Five-Year Survival and Median Survival Time of Nasopharyngeal Carcinoma in Hospital Universiti Sains Malaysia

  • Background: Nasopharyngeal carcinoma (NPC) is the fourth most common cancer in Malaysia. The objective of this study was to determine the five-year survival rate and median survival time of NPC patients in Hospital Universiti Sains Malaysia (USM). Methods: One hundred and thirty four NPC cases confirmed by histopathology in Hospital USM between $1^{st}$ January 1998 and $31^{st}$ December 2007 that fulfilled the inclusion and exclusion criteria were retrospectively reviewed. Survival time of NPC patients were estimated by Kaplan-Meier survival analysis. Log-rank tests were performed to compare survival of cases among presenting symptoms, WHO type, TNM classification and treatment modalities. Results: The overall five-year survival rate of NPC patients was 38.0% (95% confidence interval (CI): 29.1, 46.9). The overall median survival time of NPC patients was 31.30 months (95%CI: 23.76, 38.84). The significant factors that altered the survival rate and time were age (p=0.041), cranial nerve involvement (p=0.012), stage (p=0.002), metastases (p=0.008) and treatment (p<0.001). Conclusion: The median survival of NPC patients is significantly longer for age ${\leq}50$ years, no cranial nerve involvement, and early stage and is dependent on treatment modalities.

원발성 폐암 수술의 장기성적 (Long-term Results of Surgical Treatment for Primary Lung Cancer)

  • From Nov. 1980 to Jun. 1987, 270 primary lung cancer patients were operated on at the department of Thoracic & Cardiovascular Surgery, Seoul National University Hospital. There Were 223 males & 47 females with 55.5 years of mean age. There were 151 [55.9%] squamous cell ca., 43 [18.8%] adenoca., 8 [3.5%] undiff. large cell, 9 [3.9%] undiff. small cell ca. & 18 [7.9%] mixed type, and also composed of 65 [28.3%] stage I, 31 [13.5%] stage II and 133 [58.1%] of stage III cases. They received 78 [34.1%] lobectomies, 62 [27.1%] pneumonectomies and 60 [26.2%] exploratory thoracotomies with 70% resectability rate. The five year actuarial survival rate of all patients was 37%. According to TNM stage, five year survival rate of the patients in stage I was 71%, those of stage II was 29% and of stage III 21%. According to histological cell type, five year actuarial survival rate of the squamous cell ca. was 35%, of adenoca. 49%, of undiff. large cell. 22%, 2 year survival rate of undiff. small cell was 31% and 3 year survival rate of mixed type was 47%. Hospital death was 2 case with a 1.3% early postop. mortality rate.

한국인에서 구강 편평세포암종의 5년 생존율 (OVERALL FIVE-YEAR SURVIVAL RATE IN SQUAMOUS CELL CARCINOMA OF ORAL CAVITY)

  • The purpose of this epidemiologic study was to provide clinically useful information on the fundamentals for both the diagnosis and treatment planning of oral squamous cell carcinoma, which comprises $80{\sim}90%$ of all oral cancers. One hundred and forty two patients diagnosed with oral squamous cell carcinoma were selected from a total of 220 patients with oral malignancies. The patients' medical and follow-up records were reviewed and their survival was traced. The highest occurrence rate was observed in those aged between 60 and 69 years. The tongue was the most common primary site(31.7%) for oral squamous cell carcinoma. The survival rate was calculated using the Kaplan-Meier method. The overall five-year survival rate of oral squamous cell carcinoma patients was 66.90%. The 5-year survival rate according to stage was 85.82% for stage I, and 49.98% for stage IV. The five-year survival rate according to the originating site was 91.67% for the retromolar trigone, 75.30% for the tongue, and 62.41% for the maxillary gingiva. In terms of cell differentiation, the majority(58.5%) was the well-differentiated type, which had a 5-year survival rate of 70.62%.

갑상선암의 예후적 인자와 생존율 (The Prognostic Factors and Survival Rate in Thyroid Carcinoma)

  • A multivariate analysis of the prognostic factors and five year survival rate was carried out on a series of 72 thyroid cancers treated surgically from 1980 to 1987 and followed for 1 to 11 years. The prognosis of the disease was significantly influenced by age at diagnosis, extracapsular invasion, angioinvasion and pathologic type, but the disease was not influenced by sex, lymph node metastasis, and 'risk' category. The overall five year survival rate was 89.7%. Six patients were dead of tumor after surviving for six months to two and half years, and the cause of death was local recurrence in three, lung metastasis in two and bone metastasis in one patient. Five year survival rate in age above fifty, presence of capsule and angioinvasion, follicular carcinoma, and extrathyroidal lesion was significantly shorter than that of patients with age below fifty, absence of capsule and angioinvasion, papillary carcinoma, and intrathyroidal lesion. Patients at low risk or with small carcinomas had long survival over 5 years with only lobectomy. Lymph node dissection was done with a limited type in no jugular metastasis, radical neck disscetion was performed only therapeutically in proved jugular node metastasis. Thyroid hormone was administered for the period of 3 to 5 years to suppress endogenous TSH production.

Survival Rates for Breast Cancer in Iranian Patients: a Meta-Analysis

  • Background: Breast cancer is the most common cancer among Iranian women. Since development of the disease in Iranian women occurs relatively early, the survival rate matters considerably. In different countries, survival of breast cancer patients varies considerably. Therefore, the one-year, three-year, five-year, and ten-year survival rates for breast cancer in Iran were assessed using a meta-analysis. Materials and Methods: This systematic review and meta-analysis was based on valid Iranian sources including SID, MAGIRAN and IRANMEDEX, along with reliable English databases, namely, PUBMED and SCOPUS. In domestic databases, a search was conducted based on key words of breast cancer and survival rate, and in international databases, with "breast cancer" and the equivalent of "neoplasm" of Mesh Word, "survival rate" and "Iran." Then all reviewed papers and theses which met the inclusion criteria were selected for investigation. To conduct the analysis STATA 11.2 software and random-effects models were used. Results: In 24 studies, 22,745 participants were included. The one-year, three-year, five-year and ten-year survival rates were 0.956, 0.808, 0.695 and 0.559, respectively. The minimum and maximum survival rates for 5-years were 0.48 and 0.87. The average age of the onset of the disease was 48.3. Conclusions: As in Iran, since the onset of the disease is at low age, in spite of the relatively high survival rate as compared to other cancers, prevention and screening programs at early age for early stage diagnosis seems necessary.

성문상부암 방사선치료 15년 성적 (The Result of Radiation Therapy of Supraglottic Laryngeal Cancer for 15 rears)

  • To assess the result of radiation therapy for fifteen fears experience, a total of 125 cases of pathologically proven supraglottic laryngeal cancer had been analyzed according to patient survival retrospectively. All the patients had been treated with radiation therapy in curative aim using Co-60 teletherapy machine. The results are as follows ; 1. According to AJCC staging, five year survival rate was $58.3\%$ in stage I, $44.4\%$ in II, $31.8\%$ in II, and $28.6\%$ in IV. 2. According to T-staging, five year survival rate was $57.1\%$ in T1, $40.5\%$ in T2, $34.0\%$ in T3, and $19.0\%$ in T4. 3. According to N staging, five year survival rate was $43.5\%$ in negative node group and $26.8\%$ in positive node group. 4. According to the histologic grade, the better in differentiation, the more in number of cases and the better in prognosis. 5. In summary, five year actuarial survival rate was $37.3\%$ and ten year surrival rate was $34.2\%$, and ten year survivors totalled 16 cases.

성문암 방사선치료 15년 성적 (The Result of Radiation Therapy of Vocal Cord Cancer for 15 Years)

  • To assess the result of radiation therapy for fifteen years experience, a total of 81 cases of pathologically proven vocal cord cancer had been analysed according to patient survival retrospectively. All the patients had been treated with radiation therapy using Co-60 teletherapy unit in curative aim. The results are as follows ; 1. According to T-staging, five year survival rate was $75.0\%$ in T1, $73.1\%$ in T2, $24.3\%$ in T3, and $25.0\%$ in T4. 5, According to nodal status, five year survival rate was $59.4\%$ in negative node group and $14.2\%$ in positive group. 4. According to the histologic grade, the better in differentiation, the more in number of cases and the better in prognosis. 5. In summary, five year actuarial survival rate was $55.5\%$ and ten year survival rate was $49.8\%$ and ten year survivors totalled 12 cases.

단근, 줄기 절단과 식재 밀도에 따른 음나무(Kalopanax septemlobus) 묘목의 활착 및 생장 특성 (Effects of Root Pruning, Stem Cutting and Planting Density on Survival and Growth Characteristics in Kalopanax septemlobus Seedlings)

  • Kalopanax septemlobus (Thunb. ex Murray) Koidz. is natively distributed in Korea. The importance of this species has been increased not only for high quality timber but for medicinal and edible uses. However, increasing market demand of K. septemlobus with illegal cutting and overexploitation has resulted in its rapid depletion and destruction of natural habitat. This study was conducted to understand the survival rate and growth characteristics of planted K. septemlobus seedlings with treatment of root pruning, stem cutting and planting density. The survival rate and growth of height and root-collar diameter for one- and two-year old seedlings with different planting densities were investigated in the clear-cut area of a Pinus densiflora stand for five years. One-year-old seedlings were treated with or without root pruning and planted with three density levels (5,000 trees $ha^{-1}$, 10,000 trees $ha^{-1}$, and 40,000 trees $ha^{-1}$). Two-year-old seedlings were treated with and without stem cutting and planted with the density of 5,000 trees $ha^{-1}$. The survival rate of one-year-old seedlings with root pruning treatment in the density of 10,000 trees $ha^{-1}$ was 92%, while that without root pruning in the density of 40,000 trees $ha^{-1}$ was 67% after five years. The height of one-year-old seedlings has been significantly affected only by planting density in the $5^{th}$ year. The survival rate of the two-year-old seedlings with stem cutting was 75.5% and greater than control (67.3%) in the $5^{th}$ year but no difference in height was shown between the two treatments from three years after plantation.

초기 자궁경부암의 수술후 방사선 치료 (Postoperative Radiotherapy for the Early Stage Carcinoma of the Uterine Cervix)

  • This is a retrospective cohort analysis of 58 patients who treated with postoperative radiation therapy following radical hysterectomy and bilateral pelvic adenectomy for early stage carcinoma of uterine cervix between January 1988 and December 1990 at department of radiation oncology, Keimyoung University Hospital. Sixteen percent of patients (9/58) had chemotherapy. Most patients were FIGO I b (47 patients), and FIGO I a and II a were one and ten patients, respectively. The median follow-up periods were 48.5 months. The indications for radiation therapy included pelvic lymph node metastasis, large tumor size, deep stromal invasion, lymphovascular invasion, positive surgical margin, endometrial invasion and parametrial invasion. Eighty five percent of the patients had more than one risk factor. The actuarial overall five year survival rate (5 YSR) and five year disease free survival rate (5 YDFSR) were $89.5\%,\;and\;87.8\%,$ respectively. Their overall recurrence rate was $12.1\%,$(758). Distant metastasis was the most common cause of treatment failure $(71.4\%:5/7).$ The univariate analysis of prognostic factors affecting to five year survival rate disclosed pelvic lymph node status (negative: $95.5\%,\;positive:69.2\%,$ p=0.006) and hemoglobin level $(\le11 :75\%,>11g/dl:93.3\%,p=0.05)$ as significant factor. The age status was marginally significant $(\le40:96.0\%,\;>\;40:84.3\%p=0.15).$ Multivariate analysis clarified three independent prognostic factors: pelvic lymph node metastasis (p=p.006), hemoglobin level (p=0.015) and age (p=0.035). Multivariate analysis of prognostic factor affecting to five year disease free survival rate disclosed pelvic lymph node status (p=0.0078) and status of surgical margin (p=0.008). Complications relating to radiotherapy were $10.3\%,(6/58).$ There were no severe major complication requiring surgical intervention or a long hospital stay. It is our opinion that the benefit of postoperative pelvic radiotherapy may be gained in such a high risk patient population with acceptible morbidity.

원발성 외이도암의 재발 양상 (The Recurrence Pattern of Primary External Auditory Canal Cancer)

  • Background/Objective: The survival and recurrence pattern of the patients with primary cancer of the external auditory canal was evaluated. Materials & Methods: Seventeen patients with primary cancer of the external auditory canal from 2002 to 2013 was analyzed retrospectively. Overall survival, recurrence free survival, local recurrence free survival and distant metastasis free survival was calculated by Kaplan-Meyer's method. Results: Five year overall survival rate, 5 year recurrence free survival rate, 5 year local recurrence free survival rate and 5 year distant metastasis free survival rate were 49.3% 54.9%, 64.7% and 69.6% respectively. The recurrence pattern is different to the pathologic type, squamous cell carcinoma or adenoid cystic carcinoma. The patients with squamous cell carcinoma showed local recurrence and the patients with adenoid cystic carcinoma showed distant metastasis mainly. Conclusion: Primary cancer of external auditory canal showed different clinical course depend on the pathologic diagnosis.
