수상태양광 정책의제설정 연구 - 기술과 제도의 공진화 관점 - (Policy Agenda Setting of Floating Solar PV - Based on the Co-evolution of Technology and Institutions -)

  • Floating solar photovoltaic (hereinafter PV) power generation is emerging as a proper alternative to overcome various environmental limitations of existing offshore PV generation. However, more government-led policy design and technical and institutional development are still required. Based on the policy agenda setting theory and technological innovation theory, this study contains the research questions concerning the co-evolution of technology and the floating solar PV policy. This study primarily evaluates the technological and institutional development level of floating solar PV policy through a survey of domestic floating solar PV experts. Secondly, we also analyze the kind of policy agenda that should be set a priori. Analyzing the priorities to be considered, the first environmental enhancement needs to be considered from both the technical and institutional aspects. The second candidate task for the policy agenda is residents' conflict and improvement of regulations. Both candidate tasks need to be actively considered in the policy agenda from the institutional point of view. The third is publicity, profit sharing, follow-up monitoring, and cost. Among them, public relations and profit sharing are tasks that need to be considered in the policy agenda from the institutional point of view. On the other hand, the cost of follow-up monitoring should be considered as a policy agenda in terms of technology, system, and common aspects. Finally, there are technical standards. Likewise, technical standards need to be considered in the policy agenda in terms of both technical and institutional commonality.

  • Although the era of very large telescopes has come, small telescopes still have advantages for fast follow-up and long-term monitoring observation. Intensive monitoring survey of nearby galaxies (IMSNG) aims to understand the nature of the supernovae (SNe) by catching the early light curve from them with the network of small telescopes from 0.4-m to 1.0-m all around the world. To achieve the scientific goals with heterogeneous facilities, three factors are important. First, automatic processes as soon as data is uploaded will increase efficiency and shorten the time. Second, searching for transients is necessary to deal with newly emerged transients for fast follow-up observation. Finally, the Integrated process for different telescopes gives a homogeneous output, which will eventually make connections with the database easy. Here, we introduce the integrated pipeline, 'gppy' based on Python, for more than 10 facilities having various configurations and its performance. Processes consist of image pre-process, photometry, image align, image combine, photometry, and transient search. In the connected database, homogeneous output is summarized and analyzed additionally to filter transient candidates with light curves. This talk will suggest the future work to improve the performance and usability on the other projects, gravitational wave electromagnetic wave counterpart in Korea Observatory (GECKO), and small telescope network of Korea (SOMANGNET).

Monitoring Posterior Cerebral Perfusion Changes With Dynamic Susceptibility Contrast-Enhanced Perfusion MRI After Anterior Revascularization Surgery in Pediatric Moyamoya Disease

  • Objective: To determine whether dynamic susceptibility contrast-enhanced (DSC) perfusion magnetic resonance imaging (MRI) can be used to evaluate posterior cerebral circulation in pediatric patients with moyamoya disease (MMD) who underwent anterior revascularization. Materials and Methods: This study retrospectively included 73 patients with MMD who underwent DSC perfusion MRI (age, 12.2 ± 6.1 years) between January 2016 and December 2020, owing to recent-onset clinical symptoms during the follow-up period after completion of anterior revascularization. DSC perfusion images were analyzed using a dedicated software package (NordicICE; Nordic NeuroLab) for the middle cerebral artery (MCA), posterior cerebral artery (PCA), and posterior border zone between the two regions (PCA-MCA). Patients were divided into two groups; the PCA stenosis group included 30 patients with newly confirmed PCA involvement, while the no PCA stenosis group included 43 patients without PCA involvement. The relationship between DSC perfusion parameters and PCA stenosis, as well as the performance of the parameters in discriminating between groups, were analyzed. Results: In the PCA stenosis group, the mean follow-up duration was 5.3 years after anterior revascularization, and visual disturbances were a common symptom. Normalized cerebral blood volume was increased, and both the normalized time-topeak (nTTP) and mean transit time values were significantly delayed in the PCA stenosis group compared with those in the no PCA stenosis group in the PCA and PCA-MCA border zones. TTPPCA (odds ratio [OR] = 6.745; 95% confidence interval [CI] = 2.665-17.074; P < 0.001) and CBVPCA-MCA (OR = 1.567; 95% CI = 1.021-2.406; P = 0.040) were independently associated with PCA stenosis. TTPPCA showed the highest receiver operating characteristic curve area in discriminating for PCA stenosis (0.895; 95% CI = 0.803-0.986). Conclusion: nTTP can be used to effectively diagnose PCA stenosis. Therefore, DSC perfusion MRI may be a valuable tool for monitoring PCA stenosis in patients with MMD.

  • In this paper, we have developed PPG pillow system for unconstrained respiration monitoring during sleep. The system employs a pillow containing a PPG sensor and a simple respiration extraction algorithm. The results showed that the extracted respiratory rhythm was found to have close relations with the reference signal. The system has an advantage of processing simplicity. A follow-up study should be performed to evaluate the system in terms of breath intake.

Achalasia Previously Diagnosed as Gastroesophageal Reflux Disease by Relying on Esophageal Impedance-pH Monitoring: Use of High-Resolution Esophageal Manometry in Children

  • Gastroesophageal reflux disorder (GERD) is the most common esophageal disorder in children. Achalasia occurs less commonly but has similar symptoms to GERD. A nine-year old boy presented with vomiting, heartburn, and nocturnal cough. The esophageal impedance-pH monitor revealed nonacidic GERD (all-refluxate clearance percent time of 20.9%). His symptoms persisted despite medical treatment for GERD, and he was lost to follow up. Four years later, he presented with heartburn, solid-food dysphagia, daily post-prandial vomiting, and failure to thrive. Endoscopy showed a severely dilated esophagus with candidiasis. High-resolution manometry was performed, and he was diagnosed with classic achalasia (also known as type I). His symptoms resolved after two pneumatic dilatation procedures, and his weight and height began to catch up to his peers. Clinicians might consider using high-resolution manometry in children with atypical GERD even after evaluation with an impedance-pH monitor.

  • Purpose: The purpose of this experimental study have been testifying about reactive muscle control regarding sternocleidomastoid and psoas. Low back pain patient do not sit-up exercise without abnormality of trunk flexor. The reason of problem is unbalance sternocleidomastoid and psoas. According to George J, Goodheart who have been developing Applied Kinesiology (A. K.) since reactive muscle recognized gait testing in 1964. Materials and Methods: From September, 2004 to February, 2005, I have controled sternocleidomastoid and psoas. It is applied to the patients who are unable sit up exercise at Yang-Dong local clinic, Yang-Pyung county, Kyung-Gi Do province in Korea. 24 Patients divided 2 groups. A group is applied general physical therapy. Also B group is added reactive muscle control from M.E.M.P.T.(Korean society of muscle and energy monitoring physical therapy) Results : The results of this study follow. 1. Possible sit up group 8 persons of the second group. 2. Not possible sit up group the others. Conclusion: The study present that reactive muscle control sternocliedomastoid and psoas is useful in patient with low back pain is difficult to sit up.

Globus Pharyngeus의 진단 및 치료 성적 (Diagnostic and Therapeutic Analysis of Globus Pharyngeus)

  • Globus pharyngeus is a common problem comprising between 3% and 4% of new otolaryngology outpatient referrals. We do not understand the etiology of globus exactly and it is remained a disease of exclusion. The treatment of globus pharyngeus is still not established. The aim of this study is to understand the etiologic factors and determine the reliable guide for selecting method of choice of evaluation and improving therapeutic response of globus pharyngeus. A total of 141 patients were investigated by authors. 25 of 141 patients were excluded from the study because they could not satisfy the definition criteria of this study. After detailed Interview and comprehensive physical examinations, all the 116 patients had underwent barium esophagogram, fiberoptic esophagogastroscopy, esophageal manometry and 24-hour pH monitoring. They could follow up for at least 3 months. There were 43 male and 73 female subjects and the mean age was 46.5 year. Esophagogram revealed normal in 94(81%) subjects. 78 patients(67.2%) were normal in esophageal manometry. Gastroesophageal reflux(GERD) was found in 24 cases and borderline GERD was found in 25 cases showing an overall incidence of 42.2% for 24-hour pH monitoring. Especially 44(89.8%) of the 49 patients with proven reflux on 24-hour pH monitoring showed therapeutic response whereas 48(71.6%) of the 67 subjects without reflux showed response.

A Case-Study of Implementation of Improved Strategies for Prevention of Laboratory-acquired Brucellosis

  • Background: In 2012, the Alaska Section of Epidemiology investigated personnel potentially exposed to a Brucella suis isolate as it transited through three laboratories. Methods: We summarize the first implementation of the United States Centers for Disease Control and Prevention 2013 revised recommendations for monitoring such exposures: (1) risk classification; (2) antimicrobial postexposure prophylaxis; (3) serologic monitoring; and (4) symptom surveillance. Results: Over 30 people were assessed for exposure and subsequently monitored for development of illness. No cases of laboratory-associated brucellosis occurred. Changes were made to gaps in laboratory biosafety practices that had been identified in the investigation. Conclusion: Achieving full compliance for the precise schedule of serologic monitoring was challenging and resource intensive for the laboratory performing testing. More refined exposure assessments could inform decision making for follow-up to maximize likelihood of detecting persons at risk while not overtaxing resources.

Intraoperative Nerve Monitoring during Minimally Invasive Esophagectomy and 3-Field Lymphadenectomy: Safety, Efficacy, and Feasibility

  • Background: The objective of this study was to demonstrate the safety, efficacy, and feasibility of intraoperative monitoring of the recurrent laryngeal nerves during thoracoscopic and robotic 3-field esophagectomy. Methods: This retrospective analysis details our initial experience using intraoperative nerve monitoring (IONM) during minimally invasive 3-field esophagectomy. Data were obtained from a prospectively maintained database and electronic medical records. The study included all patients who underwent minimally invasive (video-assisted thoracic surgery/robotic) transthoracic esophagectomy with neck anastomosis. The patients were divided into those who underwent IONM during the study period and a historical cohort who underwent 3-field esophagectomy without IONM at the same institution. Appropriate statistical tests were used to compare the 2 groups. Results: Twenty-four patients underwent nerve monitoring during minimally invasive 3-field esophagectomy. Of these, 15 patients underwent thoraco-laparoscopic operation, while 9 received a robot-assisted procedure. In the immediate postoperative period, 8 of 24 patients (33.3%) experienced vocal cord paralysis. Relative to a historical cohort from the same institution, who were treated with surgery without nerve monitoring in the preceding 5 years, a 26% reduction was observed in the nerve paralysis rate (p=0.08). On follow-up, 6 of the 8 patients with vocal cord paralysis reported a return to normal vocal function. Additionally, patients who underwent IONM exhibited a higher nodal yield and a decreased frequency of tracheostomy and bronchoscopy. Conclusion: The use of IONM during minimally invasive 3-field esophagectomy is safe and feasible. This technique has the potential to decrease the incidence of recurrent nerve palsy and increase nodal yield.

Remote digital monitoring during the retention phase of orthodontic treatment: A prospective feasibility study

  • Objective: To evaluate if a remote digital monitoring system added at the end of orthodontic treatment could positively influence the retention phase by reducing the occurrence of misfit of removable appliances, number of emergency appointments (EA), and orthodontic relapse. Methods: Twenty-seven patients who completed active orthodontic treatment were divided into the study and control groups. In addition to the standard chairside follow-up appointments at month 1 (T1), month 3 (T2), month 6 (T3), the study group patients were monitored using Dental Monitoring® with monthly intra-oral scans. Occurrence of misfit of removable retainers, number of EAs, and intercanine width change were recorded for both groups. Differences in EAs and retainer fit were assessed using the chi-square test. Intra-group and inter-group differences in the intercanine width were assessed with Friedman test and Mann-Whitney U test, respectively (α = 0.05). Results: The study group showed a significantly lower occurrence of misfit of removable retainers (p = 0.027) compared to the control group. No significant inter- and intra-group difference was found in the EAs and intercanine width change at each time-point. Conclusions: Integrating remote monitoring systems, such as Dental Monitoring®, to the retention phase of the orthodontic treatment may lower the occurrence of misfit of removable retainers. However, a small sample size and a short observation period limit the strength of this evidence. These preliminary results tentatively suggest that remote monitoring technologies may be beneficial, especially during the COVID-19 pandemic, when the regularity of in-office visits might be disrupted.
