A Case Report on Chronic Intestinal Pseudo-obstruction Presenting as Unspecified Abdominal Pain in an Adult (만성 거짓 장폐쇄로 진단한 상세불명의 복통 환자 증례 보고)

  • Objectives: This study examined a case of chronic intestinal pseudo-obstruction (CIPO) in an adult with unspecified abdominal pain.Methods: The patient was treated with herbal medicine, acupuncture, and moxa. Treatment progress was evaluated by follow-up monitoring of the intensity, frequency, and duration (in hours) of abdominal pain and body weight. Results: The average abdominal pain level on the Numerical Rating Scale (NRS) and its incidence decreased. The duration of pain was 15 hours maximum and 2.5 hours minimum. The body weight repeatedly increased and decreased but increased overall. Despite no remarkable findings from X-rays, air-fluid levels and coil-shaped folds of the small bowel were observed. Conclusions: Korean medical treatment based on a dialectic effectively relieved chronic abdominal pain and suspected CIPO-a disease that requires continuous management and therapeutic intervention.

Coexistence of Excessive Weight Gain and Celiac Disease in Children: An Unusual Familial Condition

  • Excessive weight gain in children diagnosed with celiac disease (CD) is becoming more common. We describe 2 siblings (9-year and 6 months-old female and 6-year and 9 months-old male) with obesity showing attenuated gastrointestinal and atypical symptoms in which CD was diagnosed in the absence of a known family history of CD. After children's diagnosis, CD in their parents was also investigated. It was detected in their father affected by overweight. The presentation of patients with CD has changed. While patients with overweight and obesity commonly have symptoms such as abdominal pain, reflux, headache, and constipation due to lifestyle factors, CD should also be considered in patients with or without a family history of CD. Careful nutritional status assessment and follow-up monitoring after the diagnosis of CD are mandatory, especially in subjects who are already overweight at the presentation of this disease.

Basic Research on Mid-to-Long-term Management Plan of Purchased Land by Evaluating Ecological Function of Waterfront Area of Geum River (금강수계 수변구역의 생태 기능 평가를 통한 매수토지 중장기 관리 방안 기초 연구)

  • The purpose of this study was to propose an efficient management plan for purchased land considering the hydrological, watershed characteristics and ecological functions of the target land and surrounding area based on the result of monitoring the water quality improvement effect obtained by purchasing the land in the watershed area in the Geum river system. For this purpose, this study investigated through literature review, and examined ways to apply them to this research by deriving implications from a comprehensive analysis of previous research cases. After that, the components of assessment were derived to evaluate the ecological function of the purchase land, and the ecological function evaluation model for each land and area was proposed. In order to select purchase and restoration priorities of the land, this study analyzed the ecological status of the purchased land in main watersheds and tributaries using Arc GIS ver 10.1. Through this, a process to select restoration priorities was developed. And this study constructed the integrated management process with proposing a mid - to long - term plan by integrating the purchased land valuation and restoration priority selection process. Based on this process, this study suggested an effective management plan for purchased land through the integrated ecological management system of lands purchased. It can be used systematically in appraisal valuation, land purchase system, restoration project, and follow-up management of land purchase.

Diagnostic Accuracy of Dermoscopy for Scabies

  • The diagnostic accuracy of dermoscopy (DS) for scabies, a highly contagious parasitic disease, remains disputed. This study aimed to assess the diagnostic accuracy of DS in scabies, analyze the factors influencing DS, and explore its role in post-treatment evaluation. Patients with suspected scabies were randomly divided into 2 groups: 71 patients in the skin scraping (SS) group and 73 patients in the DS group. The diagnostic efficiencies of SS and DS in these groups were calculated. We also analyzed the influence of body part and investigator competence on the accuracy of DS. Then 16 body parts with typical signs of scabies were monitored by DS 2 and 4 day after sulfur ointment treatment. The sensitivity and specificity of DS were 98.3% and 88.5%, respectively. Hands, arms, and the abdomen had higher positivity rates than other body parts (P<0.001). The accuracy of dermatologists' interpretations of images negative for scabies in the intermediate- and high-level groups was higher than that in the low-level group (P<0.001). At follow-up, the mites were still visible on 43.8% to 62.5% of the skin lesions 2 and 4 day after sulfur ointment treatment. These results showed that DS could significantly increase the accuracy of diagnosing scabies owing to its high sensitivity and specificity. Therefore, it may be useful for monitoring clinical responses to anti-parasitic treatment.

A Study on the Evaluation of Woody Tree Vitality of Artificial Ground: Case Study of Seoullo 7017

  • Background and objective: This study examined, compared, and analyzed the tree vitality of the trees growing on the artificial ground of Seoullo 7017 that transformed the overpass that was to be demolished into a "sky garden" using portable tree pots. Methods: Based on the summer season when the metabolic activity of plants is most active, this study measured the cambial electrical resistance in four directions(east, west, south and north), using the Shigometer (model OZ-93, Osmose) and compared the location and analysis of pots according to their means and standard errors. Results: Meanwhile, according to the analysis, vitality was relatively superior in pots with a big diameter, trees planted individually than in groups, trees of the ramp section rather than the bridge section, and in the southwest direction of the cambium. Conclusion: This study revealed the improper condition of the planting plan and implementation on the site, where various species of trees are displayed in a poor environment. Despite the significant assessment of the vitality of various trees introduced within Seoullo 7017 for the first time, this study is limited in that the data used were measured only once in summer. In this regard, it raised the need for continuous interest in and monitoring of a special plant environment and development of proper maintenance and management techniques, along with follow-up research on seasonal and temperature conditions, soil moisture and root development conditions to supplement this research.

  • Blood pressure is one of the key physiological parameters for determining human health, and can prove whether human cardiovascular function is healthy or not. In general, what we call blood pressure refers to arterial blood pressure. Blood pressure fluctuates greatly and, due to the influence of various factors, even varies with each heartbeat. Therefore, achievement of continuous blood pressure measurement is particularly important for more accurate diagnosis. It is difficult to achieve long-term continuous blood pressure monitoring with traditional measurement methods due to the continuous wear of measuring instruments. On the other hand, radar technology is not easily affected by environmental factors and is capable of strong penetration. In this study, by using machine learning, tried to develop a linear blood pressure prediction model using data from a public database. The radar sensor evaluates the measured object, obtains the pulse waveform data, calculates the pulse transmission time, and obtains the blood pressure data through linear model regression analysis. Confirm its availability to facilitate follow-up research, such as integrating other sensors, collecting temperature, heartbeat, respiratory pulse and other data, and seeking medical treatment in time in case of abnormalities.

A plunging ranula in a child with holoprosencephaly: a case of unique pathophysiology and difficult airway management

  • A ranula is a pseudocyst that originates from the sublingual gland after trauma. Acute cases of ranulas that progress rapidly and cause respiratory distress are rare. Holoprosencephaly is a complex brain malformation caused by incomplete cleavage of the prosencephalon. Children with holoprosencephaly may experience upper airway obstruction due to the associated dentoalveolar malformations and oromotor dysfunctions. We present the case of an eight-year-old female patient with holoprosencephaly and a plunging ranula that manifested as an acute course due to difficult airway management. She required gastrostomy for oromotor dysfunctions related to feeding and swallowing and difficulty managing oral secretions. The sublingual gland and ranula were removed under general anesthesia. Postoperatively, urgent reintubation and close monitoring in the intensive care unit were required due to upper airway obstruction. We successfully managed the patient with close cooperation of a pediatrician and an anesthetist, and no recurrence was observed at the one-year follow-up. A ranula can be caused by trauma to the floor of the mouth in association with lingually inclined mandibular teeth, a type of dentoalveolar compensation seen in maxillary hypoplasia associated with holoprosencephaly. Careful consideration is needed in such cases since airway management can be difficult due to postoperative swelling and oromotor dysfunctions.

A Study on Optimization of Classification Performance through Fourier Transform and Image Augmentation (푸리에 변환 및 이미지 증강을 통한 분류 성능 최적화에 관한 연구)

  • Purpose: This study proposes a classification model for implementing condition-based maintenance (CBM) by monitoring the real-time status of a machine using acceleration sensor data collected from a vehicle. Methods: The classification model's performance was improved by applying Fourier transform to convert the acceleration sensor data from the time domain to the frequency domain. Additionally, the Generative Adversarial Network (GAN) algorithm was used to augment images and further enhance the classification model's performance. Results: Experimental results demonstrate that the GAN algorithm can effectively serve as an image augmentation technique to enhance the performance of the classification model. Consequently, the proposed approach yielded a significant improvement in the classification model's accuracy. Conclusion: While this study focused on the effectiveness of the GAN algorithm as an image augmentation method, further research is necessary to compare its performance with other image augmentation techniques. Additionally, it is essential to consider the potential for performance degradation due to class imbalance and conduct follow-up studies to address this issue.

Replantation of autotransplanted mature third molar in anterior open bite patient: case report (개방 교합 환자에서 자가 이식된 치아의 재식)

  • Autotransplantation of third molars with completely formed roots is known to be effective and provide a high long-term success rate. However, in case of severe mobility or unexpectedly extraction is observed during the monitoring period after surgery, it is generally considered as a failure. This case report describes successful replantation of autotransplanted mature third molar into surgically created molar socket. 1 year follow up of transplanted tooth showed clinically normal periodontal pocket depth and tooth mobility. Root resorption or bone loss were not observed. Provided that there is no apparent sign of inflammation, re-insertion into socket is a viable alternative to immediate determination of extraction.

  • For an earthquake-safe urban environment, the Republic of Korea conducts seismic performance evaluation in accordance with laws and guidelines to assign safety ratings and implement necessary management measures such as repairs and reinforcements. In the seismic performance evaluation result, structures lacking in preparation for earthquakes are prioritized and classified into measures such as repair, reinforcement, or careful observation to respond to physical risks such as earthquakes. Such repair and reinforcement work is not a one-time thing, but it is necessary to further enhance the effect through continuous follow-up observation. In this study, the location of the vertical and horizontal displacement measuring part of the construction part is displayed so that the post-construction status of the reinforcement construction part can be visually checked by identifying the problems in the process of post-monitoring in 2022 for the maintenance and reinforcement work of local governments' public facilities carried out in 2021. We propose a plan to institutionalize the installation of, inspection tools, and crack gauges at certain locations in the construction department, and to have facility managers periodically inspect and manage them with a smartphone program or the 'Facility Autonomous Safety Inspection' app.
