PURPOSE: The purpose of this study was to examine changes in the neck flexion angle according to posture while using a smartphone and the duration of smartphone usage. METHODS: The subjects in this study were 16 healthy young students in their 20s. The subjects wore a cervical range of motion instrument. They were instructed to use a smartphone while standing, sitting on a chair, and sitting on the floor. In all postures, they could use the smartphone and use their arms freely while keeping their back connected to the wall or the back of the chair. When sitting on the floor, they assumed a cross-legged position. The neck flexion angle was measured at zero, three, six, and nine minutes for each posture. RESULTS: Neck flexion is affected by the posture while using a smartphone (p<.05). Neck flexion in the standing position is larger than that in the sitting on the floor position. Neck flexion was affected by smartphone usage duration (p<.05). In general, as usage time increases, the neck flexion angle increases as well. CONCLUSION: This study suggests that using smartphone in the standing position and for a short period of time is a method to reduce the neck flexion angle.
Background: Flexion of cervical and lumbar joints is required when viewing a smartphone screen. Thus, these joints are overused, together with the surrounding joints and muscles. Long-term use of smartphones will cause changes in cervical and lumbar posture. The effect on forward head posture will vary, depending on the angle of cervical flexion start position in relation to the smartphone. This study investigated how forward head posture changes over time when using a smartphone at 20° and 40° cervical flexion start positions. Design: Cross-sectional study. Methods: Twenty-five subjects with a forward head posture angle of 35° or less participated in the study. A Forward Head Posture app on the participants' smartphones measured forward head posture 5 and 10 minutes after watching videos on their smartphones. Cervical range of motion was used to set a smartphone watching start posture of 20° and 40° of cervical flexion. Results: There was no significant difference in forward head posture, irrespective of cervical flexion start position, but the angle of forward head posture increased more at cervical flexion of 40° than at cervical flexion of 20°. There was no significant difference in what according to smartphone video viewing times, but the angle of forward head posture increased over time. Conclusion: An increase in forward head posture over time with smartphone usage poses a potential risk of neck and shoulder pain. Therefore, smartphone users should avoid prolonged screen time.

Purpose: The purpose of this study was to investigate the muscle contraction onset time characteristics of the gluteus maximus, semitendinosus, and biceps femoris muscles at different knee flexion angles in individuals with shortened or over-lengthened hamstrings performing prone hip extension. Methods: Twenty-six participants were divided into a hamstring shortened group (n = 12) and hamstring lengthened group (n = 14). Wireless surface electromyography was used to verify the muscle onset time of the gluteus maximus, semitendinosus, and biceps femoris when performing prone hip extension at different knee flexion angles. Results: There were significant differences in the muscle onset times of the semitendinosus and biceps femoris between the hamstring shortened group and hamstring lengthened group (p < 0.05). In addition, there was a significant difference in the muscle contraction onset times among of the gluteus maximus, semitendinosus, and biceps femoris muscles when performing prone hip extension at a knee flexion of 90° in the hamstring shortened group (p < 0.05) and a knee flexion angle of 0° in the hamstring lengthened group (p < 0.05). Conclusion: In all groups, there was no effect on the onset time of the gluteus maximus muscle according based on the knee angle. In addition, the knee flexion angles affected the onset time of the muscle contraction of the gluteus maximus muscle in the hamstring shortened group and hamstring lengthened group with an abnormal length of the hamstring muscle.
Background: Squatting is one of the best functional exercises to strengthen the quadriceps muscle in independent gait and activities of daily living. Although the use of a gym ball during squat exercise is the most common way of strengthening the vastus medialis oblique (VMO) muscle, published data on this subject are limited. Objects: The purpose of this study was to compare the sequential muscle activation of the VMO and vastus lateralis (VL) muscles during squat exercise using a gym ball at different knee flexion angles. Methods: Forty healthy adults were randomly divided into experimental (squat exercise using a gym ball) and control (squat exercise alone) groups, in which squats were performed at $45^{\circ}$ and $90^{\circ}$ knee flexion. Electromyographic (EMG) activity data were collected over 10 seconds under the 2 angles of knee flexion ($45^{\circ}$ and $90^{\circ}$). Results: There was significant group and time interaction effect for VMO and VL muscle activation at $45^{\circ}$ knee flexion. This was similarly demonstrated at $90^{\circ}$ knee flexion. No significant group main effect and time main effect for VMO and VL muscle activation were noted at $45^{\circ}$ knee flexion, respectively. In contrast, there was significant group main effect and time main effect for VMO and VL muscle activation at $90^{\circ}$ knee flexion. These significant differences were demonstrated through two-way analysis of variance over repeated measurements, suggesting that the EMG activity of the VMO muscle during squatting with a gym ball showed remarkable improvement compared to that of the VL muscle. Conclusion: This research suggests that squat exercise using a gym ball may be more beneficial in improving the activity of VMO than of the VL muscle at both $45^{\circ}$ and $90^{\circ}$ of knee flexion, respectively. We highly recommend squat exercises with a gym ball for selective strengthening of the VMO muscle in knee rehabilitation.

Purpose : The purpose of this study was to examine if there is any correlation between pelvic tilt angle and trunk motion and trunk extensor during trunk forward flexion and to measure trunk motion, onset time of trunk motion, and onset time of trunk extensor activation. Methods : The subjects of this study were 42 healthy adults. The subjects had no back pain due to neurological disease and no experience of back surgery. After pelvic tilt angle was measured, each trunk forward flexion was performed three times. Trunk motion and onset time of trunk motion were measured using Myomotion. Four sensors were used, with one located at the upper thoracic (below $C_7$), the lower thoracic ($T_{12}-L_1$), the sacrum ($S_1$), and at the center of the anterior femur. Onset time of trunk extensors (spinalis, longissimus, gluteus medius, gluteus maximus, biceps femoris, and gastrocnemius) activation was measured using a wireless surface EMG. The EMG amplitude was normalized by using the reference voluntary contraction (RVC). The statistical significance of the results were evaluated using Pearson's correlation test. Results : The correlation between pelvic tilt angle and lumbar motion, onset time of pelvis motion, and onset time of gluteus medius activation was statistically significant in a positive direction (p<.05). The correlation between pelvic tilt angle with pelvis motion, onset time of lumbar motion, and onset time of longissimus activation showed a statistically significant negative correlation (p<.05). Conclusion : The study results provide a significant contribution to our understanding of the lumbar load at the initial stage of trunk flexion. Therefore, it may be possible to provide basic data for evaluation and treatment, such as orthodontic treatment for alignment of the spine and back pain. In addition, it is necessary to focus on normal exercise pattern reeducation as well as pelvic correction during exercise in daily life or in industrial fields.
Ganglion in the anterior cruciate ligament is quite rare finding at the time of knee arthroscopy. We experienced a 58 years old female pateint with a ganglion in the anterior cruciate ligamentwho complained gradual development of flexion contracture of left knee joint and treated with arthroscopic excision of ganglion resulted in freedom from flexion contracture of her knee joint.

This study aimed to empirically investigate perceived discomfort depending upon external load, upper limb postures and their holding time. Discomfort was obtained through an experiment, in which external load, wrist flexion/extension, elbow flexion, shoulder flexion and adduction/abduction were used as experimental variables. The subjects were instructed to hold given postures for 60s and to rate their subjective discomfort scores at 5s, 20s, 40s and 60s by using the free modulus method of magnitude estimation. The results showed that while only external load and elbow flexion were statistically significant at the holding time of 5s at ${\alpha}=0.05$ or 0.10, external load and upper limb postures excluding shoulder adduction/abduction significantly affected discomfort ratings at 20s, 40s and 60s at ${\alpha}=0.01$ or 0.05. Discomfort scores were also significantly different between four posture holding times at ${\alpha}=0.01$. The effects of external load and holding time were much larger than those of upper limb postures. Based on the results of this study, it is recommended that external load and holding time as well as working postures betaken into consideration to precisely quantify postural load in industry.
Objectives : This study is to evaluate the effect of flexion-distraction technique by measuring surface EMG in low back pain groups. Methods : 5 low back pain patients' constact time, power, fatigue, recovery of muscle were measured before and after flexion-distraction technique. Results : The asymmetry ratio of surface EMG in low back pain groups were Increased, but decreased after flexion-distraction technique. Conclusions : Surface EMG might be used for evaluating the effect of flexion-distraction technique.
Purpose : The aim of this study was to evaluate the efficacy of arterialized venous flap in finger flexion contracture correction. Materials and methods : From 2002 to 2004, we have performed 10 arteriaized venous flap for treatment of severe flexion contracture in digit. The duration of flexion contracture was from 1 year to 50 years. The cause of contracture were bum scar(7 cases), postoperative contracture(2 cases) and other(l case). We evaluated the survival of flap, flap size, recovery of flexion contracture and subjective satisfaction. Results : All arterialized venous flap survived. The marginal minimal skin necrosis developed in 2 cases. The flap size was average $5.2{\times}3.5cm$. The recovery of flexion contracture was 87% compared with non affected side. 9 patients(90%) satisfied the results of operation. Conclusion : Arterialized venous flap is one of the useful procedure in treatment of finger flexion contracture because it has many advantages such as thin and good quality, variable length of pedicle, preservation of major vascular pedicle, less operation time and in addition possibility of various modifications.
This study examined whether there is a transfer effect to other joint angles and the angular specificity of muscle fatigue after 6 weeks of isometric training of the vastus medialis. Twenty subjects were randomly assigned to 30° and 90° knee flexion groups and were trained at 80% maximal voluntary isometric contraction(MVIC) three times a week for 6 weeks. The pre-and post-training values of the 80% holding time(endurance time) of MVIC, the Fatigue Index(FI), and the MVIC at 30°, 60°, and 90° were compared. After isometric training for 6 weeks, in the 30° knee flexion group, FI decreased significantly(p<0.05) only at 30°, which was the training angle; there was no change at other angles. By contrast, in the 90° knee flexion group, FI decreased significantly(p<0.05) at both the trained angle and at the other angles, indicating a transfer effect of training. MVIC did not increase significantly(p<0.05) at any trained angle in either the 30° or 90° knee flexion groups after 6 weeks of isometric training, neither did the 80% holding time of MVIC differ significantly compared with pre-training in either group. These findings suggest that training at 90° of knee flexion is more effective than training at 30° of knee flexion for obtaining a training transfer effect on muscle fatigue in the vastus medialis.
