Associations of dietary intake and metabolic syndrome risk parameters in Vietnamese female marriage immigrants in South Korea: The KoGES follow-up study

  • BACKGROUND/OBJECTIVES: This study was conducted to compare the overall changes in dietary intake and metabolic syndrome risk parameters in Vietnamese marriage-based female immigrants over time. SUBJECTS/METHODS: The subjects of this study were 581 Vietnamese marriage-based female immigrants, who were recruited from local clinical centers in Korea. Baseline data were collected from 2006-2011 and the follow-up data were collected from 2012-2014. Dietary food intake was assessed by a 1-day 24-hour recall method. RESULTS: Compared to the baseline, the mean body weight, body mass index, waist circumference, high density lipoprotein (HDL)-cholesterol, systolic blood pressure and diastolic blood pressure increased while the fasting blood sugar, triglycerides and low density lipoprotein-cholesterol decreased at follow-up. The total consumption of foods such as vegetables/fruits/seaweeds/mushrooms, oil and fat, and eggs decreased during the follow-up period, whereas the consumption of sugars, beverages and meats increased. Partial correlation between the rate of change [(Follow-up - Baseline) / Baseline ${\times}100$] in metabolic syndrome risk parameters and food intake after controlling for confounding factors showed that the waist circumference was positively correlated with either the total plant food intake (r = 0.1042, P = 0.0129) or the total food intake (r = 0.0880, P = 0.0359). The plasma levels of total cholesterol (r = -0.1918, P = 0.0289) and HDL-cholesterol (r = -0.1424, P = 0.0007) were negatively correlated with the percentage of total intake from plant food, and HDL-cholesterol levels were positively correlated with the intake of total animal food (r = 0.0980, P = 0.0217). The serum C-reactive protein levels were positively correlated with the total intake of animal food (r = 0.2374, P < 0.0001) or the percentage of total intake from animal food (r = 0.1346, P = 0.0016). CONCLUSIONS: These results suggest that the change rate of dietary intake such as total plant food or animal food is associated with the change rates of metabolic syndrome risk parameters.

Analysis of Patients with Facial Lacerations Repaired in the Emergency Room of a Provincial Hospital

  • Background Facial laceration is the most common injury encountered in the emergency room in the plastic surgery field, and optimal treatment is important. However, few authors have investigated this injury in all age groups or performed follow-up visit after repair. In the present study, the medical records of patients with lacerations in the facial area and underwent primary repair in an emergency room over a 2-year period were reviewed and analyzed. Methods Medical records of 3,234 patients with lacerations in facial area and underwent primary repair in an emergency room between March 2011 and February 2013 were reviewed and identified. Results All the 3,234 patients were evaluated, whose ratio of men to women was 2.65 to 1. The forehead was the most common region affected and a slip down was the most common mechanism of injury. In terms of monthly distribution, May had the highest percentage. 1,566 patients received follow-up managements, and 58 patients experienced complications. The average days of follow-up were 9.8. Conclusions Proportion of male adolescents was significantly higher than in the other groups. Facial lacerations exhibit a 'T-shaped' facial distribution centered about the forehead. Careful management is necessary if a laceration involves or is located in the oral cavity. We were unable to long term follow-up most patients. Thus, it is necessary to encourage patients and give them proper education for follow-up in enough period.

Mid-term Results of Total Hip Arthroplasty for Posttraumatic Osteoarthritis after Acetabular Fracture

  • Purpose: The prognosis of total hip replacement (THR) after open reduction and internal fixation (ORIF) versus THR following non-operative treatment of acetabular fractures is unclear. Few studies have been conducted in this regard. Therefore, the purpose of the current study was to perform an assessment and compare the functional outcomes for study subjects in the ORIF and non-ORIF groups during the follow-up period compared to baseline. Materials and Methods: This longitudinal comparative study, which included 40 patients who underwent THR for either posttraumatic arthritis after fixation of an acetabular fracture or arthritis following conservative management of a fracture, was conducted for 60 months. Twenty-four patients had undergone ORIF, and 16 patients had undergone nonoperative/conservative management for acetabular fractures. Following THR, the patients were followed up for monitoring of functional outcomes for the Harris hip score (HHS) and comparison between the ORIF and non-ORIF groups was performed. Results: The HHS showed significant improvement in both ORIF and non-ORIF groups. At the end of the mean follow-up period, no significant variation in scores was observed between the groups, i.e., ORIF group (91.61±6.64) compared to non-ORIF group (85.74±11.56). A significantly higher number of re-interventions were required for medial wall fractures and combined fractures compared to posterior fractures (P <0.05). Conclusion: THR resulted in improved functional outcome during follow-up in both the groups; however, the ORIF group was observed to have better functional outcome. Re-intervention was not required for any of the posterior fractures at the end of the mean follow-up period.

평창동계올림픽 사후관리를 위한 기록화전략 제안 (A Recording Strategy Proposal for the Post-Management of the Pyeongchang Winter Olympics)

  • 본 연구의 목적은 2018년 2월 개최된 평창동계올림픽 사후관리를 위한 기록화전략을 제안하는 데 있다. 연구목적을 달성하기 위해 평창동계올림픽 사후관리 기록의 가치와 기록의 유형을 파악하고 국내와 국외에서 진행된 메가 스포츠이벤트 사후관리 사례를 선정하여 분석하였다. 이를 바탕으로 평창동계올림픽 사후관리를 위한 기록화전략으로서 3가지를 제안할 수 있다. 첫째, 기록화 대상과 주체, 주제별, 계층별 범위를 명확하게 설정한 후 사후관리를 실시해야 한다. 둘째, 제한적인 문헌기록의 보완을 위해 구술기록을 통한 사후관리를 실시한다. 셋째, 홈페이지와 온라인 플랫폼을 통한 서비스시스템을 구축하여 사후관리에 활용해야 한다. 본 연구결과는 향후 평창동계올림픽기념관 건립과 국내 메가 스포츠이벤트의 사후관리를 위한 기록화 전략 마련에 활용될 수 있을 것이다.

개발사업에 의한 자연경관 영향 저감방안 중요도 분석에 관한 연구 (A Study on the Analysis of the Importance of Natural Landscape by the Development Project)

  • Environmental impact assessment (EIA), which predicts, evaluates, and manages the influences on natural landscape, plays a role of monitoring natural resources for systematic management of natural landscape. However, the function of verification and correction of the system is still insufficient and feed-back, one of the most important features of EIA follow-up, has not been introduced in Korea's EIA system yet. As a procedure, it is required to check if the opinions of the evaluators are properly reflected to the outcomes of the project through a reviewing process after assessing environmental impacts of a development project. In reality, despite the awareness about the importance of follow-up inspection of the conformity with, the system mainly focuses on the agreement during the planning stage of the development project and fails to continuously manage after its completion. There have been various preceding studies related to prediction, evaluation, and management of environmental impacts on natural landscape for better management. They primarily dealt with the problems in the EIA process and suggested improvement measures, including directions for institutional development, step-by-step goals, and operation methods, to address the problems which arise in the EIA follow-up process. However, suggested measures are not actively applied with the focus only put on institutional operation, there are virtually no standardized methods to predict and assess landscape changes due to the development project and to manage landscape after the project. Against this backdrop, this study aims to explore the existing methods to analyze the impacts natural landscape and to establish a system where landscape management is continued after the development project. To this end, we will suggest reducing methods according to the predicted changes in landscape for post-project management of natural landscape. Characteristics of reduction methods by project type were examined through reviewing the guide to natural landscape rating and the importance of development project impacts on natural landscape by type of reduction was evaluated through questionnaire for experts. Evaluated types of reduction are classified and presented by characteristics of each development project and content of reduction type.

현장 조사를 통한 가축전염병 대응 정책개선 방안 연구 (Study on Policy Improvement Measures to Respond to Infectious Diseases of Livestock through Field Investigation)

  • 연구목적: 본 연구는 가축전염병 가축사체처리 및 방역관리에 대한 특성을 분석하고, 가축전염병 방역과 사후 관리의 실효성을 제시하는 것을 목적으로 한다. 연구방법: 실제로 가축전염병이 발생하여 가축 사체를 처리한 매몰지 및 매몰 관리시설을 현장 방문하여 관리현황과 문제점을 분석하였다. 연구결과: 가축전염병 발생 가축의 매몰방식과 그에 따른 장기적인 후속 관리는 많은 인력과 예산을 투입하게 만들어 비효율적인 방역시스템임을 알 수 있었다. 결론: 2차적인 감염병 전파우려가 없고 후속 관리가 필요 없는 정부와 광역 시도 및 시군구 차원의 통합 가축전염병동물 사체처리 시설 설립 방안의 검토가 필요하다.

장애 유형별 치과 검진 및 관리에 대한 비교연구 (PATTERN OF CHECK UP AND DENTAL MANAGEMENT OF SPECIAL CARE NEEDS PATIENTS ACCORDING TO TYPES OF DISABILITIES)

  • Regular dental visit of disabled patients is an important strategy for maintenance of oral health because of the lack of awareness of oral care. But there is limited information about follow up period in disabled patients after dental treatment. The aim of this study was to investigate the pattern of dental visit and dental management of special care needs patients according to the types of disabilities. A total of 140 patients who received dental treatment at Seoul National University Dental Hospital from 2010 to 2012 were reviewed. Patients were divided 3 groups according to the types of disabilities (disorder of external physical function, disorder of internal organ, psychic disability). Patient's characteristic, dental treatment performed, follow up period and frequency were investigated. The mean follow up period was 28.2 months and average frequency of visit was 3.69 times per year. There was no difference in period and annual visit between 3 groups, but significantly difference in cooperation with medical hospital in affiliation. Regular check-up was the most common dental treatment in all groups. In psychic disorder group, the proportion of sedative treatment was significantly higher than other groups. The results of this study provide information necessary for treatment planning and dental management of disabled patients.

노부모 부양자 교육프로그램의 적용과 효과 (A Study on the Implementation of an Educational Program for Caregivers)

  • This study was conducted to reconstitute a family life education program for caregivers and to analyze the system of implementation. Based largely on a pre-existing program for caregiving families, I developed a new educational program. Personal time management and personal needs management were added in the content of the program order to help enhance self-esteem and internal control. The reconstituted educational program, which consists of 6 sessions, was implemented on a small group of caregivers in Jeollabukdo Province who support their elderly parents. The effectiveness of the program was evaluated by a pre-test, a post-test, a follow-up test and open-questionnaire reports. The results from the post-test suggest that the program has been effective in improving the knowledge on aging process, internal control and self-esteem. But the follow-up test results show the program has been effective only in improving the knowledge on aging process. According to the open-questionnaire reports, the program seems to have been effective in improving interpersonal relationships and changing attitudes toward their elderly parents.

Extended duration pulsed radiofrequency for the management of refractory meralgia paresthetica: a series of five cases

  • Meralgia paresthetica (MP) is a sensory mononeuropathy, caused by compression of the lateral femoral cutaneous nerve (LFCN) of thigh. Patients refractory to conservative management are treated with various interventional procedures. We report the first use of extended duration (8 minutes) pulsed radiofrequency of the LFCN in a case series of five patients with refractory MP. Four patients had follow up for 1-2 years, and one had 6 months follow up. All patients reported remarkable and long lasting symptom relief and an increase in daily life activities. Three patients came off medications and two patients required minimal doses of neuropathic medications. No complications were observed.

Conservative surgical approach to aggressive benign odontogenic neoplasm: a report of three cases

  • Aggressive benign odontogenic neoplasms have substantial potential to grow to an enormous size with resulting bone deformities, and they often invade adjacent tissues and spread beyond their normal clinical and radiographic margins; as such, they have a high rate of recurrence. Historically, management (conservative versus aggressive) on the basis of clinical, radiographic and/or histopathologic characteristics has been controversial. However, recent advances in the understanding of the biological features of these lesions may provide greater evidence of the benefits of conservative management. Three patients with different complaints and final histopathologic diagnoses were enrolled in the study. All three cases were treated by a single operator with similar conservative surgical procedures. During follow-up, the patients had uneventful secondary healing and bone regeneration, less packing time than previously reported, no clinical or radiographic evidence of recurrence and no apparent deformity. The aggressive behavior of these lesions requires long clinical and radiographic follow-up. Conservative surgical management may be an option to reduce recurrence and morbidity and increase the probability of uneventful secondary healing and bone regeneration.
