • Given the central role of sense of control in physical and emotional well-being, the present study examined the determinants of sense of control among Korean elders. We considered sociodemographic characteristics (age, gender, marital status, education, and self-perceived economic status), health-related factors (chronic conditions, functional disability, and sensory impairment), stressful life events, and network-related resources (network of relatives, network of friends, and filial satisfaction) as potential predictors. Sense of control was measured with Pearlin and Schooler's (1978) Mastery Scale. Hierarchical regression analyses with 291 Korean elders aged 60 or older (M age = 69.9) identified lower self-perceived economic status, more chronic conditions, greater functional disability, greater vision impairment, more stressful life events, and lower filial satisfaction as significant risk factors for a diminished sense of control. Because most of the identified risks represent normative experiences during later life, the results underscored the importance of preserving or promoting sense of control for older adults. Implications of the findings and recommendations for future studies are discussed.

Factors Influencing OTT Continued Use: Focusing on the Behavioral Economics Perspective (행동경제학 관점에서 본 온라인 동영상 서비스(OTT)의 지속적 이용에 영향을 미치는 요인에 관한 연구)

  • The study explored the key factors(content satisfaction factor, functional satisfaction factor, inertia factor and situational factor) that influence OTT users' continued use from the perspective of Behavioral Economics. In addition to rational factors that make OTT users feel satisfied after experiencing the contents and functions by the platform, we assumed that it would also affect habitual factors from the 'status quo bias'. To verify this, an online survey was conducted on 523 adults in 20s and 40s who have subscribed to the Top3 OTT platform(Netflix, Wavve, and Tving). As a result of regression analysis, 'content satisfaction factor', 'functional satisfaction factor' and 'inertia factor' were significant in OTT continued use. 'Inertia factor' did not affect domestic OTT users. Unlike previous studies that supposed OTT service users as beings with complete rationality, this study is meaningful to explain OTT continued use by applying 'bounded rationality'.
