족부족관절 영역에서 음압창상처치의 적용 (Application of Negative Pressure Wound Therapy in the Foot and Ankle Field)

  • Negative pressure wound therapy (NPWT) has emerged as a valuable tool for managing complex wounds within the foot and ankle field. This review article discusses the expanding applications of NPWT in this specialized field. Specifically, it discusses the efficacy of NPWT for various wound types, including diabetic foot wounds, traumatic wounds, surgical wounds, and wounds involving exposed bone or soft tissue defects. NPWT demonstrates versatile utility for foot and ankle wound management by promoting healing, potentially reducing the need for secondary surgery, improving diabetic and neuropathic ulcer healing times and outcomes, and optimizing the healing of high-risk incisions. In addition, this review explores the underlying mechanisms through which NPWT might enhance wound healing. By synthesizing current evidence, this review provides a comprehensive overview of the role of NPWT in foot and ankle surgery and offers valuable insights to clinicians navigating the complexities of wound care in this challenging anatomical area.

할룩스 포인트 인솔이 발의 접촉면적 및 압력에 미치는 영향 (Effect of Hallux Point Insole on Foot Contact Area and Pressure)

  • Purpose: The purpose of this study was to determine the changes in foot contact area and pressure when walking with a functional insole that emphasizes the Hallux point as compared to a general insole. Methods: In this study, an experiment was conducted to investigate changes in plantar pressure and contact area for a functional insole that emphasized the Hallux point as compared to a general insole. A lower extremity robot was used for walking reproduction. First, the gait sequence according to the two insoles was determined through a randomized controlled trial comparison. According to the sequence procedure, the insole was attached to the shoe and then worn on the right side of the lower extremity robot for gait reproduction at a normal gait speed of 20 steps per minute. After programming the robot to walk, the experiment was carried out. The result value was determined by averaging the pressure and area data of the fore and rear foot measures after walking at 20 steps per minute. Results: The functional insole that emphasized the hallux point significantly increased the forefoot and rearfoot contact area (p < 0.05) and significantly decreased the forefoot and rearfoot contact pressure (p < 0.05) compared to the general insole. Conclusion: A functional insole that emphasizes the hallux point does not collapse the medial longitudinal arch during gait, increasing foot stability and reducing fatigue. Thus, this functional insole needs to be widely used clinically.

숙녀화 착용시 발의 형태요인과 장해부위와의 상관 (Correlation Between Factors Related to the Foot Shape and the Foot Abrasion in Wearing Ladies' Shoes)

  • The purpose of this study was to examine the correlation between the shape of the foot and foot abrasion. and to provide preliminary data for shoe manufacturing by comparing the length of a woman's feet and the shoe size. The Martin calibrator and measuring tapes were utilized to measure the shapes of 163 female college students' feet. Indirect measurements were also obtained by line drawing of the feet for additional analysis. Brief questionnaires about what type of shoes are worn were given to the subjects of the study. Data analysis was presented by frequency, percentage, and standard deviations. Factor analysis and correlation co-efficiency of data was conducted on the significance level of p〈.05. The results of the study are as follows (1) The average number of days per week in which the subjects wear ladies' shoes per week were 4.5. Little over half of the subjects (53.87% ) answered that they wear heeled shoes for more than 8 hours per day. The most preferred height of a ladies'shoe heel was between 2 cm and 4 cm, as replied by 41.0% of the subjects. (2) The foot area where abrasion occurs most often was around the heel (51.2%) and the middle part of the sole (50.9%) (3) Factors correlated to the heel abrasion included the circumference and the breadth of the foot. The abrasion on the side of the first toe of mid-sized feet was most affected by the angle of the first the and inner foot line. The height of the foot heel was a significant factor for the abrasion near the anklebone. In conclusion, the abrasion resulted from the inappropriate fitting of the shoe and the foot. Also, the circumference and the width factors were more influential rather than the foot length. Therefore, more database on this should be systemized and available to the footwear manufacturers and the customers for more practical use of shoe size and public education.

발의 형태, 구두 디자인, 구두 착용태도에 따른 발의 불편감 (Foot discomfort by foot shape, shoe design and wearing attitude)

  • As women wear shoes for a long time due to aesthetic elements and working environment, many women experience discomfort and deformation in their feet due to their shoes and the production of comfortable shoes is becoming an important issue. The purpose of this study is to investigate the factors affecting the grip of shoes by foot type, shoe design, and wearing attitude of shoes. Through this, we suggest solutions for foot discomfort due to wearing shoes and help to prevent foot related diseases. The study results are as follows; first, a wide foot, square-type, and high or low arch (hollow foot or flat foot) among foot shapes influenced the fatigue and pain of feet, and big feet over 250mm-long, wide feet, square-type feet, and high-arch feet (hollow foot) had an influence on distortion and side effects. Second, among the characteristics of wearing shoes influencing foot discomfort, the higher the shoe heels were, the narrower the surface covering the feet was (pumps and mule), and the shoes with high front heels and narrow back heel area, the shoe wearer had a higher possibility of experiencing fatigue and pain and even had not only fatigue and pain but also side effects when she wore pointed shoes. Third, a shoe wearer experiences fatigue and pain if she wears shoes for a shorter period of time, stay stood while wearing shoes for a short time, and wears shoes that are bigger or smaller than the actual shoe size. Fourth, fatigue and pain experiencers and distortion and side effects experiencers all responded that they change into other shoes to deal with foot discomfort and that they directly massage their feet.

발바닥 특정 부위 자극이 뇌파에 미치는 효과에 대한 비선형 분석 (Nonlinear analysis of the effects on the brain waves of the stimulation on specific area of the sole of the foot)

  • The brain is one of the most complex systems in nature. Brain waves, or the "EEG", are electrical signals that can be recorded from the brain, either directly or through the scalp. The kind of brain wave recorded depends on the behavior of the animal, and is the visible evidence of the kind of neuronal (brain cell) processing necessary for that behavior. But, EEG had been considered as a virtually infinite-dimensional random signal. However, nonlinear dynamics light on dynamical aspects of the human EEG. The methods of nonlinear dynamics provide excellent tolls for the study of multi-variable, complex system such as EEG. In this study, 20 persons seperated in 2 groups were examined with EEG, one group stimulated on specific area of the sole of the foot with footbed inside the shoes. This experiment resulted in at the group stimulated on specific area of the sole of the foot correlation dimension of P4 and O1 channels increased significantly. Therefore. we obserbed that stimulation on specific area of the body had a constant effections on the specific channels.

성인 당뇨병 환자의 족부와 신발에 대한 조사 (Foot and Shoe Survey in Adult Patients with Diabetes Mellitus)

  • Purpose: To investigate the status of foot, preventive foot care and appropriateness of shoe size in patients with diabetes. Materials and Methods: One hundred and sixty-five patients who visited endocrinology clinic due to clinically proven diabetes mellitus were studied. Average age was 59.3 years (range, 36-90 years) and average duration of diabetes was $9.6{\pm}8.2$ years. Patients were questioned about their basic educational level and their foot and ankle problems. Physical examination of the foot was done including sensation testing of four areas of plantar surface with the 5.07 monofilament. Both feet and shoes were measured for width, length and the height of toes and toebox. Shoes were defined as properly fitting the foot if it is 5mm wider than the foot, and $20{\pm}30\;mm$ longer than the foot, and the height of toebox is more than 5mm higher than the height of the toes. Results: 48.5 percent of the patients were educated about how to select an appropriate shoewear. 47.3 percent of the patients had symptoms of neuropathy and 42.4 percent of patients were unable to feel the monofilament in at least one area. 25.5 percent of the patients had appropriate shoes in length, 63.6 percent of the patients in width, and 72.7 percent of the patients in the height of toebox. Conclusion: This study suggets that more patients needs to be educated about foot care and appropriate shoes which have adequate height of toebox as well as the length and width.

인솔 장착형 단하지 보조기의 생체 역학적 분석을 통한 보행 영향성 평가 (Evaluation of Insole-equipped Ankle Foot Or thosis for Effect on Gait based on Biomechanical Analysis)

  • The purpose of this study was to evaluate the effects of insole-equipped ankle-foot-orthoses (AFO) on gait. 10 healthy males who had no history of injury in the lower extremity participated in this study as the subjects. The foot of each subject was first scanned, and the insole fit to the plantar was made using BDI-PCO(Pedcad Gmbh, Germany). The subject then was made to walk on a treadmill under four experimental conditions: 1) normal walking, 2) walking wearing AFO, 3) walking wearing AFO equipped with the insole, 4) walking wearing pneumatic-ankle-foot-orthosis (pAFO) equipped with the insole. During walking, foot pressure data such as maximum force, contacting area, peak pressure, and mean pressure was collected using Pedar-X system (Novel Gmbh, Germany) and EMG activity of lower limb muscles such as gastrocnemius medial head, gastrocnemius lateral head, and soleus was recorded using MP150 EMG module (BIOPAC System Inc., USA). Collected data was then analyzed using paired t-test in order to investigate the effects of the insole. As a result of the analysis, when insole was equipped, overall contacting area was increased while both the highest peak pressure and the mean pressure were significantly decreased, and EMG activity of the lower limb muscles was decreased. On the contrary, the cases of wearing AFO showed the decreased contacting area and the increased pressures. Therefore, the AFO equipped with a proper insole fit well to the foot can help comfortable walking by spreading the pressure over the entire plantar.

  • The authors have diagnosed and treated patients with May-Thurner syndrome who presented with recurrent edema and redness in the left lower leg and foot. Although May-Thurner syndrome is a rare vascular disease, its primary symptoms manifest as edema and redness in the left lower leg and foot, leading the patients to seek foot and ankle surgery. Suspicion should be directed towards May-Thurner syndrome if an obese individual who spends prolonged periods sitting repeatedly complains of edema and redness in the left lower leg and foot area, in which a blood clot forms due to compression of the left common iliac vein within the pelvis.

발계측 자료에 기초한 신골 할출 시스템의 검증에 관한 연구 (A Study on Verification of Shoe Last Grading System Based on Foot Measuring Data)

  • Shoe's size and shape are determined by the last that takes shape of foot because last is the mold of shoe in development and manufacturing process. Then adaptation between foot and shoe is dependent on the last. In mass shoe production, model size is developed in the first place, other sized lasts are made through the grading process based on model size. The most important factor in grading system is grading deviation that must be same amount induced from foot measuring database. At present, most of the last manufacturing companies in korea using 260mm as a standard foot model size. When length grading deviation is 5mm, the ball girth grading deviation is 3.7mm and the ball width grading deviation is 1.2mm. I verified existing grading system by comparing grading results with foot measuring data. Also, I proposed reasonable grading deviation and application method of grading system. From the analysis of foot measuring database, reasonable grading deviations are 1.22mm in ball width and 0.84mm in ankle height in case of length grading deviation is 5mm. I confirmed that the current grading system is very accurate. When we grade last from 230mm to 290mm by current grading system based on model size 260mm, there is grading error over 1mm in the front outside area of foot. This error level of 1mm is no problem in normal walking shoe's last, but it induces adaptation problems in sports and special purposed shoe's last. Therefore using of three standard model size is recommended in grading men's last for reducing grading deviation error under the level of 1mm. It is specifically described as 235mm in 225-245mm, 260mm in 250-270mm, 285mm in 275-295mm. According to the above recommended grading system, it is enough to measure only three foot sizes in case of foot measuring project for men's last development.

아동화 설계에 요구되는 치수 및 구조요인의 정량적 분석 -학령기 여아를 대상으로- (Quantitative Analysis of the Size and the Structural Factors of the Feet for Elementary School Girls' Shoe Design)

  • This study was performed to provide the analysis on their size and the structural factors required in the process of design and manufacture of school girls' shoes. 371 elementary school girls in Kyungin and Youngnam area were participated in the size measurement. 25 foot items and 6 main body items were measured directly or indirectly using a digital photography. The results of the study are as follows: first, by most of measured items, the range of their foot size was very wide from the size of toddlers to adults'. That shows that the change of school girls' foot size occurred with their growth is pretty big. Second, from the structural factor analysis on 25 foot items, five factors were extracted such as 'the size of the foot', 'the volume of the foot,' 'the height and inclination of the foot,' 'the shape of the foot,' and 'the inside and outside inclination of the foot'. Third, from the cluster analysis, three clusters were classified: Cluster 1 was the group of 10 to 11 year old girls who had big-sized feet. The elementary school girls in the fourth to sixth grade belonged to this group. Cluster 2 consisted of girls who had small-sized and big-volumed feet. Cluster 3 had medium-sized and slim-shaped feet. Most of 6 to 7 year old elementary school girls belonged to this group. The above-mentioned results imply that many continual researches are required on children's shoe production reflecting the change of elementary school girls' feet size owing to their growth. The quantitative data on elementary school girls' feet size in this study could be used as basic information for the development of children's shoe design and its production.
