Preliminary Study on Development of Korean Medicine Clinical Practice Guideline for Esophageal Cancer (식도암 한의 임상진료지침 개발 예비 연구)

  • Objectives: The purpose of this study was to suggest the direction for the development of a clinical practice guideline (CPG) for esophageal cancer (EC). Methods: We collected and analyzed CPGs and related data about esophageal cancer by searching the database of domestic and foreign articles. Results: There were little contents or proposals related to Korean Medicine (KM) or Traditional Chinese Medicine (TCM), those related to KM or TCM can be found only in "Guidelines of Diagnosis and Therapy in Oncology with Traditional Chinese Medicine (惡性腫瘤中醫診療指南)". They were symptom factors, syndrome differentiation (SD) and 'treatment process of esophageal cancer by combination of western medicine and Korean medicine'. Conclusions: The topics of the development Korean medicine CPG for EC are (1) the method and procedure about combination of western medicine and Korean medicine (2) the process of SD and diagnosis (3) safety and efficacy of the herbal medicine and preparation (4) availability and timing of the tools related to KM or TCM like acupuncture, moxibustion, massage, etc.

Life-Style Habits in a High-Risk Area for Upper Gastrointestinal Cancers: a Population-Based Study from Shanxi, China

  • Background: Cancer is a burden on humanity and ranks as a leading cause of morbidity and mortality in China. Shanxi province has its unique cancer patterns and the burden is increasing. In this study, we aimed to assess the pattern of dietary habits and life-style in Shanxi, a high-risk area for upper gastrointestinal cancers in China and further evaluate the trends in cancer incidence and mortality based on registered data. Materials and Methods: Data on lifestyle, diet, physical activity were obtained from the household health survey at Zhongyang from 2013 to 2015. Cancer diagnoses were reported to Shanxi Center for Disease Control and Prevention (SCDCP). Population-based cancer incidence data and mortality data of 2012 were collected from the SCDCP. All incidence and death rates were expressed per 100,000 populations. Univariate analysis was performed using the Chi-squared test or Fisher's exact test. Results: Overall, deficiencies in fresh fruits and vegetable food, and intake of hot food, salted food, or pickled food are serious problems in Shanxi, especially in rural areas. Upper gastrointestinal cancers were the most commonly diagnosed cancers, and the incidence in rural areas is higher than those in urban areas. Cervical cancer is the most common cancer for females. Moreover, the agespecific incidence exhibited an increased trend before 40 years old. Consistent with the previous literature, our epidemiological investigation results suggest that lifestyle, nutrition deficient, and infections were major risk factors for upper gastrointestinal cancers or cervical cancer in Shanxi. Facing a serious situation, we further explored defensible recommendations for the general public in order to promote changes in environments that support healthful eating and physical activity habits, to reduce cancer risk. Conclusions: Our results present the current cancer trends in Shanxi and its related etiologic risk factors and provide a theoretical basis to guide public health efforts to prevent and control cancers in the province.

A Study on the Spatial Composition and Area Planning of the Intensive Care Units in the Public District Hospitals (지역거점공공의료원 중환자부 공간구성 및 면적계획에 관한 연구)

  • Purpose: This study aims to suggest the spatial composition and area of the intensive care units through analysis of current situation for the standards establishment of the intensive care units in public district hospitals in the future. Methods: Three Methods have been used in this paper. 1) Literature reviews about composition, location, foreign standards on intensive care units. 2) Comparative analysis on the architectural drawing for the space program and the area. 3) Field survey in order to confirm differences between architectural drawing and current situation. Results: 1) The intensive care units is required to be linked by the operating department, the emergency department and the sterile supply department upon considerations of accessibility of emergency patients and the infection control. 2) The intensive care unit consists of five areas such as the patient area, the medical staff area, the material and equipment area, the patient family area, and the public area. 3) The location of spaces in the intensive care unit is classified by three methods such as connection by view, circulation and connection to corridor. The [table 20] summarizes the type of space and the average size of the space in intensive care unit. Implications: This study is a basic research for guideline of intensive care units, and need to be followed by further study using various perspectives and methods in the future.

Gastric Phytobezoar Treated by Oral Intake and Endoscopic Injection of Coca-Cola (코카 콜라 경구 투여와 내시경적 주입법을 이용한 위석의 치료 1 예)

  • Bezoars are collections or concretions of indigestible foreign material that accumulate and coalesce in the gastrointestinal tract; they usually occur in patients who have undergone gastric surgery and have delayed gastric emptying. Treatment options include dissolution with enzymes, endoscopic fragmentation with removal or aspiration, and surgery. Recently, the efficacy of nasogastric lavage or endoscopic infusion of Coca-Cola for the dissolution of phytobezoar have been reported. We report a case of phytobezoar successfully treated by oral administration and endoscopic injection of Coca-Cola. A 62-year-old woman was referred to Yeungnam University Hospital for epigastric pain. Upper gastrointestinal endoscopy revealed one very large, dark-greenish, solid bezoar in the stomach with gastric ulcer and duodenal bulb deformity. We performed endoscopic injection of Coca-Cola into the bezoar. The patient was instructed to drink four liters of Coca-Cola per day. At endoscopy two days later, the phytobezoar was easily broken into pieces. At endoscopy on the $11^{th}$ day of admission, the phytobezoar was decreased in size and removed by endoscopic fragmentation with a polypectomy snare. At follow up endoscopy after 13 days, the bezoar was completely dissolved.

Cervical Cancer Screening: Recommendations for Muslim Societies

  • Background: The overall cervical cancer incidence rate is low in Iran; however, because of a higher risk of death for cervical cancer, a disease that kills women in middle age, a cervical cancer control program is needed. The aim of this study was to provide consensus recommendation for cervical cancer prevention in Iran and other Muslim societies with low incidences of cervical cancer. Materials and Methods: Through a practical guideline development cycle, we developed six questions that were relevant to produce the recommendation. We reviewed 190 full text records of cervical cancer in Iran (1971 to 2013) of which 13 articles were related to the data needed to answer the recommendation questions. We also reviewed World Health Organization, IARC, GLOBOCAN report, Iran Ministry of Health cancer registry report and 8 available foreign countries guidelines. Lastly, we also evaluated the Pap smear results of 825 women who participated in the Iranian HPV survey, of whom 328 were followed-up after a 5-year interval. Results: The obtained data highlighted the burden of HPV and cervical cancer situation in Iran. Experts emphasized the necessity of a cervical cancer screening program for Iranian women, and recommended an organized screening program with a cytological evaluation (Pap smear) that would start at the age of 30 years, repeated every 5 years, and end at the age of 69 years. Meanwhile, there is no need for screening among women with a hysterectomy, and screening should be postponed to post-partum among pregnant women. Conclusions: An organized cervical cancer screening is a necessity for Iran as more than 500-900 women in middle age diagnosed with an invasive cervical cancer every year cannot be ignored. This recommendation should be taken into account by the National Health System of Iran and Muslim countries with shared culture and behavior patterns. CUBA HPV test could be consideration in countries Muslim country with appropriate budget, resources and facility.

Chuna Manual Therapy for Adhesive Capsulitis : A Systematic Review and Meta Analysis (유착성 관절낭염에 대한 추나요법의 효과: 체계적 문헌고찰과 메타분석)

  • Objectives : To evaluate the effectiveness of Chuna Manual Therapy for adhesive capsulitis by a systematic review manner. Methods : Five foreign electronic databases (Pubmed, Ovid-medline, Embase, Cochrane library, Chinese Academic Journals (CAJ)) and two Korean medical electronic databases (Oriental Medicine Advanced Searching Integrated System (OASIS) and Research Information Sharing Service (RISS)) were searched to find all randomized controlled trials (RCTs) using Chuna Manual Therapy as a treatment for adhesive capsulitis. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results : Twenty-one RCTs met our inclusion criteria. The meta-analysis showed positive results on the use of Chuna Manual Therapy combined with acupuncture or exercise therapy in terms of the efficacy rate and pain using the Visual Analogue Scale (VAS), compared to acupuncture or exercise alone [SMD-1.81 (95% CI & -2.54, -1.08), P<0.001, RR 1.19 (95% CI 1.14, 1.23), P<0.001)]. Positive results in terms of efficacy rate were also obtained comparing Chuna Manual Therapy to acupuncture alone [RR 1.10 (95% CI 1.02, 1.18), P=0.01]. Conclusions : Our systematic review found favorable results on the effectiveness of Chuna Manual Therapy for pain and efficacy rate of adhesive capsulitis. However, evidence was limited due to the lack of well-designed RCTs. More qualified clinical trials are needed to obtain stronger evidence.

A report of 42 unrecorded actinobacterial species in Korea

  • During a study to discover indigenous prokaryotic species in Korea in 2016, a total of 42 actinobacterial isolates were recovered from various environmental samples collected from natural cave, squid, sewage, sea water, trees, droppings of birds, freshwater, eelgrass, mud flat, sediment and soil. On the basis of a tight phylogenetic clade with the closest species and high level of 16S rRNA gene sequence similarity, it was shown that each isolate was assigned to independent and previously described bacterial species which were assigned to the phylum Actinobacteria. The following 42 species have not been reported in Korea: eight species in two genera n the order Corynebacteriales, 26 species of 16 genera in the Micrococcales, one species of one genus in the Micromonosporales, one species of one genus in the Propionibacteriales, four species of two genera in the Streptomycetales and two species of two genera in the Streptosporangiale. Cell morphology, Gram staining reaction, colony colors and features, the media and conditions of incubation, physiological and biochemical characteristics, origins of isolation and strain IDs of 42 unrecorded actinobacterial species are presented in the species description.

Report on 30 unrecorded bacterial species of the phylum Firmicutes isolated from Korea in 2016

  • During the course of investigation of indigenous prokaryotic species in Korea, a total of 30 bacterial strains belonging to the phylum Firmicutes were isolated from diverse environmental sites such as soil, avian feces, wastewater treatment plants, fermented vegetables, seawater, algae, sea cucumber, octopus and tidal flat sediment. Phylogenetic analysis based on 16S rRNA gene sequences revealed that each strain showed high sequence similarity (${\geq}98.7%$) to the closest type strain and formed a robust phylogenetic clade with the most closely related species in the phylum Firmicutes. To date, there is no official record of these 30 species in Korea. Therefore, we report 26 species of 12 genera in the order Bacillales and 4 species of 4 genera in the order Lactobacillales which have not been reported in Korea. Morphological and biochemical characteristics, isolation sources and NIBR deposit numbers are described in the species descriptions.

  • Purpose : We evaluated children with Henoch-$Sch{\ddot{o}}nlein$ purpura(HSP) in terms of epidemiology and clinical characteristics. Methods : A total of 424 medical records of children with HSP admitted to The Catholic University of Korea, Daejeon St. Mary's Hospital, from 1987 to 2003 were retrospectively analyzed. Results : The mean annual number of cases was $25.1{\pm}7.9$ and no one year showed an outbreak. There was a steady number of patients throughout the year with a decrease during the summer season. The male-to-female ratio was 1.3 : 1 with the median age of the patients being 6-years-old. The age distribution showed a peak at age 6 in a bell-shaped distribution curve. Purpura was noted in 100 percent of the patients, gastrointestinal involvement in 53.8 percent, joint involvement in 40.8 percent, and renal involvement in 18.9 percent. Nephrotic syndrome occurred in 1 percent of all patients. Conclusion : The epidemiologic and clinical features of HSP were similar to those of other regions in Korea and foreign nations, irrespective of time.

The Evaluation of Video-Assisted Thoracic Surgery (비디오 흉부수술의 평가)

  • Over the past few years, video-assisted thoracic surgery [VATS] has been used increasingly for intrathoracic pathologic problems as a less invasive operative techniques. Today it is viewed as a sparing and safe alternative to thoracotomy for a wide spectrum of indications. Using video-assisted operative thoracoscopy, we performed consecutive 150 operations on 148 patients during the initial 2 years of our experience from July 1992 with the following indications: pneumothorax [n=53], hyperhidrosis [n=29], mediastinal mass [n=23], pleural disease [n=13], diffuse parenchymal or interstitial lung disease [n=12], benign pulmonary nodule [n=7], metastatic lung mass [n=3], primary lung cancer [n=3], bronchiectasis [n=2], malignant pericardial effusion [n=2], endobronchial tuberculosis [n=1], esophageal achalasia [n=1], and pulmonary parenchymal foreign body [n=1]. There were no death, and overall complicaton rate was 24.0%[n=36]. The most prevalent complication was persistent air leakage [longer than 5 days] in 14 cases [9.3%]. Persistent pleural effusion [longer than 5 days] occurred in 6 cases [4.0%]. Six patients were converted to an open thoracotomy because of inability to control the operative bleeding [n=3], failed adhesiolysis in bronchiectasis [n=2], and radical excision of an lung cancer [n=1]. Pneumothorax recurred in 3 cases[2.0%]. Other complications were Horner`s syndrome, diaphragm tears, temporary phrenic nerve palsy, hoarseness, subsegmental atelectasis, transient respiratory difficulty, and esophageal mucosal tear. The advantages of this minimally traumatizing operative technique lie in improved visualization, decreased pain, shortened hospital stay, and less postoperative morbidity. The indications of VATS has been extended increasingly to intrathoracic pathologies, but its role in the managements of primary lung cancer and esophageal disease remains to be defined.
