The Effect of Swiss Ball Program on Lower Extremity Function of Elderly with Mild Cognitive Impairment

  • The purpose of this study was to determine the effects of swiss ball program training on lower extremity function of old elderly with mild cognitive impairment. The subjects were 30 mild cognitive impairment eldrly people aged between 65 and 80, who were divided into the swiss ball program training group(n=15) and the control group(n=15). The swiss ball program training group engaged in a 50-minute exercise session using. Lower extremity function scale of the two groups were measured before and after the intervention. The results are as follows. According to the swiss ball program training conducted to examine the effects of the training on lower extremity function, Moreover, the two groups showed significant differences in lower extremity function. These results indicate that virtual reality training is effective in improving lower extremity function. To conclusion, swiss ball program was found to have a positive effect on elderly lower extremity function. Swiss ball program training can be proposed as a form of fall prevention exercise for the mild cognitive impairment. Swiss ball program may be helpful to reduce the incidence of dementia and behavioral complications.

  • Purpose: The purpose of this study was to assess the effectiveness of Extracorporea Shock - Wave Therapy (ESWT) on the pain and improvement of shoulder joint function which is one of the available medical treatment in patients with frozen shoulder. Object and Methods: 26 patients with frozen shoulder, with 26 patients assigned to two groups; a control treatment group (n=12) and a ESWT application group (n=14). In the ESWT application group, the patients received general physical therapy, and then they received ESWT. In the control treatment group, the patients received only general physical therapy. These medical treatments was conducted total 6 time for 3 weeks. To investigate the effectiveness of the treatment after the each group's treatment. Pain was measured by Visual Analogue Scale (VAS) which is divided into 10 ranks and shoulder joint function was measured by CSA(Constant Shoulder Assessment scale). Results: 1. There was a statistical significance of measured by shoulder pain degree in both group but, the result of a ESWT application group shows more significance than that of a control treatment group. 2. There was a statistical significance of measured by CSA in both group but, the result of a ESWT application group shows more significance than that of a control treatment group. Conclusion: We think that treatment with General physical therapy and ESWT together for the patients with frozen shoulder is more effective and more safe method to shoulder joint function improvement and decrease of pain.

Difference of Early Muscle Strengthening Exercises on Pain, Function and Sleep Quality for Rotator Cuff Partial Tear Patients (회전근개 부분 파열환자의 조기 근력강화운동이 어깨관절의 통증, 기능 및 수면의 차이)

  • Purpose : The purpose of this study was to evaluate the difference of early muscle strengthening exercise(starting at 1 week) on pain, function and sleep quality for rotator cuff partial tear patients. Method : Experimental group 15 people, control group 15 people, was a total of 30 people. Group-specific arbitration method, was applied to early muscle strengthening exercise (experimental group) and general movement treatment(Starting at 6 week) (control group). Both groups were subjected to 40 minutes of basic physical therapy interventions four times a week over an six-week intervention period. In this study, Pain, function and sleep quality measured by VAS, PPT and ROM, CMS, SST and PSQI. Result : 1. The VAS and PPT in the experimental group and control group before and after treatment were significant difference(p<.01). Comparison between the two groups in the change in motion before and after treatment also showed a statistically significant(p<.01). 2. Comparison functional Assessment of the shoulder ROM, CMS and SST in the experimental group and control group before and after treatment were significant difference(p<.01). ROM was not significant differences between the groups but CMS and SST were significant differences between the groups. 3. PSQI in the experimental group and control group before and after treatment were significant difference(p<.01). Comparison between the two groups in the change in motion before and after treatment also showed a statistically significant(p<.01). Conclusion : Therefore, early muscle strengthening exercise is useful to improve the shoulder function and pain for rotator cuff tear patients.

The Effects of Inspiratory Muscle Training with Chest Expansion on Pulmonary Function, Maximal Inspiratory Pressure, and Gait in Individuals with Stroke (흉곽확장을 병행한 흡기근훈련이 뇌졸중 환자의 폐기능과 최대흡기압력 및 보행지구력에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate and compare the effects of inspiratory muscle training with chest expansion exercises on pulmonary function, maximal inspiratory pressure, and gait in individuals with stroke. Methods: The participants in this study included 36 stroke patients. These patients were randomly divided into three groups: an inspiratory muscle training (IMT) with chest expansion (CE) group (n=12), an IMT group (n=12), and a control group (n=12). Participants in the IMT with CE group underwent IMT and CE exercises 5 times per week for 30 minutes over 4 weeks, whereas those in the CE group and the control group received IMT and conventional physical therapy, respectively, for the same duration. The investigator measured the patients' pulmonary function, maximal inspiratory pressure, and gait endurance. Results: After the intervention, the change values for the forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF), maximal inspiratory pressure (MIP), and six-minute walk test (6MWT) in the IMT with CE group and the control group were significantly greater than those of the control group (p<0.05). Similarly, after the intervention, the change values of the FVC, FEV1, PEF, MIP, and 6MWT in the IMT with CE group were significantly greater than those in the IMT group (p<0.05). Conclusion: These findings suggest that IMT with CE could be used to increase pulmonary function, maximal inspiratory pressure, and gait endurance in stroke patients.

Effects of Inspiratory Muscle Exercise Using Biofeedback on Inspiratory Muscle Activity and Pulmonary Function in Patients with Stroke

  • Purpose: This study was conducted to determine the influence of inspiratory muscle exercise using visual biofeedback and inspiratory muscle exercise with diaphragm breathing retraining in stroke patients in regard to inspiratory muscle activity and respiratory function and to provide fundamental information on intervention for improvement of pulmonary function in stroke patients. Methods: The current study measured and analyzed inspiratory muscle activity and pulmonary function of 15 randomly selected subjects in a Biofeedback inspiratory muscle exercise (BIE) group that uses visual feedback and 15 subjects in the Diaphragm breathing exercise (DBE) group that uses breathing retraining before and after intervention. Intervention was performed for 30 minutes, 5 times a week, for 8 weeks. Subjects were measured for muscle activity of upper trapezius muscle and lattisimus dorsi muscle using a surface electromyography system and maximum inspiratory pressure was measured using a respiratory measurement device. For homogeneity test of subjects, independent t-test was performed and ANCOVA was performed for comparison of inspiratory muscle activity and pulmonary function between groups. Results: In the study results, the BIE group showed more significant muscle activity than the DBE group in upper trapezius muscle and lattisimus dorsi muscle (p<0.001). In addition, the BIE group showed more pressure than the DBE group in maximum inspiratory pressure (p<0.001). Conclusion: Based on the current study, performing biofeedback respiration exercise simultaneously with breathing retraining in stroke patients can provide more efficient respiratory physical therapy. In addition, it is considered that consistent study on the effectiveness is necessary to further improve clinical availability.

Improving Lung Function and Quality of Life in Smokers after Taking Herbal Medicine (한약 사용 후 흡연자에서 폐기능 향상과 삶의 질 개선)

  • We investigated the lung function change and quality of life in smokers after taking herbal medicine. The patients were divided into two groups(smoking and non-smoking). We measured lung function change by VRI and quality of life by BDI and Fatigue Scale Score before and after herbal medicine intake. And we examined changes of nicotine dependence, total smoking amount, tobacco flavor in smoking group. The VRI energy bar could not represent the change of the lung function in smoking group and non-smoking group. And there was no change of lung function by VRI in two group patients before and after herb medication. But after taking herb medicine, the patients who have abnormal sound in VRI were improved. The herbal medicine does not affect to the amount of smoking, tobacco flavor, nicotine dependence in Smoking patients. Smoking group were higher than non-smokers in BDI score and Fatigue Scale score. Smoking group were more improved than the non-smokers in BDI and Fatigue Scale Score after taking herbal medicine.

The Effect of Group Occupational Activity Program on Visual Perception and Motor Function of Children in Community Children Center (집단 작업 활동 프로그램이 지역아동센터 아동의 시지각 및 운동기능에 미치는 효과)

  • Objective : The current study investigated the effectiveness of group occupational activity program in increasing visual-perception and motor function of children in Community Children Center. Methods : Five children aged between 6-9 years in a community children center participated in the group occupational activity program. The program was designed to facilitate children's visual-perception and motor function based on play occupations. We examined performances of MVPT-3 and BOT-2 before and after the program. Results : Children who participated in the group program showed significant increases in visual perception and motor function. Conclusion : The study revealed the effectiveness of group occupational activity program in promoting visual perception and motor function of children in a community children center, which suggests the possibility of application of occupational activity toward low-income children in the community.

Use of Quantitative CT to Predict Postoperative Lung Function (Comparison of Quantitative CT and Perfusion Lung Scan) (폐절제술후의 폐기능 예측에 대한 나선식 정량적 CT의 유용성 (나선식 정량적 CT와 폐관류스캔과의 비교))

  • Background : the prediction on changes in the lung function after lung surgery would be an important indicator in terms of the operability and postoperative complications. In order to predict the postoperative FEV1 - the commonly used method for measuring changes in lung function- a comparison between the quantitative CT and the perfusion lung scan was made and proved its usefulness. Material and Method : The subjects included 22 patients who received perfusion lung scan and quantitative CT preoperatively and with whom the follow-up of PFT were possibles out of the pool of patients who underwent right lobectomy or right pneumonectomy between June of 1997 and December of 1999. The FEV1 and FVC were calibrated by performing the PFT on each patient and then the predicted FEV1 and FVC were calculated after performing perfusion lung scan and quantitative CT postoperatively. The FEV1 and FVC were calibrated by performing the PFT after 1 week and after 3 momths following the surgery. Results : There was a significant mutual scan and the actual postoperative FEV1 and FVC at 1 week and 3 months. The predicted FEV1 and FVC(pneumonectomy group : r=0.962 and r=0.938 lobectomy group ; r=0.921 and r=913) using quantitative CT at 1 week postoperatively showed a higher mutual relationship than that predicted by perfusion lung scan(pneumonectomy group : r=0.927 and r=0.890 lobectomy group : r=0.910 and r=0.905) The result was likewise at 3 months postoperatively(CT -pneumonectomy group : r=0.799 and r=0.882 lobectomy group : r=0.934 and r=0.932) Conclusion ; In comparison to perfusion lung scan quantitative CT is more accurate in predicting lung function postoperatively and is cost-effective as well. Therefore it can be concluded that the quantitative CT is an effective method of replacing the perfusion lung scan in predicting lung function post-operatively. However it is noted that further comparative analysis using more data and follow-up studies of the patients is required.

The Effects of One Year Exercise Program on Exercise Capacity & Cognitive Function in Male Patients with Dementia (1년간의 복합 운동프로그램이 남성 치매환자의 운동능력과 인지기능에 미치는 영향)

  • The purpose of the present study is to investigate the effects of long-term exercise program on exercise capacity and cognitive function (MMSE) in male patients with dementia. 24 male patients were divided into two experimental groups : the exercise group (n=12) and the control group (n=12). The exercise group participated in regular exercise program for 12 months, and their exercise capacity (cardiopulmonary function, muscle strength, muscular endurance, flexibility, balance, agility) and MMSE (Mini-mental state examination) levels were evaluated at baseline (pre), after 6 months (mid) and after 12 months (post). The subjects carried on group-exercise of $VO_{2}max\;30\~60\%$, $30\~60$ minute a day, $2\~3$ times per week. Statistical techniques for data analysis was paired samples t-test. The level of statistical significance was ${\le}.05.$ The results of this study were summarized as follows: In the exercise group, there were significant differences in cardiopulmonary function, muscle strength, muscular endurance and MMSE at the times of pre & mid and pre & post, there were significant differences in balance and agility at the time of pre & post only, whereas there was no significant difference in flexibility following the long-term exercise. In the control group, there were no significant differences in all the times. Based on the results of this particular study, one year exercise program increases on the cognitive function & exercise capacity in male patients with dementia.

Comparative Study on the Effects of Herbal Medicine and Western Medicine on the Liver Function Based on Cross-Sectional Design

  • Objective : The purpose of this study is to examine safety of herbal medicine on liver function and compare with western medicine. Method : 303 subjects of skin disease(vitiligo and psoriasis) were chosen at a local oriental medical clinic, and tested with a spot liver function test from Sept 1, 2006 to Dec 31, 2006. Subjects were grouped into untreated, herbal medicine, western medicine, and combination group by treatment record. Total bilirubin, AST, ALT and LDH was compared in each group, odds ratio and regression coefficient was calculated. Results : Subjects comprised of 116 individuals receiving western medication(38.28%), 54 receiving herbal medication(17.82%), 107 receiving combined forms of medication(35.31%), and 26 individuals without any types of medication(8.58%). With the mean age of 37.0 yr. 204 were male(67.3%), 99 were female(32.7%). Comparing variables of liver functions, there was no significant difference between the control and experiment groups. After adjusting potential confounders, monthly $\beta$(SE) of multiple regression -0.009~0.000 for the herbal group, -0.005~0.000 for the western group, and -0.001~0.001 for the combination group. No significant difference between the groups. OR of T-bilirubin were 1.02, 1.05, and 1.04. AST were 0.92, 0.94, and 0.98. ALT were 0.85, 0.99, and 0.97. LDH were 0.96, 1.06, 1.00 for the herbal, western, and combination group, respectively. Conclusion : Comparing with western medication, herbal medication did not cause special ill-effect on the liver function based on liver function tests.
