The Comparison of the Effects of Joint Mobilization, Incline Board and PNF stretching to Increase the Dorsiflexion of the Ankle Joint on Ankle Dosiflexion and the Muscle Tone of the Plantar Flexor the Ankle in Subjects with Stroke (발등 굽힘 관절가동범위를 증가시키기 위한 관절가동술과 경사대, PNF 스트레칭이 뇌졸중 환자의 발등 굽힘 및 발바닥 굽힘근의 근 긴장도에 미치는 효과 비교)

  • Background: This study was conducted to compare the effects of an incline board, proprioceptive neuromuscular facilitation (PNF) stretching, and joint mobilization on the dorsiflexion of the ankle joint and muscle tone of the plantar flexor and ankle in stroke patients. Methods: Forty-five stroke subjects were randomly divided into three groups: each JMG (Joint Mobilization Group), IBG (Inclined Board Group) and PSG (PNF Stretching Group). The experiment was performed for eight weeks between August 1 and October 31, 2018. Results: The dorsiflexion of the ankle joint was significant between the groups and by period (p<.05). JMG had a significant change by period (p<.05). Differences between groups by period increased after 8 weeks in the PSG and the JMG (p>.05). The gastrocnemius lateral muscle tone was significant between the groups (p<.05), and medial muscle tone was interacted between groups and time (p<.05). Conclusion: An effective intervention method to increase the dorsiflexion of the ankle joint and decrease the muscle tone when performing PNF stretching, and joint mobilization in stroke patients was confirmed. Future studies are recommended to variety research of PNF stretching and joint mobilization.

Effects of Muscle Fatigue of Upper Extremity Flexor on Joint Sense (상지 굴곡근피로가 주관절의 위치 감각에 미치는 영향)

  • Background and Purpose : This study was to analysis effects of muscle fatigue on error of elbow joint sense. Methods : A total 19 healthy student(men = 10, women = 9) who did't have any problem of musculoskeletal system in upper extremities participated this study. we divided two groups into young group(n = 10, $19.67{\pm}.5$) and old group(n = 9, $28.56{\pm}1.5$). In order to evoke muscle fatigue of elbow flexor, we used Biodex, and participations performed concentric contraction of elbow flexor 150 numbers as well as we measured error of joint sense using by Biodex. We collected data just after, 30min, 2hour, 24hour after evoked muscle fatigue, and we finanlly acquired average value of three times measured joint sense of elbow joint. And we calculated value of percentage of error of joint sense. We analyzed collected data by repeated ANOVA, ANOVA using by SPSS ver.12.0 program. Result : This study showed that there was no significantly effects between groups and within groups, we could see that there was significantly difference among duration by each group of age, and sex(p<.05). Conclusion : The error of joint position sense presented highest value just period after evoked muscle fatigue compared after 30 min, 2 hours, 24 hours, and we can't find out interaction between duration and age and sex.

Analysis of Onset Time of Muscle Contractions of the Biceps Brachii in Supination According to the Presence or Absence of Elbow Flexor Activity (팔꿉관절 굽힘 작용 배제 유무에 따른 뒤침 동작시 위팔두갈래근의 근수축 개시시간 분석)

  • Purpose: This study aims to examine the functional difference in the long and short heads of the biceps brachii by investigating the onset time of muscle contractions in the biceps brachii in the supination motion according to whether the flexor of the elbow joint is excluded. Methods: This study was conducted with 21 healthy men aged in their 20s. While performing forearm pronation at an elbow flexion angle of 90 degrees, the onset time of muscle contractions in the long and short heads of the biceps brachii was measured and compared in a posture where the humerus is placed on a table and the posture is lifted against gravity. Using an independent samples t-test, the difference in the onset time of muscle contractions in the long and short heads of the biceps brachii was analyzed. Results: The onset time of the long head was shorter if the flexor activity of the elbow joint was excluded, while that of the short head of the biceps brachii was shorter if it was not excluded. Conclusion: It is noted that the long head of the biceps brachii mainly functions as a supinator muscle, while the short head of the biceps brachii plays a role in stabilizing and maintaining flexion of the elbow joint.

Effects of a Wrist Extension Splint on Muscle Power and Activities of the Forearm Muscles: Comparison of Day Versus Nighttime Wear Instructions

  • The objective of this study was to compare the differences on the activity and power of the wrist flexors and extensors in subjects before the use of a wrist extension splint, after nighttime wearing of the splint, and after daytime wearing of the splint. Ten healthy male and ten healthy female students (mean: $22.4{\pm}1.2$ years old) volunteered to wear custom-made wrist splints either during the night or during the day, The hand force of the wrist flexor and extensor, and grip force were measured by PowerTrack II and Dynatron, respectively. At the same time, the activities of the wrist flexor and extensor were recorded by' surface electromyography. The maximal hand force and motor unit recruitment of the flexor carpi ulnatis (FCU) increased significantly (p<.05) when tile subjects wore the wrist splints during the daytime, but the maximal hand power of the FCU decreased with nighttime use of the splints. The maximal hand power and motor unit recruitment of the extensor carpi radialis (ECR) and the ECR/FCU ratio decreased both during nighttime and daytime use. The decrement of the ECR/FCU ratio was significant (p<.05). Wearing a wrist extension splint during nighttime led to the maintenance of a lengthened position of the wrist flexor, resulting in the wrist flexor becoming weak. Wearing a wrist extension splint during the day induced the wrist flexors to be greater. In healthy people, the imbalance between the wrist flexors and extensors may be caused by the use of a wrist extension splint. This study indicates that therapists have to consider whether a splint will be effective, as well as the wearing time, when prescribing splints to people with problems of the musculoskeletal system.

Reliability of the Modified Modified Ashworth Scale for the Muscle Tone of Poststroke Patients (뇌졸중 환자의 근긴장도 평가를 위한 개정된 개정된 Ashworth 척도의 신뢰도)

  • Purpose : The clinical scale to assess spasticity of muscle was wildly used the modified Ashworth scale (MAS). But reliability of the MAS has been controverted for ambiguity among the grades. The purpose of this study was to establish the inter-rater reliability of the modified MAS (MMAS) translated into Korean in stroke patients. Methods : Twenty-five patients (sixteen men and nine women) with hemiplegia (ten right and fifteen left) were measured by two raters who were physical therapist in the rehabilitation hospital. The raters assessed spasticity of shoulder adductor, elbow flexor, wrist flexor, hip adductor, knee extensor, and ankle plantar flexor in the same patients according to ratings criteria of the MAS and the MMAS. Results : For the inter-rater reliability of the MAS, two raters agreed on 57.3% and the Kappa values were moderate ($\kappa$=0.41) between two rater. The inter-rater reliability of the MAS was fair for the wrist flexor and the hip adductor and moderate for the other muscles. The intra-rater reliability was good for the shoulder adductor and the knee extensor and moderate for the other muscles. For the inter-rater reliability of the MMAS, two raters agreed on 84.7% and the Kappa values were good ($\kappa$=0.78) between two rater. The inter-rater reliability of the MMAS was moderate for the hip adductor, and good for the shoulder adductor and the wrist flexor, and very good for the other muscles. The intra-rater reliability was good for the wrist flexor and the hip adductor and very good for the other muscles. Conclusion : This study suggests that the MMAS translated into Korean is reliable test scale for the spasticity with stroke patients in the clinical field.

  • We have conclusions after the study of muscular system about small intestine channel of hand taiyang muscle. Judging from many studies of interrelation between Meridian muscle and muscle, it is considered that Meridian muscle theory has some similarities with modern anatomical muscular system. It is considered that Small intestine channel of hand taiyang muscle contains Flexor digitorum profundus muscle, Extensor digiti minimi muscle, Abductor digiti minimi muscle, Extensor carpi ulnaris muscle, Flexor carpi ulnaris muscle, Triceps brachii muscle, Infraspinatus muscle, Levator scapulae muscle, Sternocleidomastoid muscle, Masseter muscle, Temporalis muscle. The symptoms of small intestine channel of hand taiyang muscle is similar to referred pain of modern Myofascial Pain Syndrome, and the medical treatment of "I Tong Wi Su(以痛爲輸)" is also similar to that of Myofascial Pain Syndrome. Small intestine channel of hand taiyang muscle is one of the three yang channels of hand muscle, and it has unity in extension of upper limb and trunk in the movement. And it is thought that weakness of small intestine channel of hand taiyang muscle is related with muscular system causing Round Shoulder and Head Forward Position.

Neuromuscular difference between normal subjects and low-back pain patients: Neural excitation measured by dynamic electromyography (정상인과 요통환자의 생체역학적 차이에 관한 연구:신경근육계의 동적 근전도 반응형태를 중심으로)

  • Neuromuscular difference between normal subjects and low-back pain patients has been identified in terms of neural excitation signal measured by Electromyography (EMG) under the dynamic flexion/extension trunk motion. Ten healthy subjects and ten low-back pain patients were recruited for this study. New parameters and normalization technique were introduced to quantify the muscle excitation pattern among the flexor-extensor pairs of muscles : rectus abdominis (RA)-erector spinae (ES at L1 and L5 level), external oblique (EO)-internal oblique (IO), rectus femoris (quadricep : QUD)-biceps femoris( hamstring : HAM), and tibialis anterior (TA)-gastrocnemius (GAS). Results indicated that the temporal EMG pattern such as peak timing difference between the hip flexor (QUD) and extensor (HAM) and the duration of coexcitation between ES at L5 and RA muscle pairs showed a statistically significant difference between normal subjects and low-back pain patients. Improtantly, this study presented a new technique to identify the dynamic muscle excitation pattern that canb be least affected by EMG-length-velocity relationship. Further study can performed to validate this method for clinical application to quantitatively identify the low-back pain patients in the future.

  • In this paper, we attempted to analyze the contraction patterns of elbow flexor muscle during isometric, concentric and eccentric contraction. The analysis parameters are consisted of Sequency domain parameters (mean frequency, median frequency, skewness, kurtosis) and time domain parameters (zero crossing, positive maxima, integrated EMG). As a results, the analysis parameters have specific trends for muscles, muscle contraction patterns, muscle contraction angles. Especially, at the time domain analysis, IEMG is a dominant parameter for analysis of activation patterns, and the skewness, kurtosis are useful parameters for functional recognition.

The Effects of Knee Extensor, Flexor Muscle Strength and Joint Position Sense in Squat Exercise on Variety Surface (다양한 지지면에서의 스쿼트 운동이 무릎관절 근력 및 위치감각 향상에 미치는 영향)

  • Purpose : The purpose of this study was to compare the Effects of Knee Extensor, Flexor Muscle Strength and Joint Position Sense in Squat Exercise on Variety Surface. Method : Subjuects were consisit of 30 male and female who had non disorder knee joint. we had devided 3 group(control group, balance pad group, and togu group) and we measured that knee joint $15^{\circ}/45^{\circ}$proprioception(position sense) and Peak Torque of knee extensor, flexor muslce at $60^{\circ}/sec$, $180^{\circ}/sec$ angular velocity. Result : First, knee joint $15^{\circ}$proprioception(position sense) of balance pad and togu group was significantly different after exercise(p<.05), but only balance pad group was significantly different at knee joint $45^{\circ}$. Second, knee extensor Peak Torque of balance pad and togu group was significantly different at $60^{\circ}/sec$, $180^{\circ}/sec$ angular velocity(p<.05). Third, knee flexor Peak Torque of balance pad and togu group was significantly different at $60^{\circ}/sec$, $180^{\circ}/sec$ angular velocity(p<.05). Conclusion : Squat exercises on the balance pad and togu were affects knee joint proprioception and muscle strength improvement. These results suggest that squat exercise on the unstable surface is effective for prevention of knee joint injury and functional activity.

Comparison of the effects of dynamic taping and kinesio taping on endurance and fatigue of plantar flexor (다이나믹 테이핑과 키네시오 테이핑 적용에 따른 발바닥 굽힘근의 지구력과 피로도에 미치는 효과 비교)

  • Background: The purpose of this study is to investigate the effect of plantar flexor on muscle fatigue and endurance when two different sport tapes are applied. Design: Cross-sectional design. Methods: Fifty-one healthy adults were randomly assigned to dynamic taping group (n=17), kinesio taping group (n=17), and control group (n=17). The dynamic taping group used the dynamic tape. The kinesio tape group used the kinesio tape, and the control group doesn't used tape. Every group performed heel rise test and sEMG for every during pre-test and post-test. To evaluate plantar flexor endurance, the number of times was measured during the heel rise test. Results: The number of heel rises increased significantly in the post-test in dynamic taping group compared to the pre-test, and In comparison between groups, the number of heel rises significantly increased in dynamic taping group compared to kinesio taping group and control group. PEF value was significantly increased in the post-test compared to the pre-test in dynamic taping group, and there was a significant difference between the three groups according to the taping application. Conclusion: The results of this study confirmed that dynamic taping was effective in musclular fatigue and endurance on plantar flexor in healthy adults. Based on these results, it is suggested that the application of dynamic tape can be suggested as one of the intervention methods for muscle endurance and muscle fatigue.
