  • A typical clutch system for automotive manual transmissions transfers hydraulic pressure generated by driver's pedal manipulation to the clutch diaphragm spring. The foot effort history during the period of push is different than the period of the clutch pedal's return. The effort or load difference is called clutch foot effort hysteresis. It is known that the hysteresis is caused by friction. The frictional force and moment are produced between various component contact points such as between the rubber seal and the inner wall inside the hydraulic cylinder and between the diaphragm spring and the pressure plate, etc. Understanding the clutch pedal foot effort hysteresis is essential for a clutch release system design and analysis. The dynamic model for a clutch release system is developed for the foot effort hysteresis prediction and a simulation analysis is performed to propose a tool for analysing a clutch system.

  • A clutch hydraulic system for automotive manual transmissions transfers hydraulic pressure generated by driver's pedal manipulation to the clutch mechanism. The foot effort when the clutch pedal is pushed is different than that when the clutch is returned. The effort or load difference, called hysteresis, is caused by the friction produced between rubber seal and inner wall inside the hydraulic cylinder. This clutch pedal travel foot effort hysteresis is essential for a clutch hydraulic system design and analysis. The dynamic model for a clutch hydraulic system is developed and a simulation analysis is performed to estimate the fiction coefficient as a function of the cylinder pressure. The simulation result is then compared to the measurements obtained from a clutch hydraulic system tester to ensure the reliability of the dynamic model and the coefficients estimated. Also the estimated friction coefficients at various pressure values are compared to those reported by an independent study.

  • For commercial vehicles with a manual transmission, the pneumatic and fluid pressure servo-device is widely used for transmitting large torque. However, the pneumatic and fluid pressure servo-device usually results in non-linear variation of foot-efforts, also such characteristics tend to increase physical fatigue of drivers who drive commercial vehicles for a long time. Thus, vehicle manufacturers consider the hysteresis loops in the Clutch Actuation System (CAS) design when a new vehicle model is being developed. In this study, we numerically simulate the hysteresis loops for predictable CAS design, and the experimental results confirm the simulation results.

  • Background For patients with neuropathy, vasculopathy, and impairment of wound healing, treatment of a diabetic foot ulcer poses many challenges. A large number of dermal analogues have been invented in an effort to overcome these challenges. Matriderm, a dermal analogue, is made from bovine collagen and elastin. This study was conducted in order to evaluate the effectiveness of Matriderm for treatment of diabetic foot ulcers, in comparison with skin grafting. Methods Sixty patients with diabetic foot ulcer were included in this prospective study. The average age of the patients, who had type II diabetes mellitus, was 58 years old. The patients were allocated to an experimental or control group with their consents. The patients were selected with their consent for inclusion in an experimental group and a control group. Patients in the experimental group received a Matriderm appliance and a split-thickness skin graft, while those in the control group received only a split-thickness skin graft. Results A shorter hospitalization period (7.52 weeks) was observed in the experimental group than in the control group (9.22 weeks), and a shorter period of time (8.61 weeks) was required for complete healing, compared with the control group (12.94 weeks), with statistical significance (P<0.05). A higher elasticity ratio of the affected side to the non-affected side was observed in the experimental group, compared with the control group (P<0.01). Conclusions Matriderm enables effective healing and improves elasticity in treatment of patients with diabetic foot ulcer.

  • Purpose: The purpose of this study is to evaluate the incidence and cause of reamputation with respect to the location of foot amputation. Materials and Methods: Eighty-six patients who received amputations below the ankle level from March 2002 to September 2012 with at least 1 year follow-up were enrolled in this study. We stratified the site of the initial amputation from first to fifth ray and into either the phalanx or metatarsal bone, and investigated the cause of reamputation. Results: The reamputation rate below the ankle level was 53.5%. It was highest (62.1%) in patients with first ray amputations without statistical significance. Rays were divided into two columns, first to third rays as the medial column and others as the lateral column, and reamputation was performed in 61.2% of patients with medial column amputation. Comparing the results between phalanx and metatarsal amputations, reamputation was performed in 62.1% of patients with metatarsal bone amputation. The rate of reamputation was statistically significant in both the medial column and metatarsal amputations. The most common reamputation site, in accordance to the initial site of amputation, was the adjacent ray (57.4%), which was without statistical significance. Moreover, the most common cause of reamputation was osteomyelitis and focal infection in all rays. Conclusion: This study showed that reamputation after amputation below the ankle level was relatively common with highest rate in medial column and metatarsal amputations. Hence, surgeons should be aware of the risk of reamputation and put more preventive effort during medial column and metatarsal amputations.

  • Foraminal or extraforaminal Far Lateral Disc Herniations (FLDH) extending into or beyond the foraminal zone have been recognized as between 7-12% of all lumbosacral disc herniations. Conventional posterior laminectomy may not provide good access to a herniation that lies far lateral to the lateral margin of the pedicle. Use of the endoscopic technique through a percutaneous approach to treat such FLDH patients can decrease the surgical morbidity while achieving better outcomes. We made an effort to utilize the advantages of percutaneous endoscopic lumbar discectomy (PELD) and to determine the appropriate approach for FLDH at the level between the 5th Lumbar and first Sacral vertebrae(L5-S1). The authors present a case of an endoscopically resected lumbar extruded disc of the left extraforaminal zone with superior foraminal migration at the level of L5-S1, which had led to foot drop, while placing the endoscope in the anterior epidural space without facetectomy.

  • Soft tissue reconstruction of dorsum of the foot and ankle has long presented challenging problems for the reconstructive surgeon. Limitations of available local tissue, the need for specialized tissue, and donor site morbidity restrict the options. In an effort to solve these difficult problems, we have begun to use adipofascial flap based on the perforating branch of the peroneal artery. We present our early experience of 5 patients treated with this flap. Our patients ranged from 6 to 26 years in age and included 3 males and 2 females. The etiologies of the wounds were secondary to traffic accident, and crushing injury. The flaps had reverse flow in all patients. The flap and the adjoining raw area were covered with a full-thickness skin graft, while the donor site at the lateral aspect of the leg was closed primarily without grafting. The skin graft was taken from the inguinal area, which was closed primarily. Compared with other flap, this adipofascial flap is thinner, producing less bulkiness to the recipient site and minor aesthetic sequelae to the donor site. In our opinion, this flap is versatile, effective, and an addition to the armamentarium of the reconstructive surgeon for coverage of difficult wounds of the foot and ankle.
