This study purposed to provide a scientific base for understanding the effect of therapeutic intervention using motivation on chronic stroke patients' balance and determining whether it is applicable as a new therapeutic intervention. For this study, we sampled 38 chronic stroke patients, and divided them randomly into a motivation training group(n=20) and a control group(n=18). To the control group was applied neurological physiotherapy 5 times a week, and 30 minutes each time, and to the motivation training group was applied neurological physiotherapy and then, additionally, a Nintendo Wii-Fit program 3 times a week, and 30 minutes each time. Before and after the experiment, the subjects' dynamic balance was measured with functional reach test(FRT), timed up & go test(TUG), and 10m gait test, and their static balance was measured with the Romberger Test. When dynamic balance ability was compared between before and after the experiment and between the motivation training group and the control group, significant difference was observed in the results of FRT, TUG, and 10m walking test between before and after the intervention(p<.05). As to static balance, in addition, body balance movement distance was not significantly different. Therapeutic intervention using dynamic motivation was found to be more effective than the control group in improving dynamic balance.
Purpose : The home-visiting exercise guidance service is a component of the community-based integrated care policy in District J of Busan Metropolitan City. This study aims to investigate the effects of the service provided by physical therapists on depression, pain, and balance among the elderly. Methods : Three elderly individuals were selected as study participants. Depression was assessed using the beck depression inventory (BDI) and the short geriatric depression scale-Korea (SGDS-K). Pain was evaluated using the visual analog scale (VAS). Balance was assessed using the berg balance scale (BBS), functional reach test (FRT), and timed up and go test (TUGT). The evaluation data were compared by averaging the values from the first and eighth sessions. Additionally, the overall satisfaction with the home-visiting exercise guidance service was surveyed. Results : In the depression assessment, the beck depression inventory score decreased (by 16.67±5.69 points), and the short geriatric depression scale score decreased (by 4.67±2.52 points). The visual analog scale score for pain decreased (by 2.67±0.58 points). In the balance assessment, the berg balance scale score increased (by 7.67±2.52 points), the functional reach test distance extended (by 4.67±0.58 cm), and the timed up and go test time decreased (by 5.33±0.58 s). Overall satisfaction with the home-visiting exercise guidance service was high (4.26±0.14 points). Conclusion : The home-visiting exercise guidance service received high satisfaction ratings from the participants. It was effective in reducing depression and pain while improving balance, potentially reducing fall risk factors among the elderly. These findings suggest that the home-visiting exercise guidance service, delivered by physical therapists, could significantly enhance the quality of life for elderly individuals by addressing issues related to depression, pain, and fall prevention within the community. Consequently, it is recommended that this service be continued and expanded as part of government policy.

Purpose: The objective of this study was a virtual reality-based balance training program effective for improvement in physical function, examined the balance ability and fall efficacy of elderly women with experienced falls, intending to examine the program's usefulness as an exercise program to prevent the recurrence of a fall. Methods: The participants were 30 elderly women aged 65 or older who met the conditions. The participants were randomly assigned to either a balance training group (BT) or a virtual reality-based balance training group (VT) and received the training three times per week, 30 minutes per day, for six weeks. To measure static balance, the Tetrax Portable Multiple System (Tetrax Ltd, Israel) were used. To measure dynamic balance, the Berg Balance Scale (BBS) and functional reach test (FRT) was used, and regarding fall efficacy, the Korean Fall Efficacy Scale (K-FES) was used. Results: Tetrax significantly improved after the intervention in both the BT group and the VT group (p<0.05). The comparison between the two groups was not significantly. BBS and FRT result significantly improved after the intervention in both the BT and VT groups (p<0.001), while K-FES was significantly ameliorated in the VT group only (p<.05). Comparing the groups, there were more significant changes in the BBS (p<0.05) and FRT (p<0.01) result of the VT group than the BT group. Conclusion: A virtual reality-based balance training program is considered to be usable as an exercise program to prevent recurrence of falls in elderly women.
Objective: In the present study, the effects of visual restriction and unstable base dual-task training (VUDT), stable base dual-task training (SDT), and on stroke patients' balance and concentration abilities were examined. Design: Two-group pretest-posttest design. Methods: Dual-task training was conducted for thirty persons with chronic stroke who were hospitalized or receiving physical therapy and were randomly assigned to either the VUDT group (n=15) or the SDT group (n=15). The subjects were divided into two groups of 15 participants each, the VUDT group and the SDT group. Dual-task training was administered for 30 minutes per session, three times a week for 8 weeks. The participants' balance was measured via the center of pressure migration distances, functional reach test (FRT), Berg Balance Scale (BBS), and attention was measured using the trail-making test and the Stroop test. Results: In comparisons within each group, the two groups showed significant differences before and after the training (p<0.05). In the comparisons between the groups, the VUDT group showed significant improvements in center of pressure (COP), FRT, and BBS, and TMT compared to the SDT group (p<0.05). Conclusions: It would be more effective to conduct dual-task training as a rehabilitation training program under vision restriction and unstable supporting surface conditions than to conduct the test under unstable supporting plane conditions to improve balance and attention in chronic stroke patients.
Purpose: The purpose of this study was to examine the difference between the faller and the non-faller in the test for balance and fear of falling. This was also done to determine whether the balance and the fear of falling are correlated in the elderly. Method: Forty eight subjects participated in this study, who attended the senior center in Daegu. Prior to the test, demographic data was collected. As for the balance test, Berg Balance Scale (BBS), Timed Up & Go (TUG), Short Physical Performance Battery (SPPB), Functional Reach Test (FRT), and One Leg Stance (OLS) were used. For measuring the fear of falling, Falls Efficacy Scale (FES) and Activities-specific Balance Confidence Scale (ABC) were used. Independent t-test and Pearson's correlation was performed using PASW 18.0 for windows. Result: All balance tests, except OLS, and fear of falling could discriminate between the faller and the non-faller. There existed a significant correlation between some balance test and fear of falling (r=0.64~0.86). Conclusion: The findings indicate that assessing the falling in the elderly, there needs to be a consideration of multiple aspects including the fear of falling and not only the balance test.
Purpose: This study examined the effects of ankle control training using neuromuscular electrical stimulation (NMES), leg muscle activation, and balance in stroke patients. Methods: Thirty-one stroke patients diagnosed with cerebral infarction and cerebral hemorrhage were selected for the study. The experimental group underwent ankle control training using NMES, while the control group applied NMES to the paretic tibialis anterior muscle for 30 minutes per session, five times a week for four weeks. The muscle activity changes were measured using surface electromyography, and balance parameters were evaluated using a functional reach test (FRT). Results: The intra-group comparison of the concentric muscle activity revealed improvements in the experimental group, including paretic tibialis anterior (TA) muscle (p<0.05), medial gastrocnemius muscle (MG) (p<0.01), and lateral gastrocnemius muscle (LG) (p<0.05), as well as MG (p<0.05), LG (p<0.05), soleus muscle (p<0.05) of the non-paretic side, and soleus muscle symmetry index (p<0.05). The intra-group comparison of the eccentric muscle activity showed improvements in the experimental group, including MG (p<0.01) and LG (p<0.01) of the paretic side, as well as MG (p<0.01), LG (p<0.01) of the non-paretic side, and LG symmetry index (p<0.01). The intra-group comparison of the functional reach test revealed significant differences in the test results in the experimental and control groups (p<0.05). Conclusion: Ankle control training using NMES had a positive effect on the changes in muscle activation and improved balance in patients with stroke.
PURPOSE: The purpose of the present study was to investigate the effects of whole body vibration exercise on balance, muscle strength and falls efficacy in the elderly. METHODS: In this blinded randomized allocation study, 27 elderly were assigned to a whole-body vibration exercise group (n=14), consisting of 25 min structured exercises for 2 days per week for 6 weeks and a control group (n=13) performing the same program without vibration. At baseline and after the 6-week intervention, balance was measured using the Korean version of the Berg balance scale (K-BBS), timed up and go (TUG) test and functional reach test (FRT). Muscle strength was determined using the 30-s chair stand test (CST). Fear of falling was assessed using the Korean version of falls efficacy scale (K-FES). Paired t-test and independent t-test were used for within and between group comparisons, respectively. RESULTS: After the intervention, the experimental group showed significantly higher changes in all the parameters (K-BBS score, TUG test, CST, K-FES score) (p<.05) compared with the control group. However, there was no significant change in all parameters in the control group (p>.05). CONCLUSION: The whole-body vibration exercise program may be helpful to improve balance, mobility, muscle performance and fear of falling in the elderly.
This study examines the effects of different environments on the application of hemiplegia patients circuit balance training. Group 1 performed circuit balance training without any auditory intervention Group 2 performed training in noiseless environments and Group 3 performed training in white noise environments. First, among lower extremity muscular strength evaluation items, maximum activity time(MAT) was not significantly different(p>.05). Maximum muscle strength(MMS) increased significantly in Group 3(p<.01), there was no significant difference in MMS among the groups. Average muscle strength(AMS) indexes also significantly increased in Group 3(p<.01), there was no significant difference in AMS among the groups. Second, among balancing ability evaluation items, Berg's balance scale(BBS) scores significantly increased in all groups(p<.05), BBS scores were significantly difference among the groups. Based on the results, Group 1, 2 and Group 1, 3 showed significant increases (p<.05). Functional reach test(FRT) values significantly increased in Group 2, 3(p<.05), and there was no significant difference in FRT values among the groups. Timed up and go(TUG) test values significantly decreased in Group 2, 3(p<.05), and there was no significant difference in TUG test values among the groups. Third, among walking speed evaluation items, the time required to walk 10m significantly decreased in all groups(p<.05), and there was no significant difference in the values among the groups. Average walking speeds showed significant increases in Group 1, 3(p<.05), and there was no significant difference in the values among the groups. Based on the results of this study, noise environments should be improved by either considering auditory interventions and noiseless environments, or by ensuring that white noise environments facilitate the enhancement of balancing ability.
