  • Malignant fibrous histiocytoma (MFH) of the breast is extremely rare and most of the previous reports were focused on the clinicopathological features of breast MFH, so analysis of its imaging findings have been limited. We report a case of MFH involving left breast and left axilla, metastasized from known MFH of left forearm, with focusing on imaging findings based on radiologic evaluation of the breast including mammography, ultrasound and breast MRI.

  • Delayed onset muscle soreness is a sensation of discomfort that occurs 24h after exercise, and it is associated with the performance of unfamiliar and high force muscle wor, such as eccentric contractions. The injury to the muscle has been well described but the mechanism underlying the injury is not fully understood. Although the pathophysiological processes underlying delayed onset muscle soreness are not completely understood, many researchers have investigated various treatments in a attempt to reduce the soreness. These treatments have focused on reducing the inflammation, or edema, consequent to tissue damage, and breaking up the cycle which is thought provoke tonic muscle spasm or pain. Physical therapy is the most importance thechniques to reduce delayed onset muscle soreness. Physical therapy on delayed onset muscle soreness includes massage, exercise, therapeutic ultrasound, TENS, stretching and cryotherapy, this investigation should encourage physical therapists to experiment further with various techniques to reduce delayed onset muscle soreness.

Modern acupuncture-like stimulation methods: a literature review

  • Acupuncture therapy has been proved to be effective for diverse diseases, symptoms, and conditions in numerous clinical trials. The growing popularity of acupuncture therapy has triggered the development of modern acupuncture-like stimulation devices (ASDs), which are equivalent or superior to manual acupuncture with respect to safety, decreased risk of infection, and facilitation of clinical trials. Here, we aim to summarize the research on modern ASDs, with a focus on featured devices undergoing active research and their effectiveness and target symptoms, along with annual publication rates. We searched the popular electronic databases Medline, PubMed, the Cochrane Library, and Web of Science, and analyzed English-language studies on humans. Thereby, a total of 728 studies were identified, of which 195 studies met our inclusion criteria. Electrical stimulators were found to be the earliest and most widely studied devices (133 articles), followed by laser (44 articles), magnetic (16 articles), and ultrasound (2 articles) stimulators. A total of 114 studies used randomized controlled trials, and 109 studies reported therapeutic benefits. The majority of the studies (32%) focused on analgesia and pain-relief effects, followed by effects on brain activity (16%). All types of the reviewed ASDs were associated with increasing annual publication trends; specifically, the annual growth in publications regarding noninvasive stimulation methods was more rapid than that regarding invasive methods. Based on this observation, we anticipate that the noninvasive or minimally invasive ASDs will become more popular in acupuncture therapy.

Ultrasonic Targeting of NK Cell in Vessel Bifurcation for Immunotherapy: Simulation and Experimental Validation

  • Natural killer (NK) cells play a crucial role in combating infections and tumors. However, their therapeutic application in solid tumors is hindered by challenges, such as limited lifespan, tumor penetration, and delivery precision. Our research introduces a novel ultrasonic actuation technique to navigate NK cells more effectively in the vascular system, particularly at vessel bifurcations where targeted delivery is most problematic. We use a hemispherical ultrasonic transducer array that generates phase-modulated traveling waves, focusing on an ultrasound beam to steer NK cells using blood-flow dynamics and a focused acoustic field. This method enables the precise obstruction of non-target vessels and efficiently directs NK cells toward the tumor site. The simulation results offer insights into the behavior of NK cells under various conditions of cell size, radiation pressure, and fluid velocity, which inform the optimization of their trajectories and increase targeting efficiency. The experimental results demonstrate the feasibility of this ultrasonic approach for enhancing NK cell targeting, suggesting a potential leap forward in solid tumor immunotherapy. This study represents a significant step in NK cell therapeutic strategies, offering a viable solution to the existing limitations and promising enhancement of the efficacy of cancer treatments.

An Optimization Method of Measuring Heart Position in Dynamic Myocardial Perfusion SPECT with a CZT-based camera (동적 심근관류 SPECT에서 심장의 위치 측정방법에 대한 고찰)

  • Purpose Cadmium-zinc-telluride (CZT) camera with semiconductor detector is capable of dynamic myocardial perfusion SPECT for coronary flow reserve (CFR). Image acquisition with the heart positioned within 2 cm in the center of the quality field of view (QFOV) is recommended because the CZT detector based on focused multi-pinhole collimators and is stationary gantry without rotation. The aim of this study was to investigate the optimal method for measuring position of the heart within the center of the QFOV when performing dynamic myocardial perfusion SPECT with the Discovery NM 530c camera. Materials and Methods From June to September 2018, 45 patients were subject to dynamic myocardial perfusion SPECT with D530c. For accurate heart positioning, the patient's heart was scanned with a mobile ultrasound and marked at the top of the probe where the mitral valve (MV) was visible in the parasternal long-axis view (PLAX). And, the marked point on the patient's body matched with the reference point indicated CZT detector in dynamic stress. The heart was positioned to be in the center of the QFOV in rest. The coordinates of dynamic stress and rest were compared statistically. Results The coordinates of the dynamic stress using mobile ultrasound and those taken of the rest were recorded for comparative analysis with regard to the position of the couch and analyzed. There were no statistically significant differences in the coordinates of Table in & out, Table up & down, and Detector in & out (P > 0.05). The difference in distance between the 2 groups was measured at $0.25{\pm}1.00$, $0.24{\pm}0.96$ and $0.25{\pm}0.82cm$ respectively, with no difference greater than 2 cm in all categories. Conclusion The position of the heart taken using mobile ultrasound did not differ significantly from that of the center of the QFOV. Therefore, The use of mobile ultrasound in dynamic stress will help to select the correct position of the heart, which will be effective in clinical diagnosis by minimizing the image quality improvement and the patient's exposure to radiation.

Finite element analysis for acoustic and temperature characteristics of a piezoelectric HIFU transducer at 10 MHz (10 MHz용 압전 HIFU 트랜스듀서의 음향 및 온도 특성에 대한 유한요소해석)

  • A high intensity focuses ultrasound (HIFU) is one of the emerging technologies in the biomedical field. The piezoelectric HIFU transducer is a device that utilizes the thermal energy generated by high ultrasound energy. Recently an operating frequency of the HIFU transducer is to expand above a 7 MHz. In this study, the acoustic pressures and temperature distributions in the tissue that generated by the HIFU transducer at 10 MHz were calculated with the finite element method. In addition, the pressure focusing characteristics of the device were analyzed. The geometrical variables are the piezomaterial thickness, lens shape, water height, and film thickness. The results shown that the acoustic pressure increased and saturated gradually when the height/radius (HL/RL) ratio of the lens increased. Moreover, the focal area was gradually decreases with HL/RL ratio of the lens. In case of the optimized HIFU transducer, the maximum pressure and temperature were analyzed about 19 MPa and 65℃ respectively. And the -3 dB focused distances in the axial and lateral direction are around 2.3 mm and 0.23 mm respectively.

A Study on the Effectiveness of Hand Sanitizer compared to Conventional Ultrasound Gel during Ultrasound Examination (초음파검사 시 기존 초음파젤과 비교한 손소독제의 유용성 연구)

  • This study focused on hand sanitizer as a medium that can replace ultrasonic gel, which is vulnerable to contamination by bacteria that reside on the hand. Hand sanitizer produces a strong sterilization effect from germs resident on the hands through different sterilization principles depending on the ingredients. Select products of gel type, cream type, and foam type, except for liquid type with low viscosity, and ultrasonically apply one 62% ethanol gel type and one cream type, one benzalkonium chloride 0.066% cream type and one foam type, respectively. Using ATS-539 as a medium, image evaluation was performed on the axial and lateral resolution and penetration depth, and the presence or absence of an air layer between the probe and the phantom. As a result, in the evaluation of the axial and lateral resolution and the depth of penetration, all four experimental groups met the evaluation criteria. However, in the case of the foam type, although it was suitable for the evaluation criteria of resolution and penetration depth, dark shadows appeared on both sides except for the center of observation during image evaluation. Through this experiment, it was possible to confirm the possibility that the remaining three types of hand sanitizers except the foam type could replace the ultrasonic gel.

  • In the accompanying paper, we proposed a real. time volumetric imaging method using a cross array based on receive dynamic focusing and synthetic aperture focusing along lateral and elevational directions, respetively. But synthetic aperture methods using spherical waves are subject to beam spreading with increasing depth due to the wave diffraction phenomenon. Moreover, since the proposed method uses only one element for each transmission, it has a limited transmit power. To overcome these limitations, we propose a new real. time volumetric imaging method using cross arrays based on synthetic aperture technique with linear wave fronts. In the proposed method, linear wave fronts having different angles on the horizontal plane is transmitted successively from all transmit array elements. On receive, by employing the conventional dynamic focusing and synthetic aperture methods along lateral and elevational directions, respectively, ultrasound waves can be focused effectively at all imaging points. Mathematical analysis and computer simulation results show that the proposed method can provide uniform elevational resolution over a large depth of field. Especially, since the new method can construct a volume image with a limited number of transmit receive events using a full transmit aperture, it is suitable for real-time 3D imaging with high transmit power and volume rate.

Effect of oil particle size on dispersion stability in oil in water emulsion (Oil in Water 에멀전에서 오일 입자 크기가 분산 안정성에 미치는 영향)

  • In this paper, we proposed an emulsification method without using an emulsifier and investigated the effects of particle size distribution in fluids on dispersion stability. Surfactant-free oil in water emulsion was prepared with 1 % (w/w) of olive oil by using high speed agitation, high pressure and ultrasonic dispersion methods. The particle size, microscopic observation, and dispersion stability of each sample were evaluated and dispersion stability according to various dispersion methods was compared. As a result, the emulsion dispersed by the ultrasonic dispersion method showed the smallest particle size and uniform distribution of $0.07{\sim} 0.3{\mu}m$ and was the most stable in a 7 days stability evaluation. In the above experiment, four olive oil emulsions having different particle sizes were prepared using ultrasonic dispersion technology that was capable of producing stable emulsions. The dispersion stability of each samples with oil droplet sizes of (A) 0.1 to $0.5{\mu}m$, (B) 0.3 to $4{\mu}m$, (C) 1 to $10.5{\mu}m$ and (D) 2 to $120{\mu}m$, was observed for 7 days, and the relationship between the stability and performance was studied. Emulsion (A) with particle size less than $0.5{\mu}m$ displayed the dispersion stability showing below 5 % change in a 7 days stability evaluation. In the case of (B), (C), and (D) that had larger particle than $0.5{\mu}m$, the changes of dispersion stability were 10 %, 13 % and 35 % respectively. From these results, it was proved that dispersion stability of emulsion with uniform particle size of $0.5{\mu}m$ or less was confirmed to be very stable.

Clinical characteristics of obese boys and girls in a high school: focused on abdominal fat indices, fatty liver and carotid intima-media thickness

  • Purpose: Our study aimed to evaluated sex differences in clinical features of obese high school students. Methods: One hundred three obese high school students (body mass index [BMI]${\geq}$85th percentile) and 51 control students (BMI<85th percentile) were enrolled in this study. Anthropometric measurements were performed. Fasting serum glucose, insulin, aspartate aminotransferase, alanine aminotransferase, total cholesterol, triglyceride, low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and high-sensitive C-reactive protein were measured. Abdominal fat thickness, degree of fatty liver, and carotid intima-media thickness were measured by ultrasound. Results: In control and obese groups, waist circumference was significantly longer in boys but body fat mass was Significantly higher in girls. In the control group, total cholesterol and LDL-C were higher in girls. In the obese group, however, aspartate aminotransferase, alanine aminotransferase and triglyceride were higher and HDL-C was lower in boys. Preperitoneal fat thickness was significantly higher in obese girls. In obese group, the degree of fatty liver was significantly higher in boys. Carotid intima-media thickness was not significantly different between boys and girls. Conclusion: Obese adolescents had distinguishable sex differences in body measurements, metabolic abnormalities, abdominal fat thickness and fatty liver. We can infer that these characteristics may extend into adult obesity.
