일 대학병원 간호사의 발 건강 상태 (Foot Health Condition and Related Characteristics of Nurses)

  • Purpose: This descriptive study aimed to identify the foot health condition and related characteristics of nurses. Methods: A literature-based questionnaire was developed and distributed to 472 nurses in a university hospital in B city, and data were collected from August 1 to August 15, 2020. The Foot Health Status Questionnaire (FHSQ) was used to measure foot health condition. Results: The most common foot diseases among the participants were plantar fasciitis (7.8%) and hallux valgus (7.0%). To relieve foot pain, most nurses used relaxation techniques, stretching, elevating the legs, and foot massage. The level of foot health condition among nurses was lower than that of older persons or the general public with chronic foot disease. The foot health condition of the nurses showed that the lower the age and the greater the number of foot diseases, the more severe the pain and dysfunction. Conclusion: The study shows that there is a need for interventions that will raise nurses' awareness about foot health and induce better foot health management.

Effects of Professional Body Massage on Forward Head Posture, Neck Pain, and Plantar Foot Pressure Balance in Men in their 20s

  • Objective: The purpose of this study was to investigate the effect of a 12-week professional body massage program (PMP) on forward head posture, neck pain, and plantar foot pressure balance in men in their 20s. Method: A total of 20 men with musculoskeletal diseases were recruited for this study. The participants were asked to take part in a PMP twice a week for 12 weeks. The cranial vertical angle (CVA) for forward head posture and visual analogue scale (VSC) for neck pain and right/left foot plantar pressure balances were extracted to compare between pre- and post-program differences. Results: CVAs before ($56.86{\pm}4.55^{\circ}$) and after ($62.72{\pm}4.57^{\circ}$) and VSCs before ($6.95{\pm}1.70$) and after ($1.70{\pm}1.56$) PMP revealed statistically significant differences. The right foot, after PMP, showed a significant increase in the plantar pressure balance from $46.17{\pm}2.95$ to $49.44{\pm}1.29%$, while the left foot decreased significantly from $53.83{\pm}2.95%$ to $50.56{\pm}1.29%$. Therefore, based on these results, it may be said that the foot pressure balance abilities were improved after PMP because the ideal foot pressure ratio is 50%. Conclusion: Consequently, it was suggested that a 12-week PMP could be utilized for improvement of forward head posture, neck pain, and foot plantar pressure balance in men in the 20s with musculoskeletal diseases.

  • Collagen is the most common structural protein in the connective tissues of vertebrates, playing a crucial role in maintaining the tissue structure and wound healing. Atelocollagen retains the original properties of collagen but does not induce inflammatory cytokines. Atelocollagen is extracted from various animals and humans, with bovine-derived collagen being the most commonly used. The safety and efficacy of atelocollagen have been demonstrated through multiple studies, but further research will be needed in the field of orthopedics regarding its range of use and effects. Foot and ankle diseases have attracted little research attention, highlighting the need for further studies.

정상의 발과 병적인 발에서 발보조기 연구의 비판적 고찰 (A Critical Review of Foot Orthoses in Normal and Diseased Foot)

  • The purpose of this study was to critically review biomechanical studies on foot orthoses (FO) in normal and diseased foot and provide beneficial information obtained from researches until now and future researching focus. The search was performed by Medline and Embase database including studies published in English from January 1980 to April 2007. The searching terms were foot orthoses, foot orthotics, insoles and shoe insert. 57 studies including 54 journal articles and 3 abstracts were finally selected under the conditions of having clinical trials, FO, control condition, movement, scientific measuring system. The reviewed studies were divided into 10 categories according to subject characteristics; healthy normal, excessive pronation or flexible flat foot, rheumatoid arthritis, diabetes, medial knee osteoarthritis, forefoot varus, plantar fasciitis, patellofemoral syndrome, cavus foot and finite element model. In summary, first, soft and semirigid FOs with some degree of cushioning showed much higher comfort and efficacy than rigid FO. Second, no big differences between prefabricated and custom FO were shown. Third, the full length's FO was preferable to the half length's FO or simple arch supports. Fourth, the wearing of FO combining medial arch supports and metatarsal dome made positive roles to enhance comfort and functionality and redistribute plantar pressure under the foot. Fifth, for patients with knee-related diseases lateral wedges were preferable. Sixth, measuring systems were properly applied according to the types of foot diseases.

  • 근골격계 분야에서의 초음파는 검사 비용이 비교적 낮고 동적인 검사가 가능하다는 장점에 힘입어 빠르게 발전하고 있다. 족부 및 족관절 분야는 초음파 검사가 용이하며 국소 증상이 있는 경우 그 감별 진단에 초음파가 중요한 역할을 할 수 있다. 본 종설에서는 흔하게 접할 수 있는 족부 및 족관절 질환을 중심으로 그 원인 및 초음파적 진단에 대해 설명하고자 하였다.

만성 족부 질환이 환측 하지의 골밀도에 미치는 영향 (Effect of Chronic Foot Disease to Bone Mineral Density of the Affected Lower Limb)

  • Purpose: Pain or discomfort caused by foot diseases may lead to abnormal gait, resulting in decreased bone mineral density (BMD) of the affected lower limb. We analyzed the effect of foot affection to BMD and its clinical significance. Materials and Methods: Bilateral hip BMD was evaluated in 93 patients with unilateral chronic foot disease. To minimize statistical errors, we excluded patients with medical histories that had influence on BMD. Analysis was based on the results of BMD tests at the first visit. All patients denied past medical intervention for osteoporosis. The difference in density between bilateral limbs was determined by comparing BMDs of the neck, upper neck, trochanter and total area of hip. Results: Test results revealed the decrease of BMD in the lower limb with the affected foot, compared to the unaffected side. This decrease was significant in the area of the trochanter (p <0.05). There was no marked difference of BMD in relation with duration of affection, underlying disease or age. Pertaining the location of foot affection, the hindfoot group showed significant decrease in BMD compared to the forefoot group. The group with affection in bone and joint also showed a marked decrease in BMD compared to the soft tissue group (p <0.05). Conclusion: Pain and discomfort caused by chronic foot diseases can lead to a decrease in the BMD of the affected lower limb. This may increase the risk of complications such as osteoporotic fracture and muscular atrophy.

Biological Control of Soilborne Diseases on Tomato, Potato and Black Pepper by Selected PGPR in the Greenhouse and Field in Vietnam

  • Bacterial wilt, Fusarium wilt and Foot rot caused by Ralstonia solanacearum, Fusarium oxysporum, and Phytophthora capsici respectively, continue to be severe problems to tomato, potato and black pepper growers in Vietnam. Three bio-products, Bacillus vallismortis EXTN-1 (EXTN-1), Bacillus sp. and Paenibacillus sp. (ESSC) and Bacillus substilis (MFMF) were examined in greenhouse bioassay for the ability to reduce bacterial wilt, fusarium wilt and foot rot disease severity. While these bio-products significantly reduced disease severities, EXTN-1 was the most effective, providing a mean level of disease reduction 80.0 to 90.0% against bacterial wilt, fusarium wilt and foot rot diseases under greenhouse conditions. ESSC and MFMF also significantly reduced fusarium wilt, bacterial wilt and foot rot severity under greenhouse conditions. Bio-product, EXTN-1 with the greatest efficacy under greenhouse condition was tested for the ability to reduce bacterial wilt, fusarium wilt and foot rot under field condition at Song Phuong and Thuong Tin locations in Ha Tay province, Vietnam. Under field condition, EXTN-1 provided a mean level of disease reduction more than 45.0% against all three diseases compared to water treated control. Besides, EXTN-1 treatment increased the yield in tomato fruits 17.3% than water treated control plants.

Age-dependent immune response in pigs against foot-and-mouth disease virus in vitro

  • Foot-and-mouth disease, one of the most contagious diseases in cloven-hoofed animals, causes significant economic losses. The pathogenesis of foot-and-mouth disease virus (FMDV) infection is known to differ with age of the animals. In this study, we aimed to reveal the difference in immunological response in the initial stage of FMDV infection between piglets and adult pigs. Peripheral blood mononuclear cells (PBMCs) were isolated from 3 piglets (8 weeks old) and 3 pigs (35 weeks old) that were not vaccinated against FMDV. O-type FMDV (2 × 102 median tissue culture infectious dose) was inoculated into porcine PBMCs and the cells were incubated at 37.0℃ under 5% CO2 for various time periods (0, 1, 3, 6, 12, 24, and 48 h). The total RNA was obtained from the FMDV-inoculated PBMCs after each time point, and the virus titer was investigated in these RNA samples. Furthermore, dynamics of mRNA expression of the six tested cytokines (interferon [IFN]-α, IFN-γ, interleukin [IL]-6, IL-8, IL-10, and tumor necrosis factor [TNF]-α) in FMDV-inoculated porcine PBMCs were evaluated by time-series analysis to determine the differences, if any, based on the age of the pigs. The PBMCs of piglets contained the highest quantity of FMDV mRNA at 6 hours post-inoculation (hpi), and the PBMCs of pigs had the highest quantity of FMDV mRNA at 3 hpi. The mean cycle threshold-value in the PBMCs steadily decreased after the peak time point in the piglets and pigs (6 and 3 hpi, respectively). The dynamics of mRNA expression of all cytokines except TNF-α showed age-dependent differences in FMDV-inoculated PBMCs. The mRNA expression of most cytokines was more pronounced in the piglets than in the pigs, implying that the immune response against FMDV showed an age-dependent difference in pigs. In conclusion, within 48 hpi, the 8-week-old piglets responded more rapidly and were more sensitive to FMDV infection than the 35-week-old pigs, which could be associated with the difference in the pathogenesis of FMDV infection among the pigs. These results provide valuable insights into the mechanisms underlying the age-dependent differences in immune response in pigs against FMDV infection.

당뇨병성 족부궤양의 재발과 연관된 위험인자: 심리사회적 위험인자를 포함한 후향적 연구 (The Risk Factors Associated with Foot Re-Ulceration in Diabetes: A Retrospective Study Including Psychosocial Risk Factors)

  • Purpose: To evaluate several risk factors related to re-ulceration of diabetic foot including psychosocial aspects such as familial support and degree of independence of patients' activity. Materials and Methods: We reviewed medical records and performed telephone interview with eighty-five patients who had a history of hospitalization in our hospital due to diabetic foot ulceration from year 2002 to 2010. Based on the collected data, we analyzed several factors such as age, gender, prevalence duration, accompanying diseases, HbA1c level, degree of independence and familial support. Results: The mean age was 61.4 years and most common in the 4th decade. There were 57 cases (67%) of recurrence, predominance of male. Eleven patients with recurrent diabetic foot ulceration had undergone major amputations. Psychosocial problems such as depression, insufficient familial support and mortality were more frequently observed in recurrent group. Conclusion: This study shows that psychosocial factor such as familial support for patient with diabetic foot could be important to reduce the recurrence rate of diabetic foot ulceration. Therefore, we should pay attention to strategic plans for prevention, screening, treatment, and aftercare through the prospective studies including psychosocial risk factor in diabetic foot ulceration.
