양측 액와개흉을 통한 양측 폐기낭 동시절제 (Simultaneous bilateral bleb resection through bilateral trans-axillary thoracotomy)

  • Simultaneous bilateral bleb resection was done through bilateral transaxillary thoracotomy in 10 patients with spontaneous pneumothorax during the period from May 1991 to Novemver 1992 in whom bilateral bulla or bleb was detected with using simple chest X-ray and chest CT scanning. To compare the effectiveness of bilateral transaxillary thoracotomy, we investigated 10 unilateral transaxillary thoracotomy patients with spontaneous pneumothorax and two clinical reports from other institutes which dealt the results of bilateral bleb or bulla resection through median sternotomy also. In bilateral transaxillary thoracotomy group,mean operation time was 115 minute,mean intraoperative bleeding was 329 cc, mean postoperative hospital stay was 7.5 days. Postoperative ABGA[Arterial Blood Gas Analysis] was in normal range and postoperative recovery rates of FVC[Forced Vital Capacity], FEV1[Forced Expiratory Volume at 1 second], TV[Tidal Volume] were 84.3%, 93.4%, 88.7%,respectively. In median sternotomy group,mean operation time was 129 minute,mean intraoperative bleeding was 490 cc, mean postoperative hospital stay was 12.4 days. Postoperative ABGA was in normal range and postoperative recovery rates of FVC, FEV1 were 97.3%, 97.4%, respectively. In unilateral transaxillary thoracotomy group, postoperative ABGA was in normal range also and postoperative recovery rates of FVC, FEV1, TV were 91.6%, 99.0%, 96.0%,respectively. In conclusion, simultaneous bilateral bleb resection through bilateral transaxillary thoracotomy should be considered in pneumothorax patients with bilateral bleb or bulla because of cost-effectiveness[reducing hospital days] and better cosmetic result without any impairment in recovery of respiratory function.

노인환자의 기도가역성 검사에서 FEV6의 보완적 지표로서의 역할 (The Complementary Role of FEV6 in Bronchodilator Reversibility Test for the Old Age)

  • 연구배경: 기도가역성 측정시, 1초간 노력성 호기량(forced expiratory volume in one second, $FEV_{1}$)과 노력성 폐활량(forced vital capacity, FVC)은 흔히 사용되는 지표인데 양성반응의 권장기준은 기저치보다 증가치가 절대량이 200ml이상 그리고 백분율로는 12%이상의 두 조건을 다 만족하는 것이다. 그러나 노인 환자는 천식에 합당한 병력을 가지고 있어도 기도가역성의 진단기준을 만족시키지 못하는 경우가 종종 있다. 저자들은 6초간 노력성 호기량($FEV_{6}$)이 노인의 기도가역성 검사에서 역할을 알아보고자 하였다. 방 법: 65세 이상의 노인환자 중 폐기능검사에서 $FEV_{1}$/FVC가 80%미만인 236명에게 폐활량측정법으로 기도가역성 검사를 시행하였다. 기저 $FEV_{1}$에 따라 다음과 같이 세 군으로 분류하였으며 각 군에서의 FVC, $FEV_{6}$, $FEV_{1}$의 지표에 따른 기도가역성를 비교분석하였다. 제1군 : $$FEV_{1}{\geq_-}80%$$, 제2군 : $FEV_{1}$ 60~80%, 제3군 : $$FEV_{1}{\leq_-}60%$$ 결 과: 전체 환자에서 기도가역성은 각각의 지표에 따라 $FEV_{1}$에서는 33명(14.0%), $FEV_{6}$는 55명(23.3%), FVC는 55명(23.3%)로 양성반응을 보였고, 제3군($$FEV_{1}{\leq_-}60%$$)에서는 $FEV_{1}$을 지표로 할 때는 15명(22.4%), $FEV_{6}$는 30명(44.8%), FVC는 32명(47.8%)으로 나타났다. 결 론: 본 연구에서는 $FEV_{6}$가 노인환자에서 기도가역성 검사의 새로운 보완적 지표로서의 역할을 보여줬지만 이의 일반화를 위해서는 좀 더 많은 연구가 필요할 것이다.

대학생의 폐활량에 대한 기초체력과 신체활동량의 관계 (Pulmonary function is related to basic physical fitness and physical activity in college students)

  • 본 연구의 목적은 한국 대학생들의 기초체력과 신체활동량이 폐기능과 상관관계가 있는지를 분석하고, 성별에 따른 차이를 검증하는데 있다. 건강한 참여자 312명(남성 150명[평균연령: 19.29±1.72세], 여성 160명[평균연령: 19.05±1.17세])은 신체조성, 기초체력, 신체활동량 설문지, 그리고 노력성폐활량(FVC)과 1초간 강제호기량(FEV1)의 폐기능 검사를 수행하였다. 연구결과, 남학생의 폐기능은 우악력과 좌악력, 그리고 배근력과 관련이 있었고, 여학생의 폐기능은 모든 기초체력 하위 요인과 관련이 있었다. 또한 여성의 폐기능은 중강도 신체활동량과 관련이 있는 반면 남성의 폐기능은 신체활동량의 모든 하위 요인과 관련이 있었다. 본 연구의 주요 발견은 폐기능을 개선하기 위해서 남학생은 신체활동량을 증가시키고, 여학생은 기초체력을 향상시킬 필요가 있음을 제안한다. 대학생들의 폐기능을 유지하고 개선시키기 위해서는 폐기능 관련 인자에 대한 성별의 차이를 이해하고, 성별에 맞는 교육적 노력이 필요할 것이다.

New Method for Combined Quantitative Assessment of Air-Trapping and Emphysema on Chest Computed Tomography in Chronic Obstructive Pulmonary Disease: Comparison with Parametric Response Mapping

  • Objective: Emphysema and small-airway disease are the two major components of chronic obstructive pulmonary disease (COPD). We propose a novel method of quantitative computed tomography (CT) emphysema air-trapping composite (EAtC) mapping to assess each COPD component. We analyzed the potential use of this method for assessing lung function in patients with COPD. Materials and Methods: A total of 584 patients with COPD underwent inspiration and expiration CTs. Using pairwise analysis of inspiration and expiration CTs with non-rigid registration, EAtC mapping classified lung parenchyma into three areas: Normal, functional air trapping (fAT), and emphysema (Emph). We defined fAT as the area with a density change of less than 60 Hounsfield units (HU) between inspiration and expiration CTs among areas with a density less than -856 HU on inspiration CT. The volume fraction of each area was compared with clinical parameters and pulmonary function tests (PFTs). The results were compared with those of parametric response mapping (PRM) analysis. Results: The relative volumes of the EAtC classes differed according to the Global Initiative for Chronic Obstructive Lung Disease stages (p < 0.001). Each class showed moderate correlations with forced expiratory volume in 1 second (FEV1) and FEV1/forced vital capacity (FVC) (r = -0.659-0.674, p < 0.001). Both fAT and Emph were significant predictors of FEV1 and FEV1/FVC (R2 = 0.352 and 0.488, respectively; p < 0.001). fAT was a significant predictor of mean forced expiratory flow between 25% and 75% and residual volume/total vital capacity (R2 = 0.264 and 0.233, respectively; p < 0.001), while Emph and age were significant predictors of carbon monoxide diffusing capacity (R2 = 0.303; p < 0.001). fAT showed better correlations with PFTs than with small-airway disease on PRM. Conclusion: The proposed quantitative CT EAtC mapping provides comprehensive lung functional information on each disease component of COPD, which may serve as an imaging biomarker of lung function.

축구 선수에서 집중훈련이 환기기능에 미치는 영향 (Effect of Intensive Training on Ventilatory Functions in Soccer Players)

  • To evaluate the effect of intensive training on ventilatory functions in soccer players, comparisons of various ventilatory parameters were made before and after 5-5.5 months of intensive training. The subjects were 15 members of a university soccer team with mean age and career of 19.9 and 8.3 years, respectively. Ventilatory parameters studied were those obtained by the analyses of forced expiratory volume and maximal expiratory flow-volume curves, as well as spirometric measurements of VC and MVV. After intensive training, volume parameters, such as VC, FVC and $FEV_{1&3}$ as well as flow parameters, effort-dependent and effort-independent, such as MVV, FEF200-1200, FEF25-75%, PEF, FEF25%, FEF50%, FEF75% showed significant increase. However, when the observed values of flow parameters were volume-adjusted to FVC, the differences before and after intensive training became insignificant. This suggests that enhanced ventilatory functions in soccer players after intensive training are primarily due to increase in FVC caused by increase in respiratory musle strength.

Effects of Modified Cervical Exercise on Respiratory Functions in Smartphone Users with Forward Head Posture

  • Purpose: This study was conducted to investigate the effects of modified cervical exercise on respiratory functions in smartphone users with forward head posture. Methods: Thirty-three smartphone users with forward head posture participated in this study. Subjects were divided into three groups that performed modified cervical exercise one time a day (A group), two times a day (B group), and three times a day (C group). All subjects performed the exercise for four weeks, during which time respiratory functions were measured. Results: There were significant differences in respiratory functions such as forced vital capacity, forced expiratory volume at one second, and maximal voluntary ventilation in the C group after four weeks (p<0.05). Moreover, the forced vital capacity differed among groups, and the post hoc test revealed a significant difference between A group and C group (p<0.05). Conclusion: The results of this study confirmed that modified cervical exercise improved respiratory functions in smartphone users with forward head posture. These findings indicate that smartphone users with forward head posture should perform modified cervical exercise to build correct posture and respiratory functions.

동계훈련이 폐기능에 미치는 영향(야구선수를 중심으로) (Effect of Pulmonary Function by Winter Intensive Training (baseball players))

  • To evaluate the effect of winter intensive training on pulmonary function in high school baseball players, comparisons of various ventilatory parameters were made before and 3-3.5 months of winter intensive tranining. The subjects were 18 members of a high school baseball players with mean age and career of 6.3 and 7.3 years, respectively. The following were mainly observed by spirometry for the study ; respiratory rate, vital capacity(VC), maximum voluntary ventilation(MVV), forced vital capacity (FEV1%) and forced mid-expiratory flow (FEF25-75%). The result obtained are summarized as follow. 1) Resipratory rate, tidal volume, forced vital capacity, 1FEV%, FEF50% and FEF75% showed no significant difference between before and after. 2) MVV in after was significantly(p<0.001) increased to 166 L/min comparing with 136L/min in before. 3) 1FEV in after was significantly(p<0.05) increased to 4.46L comparing with 3.76L in before. 4) PEF in after was significantly(p<0.05) increased to 10.40 L/sec comparing with 9.18 L/sec in before.

Comparison of Vital Capacity and Balance between Elderly Women and Young Women with Forward Head Postures According to the Use of Figure-8 brace

  • Purpose: To improve pulmonary function and decrease in balance ability with increasing forward head position and vertebral curvature, we applied Figure-8 brace to confirm the immediate effect on vital capacity and balance and to see if it is applicable. Methods: A total of 34 elderly women aged 65 or older and young women in their 20s with FHP were screened to measure vital capacity, measuring the forced expiratory volume in 1 second (FEV1) and forced vital capacity (FVC), and measuring the foot pressure to see the change in balance. For statistical analysis, the difference between pre and post values was compared using pared t-test. Results: As a result of vital capacity measurements, there was no significant difference between FEV1 and FVC for women over 65 years old (p>0.05). Young women in their 20s had no significant difference in FEV1 (p>0.05), and FVC had significant differences (p<0.05). In measuring foot pressure to measure balance, both women aged 65 and above and young women in their 20s had a significant decrease in anterior foot pressure, and a significant increase in posterior foot pressure (p<0.05). Conclusion: The results of this study did not positively affect the vital capacity of elderly women with FHP. However, the significant increase in vital capacity of young women in their 20s suggests that contraction of the abdominal muscle is necessary during forced expiration. Therefore, it is believed that proper application and therapeutic interventions should be combined when applying Figure-8 brace.

Effects of different Diaphragm Breathing Methods on the Diaphragm Thickening Ratio and Pulmonary Function in Young Adults

  • PURPOSE: This study examined the effective impact of self and resistive and ultrasound-biofeedback diaphragm breathing on the pulmonary function and diaphragm thickening ratio of young adults. METHODS: Thirty normal adults were assigned randomly to three experimental groups (self- diaphragm breathing (n=9), resistive-diaphragm breathing (n=11), ultrasound-biofeedback diaphragm breathing (n=10)). Each group participated for 15 minutes for times with a two minute rest between two sets. The subjects were assessed using the pre- and post- diaphragm thickening ratio and the pulmonary function (forced vital capacity, forced expiratory volume at one second, maximal voluntary ventilation, and respiratory rate) on the thirty subjects. A paired t-test was to determine the difference between before and after the experiment in each group of diaphragm breathing before and after the exercises. One-way ANOVA was used to determine the differences between the groups. RESULTS: The forced vital capacity and maximal voluntary ventilation measurements revealed a significant difference in the resistive-diaphragm breathing group than the other two groups. On the other hand, there was no significant difference between the self-diaphragm breathing and ultrasound-biofeedback breathing groups. CONCLUSION: The resistive-diaphragm breathing group showed greater improvement in the pulmonary function than the other two groups. Therefore, resistive-diaphragm breathing will improve the pulmonary function on normal young adults.

진폐증의 조기진단에 관한 연구 (A Study on the Early Diagnosis of Pneumoconiosis)

  • In order to assess the method which is more sensitive one to detect the early change of lung tissue by the inhaled dust, we have performed the various medical examinations such as chest radiography, pulmonary function test, high resolution chest CT, brnchoalveolar lavage and lung biopsy used bronchoscope and ultrathin bronchoscopy examination to 48 persons. The control group were 8 persons who did not exposed to dust, 40 cases of the experimental group have professionally exposed to the mineral dust. The results were as follows 1. The total number of cells in bronchoalveolar lavage was significantly increased in all of the pneumoconiosis group classified by chest and high resolution chest CT. 2. The composition rate of macrophage to the total number of cells in bronchoalveolar lavage fluid was significantly decreased in all of the pneumoconiosis group compared with the control group. 3. The composition rate of neutophils and lymphocytes to the total number of cells in bronchoalveolar lavage fluid was significantly increased in all of the pneumoconiosis group compared with the control group. 4. The forced expiratory volume in one second ($FEV_{1-0}$), maximal mid-expiratory flow (MMF), and maximal voluntary ventilation (MVV) were significantly increased only in the group of the progressed pneumoconiosis relatively. 5. We observed submucosal edema, anthracotic pigmentation and granuloma formation in transbronchial lung biopsy of the suspected pneumoconiosis (category 0/1) case which is thought to the early change of coal workers' pneumoconiosis.
