THE EFFECT OF PRIMER ON PENETRATION OF SEALANT (치면열구전색제의 열구 침투에 primer의 효과)

  • The objective of this study is to confirm the effect of dentine bonding primer application on penetration of sealant. Extracted permanent molars were used to compare penetration success rate of control group (sealant application only) and experimental groups (sealant application after applying the primers of $Scotchbond^{TM}$ Multi-Purpose system and $All-Bond^{(R)}$ 2 system). The following results were obtained: 1. The experimental groups using the primers showed increased sealant penetration success rate to the base of fissure when compared to control group but there was no statistically significant differences(p>0.05). 2. The depth, width and 'depth/width' value of fissure had statistically significant effect on sealant penetration success rate(p<0.05). 3. The penetration success rate decreased about 0.9 times as the depth of fissure increased every $25{\mu}m$, and increased about 1.1 times as the width of the fissure orifice increased every $25{\mu}m$ and decreased about 0.6 times as the 'depth/width' value increased every 1. From the above results, it can be concluded that assure morphology had a great effect on sealant penetration and for better penetration, use of dentine bonding primer can be helpful but it needs more study in clinical bases.

Evaluation of Microleakage and Penetration Ability of Flowable Resin in Occlusal Fissure (유동성 레진의 교합면 열구내 침투도와 미세누출 평가)

  • The aim of this study was to compared the penetration scores and microleakage levels of flowable resin in comparison to resin-based pit and fissure sealant. A total 120 extracted premolars were used and classified with group I (resin-based pit and fissure sealant), group II (flowable resin), group III (bonding agent + flowable resin), IV (fissurotomy + bonding agent + flowable resin) depending on the materials and the application methods. The penetration scores of the group treated with flowable resin following fissurotomy were similar to those of the group treated with resin-based sealant, but higher compared to those of the other groups treated with flowable resin without fissurotomy or bonding agent. The group treated with flowable resin following fissurotomy also exhibited the lowest microleakage levels. The group treated with resin-based sealant and the one treated with a bonding agent prior to the application of flowable resin showed similar microleakage levels. In conclusion, flowable resin may be applied as a pit and fissure sealant, and its application along with fissurotomy could increase the penetration scores.

THE EFFECT OF FLUORIDE PRETREATMENT ON SHEAR BOND STRENGTH BETWEEN ENAMEL AND FISSURE SEALANT (불소 전처리가 법랑질과 치면열구전색재의 전단결합강도에 미치는 영향)

  • This study seeks to know the effect of fluoride topical application on the shear bond strength between enamel and fissure sealant. On group 1,2,3,4, Teethmate A(unfilled sealant) and on group 5,6,7,8 Ultraseal XT(filled sealant) were used. Group 1 and 5 were not pre-treated with fluoride and group 2 and 6 were pre-treated with 1.23% APF for 4minutes. Group 3 and 7 were pretreated with 2.0% NaF for 4 minutes. and group 4 and 8 were pumiced with abrasive containing fluoride for 10 seconds. Prepared specimens were acid etched for 30 seconds using 35% phosphoric acid, and then sealant was cured to the specimen using a 3mm diameter by 2mm height mold. They went through thermocycling. Its shear bond strength was measured, and then acid etched both groups that were pretreated with fluoride and that were not pretreated with fluoride. Then we examined the surface of the specimen with EM and came up with these results. In groups using unfilled sealant(group 1,2,3.4) there were no significant differences between oops pretreated with fluoride and groups not pretreated with fluoride. In groups using filled sealant(group 5,6,7,8), groups 6 and 7(treated with 1.23% APF 2.0% NaF respectively) showed significantly lower shear bond strength, compared to group 5 (not pretreated with fluoride) and group 8 (pumiced with abrasive containing fluoride) (p<0.05).

Antibacterial Properties of Pit and Fissure Sealant Containing S-PRG filler on Streptococcus mutans (S-PRG filler를 포함한 치면열구전색제의 Streptococcus mutans에 대한 항미생물 특성에 관한 연구)

  • The purpose of this study was to evaluate the antibacterial properties of a sealant containing S-PRG filler compared to those of two contemporary commercial sealants to determine the inhibition of bacterial growth in broth culture and biofilm formation using the CDC Biofilm Reactor. The BeautiSealant containing S-PRG filler, the fluoride releasing Clinpro$^{TM}$ sealant, which are known to have higher antibacterial effects, and the non-fluoride releasing Concise$^{TM}$ sealant were selected for this study. A Streptococcus mutans culture in BHI broth without sealant served as a negative control in the planktonic growth inhibition test. As a result, bacterial growth was inhibited in all three sealant groups compared to that in the control. The Clinpro$^{TM}$ sealant showed a significantly reduced number of CFUs compared to those of the BeautiSealant and Concise$^{TM}$ sealants. However, no significant difference was detected between the BeautiSealant and Concise$^{TM}$ sealants. The Clinpro$^{TM}$ sealant significantly decreased biofilm formation compared to that by the BeautiSealant and Concise$^{TM}$ sealants. No significant difference was observed between the BeautiSealant and Concise$^{TM}$ sealants. In conclusion, the sealant containing S-PRG filler had a less potent anti-bacterial property and increased biofilm formation capacity compared to those of the fluoride releasing Clinpro$^{TM}$ sealant.

A SURVEY ON THE USING STATUS AND PERCEPTION OF PIT AND FISSURE SEALANT (치면열구전색제 사용실태와 인식에 관한 조사)

  • The property of pit and fissure sealant has been improved and many studies on the bond strength, penetration, microleakage have been published. But there are few studies on the using status and perception of pit and fissure sealant within the country. Therefore, this study made a survey on it. Pedodontists and non-pedodontists were surveyed by interview. The Results were as follows; 1. On caries prevention effect, 96.7% of the pedodontists replied that sealants were effective on both permanent teeth and primary teeth. On the other hand, 13.5% of the non-pedodontists replied that sealants weren't effective on both. 2. All of the pedodontists and 27% of the non-pedodontists used rubber dams. 83.3% of the pedodontists and 40.5% of the non-pedodontists used bonding agents. 3. Non-pedodontists used enameloplasty more frequently than Pedodontists but the pattern was not significantly different. 4. The causes of sealant failures included salivary contamination, caries under sealant, low strength, low flowability, overfilling. 5. In the pedodontists, 90% replied that PRR application was desirable and PRR applications were more frequent than sealant application.

Evaluation of sealant microleakage using Swept-Source Optical Coherence Tomography (치면열구전색치아의 미세누출 평가를 위한 Swept-Source Optical Coherence Tomography의 타당도 평가)

  • Purpose: The aim of this study was to evaluate the validity of swept-source optical coherence tomography (SS-OCT) for detecting sealant microleakage. Materials and Methods: A total of 31 extracted sound human molar, assigned as suitable for sealant application, were chosen and divided into two groups: (1) no microleakage group and (2) microleakage group by applying sealant using the different methods. All specimens were immersed in 1% methylene blue for 24 h and sectioned to confirm the absence or presence of sealant microleakage as a gold standard method. Subsequently, all the sectioned specimens were digitally photographed using a microscope with a magnification of ${\times}50$. Presence of microleakage was evaluated on a 2-point rating scale. The association of histological method with conventional methods (visual and tactile assessment) and SS-OCT was assessed using a chi-squared test. The intra- and inter-examiner reliability was calculated using Cohen's Kappa. Results: The SS-OCT showed a sensitivity of 0.73 and a specificity of 1.00, while visual and tactile assessment showed a sensitivity of 0.67 and a specificity of 0.86. The inter-examiner reliability of SS-OCT was 0.79, whereas that of the visual and tactile assessment was 0.53. Conclusions: SS-OCT can be used to non-invasively detect sealant microleakage and to monitor pit and fissure sealant in the clinics.

  • It is introduced that pit and fissure sealant is the most universal and effective to prevent occlusal dental caries. In processing of being applied the pit and fissure sealant, the various kinds of methods are developed to remove organic matters, plaque, microflora and debris in the pit and fissure for increasing the rates of maintain the sealant. Recently, the Er:YAG laser has been used as a new enamel surface treatment method. The purpose of this thesis is compared whether that enamel surface treatment method is superior to other methods or not. 1. 100mJ 5Hz Er:YAG lased enamel surface was similar to acid-etched enamel in SEM evaluation. 2, Mechanical preparation showed decreased microleakage when compared with acid-etching only, but no significant differences in both method. 3. After laser and acid-etching method showed decreased microleakage when compared with acid-etching only.

Experimental Research on the Surface Changes and the Abrasion Resistance of Pit and Fissure Sealant by Fluoride (불소도포제에 의한 치면열구전색제의 마모저항성과 표면 변화에 관한 실험연구)

  • To test the effects of representative fluoride vanishes-1.23% APF gel and 5% NaF Fluoride Varnish-on the surface structure of pit and fissure sealant, this study classified samples of pit and fissure sealant into five groups: Group I deposited in distilled water for ten minutes, Group II treated with APF gel for one minute, Group III treated with APF gel for four minutes, Group IV treated with Fluoride Varnish for one minute, and Group V treated with Fluoride Varnish for four minutes. An abrasion test was carried out to measure changes in weight, along with observation of the surface structure by using an optical microscope, consequently drawing the following conclusions. 1. The results of the abrasion test using pit and fissure sealant, Concise, showed that Group III had the reduction in weight more increased than Group I and that Group V had less reduction in weight (p<0.05); the results of the abrasion test using Eco-S showed Group III had the reduction in weight more increased than Group I and that Group V had less reduction in weight (p<0.05). 2. The results of observation using an optical microscope showed that application of APF gel made the filler remarkable due to loss of substrate and that Group III treated with APF gel for four minutes had the toughest surface, followed by Group II treated with APF gel for one minute, Group I deposited in distilled water for ten minutes, Group IV treated with Fluoride Varnish for one minute, and Group V treated with Fluoride Varnish for four minutes.

Reliability of Q-Ray View for Assessing Retention Status of Pit and Fissure Sealant (Q-Ray View를 이용한 치면열구전색재의 유지상태 평가)

  • Purpose: To evaluate reliability of Q-ray view (Aiobio Inc,. Seoul, Korea) for assessing retention status of pit and fissure sealants. Methods: Pit and fissure sealants of 58 permanent molars from 15 third-grade students were examined. Posterior teeth with ≥1 pit and fissure sealants applied to the occlusal surface for >6 months were examined. The teeth were examined using traditional visual-tactile assessments and combined Q-ray view. Pit and fissure sealants were evaluated by assessing marginal plaque, marginal discoloration, marginal integrity, retention, and presence of caries. Fleiss kappa and Cohen's kappa values were calculated to compare inter- and intrarater agreements between visual-tactile and combined Q-ray view assessments. Results: Regarding interrater agreement in visual-tactile assessments, K values of Cohen's kappa for marginal plaque, marginal discoloration, and presence of caries were 0.22-0.57, 0.36-0.57, and 0.43-0.61, respectively, and agreements ranged from slight to moderate. When combined with Q-ray view, the values were 0.81-0.89, 0.69-0.88, and 0.80-0.90, respectively, and agreements ranged from substantial to nearly perfect level, indicating statistical significance. Marginal plaque (0.81-0.83), marginal discoloration (0.57-0.89), and presence of caries (0.69-0.91) showed higher agreements in combined Q-ray view than in visual-tactile assessments, and kappa values of marginal plaques were significantly higher in combined Q-ray view than in visual-tactile assessments. Conclusion: Evaluating retention status of pit and fissure sealants using Q-ray view showed higher reliability than using visual/tactile assessments for marginal plaque, marginal discoloration, and presence of caries. Therefore, Q-ray view may be used to assess the retention status of pit and fissure sealants.

Effect of Physical Properties and Bacterial Adherence Inhibition of Pit and Fissure Sealant Containing Bioactive Glass Nano Particles(BGn) (생체활성 유리 나노입자 첨가량에 따른 치면열구전색제의 물성평가와 세균부착 억제 효과)

  • In this study BGn-incorporated non-fluoride release of pit and fissure sealant $Concise^{TM}$ was developed to improve the mechanical properties and promote antibacterial effect of fit and fissure sealant with the original material. The mechanical properties and antibacterial activity of BGn incorporating vary-ing amounts bioactive glass nano particles(BGn) (0,0.5,1.0 and 2.0 wt% in sealant) were characterized composition of the resulting were investigated. The solubility to aid absorption was calculated by weighing specimens with a diameter of 10 mm and a thickness of 2 mm according to ISO 4049 (2009). The antimicrobial effect was evaluated using three strains of S. mutans, S. aureus and E. coli. The absorbance of the test results was as high as the addition of BGn increased, and the lower the solubility as the solubility was added(p<0.05). Adhesion experiment results S. mutans in contrast to the control group $Concise^{TM}$, BGn-added experimental group showed a somewhat lower adherent surface but no statistically significant difference was observed (p<0.05). However S. aureus and E. coli statistical analysis indicated a significant difference for antibacterial agents between control and BGn containing(p<0.05). It seems that this BGn proved that even a antibacterial effect was demonstrated. Therefore, it was suggest that the additional effects of BGn and research on a wide range of substances.
