노인 걷기운동이 하지근지구력, 전신지구력과 상체유연성에 미치는 효과: 메타분석 (A Meta-analysis of the Effect of Walking Exercise on Lower Limb Muscle Endurance, Whole Body Endurance and Upper Body Flexibility in Elders)

  • Purpose: The purpose of this study was to determine whether walking exercise improved physical function in elderly people using meta-analysis. Methods: Medical and nursing literature databases were searched to identify the studies on the effectiveness of walking exercise on physical function. In the databases, there were 16 articles reporting 21 interventions. Overall effect sizes for three outcome variables, elders' physical function in lower limb muscle endurance, whole body endurance and upper body flexibility, were calculated. Effects of study characteristics on outcome variables were analyzed. Results: The meta-analysis showed that walking exercise generally had positive effects on CST (chair stand test), 6MW (6 min walking), and SRT (standing or sitting reach test) with overall weighted effect sizes of 1.06, 0.41 and 0.29 respectively. This study also showed that the chronic disease status of the elders, intervention methods, and type of residence had different effects on CST, 6MW and SRT. Conclusion: The results indicate that walking exercise improves physical function in elders. Walking exercise which can be done at any time and any location is indeed a very effective exercise for elderly people.

운동학습에 따른 대뇌 보조운동영역의 활성화 변화: fMRI 사례연구 (Change of activation of the supplementary motor area in motor learning: an fMRI case study)

  • Purpose: The contribution of the supplementary motor area (SMA) to the control of voluntary movement has been revealed. We investigated the changesin the SMA for motor learning of the reaching movement in stroke patient using functional MRI. Methods: The subject was a right-handed 55 year-old woman with left hemiparesis due to an intracerebral hemorrhage. She performed reaching movement during fMRI scanning before and after reaching training in four weeks. The motor assessment scale and surface EMG were used to evaluate the paretic upper limb function and muscle activation. Results: In the fMRI result, contralateral primary sensorimotor cortex (SM1) was activated before and after training. SMA was only activated after training. In addition, muscle activation of the paretic upper limb was similar to that of the unaffected upper limb after training. Conclusion: These findings suggest SMA is related to the execution of a novel movement pattern resulting in motor learning in stroke patients.

가속도계를 활용한 뇌졸중 환자의 환측과 건측의 사용량이 라이프 케어 증진을 위한 상지 기능 평가에 미치는 영향 (The Effect of Amount of Use on the Affected and Unaffected Side Using an Accelerometer in Stroke Patients on the Upper Limb Function Assessment for Improving Life Care)

  • 본 연구의 목적은 만성 뇌졸중 환자를 대상으로 가속도계를 활용하여 측정된 환측과 건측의 사용량과 상지 기능 간 상관관계를 알아봄으로써, 임상 작업치료를 시행하는데 있어 유용한 자료를 제공하고자 하는 것이었다. 본 연구는 2019년 5월 2일부터 2020년 7월 18일까지 원주시에 위치한 Y병원에 내원해 있는 뇌졸중 환자를 40명을 대상으로 가속도계(Accelerometer), 퍼글 마이어 평가(Fugle-Meyer Assessment: FMA), 운동 활동 척도(Motor Activity Log: MAL), 연세 양손 활동 검사(Yonsei-Bilateral Activity Test: Y-BAT)을 측정하였다. 연구 결과, 건측의 사용량과 FMA, MAL의 사용량(Amount of Use: AOU), 움직임의 질(Quality of Movement: QOM), Y-BAT를 통한 양손 수행의 질과 만족도는 유의한 음의 상관관계를 나타냈으며, 환측의 사용량은 FMA, MAL의 AOU, QOM, 양손 수행의 질과 유의한 양의 상관관계를 나타냈다. 본 연구는 뇌졸중 환자에게 가속도계를 활용하여 측정한 환측과 건측의 실제 사용량이 상지기능 평가항목으로써 활용될 수 있는 가능성을 제시했다는 측면에서 연구의 의의가 있다고 할 수 있다.

뇌졸중 환자의 인지활성화를 위한 과제 훈련이 상지기능 및 일상생활동작에 미치는 영향 (Effects of Task Training for Cognitive Activation of Stroke Patients on Upper Function and Activities of Daily Living)

  • 목적: 뇌졸중 환자의 인지활성화를 위한 과제 훈련이 상지기능 및 일상생활동작에 미치는 영향을 알아보고, 재활 치료 시 중재를 제언하고자 한다. 방법: 2018년 1월부터 2월까지 뇌졸중 환자 9명을 대상으로 4주 동안 주 5회 30분씩 중재하였다. 실험군은 5가지 과제를 치료사의 언어적 가이드를 통해 환자가 인지전략을 사용하도록 유도하고 환자 스스로 생각하고 말하는 과정을 통해 과제수행의 문제점을 해결하도록 하였다. 대조군은 상지근육과 관절의 경직을 감소시키기 위한 수동적 관절 가동 범위 운동과 도구를 이용한 능동적 관절 가동 범위 운동을 시행하였고, 근력운동으로는 일상생활활동에서 가장 많이 쓰이는 위팔 두갈래근, 위팔 세갈래근, 삼각근을 근력수준에 맞추어 실시하였다. 결과: 실험군에서 MBI는 최대 10점 향상되었고, K-AMPS의 운동기술은 최대 1.0 logit의 향상이 나타났으며 처리기술에서는 최대 0.6 logit의 향상이 나타났다. MFT에서는 최대 2점의 향상이 나타났다. 대조군에서 MBI는 최대 5점 향상되었고, K-AMPS의 운동기술은 최대 0.2 logit의 향상 나타났으며 처리기술에서는 최대 0.3 logit의 향상이 나타났다. MFT에서는 점수 변화가 없었다. 결론: 뇌졸중 환자에게 인지활성화를 위한 과제 훈련이 상지기능 및 일상생활동작에 긍정적인 영향을 준다.

  • 본 연구의 목적은 가상현실재활프로그램이 뇌졸중 환자의 상지기능, 인지와 일상생활활동에 미치는 영향을 조사하는 것이다. 프로그램 참여한 25명 중 무작위로 실험군 13명과 대조군 12명으로 분류하였다. 실험군 가상현실재활훈련 기기를 활용하여 재활훈련을 주 3회와 일반 작업치료 주 2회 적용하였고, 대조군은 일반 작업치료만 매주 5회, 회당 30분씩 진행하였다. 가상현실재활프로그램 진행한 결과 인지기능은 가상현실재활프로그램 실시한 실험군은 3.39점 증가 하였고, 일반 작업치료만 실시한 대조군은 실험 후 1.5점이 증가하였다, 상지기능은 가상현실재활프로그램 실시한 실험군은 4.84점 증가 하였고, 일반 작업치료만 실시한 대조군은 1.34점이 증가하였다, 일상생활활동은 가상현실재활프로그램 실시한 실험군은 20.38점 증가 하였고, 일반 작업치료만 실시한 대조군은 7.61점이 증가하였다, 이 결과를 통해 가상현실프로그램이 뇌졸중 환자의 상지기능, 인지와 일상생활활동도 효과가 있다는 것을 확인할 수 있다.

상지역동학 치료가 수근관 증후군 환자의 통증, 악력, 신경전도속도에 미치는 효과 (The Effects of Upper Limb Neuro Dynamic Treatment on the Pain, Grip Strength and Nerve Conduction Velocity of Patients with Carpal Tunnel Syndrome)

  • Background: When applying the upper limb neurodynamic treatment ULNT1 and upper limb neurodynamic treatment ULNT2, The object of this study evaluates the pain and grip strength, nerve conduction velocity and tries to present the treatment that is effective in the carpal tunnel syndrome through the comparison of effect between tests. Methods: The ULNT1 (n=10) and ULNT2 (n=10) was applied to the carpal tunnel syndrome in patient of 20 subject for 10 time 5 set during 4 weeks. Both groups received conventional physical therapy (hot pack with 30 minutes and ICT with 15 minutes and ultra sound with 15 minutes). Results: The ULNT1 showed the difference which the result of this study significantly in the median nerve motor latency, median nerve motor amplitude, median nerve sensory latency, median nerve sensory amplitude, grip strength and pain. ULNT2 showed the difference significantly in median nerve motor latency, median nerve motor amplitude, median nerve sensory latency, median nerve sensory amplitude and pain. Conclusions: If it implements by applying the traditional physiotherapy and ULNT1 and ULNT2 to the carpal tunnel syndrome subjects according to the subject state and function, the remedial value can be more enhanced.

  • Need to develop human body's posture supervised robots, gave the push to researchers to think over dexterous design of exoskeleton robots. It requires to develop quantitative techniques to assess human motor function and generate the command to assist in compliance with complex human motion. Upper limb rehabilitation robots, are one of those robots. These robots are used for the rehabilitation of patients having movement disorder due to spinal or brain injuries. One aspect that must be fulfilled by these robots, is to cope with uncertainties due to different patients, without significantly degrading the performance. In this paper, we propose chattering free sliding mode control technique for this purpose. This control technique is not only able to handle matched uncertainties due to different patients but also for unmatched as well. Using this technique, patients feel active assistance as they deviate from the desired trajectory. Proposed methodology is implemented on seven degrees of freedom (DOF) upper limb rehabilitation robot. In this robot, shoulder and elbow joints are powered by electric motors while rest of the joints are kept passive. Due to these active joints, robot is able to move in sagittal plane only while abduction and adduction motion in shoulder joint is kept passive. Exoskeleton performance is evaluated experimentally by a neurologically intact subjects while varying the mass properties. Results show effectiveness of proposed control methodology for the given scenario even having 20 % uncertain parameters in system modeling.

The Functional Results of Forearm and Upper Arm Replantation: Report on Two Cases

  • Upper extremity replantation is relatively less commonly performed than finger or hand replantation. We have experienced one case of forearm replantation and one case of upper arm replantation. After the replantation, limb volume at the biceps brachii muscle level below the replantation level appeared to be appropriate, however, the motor function of the muscles and the sensitivity were disappointing. For replantation of forearm and upper arm, restoration of the motor function and sensitivity of the extremity below the amputation level as well as the morphologic reconstruction have to be considered.

Effects of Mental Practice on Function and Muscle Activation of Upper Extremity in Stroke Patients

  • PURPOSE: The objective of this research is to investigate the effectiveness of mental practice on function and muscle activation of upper extremity (UE) in stroke patients. METHODS: The mental practice was conducted on 12 subjects for 10 minutes and the related existing work treatment for 20 minutes. The evaluation for the function of UE proceeded with Fugl-Meyer assessment of motor function (FMA) and muscle activity analysis instrument. Furthermore we analyzed patients' reaching activity by two stages, reaching phase and returning to original position phase. RESULTS: According to the research results, the subjects' upper limb function improved in all of the measured items and the total scores after the mental practice (p<.05). In muscle activity, CCR value was used to efficiently analyze the patients' reaching activity in the two stages of reaching stage and returning to the original position phase. While the elbow movement only had a bit of positive change (1.1%) in the reaching stage, both the shoulder (-12.3%) and elbow (-18.2%) movements had a positive change in the returning to original position phase. CONCLUSION: This research confirmed that the mental practice is effective for the enhancement of UE function for stroke patients. The result of this research can contribute to the progress of mental practice in clinical environment.

Repetitive Transcranial Magnetic Stimulation Combined with Task Oriented Training to Improve Upper Extremity Function After Stroke

  • The purpose of the present study was to investigate the effect of repetitive transcranial magnetic stimulation (rTMS) in conjunction with task oriented training, on cortical excitability and upper extremity function recovery in stroke patients. This study was conducted with 31 subjects who were diagnosed as a hemiparesis by stroke. Participants in the experimental (16 members) and control groups (15 members) received rTMS and sham rTMS, respectively, during a 10 minutes session, five days per week for four weeks, followed by task oriented training during a 30 minutes session, five days per week for four weeks. Motor cortex excitability was performed by motor evoked potential and upper limb function was evaluated by motor function test. Both groups showed a significant increment in motor function test and amplitude, latency in motor evoked potential compared to pre-intervention (p < 0.05). A significant difference in post-training gains for the motor function test, amplitude in motor evoked potential was observed between the experimental group and the control group (p < 0.05). The findings of the current study demonstrated that incorporating rTMS in task oriented training may be beneficial in improving the effects of stroke on upper extremity function recovery.
